ABSTRACT

STATEMENT OF PROBLEM: Lichen planus is a chronic inflammatory immunologic-based disease involving skin and mucosa. This disease is generally divided into two categories: erosive and non-erosive. Many etiologic factors are deliberated regarding the disease; however, the disorders of immune system and the role of cytotoxic T-lymphocytes and monocytes are more highlighted. Zinc is an imperative element for the growth of epithelium and its deficiency induces the cytotoxic activity of T-helper2 cells, which seems to be associated with lichen planus. PURPOSE: This study was aimed to evaluate the levels of serum zinc in erosive and non-erosive oral lichen planus (OLP) and to compare it with the healthy control group to find out any feasible inference. MATERIALS AND METHOD: A total of 22 patients with erosive oral lichen planus, 22 patients with non erosive OLP and 44 healthy individuals as the control group were recruited in this descriptive-comparative study. All the participants were selected from the referees to the department of oral medicine, school of dentistry, Tabriz University of Medical Sciences. Serum zinc level was examined for all the individuals with liquid-stat kit (Beckman Instruments Inc.; Carlsbad, CA). Data were analyzed by adopting the ANOVA and Tukey tests, using SPSS 16 statistical software. RESULTS: The mean age of patients with erosive and non-erosive LP was 41.7 and 41.3 years, respectively. The mean age of the healthy control group was 34.4 years .The mean serum zinc levels in the erosive and non erosive lichen planus groups and control groups were 8.3 (1.15), 11.15 (0.92) and 15.74 (1.75) µg/dl respectively. The difference was statistically significant (p< 0.05). CONCLUSION: The serum zinc levels were decreased in patients with erosive oral lichen planus. This finding may probably indicate the promising role of zinc in development of oral lichen planus.

STATEMENT OF PROBLEM: Oral and pharyngeal cancer is one of the most mortal cancers; however, its quick diagnosis and referral is a crucial factor in enhancing the survival rate of the patients. PURPOSE: The aim of this study was to inspect the referral conditions and the reasons for the delay in curing the patients referred to the educational hospitals in Tehran. MATERIALS AND METHOD: In this retrospective -descriptive study, two hundred and fifty six files related to the oral and pharyngeal cancer were inspected. The documents were obtained from 5 educational hospitals specialized in the field of cancers. Eventually data related to the time difference between the first time of attending to lesion and diagnosing the cancer as patient's delay and until the curing as professional's delay were recorded. RESULTS: The majority of cancers were squamous cell carcinoma (SCC). The patient's delay was recorded in 110 files among the whole files. The mean of the time between the patients' first notice of the problem and the time visiting a primary care clinician was 270 days (range, 0-2520 days). The mean of the time from when the patient visited a primary- care clinician to the starting time of definitive treatment was 90 days (range, 0-270 days). CONCLUSION: In this study, like other studies, SCC was the most common occurring cancer. Delays related to the patients were more than those related to the professionals. And at last, accuracy in recording the files and training the patients were recognized to be the most imperative factors to continue the treatment successfully.

ABSTRACT

The improvement in survival and local control measures in children with neoplasm in the head and neck region may lead to increased iatrogenic adverse effects of treatment. The aim of this study was to report a new case of the long-term effects of chemoradiotherapy on oral health and dental development in a patient treated for Hodgkin's disease at an early age. In this case report, a 26-year-old female is presented, who at the age of 5 years received chemotherapy and radiotherapy for Hodgkin's disease in the neck region. The patient consulted the Department of Oral Medicine because of dental changes and tooth loss despite adequate dental care and oral hygiene. Clinical examination revealed loose teeth and inflamed gingiva of the mandible, x-ray showed premature root resorption, V-shaped and shortened roots and alveolar bone loss. After examination, the patient was referred for extracting the mandibular teeth and then wassent to the prosthetics department. Therefore, in order to decrease dental treatment sequelae in patients who have had cured malignant disease, these cases should have life-long dental care and follow-up.
