ABSTRACT

The authors examined 65 inpatients aged 20-61 admitted to the clinic with obsessional-phobic and depressive conditions (comorbidity). According to DSM-III-R, the patients suffered from panic and major depressive disorders, dysthymia, atypical depression within schizoaffective disturbance. By the response of the patients to alprazolam (anxiolytic) and fluoketin (antidepressant) given by a special scheme, 3 types of comorbidity of obsessional-phobic and affective disturbances were recognized: 1) prevalent depression, 2) combination of dysthymia with marked obsessional-phobic disorders, 3) overcoming of obsessional-phobic and affective disorders. Type 1 and 2 patients benefited more from monotherapy with antidepressants or tranquilizers. Type 3 patients did well on combined chemotherapy with tranquilizers, antidepressants, neuroleptics.

ABSTRACT

Reactive delirious psychoses in 38 patients with slow-progressive schizophrenia (SPS) were studied. Three clinical variants of SPS were singled out while studying the course and psychopathological features of endogenic process with delirious reactions. Delirious reactions are known to form only in those patients with SPS whose endogenic process had favoured their manifestation in the presence of additional psychogenic factors. Personality traits connected with endogenic process (psychopathlike manifestations with hypoparanoic features, higher affective reactivity acquired) as well as readiness to acute delirium conditioned by endogenic process itself underlie the delirium appearance. The type, development, and outcome of psychogenic disorders are predetermined by schizophrenic process itself.

ABSTRACT

A number of schizophrenic patients with the delusion homogeneous in its contents (conformant delusion) were examined. The results of the examination have shown that the delusion of this type can develop not only simultaneously, by the inter-induction mechanism, but also by way of transfer, suggestion of the delusional idea, i.e. by the type of suggestive psychoses. The leading role in the mechanism of the conformant delusion formation belongs to endogenous factors, and namely, to the regularities of the basic disease development that determine the peculiarities of the time course and outcome of the conformant delusion. A slowly progressing course of the disease without gross negative changes, a tendency to formation of "worshipped" delusional ideas, and formation of acquired reactive lability are the peculiarities of a schizophrenic process which are the "grounds" for conformant delusion formation.
