ABSTRACT

Treatment of orbital fractures involves restoration of lost bone anatomy and orbital volume. Among the materials used for this purpose, the titanium mesh is widely used due to its effectiveness and low complication rate related to distortion especially in non-traumatic events. This study shows an atypical and late complication involving the deformation of the titanium mesh used during reconstruction of the orbital floor in a patient with orbital zygomatic complex fracture 5 months after the procedure and without traumatic etiology.

ABSTRACT

Necrotizing fasciitis (NF) is a severe bacterial infection with rapid and aggressive progression. The infection generally affects individuals with comorbid conditions that lead to immunologic and microvascular deficiencies. It is characterized by necrosis of tissues, mainly in the extremities, trunk, and perineum, and is rarely found in the head and neck. This case report describes the course of NF in a 55-year-old man, highlighting diagnosis, surgical treatment, drug therapy, and supportive measures. The patient, who had chronic alcoholism, systemic arterial hypertension, a smoking habit, and decompensated diabetes (glucose level of 490 mg/dL), was admitted to the hospital with a volume increase in the cervical and thoracic areas with a duration of about 7 days. He presented with fever, dyspnea, and inflammatory signs bilaterally in the submandibular, submental, and superior thoracic regions as well as severe trismus. The patient underwent a surgical procedure to drain the infectious process and to place drains. The patient developed cutaneous necrosis in the cervical and superior thoracic regions, diagnosed as NF. Surgical debridement of all affected tissue was performed. After resolution of the infection, the patient underwent skin grafting with a satisfactory outcome.

ABSTRACT

Os ferimentos ocasionados por mordedura humana são geralmente tidos como portadores de maior incidência de complicações infecciosas, pois possuem alto índice de contaminação. O tratamento dessas lesões é controverso, principalmente no que se refere à sutura primária e à profilaxia de doenças infecto-contagiosas originadas a partir do contato da saliva do agressor com a ferida. O atendimento desses doentes na urgência consiste no controle da infecção, reabilitação funcional e consequentemente estética, a fim de minimizar danos psíquicos e possibilitar retorno ao convívio social. O presente trabalho relata um caso de uma extensa avulsão de lábio inferior decorrente de agressão por mordedura humana. A reconstrução inicial foi possível através de avanço de retalhos laterais, associada à antibioticoterapia específica. Desta forma foi obtido um resultado estético e funcional satisfatório, sem complicações infecciosas pós operatórias ou deiscências de suturas, a continência oral foi preservada, bem como a sensibilidade e a mobilidade labial... Injuries caused by human bites are generally considered to be more incidence of infectious complications, as they have a high rate of contamination. The treatment of these lesions is controversial, especially with regard to primary suturing and disease prophylaxis. Infectious-contagious caused by contact between the aggressor's saliva and the wound. the service of these patients in the emergency room consists of infection control, functional rehabilitation and, consequently, aesthetics, in order to minimize psychic damage and enable a return to social life. The present work reports a case of an extensive avulsion of the lower lip resulting from human bite aggression. The initial reconstruction was possible through the advancement of lateral flaps, associated with specific antibiotic therapy. In this way, a satisfactory aesthetic and functional result was obtained, without complications. A luxação da Articulação temporomandibular (ATM) é definida como um movimento de avanço excessivo do côndilo além à eminência articular, não auto redutível, que pode ser clinicamente dividida em 3 grupos; luxação que ocorreu apenas uma vez, luxação recidivante e deslocamento prolongado. O deslocamento prolongado é uma condição rara com poucos casos descritos na literatura que na maioria dos casos não responde a reposição manual, necessitando de intervenção cirúrgica, e requer uma atenção especial por parte dos cirurgiões tendo em vista a peculiaridade de cada caso. The dislocation of the temporomandibular joint (TMJ) is defined as an excessive forward movement of the condyle beyond the articular eminence, not self-reducible, which may be clinically divided into 3 groups; dislocation that occurred only once, recurrent dislocation and prolonged displacement. Prolonged dislocation is a rare condition with few cases reported in the literature that in most cases does not respond to manual reset, requiring surgical intervention, and requires special attention from surgeons in view of the peculiarity of each case. ABSTRACT

Descending necrotizing mediastinitis is a rare type of deep infection of the soft tissues of the face, originating from cervical and oral infections, typically dental infections. It is associated with high mortality, due to its high invasive potential and the fact that the diagnosis is often delayed. We report the case of a 42-year-old female patient presenting with severe trismus, fever, purulent intraoral drainage, dysphagia, and dysphonia, accompanied by edema and redness in the cervical region. She was diagnosed with necrotizing descending mediastinitis secondary to a dental infection after the extraction of a mandibular third molar. The patient underwent surgical drainage and intravenous antibiotic therapy. Despite the development of septicemia, the evolution was favorable and the patient was discharged on post-admission day 20. Early and accurate diagnosis, together with prompt treatment, is imperative for better outcomes in this rare condition.
