ABSTRACT

In this study, Ag2O was synthesized on polyethylene terephthalate fabrics by using an ultrasonic technique with Ag ion reduction in an aqueous solution. The effects of pH on the microstructure and antibacterial properties of the fabrics were evaluated. X-ray diffraction confirmed the presence of Ag2O on the fabrics. The fabrics were characterized by Fourier transform infrared spectroscopy, ultraviolet⁻visible spectroscopy, and wettability testing. Field-emission scanning electron microscopy verified that the change of pH altered the microstructure of the materials. Moreover, the antibacterial activity of the fabrics against Escherichia coli was related to the morphology of Ag2O particles. Thus, the surface structure of Ag2O particles may be a key factor of the antibacterial activity.

ABSTRACT

BACKGROUND: Return to work is an important aspect for cardiac rehabilitation following a major cardiac event. OBJECTIVE: The aim was to understand the local prevalence and factors associated with returning to work in Malaysia after a cardiac event. METHODS: A cross sectional design was used. All patients attending the cardiac rehabilitation program after major cardiac event during an 11-months period (2011-2012) were included. Data relating to socio-demographic, work-related, risk factors and acute myocardial infarction were collected. The SF-36 questionnaire was used to assess quality of life. Regression analysis was used to determine the predicting factors to return to work. RESULTS: A total of 398 files were screened, 112 respondents agreed to participate giving a response rate of 47.3%. The prevalence of returned to work (RTW) was 66.1% [95% CI: 57.2-75.0]. Factors associated with work resumption were age (Adj. OR: 0.92 (95% CI: 0.84-0.99), diabetes mellitus (Adj. OR: 3.70, 95% CI: 1.35-10.12), Mental Component Summary (MCS) score (Adj. OR: 1.05 (95% CI: 1.01-1.09) and baseline angiography findings. Patients with single vessel and two vessel disease were 8.9 times and 3.78 times more likely to return to work compared to those with 3 vessels (Adj. OR: 8.90 (95% CI: 2.29-34.64) and Adj. OR: 3.78, (95% CI: 1.12, 12.74). CONCLUSIONS: We proposed a cardiac rehabilitation program to emphasize mental health as it may improve successful return to work after cardiac event.

ABSTRACT

Purpose. The aim of this present study was to investigate the ground reaction forces (GRFs) alterations in stroke survivors with diabetic peripheral neuropathy (DPN). Methods. Ten stroke survivors with DPN, 10 stroke survivors without DPN, and 10 healthy controls with matched body weight between groups participated in this case-control cross-sectional study. Three-dimensional GRFs (anterior-posterior, medial-lateral, and vertical) were collected at a comfortable walking speed using the Nexus Vicon motion analysis system and force plate. The Kruskal-Wallis test was used to analyze GRFs parameters. Results. We found significant alterations of medial-lateral forces of the nonparetic side and vertical forces of the paretic side in stroke survivors with DPN compared to stroke survivors without DPN and healthy controls. In addition, there were smaller braking and lower propulsion peak in anterior-posterior forces, smaller magnitude of medial-lateral forces, and lower first and second peak of vertical forces in stroke survivors with DPN compared to stroke survivors without DPN and healthy controls. Conclusion. The study findings identified that GRFs were affected in stroke survivors with DPN on both the paretic and the nonparetic sides. Further investigations are warranted to explore the impact of DPN on the kinematics and muscle activity related to the gait performance in stroke survivors with DPN.

ABSTRACT

INTRODUCTION: Extramedullary hematopoiesis (EMH) is the production of blood cell precursors outside the bone marrow that occur in various hematological diseases. In patients with thalassemia intermedia, ineffective erythropoiesis drives compensatory EMH in the liver, pancreas, pleura, spleen, ribs and spine. CASE PRESENTATION: We describe a patient with thalassemia intermedia who presented with acute neurological symptoms caused by paraspinal EMH, which responded well to combination therapy of steroid, hypertransfusion, laminectomy and excision of pseudotumor and hydroxyurea therapy to boost the formation of fetal haemoglobin. DISCUSSION: Prompt recognition of EMH based on clinical presentation and typical radiological findings should be made. Early treatment is recommended to prevent irreversible damage to the spinal cord.

ABSTRACT

We describe a case of 55-years-old man with a known T11 AIS C since 1985. The muscle strength of his left leg is better than the right leg and he is an active community ambulator. He walks using his right knee ankle foot orthosis without a knee lock. However, on April 2012 he had undergone a left transtibial amputation secondary to infected diabetic foot ulcer. He only had his first contact with rehabilitation team 2 months after the amputation and started on gait retraining since. Given the fact that he is a K3 level as he used to climb Batu Caves which is known to have 272 steps and he plans to continue this activity for his religious purposes, we prescribed him with prosthesis - patella tendon bearing socket, pin and lock suspension, silicone liner and energy storing foot. In conclusion, a community ambulator in dual disabilities, that is, spinal cord injury and amputee is hardly encountered due to multiple confounding factors. However, the right prosthetic prescription in patient with good prognosticating factors to ambulate will determine successful rehabilitation.

ABSTRACT

INTRODUCTION: The Gross Motor Function Classification System (GMFCS) was developed to establish uniform communication between healthcare providers, patients, and the patients' families. It is also used to prognosticate the outcome of motor function. Based on previous reports, prognostication of ambulation status in cerebral palsy is based on the motor development curve, which shows a plateau at a certain known age. CASE REPORT: This report illustrates the case of a boy with spastic triplegic cerebral palsy secondary to postnatal insult at early childhood. The patient was noted to have tremendous progressive improvement in his GMFCS level beyond 7 years old: from level IV at 4 years old to level II at 9 years old. CONCLUSION: Prognostication of ambulation in cerebral palsy based on the motor development curve provides a basis for physicians to predict motor function outcome and plan appropriate intervention. This case report shows that other important factors need to be considered in the clinical evaluation before rendering the prognostication of motor function outcome, including environmental factors as well as the etiology of cerebral palsy, for which special consideration should be given in cases of postnatal insult-related cerebral palsy.
