ABSTRACT

We report the case of an 80-years-old male patient who presented with hematochezia without hemodynamic compromise and with a history of laryngeal squamous-cell carcinoma that was operated on 2 weeks before the bleeding episode.

ABSTRACT

BACKGROUND AND AIMS: Vasoactive agents with endoscopic therapy are used to treat acute variceal bleeding (AVB). There are two main groups of vasoactive agents: terlipressin and vasopressin (T-V), and octreotide and somatostatin (O-S). However, the benefit/harm balance is unclear. Our aim was to assess the efficacy and safety of T-V versus O-S for the management of AVB. METHODS: We performed a systematic search for randomized controlled trials (RCTs) in PubMed, Scopus, and CENTRAL. Our main outcomes were mortality and adverse events. Secondary outcomes were bleeding control, rebleeding, blood transfusion, hospital stay. We evaluated the certainty of evidence using GRADE methodology. RESULTS: We included 21 RCTs. The risk of mortality (RR: 1.01; 95%CI: 0.83-1.22), bleeding control (RR: 0.96; 95%CI: 0.91-1.02; I 2 =53%), early rebleeding (RR: 0.91; 95%CI: 0.66-1.24: I 2 =0%), late rebleeding (RR: 0.94; 95 CI: 0.56-1.60; I 2 =0%), blood transfusion (MD: 0.04; 95%CI: -0.31-0.39; I 2 =68%) and hospital stay (MD: -1.06; 95%CI: -2.80-0.69; I 2 =0%) were similar between T-V and O-S groups. Only 15 studies reported adverse events, which were significantly higher in the T-V compared to the O-S group (RR 2.39; 95%CI: 1.58-3.63; I 2 =57%). The certainty of evidence was moderate for the main outcomes, and low or very low for others. CONCLUSIONS: In cirrhotic patients with AVB, those treated with T-V had similar mortality risk compared to O-S. However, the use of T-V showed an increased risk of adverse events compared to O-S.

OBJECTIVE: The ideal therapy for chronic hepatitis C is the use of direct acting antivirals (DAA). In Peru there is no data in this aspect, in that sense it is necessary to report real life experience with these drugs. MATERIAL AND METHODS: A digital survey was sent through e-mail to hepatologists, and the data of four is considered in this study. Statistical analysis was descriptive. RESULTS: We included 63 patients, mean age was 59 years, 49.21% were male, cirrhosis was present in 49.21%, and 34.92% was non-responder to PEGIFN and Ribavirin. Genotype 1 was present in 93.65%, and subtype 1a was 58.73%, there were only 2 cases with Gt 2 and one with Gt 3. There were 10 different DAA combinations used, and the most effective were Sofosbuvir/Ledipasvir, Sofosbuvir/Ledipasvir/Ribavirina and Sofosbuvir/Simeprevir, in all these cases the Sustained Viral Response (SVR) was 100%. The other combinations had SVR < 90% or only 1-2 patients included. All patients tolerated treatments and no serious adverse events occurred. CONCLUSIONS: In real life antiviral treatment for hepatitis C with AAD is effective and well tolerated. The best SVR was obtained with Sofosbuvir/Ledipasvir, Sofosbuvir/Ledipasvir/Ribavirina and Sofosbuvir/Simeprevir. This report may be useful to consider treatment strategies with focus in public health.

ABSTRACT

Objetivos: La terapia ideal para la hepatitis crónica C consiste en el uso de drogas antivirales de acción directa (DAA). En el Perú la experiencia en vida real con DAA no se conoce, por lo que el objetivo del presente estudio es reportar la alta eficacia terapéutica con estos esquemas. Material y métodos: Mediante correo electrónico se invitó a participar a través de una encuesta a médicos hepatólogos a nivel nacional. Se incluyeron los datos de 4 médicos. Los resultados fueron analizados con estadística descriptiva. Resultados: Se incluyeron 63 pacientes, la edad promedio fue 59 años, varones fueron 49,21%, cirrosis estuvo presente en el 49,21% (31/63), 34,92% había sido no respondedor a terapia con PEGIFN/RBV. El Genotipo 1 estuvo presente en 93,65% de casos, siendo el 1a el predominante (58,73%). Solo hubo dos casos de genotipo 2 y uno de genotipo 3. Se utilizaron 10 esquemas de combinación con DAA, siendo los más eficaces, Sofosbuvir/Ledipasvir, Sofosbuvir/Ledipasvir/ Ribavirina y Sofosbuvir/Simeprevir, en todos ellos se logró Respuesta Viral Sostenida (RVS) de 100%. Con los otros 7 esquemas la RVS fue menor a 90% o solo se había incluido uno o dos pacientes. La tolerancia a la terapia fue adecuada y todos los pacientes culminaron la terapia. Conclusiones: En vida real los esquemas de terapia antiviral para hepatitis C con DAA tienen alta eficacia y seguridad. Las mejores respuestas se obtuvieron con Sofosbuvir/Ledipasvir, Sofosbuvir/Ledipasvir/Ribavirina y Sofosbuvir/Simeprevir. Esta data puede ser útil para considerar estrategias de tratamiento con el enfoque de salud pública.

ABSTRACT

Objetivo: El objetivo del presente estudio fue determinar la prevalencia de seropositividad para HBsAg, Anti-HBcAg y Anti- HVC del Banco de Sangre del Hospital Nacional Daniel Alcides Carrión (HNDAC) durante el periodo 2010 al 2012. Materiales y métodos: Estudio transversal retrospectivo. Se incluyó a los potenciales donadores. Se recolectaron las características tales como edad, sexo y conductas de riesgo. Se realizó el análisis descriptivo con el programa STATA 14. Resultados: Se incluyó 13 887 potenciales donantes del HNDAC entre enero 2010 y diciembre 2012. Se identificaron 897 potenciales donantes positivos. La prevalencia de HBsAg fue 0,55%; Anti-HBcAg, 5,15%; y Anti-HVC, 1,25%. De ellos se encontró edad promedio de 37,4 años para los pacientes infectados por virus de hepatitis B y de 36,9 para los pacientes infectados por virus de hepatitis C, 31,2% fueron mujeres del total de infectados. Conclusión: La prevalencia de serología positiva para virus de hepatitis B fue similar a reportes anteriores, por otro lado la serología positiva para virus de hepatitis C fue mayor a lo reportado en nuestro país

ABSTRACT

OBJECTIVE: To describe the clinical and endoscopic features of caustics injuries in the upper gastrointestinal tract in patients of the National Hospital Daniel Alcides Carrión. MATERIALS AND METHODS: A descriptive study was conducted; the study populations were patients diagnosed with caustic ingestion who were admitted into the Gastroenterology service of the HNDAC to perform an upper endoscopy during the period of January 2009 to December 2012. We documented the type of caustic substance ingested, cause of intake, amount ingested, intake mode, signs or symptoms present, endoscopic findings as classified by Zargar, presence of complications and treatment performed. Analysis was performed comparing them based on the ingested caustic type: acid or alkali. RESULTS: We obtained 91 patients; the average age was 30.6±16.3 years. Caustic substances ingested were: bleach (sodium hypochlorite) in 71 (78%) patients, muriatic acid (hydrochloric acid) in 18 (20%) patients, caustic soda in 2 (2%) patients. The average intake was 136ml (30-500 ml). The most frequent signs and symptoms were vomiting, abdominal pain, nausea and sore throat. The 46% of patients had injuries. 100% of patients who ingested acid and 33% who ingested alkali had lesions. Five (5%) patients had oropharyngeal lesions, 24 (26%) in the esophagus, 36 (40%) in the stomach and 12 (13%) in the duodenum. Grade I lesions were found in 10 (11%) patients, 16 (18%) patients with grade II lesions and 15 (16%) of patients with grade III, 12 patients who toke acid had lesions of grade III. Esophageal stricture was found in 2 (2%) patients, gastric stenosis in 7 (8%) patients and esophageal and gastric stenosis in 3 (3%) patients. Two patients required pneumatic dilation and 10 surgical treatments. CONCLUSION: Most injuries were found in the stomach. Acidic injuries occur more frequently and with greater severity than alkaline. Acidic substances produce esophageal or gastric stenosis more frequently than alkaline.

Describir las características clínicas y endoscópicas de las lesiones producidas por cáusticos en el tracto digestivo superior en pacientes del Hospital Nacional Daniel Alcides Carrión. Materiales y métodos: Se realizó un estudio descriptivo, se incluyó a pacientes con diagnóstico de ingesta de cáusticos admitidos en el servicio de Gastroenterología del HNDAC para la realización de una endoscopia digestiva alta durante el periodo de enero del 2009 a diciembre del 2012. Se documentó el tipo de sustancia cáustica ingerida, causa de ingesta, cantidad ingerida, modo de ingesta, síntomas o signos presentes, hallazgos endoscópicos según la clasificación de Zargar, presencia de complicaciones y tratamiento realizado. Se realizó el análisis descriptivo comparándolos según la sustancia cáustica ingerida: ácido o álcali. Resultados: Se obtuvieron 91 pacientes, la edad promedio fue de 30,6 +/- 16,3 años. Las sustancias caústicas ingeridas fueron: lejía (hipoclorito de sodio) en 71 (78%) pacientes, ácido muriático (ácido clorhídrico) en 18 (20%) pacientes, soda cáustica en 2 (2%) pacientes. La cantidad promedio ingerida fue de 136ml (entre 30-500 ml). Los síntomas y signos más frecuentes fueron: vómitos, dolor abdominal, náuseas y odinofagia. El 46% de los pacientes presentaron lesiones. El 100% de los pacientes que ingirieron ácido y el 33% de los que ingirieron álcali presentaron lesiones. Cinco (5%) pacientes presentaron lesiones orofaríngeas, 24 (26%) en esófago, 36 (40%) en estómago y 12 (13%) en duodeno. Se encontró 10 (11%) pacientes con lesiones de grado I, 16 (18%) pacientes con lesiones de grado II y 15 (16%) de pacientes con lesiones de grado III; 12 pacientes que consumieron ácido presentaron lesiones de grado III. Estenosis esofágica se encontró en 2 (2%) pacientes, estenosis gástrica en 7 (8%) pacientes y estenosis esofágica y gástrica en 3 (3%) pacientes. Dos pacientes requirieron dilataciones neumáticas y 10 tratamiento quirúrgico...

ABSTRACT

OBJECTIVE: The systemic inflammation worsens circulatory disorders in cirrhotic patients and recently the systemic inflammatory response syndrome (SIRS) may be a prognostic indicator therein. The aim of the study was to determine whether the presence of SIRS at admission in hospitalized cirrhotic patients is associated with complications or mortality. MATERIALS AND METHODS: A retrospective cohorts study was conducted at the Daniel Alcides Carrion National Hospital.Hospitalized cirrhotic patients admitted from July 2008 to December 2010 without significant comorbidities, malignancy,HIV infection, or stay less than 72 hours were included. Presence of SIRS at admission and the occurrence of complications or death after 72 hours of admission were evaluated. RESULTS: 150 cirrhotic patients were admitted, six were excluded;three for lower survival at 72 hours, one for neoplasia, one for severe heart failure and two for chronic renal failure. One hundred forty four patients were included, 95 (66%) patients had SIRS at admission. There was no significant difference in age, sex, etiology, in both groups. SIRS was associated with higher scores of MELD and Child-Turcotte Pugh. Of the group of patients with SIRS, 41 (43%) had complications and 16 (16.8%) died, while the group without SIRS 5 (10.2%) had complications and two (4%) died p < 0.0001 and p=0.028 respectively. The most common complications were infections and hepatic encephalopathy. In multivariate analysis SIRS was associated with complications (p < 0.006) but not with mortality(p < 0.276). CONCLUSIONS: SIRS is common in hospitalized cirrhotic patients and is associated with in-hospital complications.

ABSTRACT

OBJECTIVE: to describe the clinical, endoscopic, and histological characteristics of rectal mucosal prolapse syndrome, formerly known as Solitary rectal ulcer, in patients from a general hospital. MATERIAL AND METHODS: All patient diagnosed as rectal mucosal prolapse syndrome during 2010-2013 was selected; the medical history war reviewed and the histological slides were reevaluated by two pathologists. RESULTS: 17 cases of rectal mucosal prolapse syndrome were selected, the majority were males under 50 years, the most common clinical findings were rectal bleeding (82%) and constipation (65%), the endocopic findings were heterogeneous,: erythema (41%), ulcers (35%) and elevated lesions (29%). All cases presented fibromuscularhyperplasia in lamina propia and crypt distortion in the microscopic evaluation. CONCLUSION: In our study of rectal mucosal prolapse syndrome. The most common clinical findings were rectal bleeding and constipation. Erythematous mucosa was the most common endoscopic finding.

ABSTRACT

Objetivo: La inflamación sistémica empeora los trastornos circulatorios en el paciente cirrótico y recientemente el síndrome de respuesta inflamatoria sistémica (SRIS) podría ser un indicador pronóstico en ellos. El objetivo del estudio fue determinar si la presencia de SRIS al ingreso en pacientes cirróticos hospitalizados está asociada a complicaciones o mortalidad. Materiales y métodos: Estudio de cohortes retrospectiva, realizado en el Hospital Nacional Daniel Alcides Carrión. Se admitieron pacientes cirróticos hospitalizados desde julio 2008 hasta diciembre 2010 sin comorbilidades importantes, neoplasia maligna, infección VIH, o estancia fue menor a 72 horas. Se evaluó presencia de SRIS al ingreso y la aparición de complicaciones o muerte después de 72 horas del ingreso. Resultados: Fueron 150 pacientes cirróticos admitidos, se excluyeron 6, tres por supervivencia menor a las 72 horas, uno por neoplasia, uno por insuficiencia cardiaca severa y dos por insuficiencia renal crónica. En total 144 pacientes ingresaron al estudio, 95 (66%) pacientes presentaron SRIS al ingreso. No hubo diferencia significativa en cuanto a edad, sexo, etiología, en ambos grupos. SRIS estuvo asociado a mayores puntajes de MELD y Child-Pugh Turcotte. De los pacientes con SRIS, 41 (43%) se complicaron y 16 (16,8%) fallecieron, mientras que del grupo sin SRIS 5 (10,2%) se complicaron y 2 (4%) fallecieron, (p<0,0001y p=0,028 respectivamente). Las complicaciones más frecuentes fueron las infecciones y encefalopatía hepática. En el análisis multivariado SRIS estuvo asociado a complicaciones (p<0,006) mas no a mortalidad (p<0,276). Conclusiones: SRIS es frecuente en pacientes cirróticos hospitalizados y está asociado a complicaciones intrahospitalarias.


Objective: The systemic inflammation worsens circulatory disorders in cirrhotic patients and recently the systemic inflammatory response syndrome (SIRS) may be a prognostic indicator therein. The aim of the study was to determine whether the presence of SIRS at admission in hospitalized cirrhotic patients is associated with complications or mortality. Materials and methods: A retrospective cohorts study was conducted at the Daniel Alcides Carrion National Hospital. Hospitalized cirrhotic patients admitted from July 2008 to December 2010 without significant comorbidities, malignancy, HIV infection, or stay less than 72 hours were included. Presence of SIRS at admission and the occurrence of complications or death after 72 hours of admission were evaluated. Results: 150 cirrhotic patients were admitted, six were excluded; three for lower survival at 72 hours, one for neoplasia, one for severe heart failure and two for chronic renal failure. One hundred forty four patients were included, 95 (66%) patients had SIRS at admission. There was no significant difference in age, sex, etiology, in both groups. SIRS was associated with higher scores of MELD and Child-Turcotte Pugh. Of the group of patients with SIRS, 41 (43%) had complications and 16 (16.8%) died, while the group without SIRS 5 (10.2%) had complications and two (4%) died (p<0.0001 and p=0.028 respectively). The most common complications were infections and hepatic encephalopathy. In multivariate analysis SIRS was associated with complications (p<0.006) but not with mortality (p<0.276). Conclusions: SIRS is common in hospitalized cirrhotic patients and is associated with in-hospital complications.

ABSTRACT

OBJECTIVE: To evaluate the diagnostic accuracy of the Serum-Ascites Albumin Gradient (GASA), Protein Concentration in the Ascitic Fluid (PTLA), Albumin Concentration in the ascitic fluid (CAA) and the Protein Ascites/Serum Ratio (IPAS) for the diagnosis of ascites due to portal hypertension. MATERIALS AND METHODS: it was an observational and retrospective study of validation of diagnostic tests. The study population was patients from a National Public Health Hospital Daniel Alcides Carrion of Callao, Peru, during the period January to December of 2012, patients over 15 years old with a diagnosis of ascites which samples were taken for study by paracentesis with an standard technique, it was analyzed total protein and albumin, as well as study of total protein and albumin in blood. We obtained the diagnostic accuracy, sensitivity, specificity, PPV and NPV of the Serum-Ascites Albumin Gradient (GASA), Protein Concentration in the Ascitic Fluid (PTLA), Albumin Concentration in the ascitic fluid (CAA) and the Protein Ascites/Serum Ratio (IPAS) for the diagnosis of ascites due to portal hypertension. To determine ascites by HTP as diagnostic tests we took into account: GASA >= 1.1, PTLA <2.5, CAA <1.1 or IPAS< 0.5. RESULTS: There were 126 patients diagnosed with ascites, 10 patients was excluded for having incomplete data. Of the 116 patients, the average age was 53.03 +/- 15.73 years old, male 65 (56%) and female 51 (44%). 61 (52%) had ascites due to portal hypertension from liver cirrhosis, and 55 (48%) of ascites due to NO HTP. The sensitivity and specificity for GASA was 93% and 47% respectively, for PTLA was 80% and 89% respectively, for CAA was 85% and 87% respectively and for the IPAS was 83% and 80% respectively. The área under the ROC curve for GASA was 0.70, ATPL was 0.84, IPAS was 0.81 and CAA was 0.86, we found statistically significant differences between GASA compared to the other three parameters (p<0.01 ). CONCLUSION: The diagnostic accuracy of CAA, ATPL and IPAS is higher than the GASA to discriminate between ascites due to HTP or NO HTP, so that they could be used in clinical practice alone or together to achieve a diagnostic approach more successful.

ABSTRACT

Objetivo: Describir el espectro clínico endoscópico e histológico de síndrome de prolapso de mucosa rectal, antes llamado ulcera rectal solitaria, en pacientes de un hospital general. Material y métodos: Se recolectaron los casos diagnosticados como síndrome de prolapso de mucosa rectal durante los años 2010-2013. Las historias clínicas fueron revisadas y las láminas fueron reevaluadas por 2 patólogos. Resultados: Se seleccionaron 17 casos de prolapso de mucosa rectal, la mayoría en varones menores de 50 años, los hallazgos clínicos más frecuentes fueron rectorragia (82%) y constipación (65%), con hallazgos endoscópicos muy variables que incluyó eritema (41%), ulceras (35%) y lesiones elevadas (29%). Todos los casos presentaron hiperplasia fibromuscular en lámina propia y distorsión de criptas en la evaluación histológica Conclusión: En nuestro estudio de síndrome de prolapso de mucosa rectal la rectorragia y la constipación fueron los hallazgos clínicos más frecuentes. El eritema mucoso fue la presentación endoscópica más frecuente.


Objective: to describe the clinical, endoscopic, and histological characteristics of rectal mucosal prolapse syndrome, formerly known as Solitary rectal ulcer, in patients from a general hospital. Material and methods: All patient diagnosed as rectal mucosal prolapse syndrome during 2010-2013 was selected; the medical history war reviewed and the histological slides were reevaluated by two pathologists. Results: 17 cases of rectal mucosal prolapse syndrome were selected, the majority were males under 50 years, the most common clinical findings were rectal bleeding (82%) and constipation (65%), the endocopic findings were heterogeneous,: erythema (41%), ulcers (35%) and elevated lesions (29%). All cases presented fibromuscular hyperplasia in lamina propia and crypt distortion in the microscopic evaluation. Conclusion: In our study of rectal mucosal prolapse syndrome. The most common clinical findings were rectal bleeding and constipation. Erythematous mucosa was the most common endoscopic finding.

Objetivo: Evaluar la exactitud diagnóstica de la gradiente albúmina sangre/ascitis (GASA), proteínas totales en líquido ascítico (PTLA), albúmina en líquido ascítico (CAA) e índice de proteínas ascitis/suero (IPAS) para el diagnóstico de ascitis por hipertensión portal. Materiales y métodos: Se realizó un estudio, observacional, retrospectivo, de validez de pruebas diagnósticas. La población estudiada fueron pacientes mayores de 15 años con diagnóstico de ascitis a los cuales se les tomó una muestra para estudio del líquido ascítico mediante la técnica estándar de paracentesis, analizando proteínas totales y albúmina, además de estudio de proteínas totales y albúmina en sangre en el Hospital de Salud Pública Nacional Daniel Alcides Carrión del Callao, Perú (HNDCA), durante el periodo de enero a diciembre del 2012. Se obtuvo la exactitud diagnóstica, sensibilidad, especificidad, VPP y VPN de la gradiente albumina sangre/ascitis (GASA), proteínas totales en líquido ascítico (PTLA), albúmina en líquido ascítico (CAA) e índice de proteínas ascitis/suero (IPAS) para el diagnóstico de ascitis por hipertensión portal o no HTP. Para determinar ascitis por HTP según las pruebas diagnósticas se tomo en cuentas: GASA≥1,1, PTLA<2,5, CAA<1,1 o IPAS<0,5. Resultados: se obtuvieron 126 pacientes con diagnóstico de ascitis a los cuales se excluyó 10 pacientes por tener datos incompletos. De los 116 pacientes finales la edad promedio fue de 53,03 ± 15,73 años, pacientes de sexo masculino fueron 65 (56%) y femenino 51 (44%). Se encontró 61 (52%) líquidos ascíticos debido a HTP por cirrosis hepática, y 55 (48%) de ascitis por NO HTP. La sensibilidad y especificidad para el GASA fue de 93% y 47% respectivamente, para PTLA fue de 80% y 89% respectivamente, para CAA fue de 85% y 87% respectivamente y para el IPAS fue de 83% y 80% respectivamente. El área bajo la curva ROC para el GASA fue de 0,70, de las PTLA fue de 0,84, del IPAS fue de 0,81 y de la CAA fue de 0,86; encontrándose diferencia estadísticamente significativa entre el GASA comparado con los otros tres parámetros (p<0,01). Conclusión: La exactitud diagnóstica de la CAA, PTLA y IPAS es superior a la del GASA para discriminar entre ascitis por HTP o NO HTP, por lo que podrían ser usados en la práctica clínica de forma aislada, o en conjunto para lograr una aproximación diagnóstica más acertada.

ABSTRACT

Objetivo. Determinar las frecuencias de erradicación de diferentes esquemas de tratamiento contra la infección por Helicobacter pylori (HP) en la Clínica Médica Cayetano Heredia (CMCH) entre los años 2003-2011. Material y Métodos. Estudio descriptivo, retrospectivo, transversal basado en la revisión de historias clínicas de pacientes con dispepsia, diagnosticados por biopsia gástrica mediante estudio histopatológico de tener infección por HP, haber recibido tratamiento anti-HP, y regresado para determinar si hubo o no erradicación mediante una biopsia de control. Se analizaron los diferentes esquemas y su eficacia. Resultados. Entre el 2003 y 2011 se hizo 4 968 endoscopias altas y biopsias gástricas a dispépticos en la CMCH; de estas, 2 504 resultaron positivas para HP (50,4%). Solo 316 pacientes regresaron para conocer si habían erradicado o no la infección después del tratamiento. De estos en 287 se obtuvo adecuada información de las historias clínicas. La frecuencia de erradicación global fue 74,6% (214/287). El esquema omeprazol, amoxicilina, y claritromicina (OAC) fue el más usado: en 105, por 10 días con 77,1% de erradicación; en 118, por 14 días con 72% de erradicación. El esquema esomeprazol, amoxicilina y claritromicina por 14 días tuvo un porcentaje de erradicación de 76,9%, y se usó en 13 pacientes. Conclusiones. Solo el 16,6 % de los que reciben tratamiento anti-HP en la CMCH regresaron a consulta para conocer si erradicaron o no la infección. El esquema OAC por 10 días tuvo la mayor erradicación.


Objective. To determine the eradication rate of the different treatment regimens against Helicobacter pylory (HP) infection in the Cayetano Heredia Medical Clinic between the years 2003-2011. Material and Methods. Descriptive, retrospective and transversal study based on the review of clinical notes of dyspeptic patients, diagnosed by gastric biopsy histology of having HP infection, having received anti-Hp treatment and returned to the health facility to determine whether eradication was obtained or not by means of a control gastric biopsy. Different schemes and their efficacy were analyzed. results. Between 2003 and 2011, 4 968 upper endoscopies were performed on patients with dyspepsia; of these, 2 504 (50,4%)were positive for HP. Only 316 patients returned to known whether eradication was achieved after treatment. Of these, only in 287 patientes proper information on the clinical notes were obtained. The global eradication rate was 74,6% (214/287). the omeprazole, amoxicilin, and claritromicyn treatment regimen was the most frequently used: In 105 it was used for 10 days with a 77,1 % eradication rate (ER); in 118 it was used for 14 days with an ER of 72%. The regimen esomeprazole, amoxicilin and claritromycin for 14 days obtained an ER of 76,9 %, it was used in 13 patients. Conclusions. Only 16,6 % of patients that received a treatment against Hp infection returned to determine eradication status. The scheme OAC for 10 days obtained the best eradication rate.

ABSTRACT

OBJECTIVES: To describe the histologic and endoscopic characteristics reported among patients diagnosed with gastric cancer in Daniel Alcides Carrion National Hospital in Callao. MATERIALS AND METHODS: We performed a case series including all patients with histological diagnosis of gastric cancer from January 2009 to December 2011. Data were obtained from the registers of the pathology service of Daniel Alcides Carrion National Hospital. Factors such as age and gender of patients, histologic type, endoscopic location, presence of intestinal metaplasia, histologic degree, and cancer morphology were evaluated. RESULTS: 120 patients were included. Mean age was 65.4 ± 13.6 years; 59 (49%) were male. Based on the histologic type, intestinal type was found among 68 (56%); diffuse type among 45 (38%), and a mixed type in 7 (6%). Regarding the site, 23 (19%) of gastric cancers were located in the fundus; 52 (43%) in the body; 39 (33%) in the antrum, and 6 (5%) in the pylorus. Patients with gastric cancer of the intestinal type were in average older than those with a diffuse type (69.1 ± 10.3 versus 59.3 ± 15.3). 60.3% of intestinal-type gastric cancers were located proximally, versus 66.6% of diffuse-type cancers. CONCLUSION: Among the studied population, diffuse-type gastric cancer appears at an earlier age than the intestinal type, and its most common location is proximal.
