ABSTRACT

BACKGROUND AND OBJECTIVES: Red blood cell (RBC) transfusion may be justified in iron deficiency anaemia (IDA) when an increase in oxygen delivery is needed, as sometimes occurs in subjects with haemoglobin <8·0 mg/dL, serious comorbidities or at risk of cardiovascular instability. Earlier investigations showed that some patients with severe IDA requiring transfusion had lower than expected post-transfusion haemoglobin levels with poorer clinical outcomes than other patients. After hypothesizing that haemoglobin responses to transfusion were different and that the underlying gastrointestinal (GI) disorders causing IDA could be a confounder explaining this association, these responses were analysed in a prospective cohort of IDA adults referred for outpatient GI investigation. MATERIALS AND METHODS: Transfused patients with proven IDA, baseline haemoglobin at referral <9·0 g/dL and no extraintestinal bleeding were eligible. To assess a homogeneous population, only GI disorders known to cause occult bleeding were considered. Haemoglobin increments per 100 mL of RBCs were investigated. RESULTS: In total, 2818 patients were enrolled over 10·5 years. On multivariable regression, diffuse angiodysplasias and GI cancer independently predicted for reduced increments in post-transfusion haemoglobin [adjusted regression coefficients: -0·082 (95% confidence interval, -0·093 to -0·072) and -0·073 (95% confidence interval, -0·081 to -0·066), respectively, P < 0·001 in both]. Haemoglobin responses in the remaining bleeding disorders were adequate and agreed with the principle that one RBC unit increases the haemoglobin an average of 1 g/dL. CONCLUSION: The potential differential impact of GI disorders on changes in haemoglobin levels after RBC transfusion could be useful for transfusing physicians, especially for diagnostic purposes.

ABSTRACT

Cada vez resulta más importante medir de forma objetiva la producción científica individual, de los grupos de investigación o de un área científica, para lo que se utilizan parámetros bibliométricos. En el presente estudio se analiza la actividad investigadora llevada a cabo por los toxicólogos españoles durante 15años, se compara con la de otras áreas científicas y con la de los toxicólogos de otros países y, finalmente, se investiga la aportación de la toxicología clínica al conjunto de la investigación toxicológica española. Se empleó como herramienta de búsqueda bibliométrica de documentos científicos publicados por toxicólogos españoles el programa Web of Knowledge del Institute for Science Information(ISI) y como base de datos el Science Citation Index (SCI). El periodo de tiempo analizado fue de 1991 a 2005. Los datos tabulados fueron: año de publicación, nombre de la revista y su factor de impacto, país del editor de la revista, tipo e idioma del documento, centro, provincia y comunidad del primer toxicólogo firmante, colaboración con otros servicios de toxicología y número de citas que recibió el artículo. Para el análisis estadístico, los datos se introdujeron en el programa informático SPSS versión 12.0. Durante los 15 años estudiados se han identificado 1904 documentos firmados por toxicólogos españoles y publicados en 523 revistas diferentes. Durante este periodo de tiempo, se ha observado un incremento progresivo y significativo del número de documentos firmados por toxicólogos, así como de los documentos encuadrados en el área de toxicología. Sin embargo, como la producción científica global en España ha mostrado un marcado aumento, el porcentaje que supone la toxicología respecto a la producción total española, ha experimentado un significativo descenso. Objective measurement of scientific production by individuals, research groups or scientific areas using bibliometric parameters is increasingly important. The aims of this study were to analyze the research made by spanish toxicologists during a 15-year period, to compare it with that of other scientific areas and toxicologists from other countries and to determine the contribution of clinical toxicology to total spanish toxicological lresearch. The Web of Knowledge programme of the Institute for Science Information (ISI) and the Science Citation Index (SCI) database were used to carry out a bibliometric search for scientific documents published by spanish toxicologists from 1991 to 2005.The data tabulated were: year of publication, names of the journal and its impact factor, nationality of the journal's editor, type and language of the document, centre, province and community of the first toxicologist author, collaboration with other toxicology services and number of citations of each article. The statistical analysis was made using the SPSS version 12.0 statistical programme. During the study period, 1904 works authored by spanish toxicologists appearing in523 different journals were identified. There was a significant, progressive increase in the number of works authored by toxicologists and the number of works in the area of toxicology during the study period. However, as global scientific production in Spain increased markedly during this period, the percentage of toxicological works as a percentage of total spanish scientific production fell significantly. There was international collaboration in16% of the toxicological studies included. ABSTRACT

El transporte intracorporal de drogas de abuso (body-packers) constituye una potencial emergencia médica, ya que la salida del contenido de los paquetes puede producir una sobredosis mortal. Para el diagnóstico de esta entidad se recurre habitualmente a la radiografía de abdomen. Se presenta el caso de un varón que fue remitido al Servicio de Urgencias con manifestaciones compatibles con una sobredosis de cocaína. Aunque se sospechó que se tratase de un body-packer, la radiografía del abdomen fue negativa. El paciente evolucionó mal en las horas siguientes, por lo que se realizó un TAC abdominal que permitió el diagnóstico de body-packer. F ue intervenido quirúrgicamente, extrayéndose tres bolsas de la cavidad gástrica. La evolución posterior fue favorable y el paciente fue dado de alta sin secuelas (AU)


The intracorporal transport of abusive substances by body-packers constitutes a potential medical emergency, as a fatal overdose may occur if the packages break inside the body. The diagnosis is usually made by abdominal radiography. We present the case of a male who was sent to the emergency department with manifestations compatible with a cocaine overdose. Although the patient was suspected to be a body-packer, the abdominal radiography was negative. The patient's condition worsened during the following hours and an abdominal CT scan was carried out which allowed the patient to be identified as a body-packer. Three packages were removed from the gastric cavity by surgery. The patient evolved favourably and was discharged without sequelae (AU)

ABSTRACT

Una mujer de 89 años fue hallada en su domicilio en coma, bradicárdica e hipotensa, tras la ingesta de bisoprolol y lorazepam en una tentativa de suicidio. Al administrar flumazenilo se obtuvo una mejoría en el estado de conciencia, pero la persistencia de la bradicardia, hipotensión y oligoanuria, motivaron una terapéutica intravenosa con fluidos y expansores del plasma y la administración sucesiva de glucagón, dopamina y adrenalina, hasta que la situación cardiocirculatoria se estabilizó a las 52 horas del ingreso. La evolución posterior fue favorable y la paciente fue dada de alta sin secuelas. Se discute el papel antidótico de estos fármacos en el tratamiento de la intoxicación por beta-bloqueantes


An 89-year-old woman was discovered unconscious at home, with bradycardia and hypotension, after ingesting bisoprolol and lorazepam in an attempted suicide. Administration of flumazenyl improved the level of consciousness, but persisting bradycardia, hypotension and oliguria led to intravenous administration of fluids and plasma expanders. Subsequently, glucagon, dopamine and epinephrine were administered until the cardiovascular function stabilized, 52 hours after admission. The patient evolved favorably and was discharged without sequelae. We discuss the role of these drugs and their effectiveness as antidotes in the treatment of beta-blocker intoxications
