ABSTRACT

OBJECTIVE: To determine the prevalence and factors associated with the development of ocular hypertension and glaucoma, in patients undergoing penetrating keratoplasty, in the Eye Clinic of the city of Bogotá. METHOD: A retrospective cross-sectional study was conducted, and 130 eyes of patients undergoing penetrating keratoplasty were analyzed at the Eye Clinic in Bogotá, between January 2015 and August 2018. Demographic and clinical data were obtained, and it was determined by bivariate analysis, the association factors and the prevalence of the pathology under study. RESULTS: Prevalence of ocular hypertension was 27.69% and glaucoma 10%. Average age 48.93 ± 18.63 years; higher frequency of presentation in men (61.5%). Statistically significant association factors were male sex (PR 2.59), presence of peripheral anterior synechiae (PR 1.83), history of trauma (PR 2.16), prior PK (PR 2.10) and graft failure (PR 2.04). Post-KP glaucoma only had statistically significant association with bullous keratopathy (PR 2.76). CONCLUSIONS: Ocular hypertension and glaucoma had a high prevalence after penetrating keratoplasty, and the association factors were similar to those reported in other international studies. Knowing these factors, allows focusing surveillance and treatment in these patients to avoid blindness due to damage of the optic nerve or corneal graft.

Objetivo Determinar la prevalencia y los factores asociados con el desarrollo de la hipertensión ocular (HTO) y el glaucoma, en los pacientes sometidos a queratoplastia penetrante (QPP), en la Clínica de Ojos de la ciudad de Bogotá. Método Se realizó un estudio retrospectivo de corte transversal y se analizaron 130 ojos de pacientes sometidos a QPP, en la Clínica de Ojos de la ciudad de Bogotá, entre enero del 2015 y agosto del 2018. Se obtuvieron datos demográficos y clínicos y se determinó mediante el análisis bivariado los factores de asociación y la prevalencia de la patología a estudio. Resultados Prevalencia de HTO del 27,69% y glaucoma del 10%. Edad promedio 48,93 ± 18,63 años; frecuencia de presentación mayor en hombres (61,5%). Los factores de asociación estadísticamente significativos fueron el sexo masculino (PR 2,59), presencia de sinequias anteriores periféricas (PR 1,83), antecedente de trauma (PR 2,16), QPP previa (PR 2,10) y fracaso del injerto (PR 2,04). El glaucoma de la queratoplastia post penetrante (post-QPP) únicamente tuvo asociación estadísticamente significativa con la queratopatía bullosa (PR 2,76). Conclusiones La HTO y el glaucoma tuvieron una alta prevalencia posterior a QPP y los factores de asociación fueron similares a los reportados en otros estudios internacionales. Conocer estos factores permite enfocar la vigilancia y el tratamiento en estos pacientes para evitar la ceguera por daño del nervio óptico o del injerto corneal (AU)

ABSTRACT

AIMS: Acetylcholinesterase (AChE) and butyrylcholinesterase (BChE) have been associated with risk factors for metabolic syndrome (MetS). Our objective was to evaluate the effect of nicotinamide (NAM) on the activities, expression and protein content of cholinesterases in a MetS model. MAIN METHODS: MetS was induced in male rats administrating 40% fructose to the drinking water for 16 weeks. Additionally, from 5th week onward, the carbohydrate solution was replaced by NAM, at several concentrations for 5 h each morning for the next 12 weeks. In the 15th week, the glucose tolerance test was conducted, and blood pressure was measured. After the treatment period had concluded, the biochemical profile; oxidant stress; proinflammatory markers; and the activity, quantity and expression of cholinesterases were evaluated, and molecular docking analysis was performed. KEY FINDINGS: The MetS group showed anthropometric, hemodynamic and biochemical alterations and increased cholinesterase activity, inflammation and stress markers. In the liver, cholinesterase activity and mRNA, free fatty acid, tumor necrosis factor-alpha (TNF-α), and thiobarbituric acid-reactive substance (TBARS) levels were increased, while reduced glutathione (GSH) levels were decreased. NAM partially or totally decreased risk factors for MetS, markers of stress and inflammation, and the activity (serum and liver) and expression (liver) of cholinesterases. Molecular docking analysis showed that NAM has a greater affinity for cholinesterases than acetylcholine (ACh), suggesting NAM as an inhibitor of cholinesterases. SIGNIFICANCE: Supplementation with 40% fructose induced MetS, which increased the activity and expression of cholinesterases, oxidative stress and the inflammation. NAM attenuated these MetS-induced alterations and changes in cholinesterases.

ABSTRACT

Hemos evaluado la determinación de troponina Ic debido a su amplio uso tras su reciente inclusión en las guías americanas de práctica clínica para el diagnóstico y estratificación del infarto de miocardio y a la gran diversidad de resultados que se obtienen al realizar la determinación por diferentes métodos. Hemos evaluado el sistema automático Vitros ECI y lo hemos comparado con los sistemas automáticos AxSYM y ACCESS en dos laboratorios diferentes. También hemos evaluado la utilidad clínica de los tres sistemas mediante curvas ROC. (AU)

En este trabajo se trata de lograr una nueva clasificacion de tecnicas oclusales, que pueda salvar algunas deficiencias de las conocidas hasta la actualidad, en nuestro medio. Para ello se modifican algunos de sus terminos por no ser suficientemente precisos y se agregan otras subdivisiones que permiten incluir tecnicas que no figuran en la misma. Respecto al cambio de algunos de sus terminos, fundamentado en la orientacion de sus planos de incidencia, se indica el reemplazo de "Especializadas normales" por "Parasagitales normales" y el de "Laterales oblicuas". Tambien se incluye la necesidad de agregar un nuevo item en esta clasificacion, con la denominacion de "Parasagitales Oblicuas", para integrar y justificar tecnicas oclusales no incluidas hasta el momento, tales como Oclusales por Fosa Multiforme por Fosa Orbitaria, por Fosa Temporal para Arco Cigomatico, para terceros molares para Glandula Submaxilar, etc., pudiendose agregar a partir de este momento, nuevas tecnicas, en razon de la incorporacion de todos los planos posibles
