La creciente complejidad de los procedimientosterapéuticos, la especialización ysegmentación de la asistencia sanitaria y laexistencia de una cultura profesional sanitariaque tiende a ocultar en lugar de favorecerla comunicación abierta de los erroresasistenciales y sus causas, son factoresque predisponen al aumento de la incidenciade errores de medicación.Con el propósito de identificar y analizarlos errores en la administración de fármacosen el ámbito de la Atención Primaria eimplantar sistemas de notificación y vigilancia,se llevó a cabo un Análisis Modal deFallos y Efectos (AMFE) sobre Administraciónde fármacos. Participaron profesionalesde cinco centros, formado por tresenfermeras, un médico de familia, un pediatray la directora de calidad del GrupoSagessa. De entre todos los procesos relacionadoscon esa intervención, la administracióndel fármaco fue el que obtuvoun Índice de Puntuación de Riesgo más alto,calculado sobre la severidad y la probabilidaddel mismo. El análisis de ese procedimientodio como resultado la puesta enmarcha de varias acciones protocolizadasque están comenzando a ser evaluadas
The growing complexity of therapeuticprocesses, the specialisation and segmentationof healthcare and the existence of ahealthcare professional culture that tendsto hide instead of favouring open communicationregarding errors and their causesin the provision of care, are factors predisposingto an increase in the number of errorswhen administering medication.With the aim of identifying and analysingerrors in the administration of drugs withinthe setting of primary care and also to implantnotification and surveillance systems,a Modal Analysis of Errors and Effects(AMFE) on the Administration of Drugswas carried out. Five centres participatedin the survey, including nurses, a familypractitioner, a paediatrician and a qualitydirector from the Sagessa Group. Amongstall the processes relating to the intervention,the administration of drugs obtainedthe highest risk score calculated on the basisof severity and the probability of error.Analysis of this procedure resulted in the creation of several protocols that are currently being assessed (AU)
