ABSTRACT

Crooke's cell adenomas are a rare type of pituitary neoplasm. They produce adrenocorticotropic hormone causing Cushing's disease or may be endocrinologically silent. These tumors are usually invasive, may exhibit aggressive clinical behavior, and often recur with a low success of cure after reoperation and/or radiotherapy. Due to their rarity, they present great difficulties in assessing prognosis, treatment, and clinical management. Neurosurgeons and physicians dealing with pituitary adenomas diagnosed as Crooke's cell adenomas have to be aware of their potential clinical aggressiveness to plan strict follow-up of patients and eventual multimodality treatment. We review here the published cases of Crooke's cell tumors, as well as the clinical and histopathological characteristics of these unusual neoplasms.

ABSTRACT

BACKGROUND: Hyperglycemia is a frequent phenomenon in hospitalized patients that is associated with negative outcomes. It is common in liver transplant patients as a result of stress and is related to immunosuppressant drugs. Although studies are few, a history of diabetes and the presentation of hyperglycemia during liver transplantation have been associated with a higher risk for rejection. AIMS: To analyze whether hyperglycemia during the first 48hours after liver transplantation was associated with a higher risk for infection, rejection, or longer hospital stay. METHODS: A retrospective cohort study was conducted on patients above the age of 15years that received a liver transplant. Hyperglycemia was defined as a value above 140mg/dl and it was measured in three different manners (as an isolated value, as a mean value, and as a weighted value over time). The relation of hyperglycemia to a risk for acute rejection, infection, or longer hospital stay was evaluated. RESULTS: Some form of hyperglycemia was present in 94% of the patients during the first 48 post-transplantation hours, regardless of its definition. There was no increased risk for rejection (OR: 1.49; 95%CI: 0.55-4.05), infection (OR: 0.62; 95%CI: 0.16-2.25), or longer hospital stay between the patients that presented with hyperglycemia and those that did not. CONCLUSIONS: Hyperglycemia during the first 48hours after transplantation appeared to be an expected phenomenon in the majority of patients and was not associated with a greater risk for rejection or infection and it had no impact on the duration of hospital stay.

Introducción: el manejo del dolor debe ser adaptado y optimizado de acuerdo con las condiciones de cada hospital, tipo de cirugía y paciente. Objetivos: ealuar la percepción del dolor de los pacientes intervenidos en el postoperatorio del Hospital Universitario San Jorge de Pereira. Materiales y métodos: Se realizó un estudio de corte, prospectivo en pacientes mayores de 18 años entre 2 de septiembre y 28 de octubre de 2011. La valoración de intensidad del dolor postoperatorio se realizó mediante Escala Visual Analógica a las 24 horas del postquirúrgico. Se consideraron variables sociodemográficas, clínicas (tipo de cirugía, anestesia, riesgo) y farmacológicas (medicamentos empleados, dosis, intervalos dosificación). El análisis se hizo con SPSS 20.0 para Windows. Resultados: Se evaluaron 153 pacientes en postoperatorio, 80 (52,3%) mujeres y 73 (47,7%) hombres, con edad promedio de 47,6 ± 20,2 años. El 38,8% de los pacientes no tenía controlado el dolor. Las variables de cirugía ortopédica, el empleo de anestesia general por vía intravenosa, y el incumplimiento de los intervalos recomendados de dosificación de los analgésicos, se asociaron de manera estadísticamente significativa con la falta de control. Discusión: El control del dolor fue inadecuado haciendo evidente la necesidad de replantear su manejo ajustado a guías de práctica clínica, formalizando el uso de medicamentos, en dosis e intervalos adecuados que garanticen una analgesia efectiva.


Introduction: Pain management should be adapted and optimized according to the conditions of each hospital, type of surgery and patient. Objectives: To evaluate the perception of pain in postoperative patients at Hospital Universitario San Jorge of Pereira. Materials and methods: A descriptive observational study in patients older than 18 years, between September 2nd to October 28, 2011. The assessment of postoperative pain intensity was performed by visual analogue scale 24 hours after surgery. We considered socio-demographic, clinical (type of surgery, anesthesia, risk) and pharmacological variables (drugs used, dosage, dosage intervals). The analysis was done with SPSS 20.0 for Windows. Results: We evaluated 153 patients in the postoperative period, 80 (52.3%) women and 73 (47.7%) males, mean age 47.6 ± 20.2 years. 38.8% of patients did not obtain pain control. The variables of orthopedic surgery, the use of intravenous general anesthesia, and suboptimal dosage was statistically significantly associated with lack of control. Conclusions: Pain control was inadequate thus showing the need to redefine its management by following clinical practice guidelines, through the use of analgesics in doses and appropriate intervals.

As variáveis de cirurgia ortopédica, o emprego de anestesia geral por via intravenosa, e o descumprimento dos intervalos recomendados de dosagem dos analgésicos, se associaram de maneira estatisticamente significativa com a falta de controle. Discussão: O controle da dor foi inadequado fazendo evidente a necessidade de repensar o seu manejo ajustado a guias de prática clínica, formalizando o uso de medicamentos, em doses e intervalos adequados que garantam uma analgesia efetiva.

En este artículo se presenta un consenso médico basado en el sistema de Bethesda del Instituto Nacionalde Cáncer (Estados Unidos) para el uso de la biopsia por aspiración con aguja fina en el manejo de nódulos tiroideos, realizado en conjunto con patólogos, radiólogos, endocrinólogos y otras especialidades médicas de Colombia, España, Chile, Venezuela, Estados Unidos y Panamá. En este trabajo se describen las indicaciones de la biopsia por aspiración con aguja fina de tiroides, requisitos previos, entrenamiento, acreditación, técnicas, terminología diagnóstica, pruebas complementarias y opciones de tratamiento. El objetivo del actual artículo es presentar ante la comunidad médica la clasificación de los reportes citológicos, el reporte de ecografía que propone usar el sistema de datos y el reporte de imágenes tiroideas (TIRADS, del inglés The Thyroid Imaging Reporting and Data System), el uso de la medición de tiroglobulina en biopsia por aspiración con aguja fina y técnicas de citología líquida;...


This article presents a medical consensus based on the Bethesda system of the National Cancer Institute (USA) for the use of fine needle aspiration biopsy in the management of thyroid nodules. This consensus was performed in conjunction with pathologists, radiologists, endocrinologists, and other medical specialties of Colombia, Spain, Chile, Venezuela, United States, and Panama. In this work was described the indications for fine needle aspiration biopsy of thyroid, prerequisites, training, accreditation, techniques, diagnostic terminology, additional tests and treatment options.
