ABSTRACT

Thirty one intervertebral space calcifications (I.S.C.) were studied in 21 children (12 boys, 9 girls), with mean age of 7 years; 25 calcifications had been followed for an average of 71.5 months. The main symptom was pain (Crisis) which relieved in a delay of 1 month in 70 per cent of cases. "Crisis" and radiographic fragmentation of the calcification are both criteria of going out of 18 calcifications, in a delay ranged from 28 days to 6 months. The calcification appears as a sign which disclose a general disease of spine, interesting vertebral body, growth plate, and disc; therefore we have observed 15 lesions due to growth disorders in 16 cervical I.S.C., at dorsal level we have described 4 cases of intrasomatic cavitation containing calcified material, opened in the intervertebral space. This growth disturbance has been noted either on first radiograph, or has appeared later, after disappearance of the I.S.C.; we found more over absence of listel and lesion of multiple plates distant from the calcification. This disease is identified to vertebral osteochondrosis where residual deformities are more worrying than the benign calcification; they have concluded management of I.S.C. as early as possible to avoid great deformities of vertebral bodies.

ABSTRACT

The authors report a case of a recent Monteggia's fracture, occurring to a one-year-old child, which is a very unusual age (2 from 286 cases in literature). Orthopedic ulnar reduction failed and intramedullary pinning was required. According to this observation, the authors propose a therapeutic procedure of Monteggia's fracture in young children.

ABSTRACT

In a series of 110 fractures of the ankle in children the authors have observed 27 cases of "at risque" lesions: Mac Farland types of fractures and crush injuries. Epiphysiodesis occurs in all cases of crush injuries and in 25% of fracture-separation of the tibial malleolus. In order to prevent this complication the only procedure is to obtain an anatomical reduction. Desepiphysiodesis is uncommunly possible (2 cases) and rarely effective. In case of varus deformity an osteotomy must be performed and repeated if the child is still young.

ABSTRACT

29 congenital radio-ulnar synostosis have been observed in 16 childrens. The authors review the clinical patterns of this affection: most often bilateral, it results in impossibility of pronosupination of the wrist which has but little functional consequence, if the hand is in an intermediary position. On the X-ray its almost always a superior radio-ulnar synostosis but the inferior radio-ulnar joint is abnormal and non functional. Only the children severely handicapped by a hand fixed in pronation should be operated upon. No good result can be hoped from a surgery that tries to restore pro-supination. The best surgical technique seems to be a simple horizontal osteotomy through the synostosis itself which allows a derotation of the forearm into the functional intermediary position. Severe complications can occur. Indications and technique must be very careful since this congenital abnormality is very well tolerated.

ABSTRACT

The authors review 66 case reports of trauma involving major vessels of the limbs in children and confirm the seriousness of these lesions. It becomes apparent that iatrogenic lesions are quite frequent especially after arterial punctures and arteriographies in general. They stress the importance of clinical diagnosis in vascular blocks. Arteriography should be exceptionally used for infants. Management should aim at restoring vascular continuity by deobstruction, suture or vein grafting. The ideal solution is often easily obtained when repair is undertaken quickly. Normal blood flow was satisfactorily obtained in only 36 patients whereas poor results in the remaining cases was explained because of delayed action. Sequellae are severe and more frequently encountered when blood flow is not restored. On the other hand, the repercussions on growth and development of the limb after chronic ischemia is still not a proven fact. Finally, the authors underline the severe consequences of thromboses after arteriographies by femoral puncture in infants.

ABSTRACT

The authors report on 23 cases of fibrons in infants who had been treated surgically, and review the therapeutic problem which arise from this particular type of bone fragility. When located in the diaphysis they had been treated by screw fixation, isolated grafts being constantly unsuccessful. Various procedures had been used in 9 cases with localization in the proximal end of the femur, and they are reviewed critically.

ABSTRACT

The authors have observed 29 cases of osteoid osteoma in children aged from 1 to 15 years. 10 cases involved the femur, 9 the tibia, 4 vertebrae. All cases were operated on. 3 recurrences were noted but the nidus had been removed in these cases. 2 cases were complicated by post-operative fractures and 5 by bone growth disturbances.
