ABSTRACT

Retroperitoneal cystic lymphangioma is an uncommon tumor. One case is reported, with surgical confirmation. Authors point out the diagnostic value of ultrasonography and computed tomography in the pre-operative diagnosis.

ABSTRACT

With reference to 115 cases of acute pancreatitis which were either operated on or confirmed, the authors stress the seriousness of the lesion to the glandular parenchyma; it is also difficult to recognise during peroperative exploration. The necrosis, which is sometimes limited to the peripheral and interstitial tissue, sparing the gland itself (its prognosis is less serious) has led many surgeons to perform surgical excision. However, the problems encountered postoperatively have given rise to doubts a posteriori as to whether this is legitimate. Reference is made to the decapsulation of the pancreas described by Romanian authors, and a method for future operative diagnosis of glandular necrosis is proposed.

ABSTRACT

The case reported is that of a young woman who underwent a termino-terminal jejno-ileal by-pass procedure for obesity which was refactory to usual forms of treatment. Eight months later, a cholecystectomy was carried out for lithiasis, presenting with abdominal pain. At the time of operation, lesions of cystic pneumatosis were discovered on the excluded length of small bowel. This complication frequently manifests itself in the form of pseudosurgical abdominal pain, or as diarrhoea. More rarly, it is a radiological finding. The pathogenesis remains a subject of discussion. The mechanical theory would seem the most logical, since colonic intraluminal pressure is higher than that in the intestine excluded from the circuit. Bacterial proliferation, classical in blind loops, would be a farourising factor.

ABSTRACT

The favourable experience of M. BROWN with noxythiolin in fecal peritonitis has prompted us, over the last seven years, to use this product in all forms of generalized peritonitis. This study involved 187 cases of generalized peritonitis : 102 males and 85 females, whose mean age was 53 years. Noxythiolin was reserved for severe cases (after having deliberately decided not to perform a comparative control series), the severity being related to the advanced age of the patients, the delay in intervention, and preexisting disorders. It was used in the form of a peroperative lavage of the peritoneal cavity and a postoperative instillation. The overall survival (71 p. 100) and that observed in particular in peritonitis of colonic origin (58 p. 100), seem to be arguments in favour of the beneficial effect of the product. Noxythiolin contributes to the sterilization of the peritoneal cavity. By virtue of its efficacy, the simplicity of its use, and its innocuousness, it merits a place in the therapeutic arsenal for treatment of generalized peritonitis, in combination with systemic treatments.

ABSTRACT

Nine hypoglycaemia-inducing pancreatic tumours were studied by electron microscopy. In 8 of these tumours, it was possible to identify, within the cell cytoplasm, secretory granules with a "paracrystalline" content, identical in appearance to the granules of the B cell of the normal human pancreas. Thus electron microscopy would appear to be a reliable and sensitive method for the morphological identification of these endocrine tumours of the pancreas.

ABSTRACT

The authors report a personal experience of 16 cases and in the light of data in the world literature, the authors discuss the controversial method of treatment of very severe obesity when medical treatment has failed. The onset of various complications linked mainly to intestinal malabsorption, should make one careful in the indications for surgery which may, however, be applicable in certain cases.

The authors analyse retrospectively 284 case records of perforation of gastric and duodenal ulcers treated between 1961 and 1972 and, from a study of the literature, the authors attempt to draw up prognostic factors which permit one to apply to this complication of ulcer disease, the most efficient short and long-term treatment.
