ABSTRACT

BACKGROUND: Guidelines recommend atezolizumab plus nab-paclitaxel (A + nP) for first-line treatment of unresectable, locally advanced, or metastatic triple-negative breast cancer expressing programmed death-ligand 1 (PD-L1) on tumor-infiltrating immune cells (IC), based on IMpassion130. We report the final overall survival (OS) and safety of that study as per the prespecified analysis plan. PATIENTS AND METHODS: Patients were randomized to nP 100 mg/m2 (days 1, 8, and 15 of a 28-day cycle) with atezolizumab 840 mg (A + nP) or placebo (P + nP; days 1 and 15), until progression or unacceptable toxicity. Coprimary endpoints were progression-free survival [intention-to-treat (ITT) and PD-L1 IC-positive populations] and OS (tested hierarchically in the ITT population and, if significant, in the PD-L1 IC-positive population). RESULTS: Each arm comprised 451 patients; 666 (73.8%) had died by the final OS analysis cut-off (median follow-up, 18.8 months; interquartile range, 8.9-34.7 months). Median OS in the ITT population was 21.0 months [95% confidence interval (CI), 19.0-23.4 months] with A + nP, and 18.7 months (95% CI, 16.9-20.8 months) with P + nP [stratified hazard ratio (HR), 0.87; 95% CI, 0.75-1.02; P = 0.077]. Exploratory analysis in the PD-L1 IC-positive population showed a median OS of 25.4 months (95% CI, 19.6-30.7 months) with A + nP (n = 185) and 17.9 months (95% CI, 13.6-20.3 months) with P + nP (n = 184; stratified HR, 0.67; 95% CI, 0.53-0.86). Safety outcomes were consistent with previous analyses and the known toxicity profiles of each agent. Immune-mediated adverse events of special interest were reported in 58.7% and 41.6% of patients treated with A + nP and P + nP, respectively. CONCLUSION: Although the OS benefit in the ITT population was not statistically significant, precluding formal testing, clinically meaningful OS benefit was observed with A + nP in PD-L1 IC-positive patients, consistent with prior interim analyses. This combination remained safe and tolerable with longer follow-up.

ABSTRACT

Background: Head and neck cancer (HNC) has a poor prognosis at advanced stages. Given the immunosuppressive tumor microenvironment in HNC, inhibition of the programmed death-ligand 1/programmed death-1 (PD-L1/PD-1) signaling pathway represents a promising therapeutic approach. Atezolizumab (anti-PD-L1) is efficacious against many tumor types. Here we report the clinical safety and activity from the HNC cohort of the phase Ia PCD4989g clinical trial. Patients and methods: Patients with previously treated, advanced HNC received atezolizumab i.v. every 3 weeks for 16 cycles, up to 1 year or until loss of clinical benefit. Patients were monitored for safety and tolerability and evaluated for response at least every 6 weeks. Baseline PD-L1 expression level and human papillomavirus (HPV) status were evaluated. Results: Thirty-two patients were enrolled; 7 patients (22%) had a primary tumor in the oral cavity, 18 (56%) in the oropharynx, 1 (3%) in the hypopharynx, 2 (6%) in the larynx, and 4 (13%) in the nasopharynx. Seventeen patients (53%) had ≥2 prior lines of therapy. Twenty-one patients (66%) experienced a treatment-related adverse event (TRAE), with three experiencing grade 3 TRAEs and one experiencing a grade 4 TRAE (per CTCAE v4.0). No grade 5 TRAEs were reported. Objective responses by Response Evaluation Criteria in Solid Tumors version 1.1 (RECIST v1.1) occurred in 22% of patients, with a median duration of response of 7.4 months (range 2.8-45.8 months). Median progression-free survival was 2.6 months (range 0.5-48.4 months), and median overall survival was 6.0 months (range 0.5-51.6+ months). Responses showed no association with HPV status or PD-L1 expression level. Conclusions: In this heavily pre-treated advanced HNC cohort, atezolizumab had a tolerable safety profile and encouraging activity, with responses observed regardless of HPV status and PD-L1 expression level. These findings warrant further investigation of atezolizumab in HNC. ClinicalTrials.gov number: NCT01375842.

ABSTRACT

INTRODUCTION: Health-related quality of life perceived by children and teenagers is important to assess the effects of therapeutic intervention. AIM: To analyze quality of life, comparing cases of attention deficit hyperactivity disorder (ADHD) treated with methylphenidate, untreated cases and controls. SUBJECTS AND METHODS: Sampling of 228 participants between 8 and 14 years-old. Consecutive sampling in ADHD according to DSM-IV criteria (ADHD Rating Scales IV) and random sampling of matched controls by sex and age. Evaluation of quality of life using KIDSCREEN-52 (children version). ANOVA with Bonferroni correction was used. RESULTS: There is a moderate significant correlation between greater intensity of ADHD symptoms and worse quality of life, except in the dimension of physical well-being. Cases of untreated ADHD have significantly worse quality of life than controls on psychic well-being, mood, autonomy school environment and social acceptance. Cases of treated ADHD present similar results, except in the school environment and psychological well-being. The cases of ADHD treated only differ significantly from ADHD not treated in having a better school environment. CONCLUSIONS: The cases of ADHD present dimensions of KIDSCREEN-52 with worse quality of life than controls and the cases of ADHD treated with methylphenidate only differ significantly from those not treated in presenting better results in the school environment.

Conclusion.

ABSTRACT

Introduction: research into drug consumption agrees on the relationship between the perception of the risk associated with the consumption of a substance and the probability of consuming it. Objective: To find out the perception of danger in the occasional consumption of conventional and electronic cigarettes in adolescents aged between 13 and 18 attending school in Castile & Leon (academic year 2014-2015), as well as their relation with other sociodemographic factors. Material and methods: a standardized, telematic and anonymous questionnaire was used with analogous questions to those used in national and international programmes to discover the risk perception concerning the consumption of drugs among adolescents. A descriptive, cross-sectional study with a random, two-stage cluster sample was also used. A probability of 50% was estimated in all our answers, with a confidence interval of 95.5% and an error probability in bilateral contrast of 1.7%. The final sample was made up of 3,311 adolescents (mean age 14.8±1.3 years). Results: we found a high perception of risk for the occasional consumption of tobacco among adolescents (74.3% considering it very dangerous, n = 2458). However, with the electronic cigarette there is a lower perception of risk (only 65.9% considered it to be very dangerous), with no statistically significant differences in relation to age or family functionality (p < 0.005 respectively). Women (odds ratio: 1.28) and adolescents from public centres (p ≤ 0.01 respectively) show a lower perception of danger. Conclusions: the e-cigarettes could become an additional way of initiatiating tobacco smoking in adolescents due to their accessibility, their attractive flavours and the low perception of danger associated with their consumption in early adolescence (AU)

ABSTRACT

Introduction: the adolescent population is particularly vulnerable to the risks related with their sexual behavior, because it is a period of maturation and experience which is part of this evolutionary stage. Teenage pregnancy and early motherhood are associated with school failure, impaired physical and mental health, social isolation, poverty and other related factors. Moreover the non-use of appropriate protective barriers increases the risk of sexually transmitted infections (STI), with serious consequences in the short and long term biopsychosocial adolescent health. Objectives: to determine the characteristics of adolescent sexuality in the province of Valladolid. Methods: cross-sectional study, using an anonymous survey to 2412 schoolchildren 13-18 years old in the academic year 2011-2012. Results: 81% of students feel they have very good or good information on sexuality. 75% of students believe that the information they have about STI is very good or good. 30.4% have had sexual intercourse. The age of first complete sexual intercourse is between 15 and 16 years (50.9%). Most of the adolescents use a contraceptive method in their relationships (91.3%), but there are still 8.7% referring not using any. Most of the students use the condom for contraceptive method (89.6%). 20.9% of teens who have had sex with penetration have used 'the morning-after pill' or emergency contraception at some time. 3.6% reported having being pregnant or having left his partner pregnant (AU)

ABSTRACT

Resultados: hemos estudiado a 563 varones y 726 mujeres.

Introduction: in the last years, sexual initiation in our young people seems to have anticipated. In this study we want to find out what risk factors have statistically significant relation to having sexual relations with penetration in adolescents. Methods: 1289 students of 2.º and 3.º grade of Secondary Education in the region of Castilla-León (Spain) were interrogated in the 2010-2011 school year. Multiple epidemiological variables were analyzed, calculating centralization and dispersion in quantitative variables, and distribution of frequencies in qualitative variables. Results: 563 boys and 726 girls were studied. The students’ media age was 14.05 years (median and mode: 14 years; range: 12-17 years). 17.5% had had sexual relations with penetration. (9% at 12; 6% at 13; 10.6% at 14; 34.2% at 15; 53.2% at 16 and 57.1% at 17 years). This percentage was greater in boys (20.2%) than girls (15.4%). Other statistically significant associated factors (p< 0.05) were being older, worse relation with his parents, divorced parents, worse relation between their parents, smoking, drinking alcohol, getting drunk, consuming drugs, worse present health, worse feeding, worse memory, having more erotophilia and being agnostic or atheist. Conclusions: adolescent sexual precocious initiation risk factors are alcohol, tobacco, drug consumption, a bad relationship with their parents and between their parents (AU)

ABSTRACT

Objectives: To determine the frequency and use of new information and communication technologies (ICT) in students of the province of Valladolid, detect risks and assess inappropriate behavior. Methods: Cross-sectional study, using an anonymous survey completed by 2,412 schoolchildren from 13 to 18 years between March and May 2012. Results: 96.6% had a mobile phone, 43% smartphone. 62.2% referred limitation for use, mainly overspending and bringing it to class. Computer use was the fourth in the ranking of the leisure activities of adolescents. 82.5% connected to Internet almost every day. Second year high school students (age 17-18) connected more frequently than second year secondary school students (aged 13-14) (92.1 vs 70.4%; p <0.001). The three main reasons for using Internet were social networks, music download and information, with differences by sex and age. Regarding the risk of the use of ICT the girls reported greater reliance, while boys manifested more risk behaviors. Conclusions: The use of ICT was almost universal in adolescents. Low perception of risk was observed. The risk factors found were overuse, dependence, access to inappropriate content, cyberbullying cases, grooming, sexting and overspending. Some differences were detected by sex, age, urban or rural habitat, type of school, socioeconomic status and academic performance. To minimize these situations, children and adolescents need information about the safe and responsible use of ICT (AU)

ABSTRACT

Introduction: one of the most important problems in our public health system nowadays is drug consumption and the risky behaviors related to it. Objective: to analyze drug consumption prevalence in adolescents attending high school in the province of Valladolid and its relation with other factors. Material and methods: a 101 question survey about alcohol consumption, school performance, leisure time, accidents, tobacco, drugs, mistreatment, relationship with other persons, food habits and sexual behavior has been used. The final number of students 13 to 18 years old surveyed was 2,412 in the Valladolid province during the year 2012. Results: had taken at least one alcoholic drink 77.2%, 36.7% had smoked tobacco, 17% had smoked cannabis, 2.5% had taken relaxing and sleeping pills with medical prescription, 1.9% had taken speed or amphetamines, 1.3% had taken relaxing and sleeping pills without medical prescription, 1.1% had taken speed, 1% had consumed cocaine and 0.7% had inhaled volatile inhalants at some time. A proportion of 38.3% of them had consumed two or more substances. The average for starting tobacco and alcohol consumption is from 13 to 14 years old, and 15 to 16 years old for illegal drugs. There has been a correlation between some risky habits and the factors that helped to develop them. Conclusions: a lot of adolescences start drug use in the pediatric age. Multidrug use is a prevalent standard that increases risks. Pediatricians in Primary Medical Care have a lot of work to do in different points such as prevention, diagnosis and treatment
