ABSTRACT

OBJECTIVES: This study aimed to evaluate the adhesive properties in dentine after the application of silver diamine fluoride (SDF) on carious dentine lesions immediately and after 2 years of water storage. METHODS: 96 human molars used were subjected to artificial dentine caries production, and then randomly divided into 12 experimental groups according to 1. application of an SDF solution (carious dentine lesion without SDF treatment [control], with 12 % silver diamine fluoride [SDF 12 %] or 38 % silver diamine fluoride [SDF 38 %]); 2. Universal adhesives (Clearfil Universal Bond Quick [CUQ] and Single Bond Universal [SBU]); 3. adhesive strategy (etch-and-rinse [ER] and self-etch [SE]). After restoration, the specimens were sectioned and submitted to the microtensile bond strength test (µTBS) and energy-dispersive X-ray spectrometry analysis (SEM/EDX). All tests were performed immediately and after 2 years of water storage. Data from the µTBS were analyzed using four-way ANOVA and Tukey's test (α = 0.05). RESULTS: Only the interaction of factors 'SDF' vs 'time' was significant (p = 0.03). After 2 years of storage, the groups where SDF was applied showed higher µTBS values compared to the control group. No significant decrease in µTBS values was observed for SBU when comparing immediate and 2-year results, but a significant reduction in µTBS values was observed after 2 years for CUQ. CONCLUSION: Independent of the adhesive strategy, the use of SDF may be a promising alternative to maintain the bonding of universal adhesives to carious dentinal lesions. CLINICAL SIGNIFICANCE: This study may clarify and support clinicians regarding the longevity of resin-based restoration in caries-affected dentine treated with silver diamine fluoride.

ABSTRACT

PURPOSE: To evaluate the effect of application time and viscosity of meta-phosphoric acid (MPA) and ortho-phosphoric acid (OPA) on the push-out resin-dentin bond strength (PBS) and nanoleakage (NL) at the adhesive-root dentin interface. METHODS AND MATERIALS: Ninety-six roots of premolars were endodontically prepared and randomly assigned into groups according to the (i) acid (OPA and MPA), (ii) viscosity (gel and liquid), and (iii) application time (7 and 15 seconds). Fiber posts were cemented to the roots, which were then transversally sectioned into serial slices. The slices (cervical, medium, and apical) were subjected to PBS or NL. Analysis of the dentin etching pattern was performed using scanning electron microscopy (SEM). The PBS (MPa) and NL (%) data were subjected to three-way repeated-measures analysis of variance (ANOVA) and Tukey's test (α=0.05). RESULTS: The cross-product interaction was significant for both PBS and NL (p<0.001). No significant difference in the PBS was observed with the use of OPA compared to MPA (p>0.05). Higher PBS values were observed in the groups etched for 15 seconds compared to 7 seconds, regardless of the acid, viscosity, or root third (p<0.05). Lower NL was observed for MPA etching when compared to OPA etching (p<0.05), and these values were not affected by increasing the application time or acid viscosity (p>0.05). A more pronounced etching pattern was observed with OPA than MPA regardless of the acid viscosity. CONCLUSION: The use of 40% meta-phosphoric acid promoted adequate bond strength without increasing dentin demineralization or void spaces in the hybrid layer.

ABSTRACT

A lipoaspiração ultrassônica vem se tornando bastante indicada no tratamento das ginecomastias, devi- do à boa qualidade dos resultados obtidos. O presente estudo relata 11 casos de ginecomastia glandular e glândulo-gordurosa, no período de 1999 a 2002, submetidos à lipoaspiração ultrassônica e lipoaspiração convencional complementar, com cânulas de médio e pequeno calibres. Foi utilizado o equipamento Lisonix 2000, numa amplitude de 5, ou seja, 50 da potência do aparelho, com cânulas de 32 cm de comprimento e 3 mm de diâmetro. O volume médio aspirado, pela lipoaspiração ultrassônica, foi de 222 ml na mama direita e de 225 ml na mama esquerda, enquanto que o volume aspirado por lipoaspiração convencional foi de 200 ml e de 205 ml, respectivamente, totalizando em média, 852 ml por paciente. O tempo médio de lipoaspiração ultrassônica foi de 7 minutos e 12 segundos. Podemos concluir que, a lipoaspiração ultrassônica tem sua indicação, tanto em ginecomastia gordurosa quanto glandular, com uma evolução pós- operatória mais rápida, sem a necessidade de uma incisão na aréola e ressecção cirúrgica da glândula ma- mária.


The ultrasound assisted liposuction is becoming more popular in treating gynecomastias. This paper reports 11 cases of fatty-glandular breasts treated from 1999 to 2002 at private clinic, which were undertaken to ultrasound-assisted lipoplasty and complementary conventional liposuction, with small and medium canulas. Lisonix 2000 was used with 32 cm length canulas and diameter of 3 mm.The medium volume aspirated by ultrasound- assisted lipoplasty in the right breast was 222 ml and 225 ml were aspirated from the left breast, while the volume aspirated by conventional liposuction was 200 ml and 205 ml, respectively, with a total of 852 ml per patient. The medium time of liposuction for each side was 7 minutes and 12 seconds. The ultrasound-assisted lipoplasty is indicated either in fatty or glandular gynecomastia, with better results when compared with conventional liposuction. It has a faster recovery period without the need for glandular tissue resection.

ABSTRACT

Realizou-se um estudo sobre o retalho transverso lombar, descrevendo sua anatomia, técnica operatória, aplicações clínicas e complicações. Baseado em dissecções anatômicas, este retalho é descrito como sendo do tipo axial, em sua porção proximal, nutrido pelos ramos perfurantes das artérias lombares e intercostais e, em sua porção distal, como sendo do tipo randomizado, baseado nos plexos vasculares subdérmicos, que cruzam a linha média da região lombar. Por ser um retalho de fácil execução, com um ótimo aporte sanguíneo e não inviabilizar futuras cirurgias, ele é indicado como primeira opção para o tratamento das úlceras de pressão da região sacral e das lesões localizadas na linha média e adjacências do terço inferior da região lombar.


This is a study concerning transverse lumbar flap that describes its anatomy, surgical technique, clinical applications and complications. Based upon anatomical dissections, this flap is described as with an axial pattern on its proximal portion, irrigated by perfurating branches of lumbar and intercostal arteries, and on its portion as with random pattern due to an uninterrupted subdermal vascular plexus across the midline of back. Being of simple execution, with a good blood supply and not interfiring in possible future operations, it can be indicated as first option for the treatment of sacral pressures sores as well as for lesions located on midline and nearby of the lower third of lumbar area.

ABSTRACT

A reconstrução do escroto, com adeqüada proteção dos testículos, representa um desafio após síndrome de Fournier. Os autores relatam o caso, de um paciente de 20 anos, apresentando lesão extensa acometendo escroto e pênis, devido a síndrome de Fournier. O tratamento do pênis consistiu de enxerto de pele, disposto de forma helicoidal, para evitar retrações. O escroto foi reconstruído utilizando-se um retalho fasciocutâneo de região supéro-medial da coxa, de pedículo inferior, 43 dias após o primeiro desbridamento. O resultado mostrou-se satisfatório, funcional e esteticamante, apesar do tratamento não estar finalizado.


Scrotal reconstruction with adequate protection of the testicles remains the challenge following Fournier’s gangrene. The author report on a 20-years-old man presented with an extensive defect of the scrotum and penile caused by Fournier’s gangrene. The penile coverage was gained by grafts helicoidally placed to avoid retractions. The scrotum was reconstructed using a fasciocutaneous supermedial thigh inferiored based flap, 43 days after the first debridement.The results were satisfactory both functionally and cosmetically, although the treatment is not complete.
