Utilidad de la tomografía de coherencia óptica Swept Source en pacientes con distrofias de fotorreceptores / Impact of Swept Source optical coherence tomography in patients with photoreceptor dystrophies

El uso de imágenes de tomografía de coherencia óptica para el diagnóstico de enfermedades que afectan de manera predominante a la capa de fotorreceptores se postula como una técnica que permite evaluar la relación entre los cambios retinianos y la pérdida de agudeza visual. Presentamos el caso de 2 pacientes que acuden a nuestra consulta aquejando una disminución bilateral de la agudeza visual y una alteración de la percepción cromática, en los que encontramos en las imágenes de tomografía de coherencia óptica un defecto hiporreflectivo focal subfoveal que se correlaciona con la banda hiperreflectante correspondiente a los elipsoides de los fotorreceptores

The use of optical coherence tomography to diagnose conditions that predominantly affect the photoreceptor layer is postulated as a technique that allows to evaluate the relationship between retinal changes and loss of visual acuity. Two clinical cases are presented on patients with a bilateral decrease of their visual acuity and alteration in chromatic perception. Optical coherence tomography revealed subfoveal focal hypo-reflective defects on the hyper-reflective band (known as the ellipsoid zone)

Infiltración leucémica y retinitis por citomegalovirus en paciente con leucemia linfoblástica aguda tipo T en remisión completa / Leukaemic infiltration and cytomegalovirus retinitis in a patient with acute T-cell lymphoblastic leukaemia in complete remission

CASO CLÍNCO:Mujer de 43 años con leucemia linfoblástica T en remisión completa, remitida por sospecha de necrosis retiniana herpética/retinitis leucémica en el ojo izquierdo (OI). La agudeza visual era de unidad y el fondo de ojo presentaba retinitis y hemorragias en periferia. Ante estudio hematológico negativo, recibió tratamiento por retinitis por citomegalovirus. Tras mejoría inicial, aparece papilitis en el OI y restricción de la motilidad en el ojo derecho. La resonancia y punción lumbar confirman la recidiva leucémica. DISCUSIÓN: La afectación ocular puede preceder a la recaída hematológica, por eso debe sospecharse ante sintomatología ocular. Además, son frecuentes las infecciones oportunistas en inmunodeprimidos

CLINICAL CASE: A 43-year-old woman in remission from T- cell acute lymphoblastic leukaemia was referred to our hospital with suspected leukaemic retinitis. The funduscopic examination of her left eye revealed multifocal yellow-white peripheral retinitis and retinal haemorrhage. The patient was treated for cytomegalovirus retinitis after an extended haematological investigation showed no abnormalities. Initial improvement was followed by papillitis in the left eye and motility restriction in the right eye. Magnetic resonance and lumbar puncture confirmed leukaemia relapse. Specific treatment was initiated with complete resolution. DISCUSSION: Ocular involvement may precede haematological leukaemia relapse. Physicians should be alerted when ocular symptoms appear in these cases

Metástasis orbitaria como primera manifestación de adenocarcinoma pulmonar / An orbital metastasis as the presentation of lung adenocarcinoma

Introducción: Las metástasis de tumores primitivos invaden la órbita generalmente por vía hemática y en el 19% de los casos representan la primera manifestación de enfermedad neoplásica.Caso Clínico: Mujer de 42 años con un nódulo de crecimiento rápido en región orbitaria anterior derecha, de consistencia pétrea, bien delimitado y asociado a leves signos inflamatorios. En la resonancia magnética se observó una imagen ovoidea con captación homogénea de gadolinio. La exéresis-biopsia del tumor mostró un carcinoma ductal y el estudio sistémico evidenció su origen pulmonar. Se inició tratamiento quimioterápico sin objetivarse en estudios posteriores de imagen signos de recidiva orbitaria.Discusión: La exploración oftalmológica ha constituido el inicio del proceso diagnóstico de un adenocarcinoma pulmonar diseminado

Introduction: Metastasis of primary tumours can involve the orbit via the blood stream. In 19% of cases this type of metastasis is the first oncologic symptom. Case report: A 42-year-old woman with a hard, well-defined nodule situated anteriorly in the right orbit and with mild associated inflammation. MR showed an oval image with homogeneous gadolinium uptake. On excision-biopsy a ductal cancer was found and determined on CAT scan to be of pulmonary origin. Chemotherapy was given and no recurrence occurred in the orbit. Conclusion: Ophthalmologic examination was the first step in the diagnosis of a metastasis of adenocarcinoma of the lung
