ABSTRACT

INTRODUCTION: Scaphoid fractures represent 5-10% of nonunion rate and the treatment options consist of an open reduction with correction of deformity, restoration of the scaphoid length with autologous wedge grafting and fixation. However, there is still no consensus in the literature on the best fixation method. Therefore, the purpose of the study is to compare plate fixation and screw fixation in treating scaphoid nonunion with humpback deformity and carpal instability. METHODS: Prospective, non-randomised study comparing the treatment of two groups of patients with scaphoid nonunion. A total of 19 patients were included in the study, the first ten patients were included in group 1 (plate fixation), subsequently nine patients were included in group 2 (screw fixation). The nonunion duration was longer than 6 months and patients did not present type III Scaphoid Nonunion Advanced Collapse (SNAC). Clinical evaluations included pain intensity, range of motion, grip strength, pinch test and functional scales Disabilities of the Arm, Shoulder and Hand (DASH) and Mayo Wrist Score. Radiographic evaluations consisted of radiographs of both wrists in AP, AP with ulnar deviation, lateral and oblique views. Patients further underwent a tomography of the affected wrist for bone deformity, carpal collapse and later consolidation evaluation. RESULTS: According to post-operative measurements, group 1 showed a significant improvement in the scapholunate angle (p = 0.011) and in the intrascaphoid angle (p = 0.002). Group 2 only showed an improvement in the scapholunate angle (p = 0.011). All patients in group 1 achieved consolidation in 8 weeks, while group 2 patients had a slower consolidation, with a mean of 14 weeks and standard deviation (SD) of 4.2, with statistical significance (p = 0.006). CONCLUSION: Our prospective study, despite the limitations, contributes to the literature for demonstrating a better fixation using plate, with a better correction of humpback deformity and Dorsal Intercalated Segment Instability (DISI) and a faster consolidation. ABSTRACT

Objectives: The purpose of this study is to describe associated factors and epidemiological aspects of Dupuytren's Disease in patients followed up in a Brazilian tertiary public hospital, at the Hand Surgery service. Methods: A cross-sectional study was performed from 2014 to 2019. Data collected included: age, gender, ancestry, associated comorbidity presence, phenobarbital, tobacco, and alcohol use, family history of Dupuytren's Disease and associated fibrotic diseases. Then, the patients underwent a clinical examination to identify and characterize the involvement of the fingers. The patients were also assessed in regard to whether they presented Dupuytren's Disease severity factors. Results: 140 patients were included, 70.7% men and 29.3% women. Only 42.3% reported being of European ancestry; 20% had first-degree relatives with the disease; 59.3% presented comorbidities, including hypertension, diabetes, chronic heart disease, dyslipidemia, epilepsy, and HIV infection; 15.8% had Ledderhose disease, 7.1% had Peyronie's disease. 31% were smokers, 16.6% were alcoholic, and 37.1% were phenobarbital users; 40% presented with a severe form of DD. Conclusion: The population studied was composed of Brazilians, most of whom did not report European ancestry; still, they presented several characteristics similar to those described in literature worldwide. BACKGROUND: tissue expanders have high relevance in plastic surgery and among indications it is worth mentioning their use in the treatment of burn reconstruction. Although it shows good results, its use requires special care because some complications can interrupt the reconstruction process. The objective of this study was to report the experience of the Clinics Hospital (University of Sao Paulo) with the use of tissue expanders to treat burn sequelae, establishing the incidence of complications, and identifying risk factors for their occurrence. METHODS: a retrospective, observational, and analytical study, evaluating the use of expanders in burns sequelae treatment from 2009 to 2018. RESULTS: 245 expanders were placed in 84 patients, 215 were female, with a mean age of 19.96 years, being 40% in the trunk and 20% in the scalp, with a predominance of rectangular shape in 76.7% of cases. Complications were classified as major and minor.Complications occurred in 17.95% of cases, and extrusion and infection were the most common. There was a higher incidence of complications in expanders used in the upper and lower limbs as well as in those who did not undergo concomitant expansion (p <0.05), with an even higher chance of major complications in patients submitted to additional expansion. From 2009 to 2018, we observed a decrease in the incidence of complications. CONCLUSION: the complication rate (17.95%) is similar to other studies of the literature, there was a higher rate of complication with expanders placed in the limbs and a higher rate of major complications when additional expansion was done.

BACKGROUND: We aimed to evaluate the use of nerve allograft preserved in glycerol. We compared the efficiency of glycerol-preserved allografts with autogenous nerve grafting, cryopreserved grafts, and detergent-processed grafts in the axonal regeneration. Secondarily, we evaluated the effectiveness of each preservation method in maintaining the extracellular matrix free of cellular components. METHODS: This was a prospective experimental, longitudinal, unblinded, nonrandomized, controlled animal model study. Three different allograft preservation techniques for the repair of sciatic nerve injuries were compared, including cold preservation, glycerol preservation, and detergent preservation. Functional assessment was performed, and histomorphometric analyses were further performed, which enabled the allograft structure evaluation and an estimation of the nerve regeneration efficacy based on the myelinated axons count and on their diameters. RESULTS: After the 14th week, all groups were already balanced and similar (P = 0.265): all groups present near-zero SFIs, thus confirming their efficiency in promoting nerve regeneration. In the histomorphometric evaluations, all groups were equivalent, presenting a similar efficiency in nerve regeneration (P = 0.716 and P = 0.577, respectively). Similarly, histomorphometric evaluations showed a reduction in the number of axons and in their diameters, but none of them effectively eliminated all cellular debris. Comparing the groups with each other, the groups preserved in glycerol and detergent solution were similar, both presenting better results than the cooled group. CONCLUSION: By evaluating the presence of cell debris after the treatment using glycerol, it was found to be similar to the treatment using detergent and significantly better than the cold-preservation treatment.

INTRODUCTION: Syndactyly has a cosmetic, functional, and psychosocial impact, and surgical treatment is indicated in most cases. This study aimed to retrospectively identify whether patients undergoing surgical release of syndactyly younger than 1 year presented different results compared with those operated older than 1 year. METHODS: Patients were assessed through photographic records by 3 independent specialist surgeons for the quality of scar, presence of web creep, rotational, flexion-extension, and lateral flexion deformities using the Withey score. RESULTS: Thirty-four patients were included in the study, totaling 51 commissures operated. The score was higher in the group operated younger than 1 year (5.83 ± 2.39) compared with the group older than 1 year (3.94 ± 1.93), being statistically significant, with a P value of 0.011. CONCLUSIONS: Children with syndactyly operated younger than 1 year have worse postoperative outcomes measured by the Withey score than those operated older than 1 year.

RESUMO Introdução: os expansores teciduais são de importante relevância na cirurgia plástica, e dentre suas indicações destaca-se seu uso no tratamento de sequelas de queimaduras. Ainda que apresente bons resultados, seu uso requer cuidados especiais pois a incidência de complicações não é desprezível. Objetivo: relatar a experiência do Hospital das Clínicas da FMUSP com a utilização de expansores teciduais para tratamento de sequelas de queimaduras, estabelecendo a incidência de complicações e identificando possíveis fatores de risco para ocorrência das mesmas. Método: estudo retrospectivo, observacional e analítico, avaliando o uso de expansores no tratamento de sequelas de queimaduras no período de 2009 a 2018 no Hospital das Clínicas da FMUSP. Resultados: no período de análise estabelecido, foram colocados 245 expansores, sendo 215 em pacientes do sexo feminino, com idade média de 19,96 anos, sendo 40% em tronco e 20% em couro cabeludo, predominando uso do retangular em 76,7% dos casos. As complicações foram divididas em maiores e menores. As complicações ocorreram em 17,95% dos casos, sendo extrusão e infecção as de maior incidência. Houve maior incidência de complicação em expansores utilizados nos membros superiores e inferiores bem como naqueles que não realizaram expansão concomitante (p<0,05), havendo ainda maior chance de complicações absolutas nos pacientes submetidos à expansão adicional. Observou-se também diminuição no número de complicações entre 2009 e 2018. Conclusão: a taxa de complicação (17,95%) é semelhante a da literatura, observando-se maior taxa de complicação nos expansores colocados em membros e maior taxa de complicações absolutas quando realizada expansão adicional.

Introdução: Lesões que acometem as mãos com importante perda cutânea frequentemente requerem retalhos para cobertura precoce, visto que permitem melhor reabilitação. Dentre as opções, o retalho interósseo posterior reverso do antebraço é o mais utilizado para defeitos no dorso da mão e punho, com baixas taxas de complicações. Normalmente, esse retalho não é utilizado para a reconstrução de defeitos em região palmar, já que geralmente não alcança esse local. Relato de caso: Apresentamos o caso de um paciente com queimadura elétrica de terceiro grau, em palma da mão direita, cuja reconstrução foi realizada com o uso do retalho interósseo posterior reverso do antebraço, após debridamentos conservadores, no 14o dia após a queimadura. O paciente apresentou boa evolução pós-operatória, sem complicações ou sequelas funcionais a longo prazo. Conclusão: O retalho interósseo posterior reverso do antebraço permite cobertura adequada de lesões em palma da mão, preservando sua funcionalidade.


Introduction: Lesions affecting the hands with significant skin loss often require flaps for early coverage, as these permit faster healing. Among the various options, the reverse posterior interosseous flap of the forearm is most commonly used for defects involving the back of the hand and wrist due to low complication rates. Normally, this flap is not used for the reconstruction of defects in the palmar region since its distal reach is insufficient. Case report: We present the case of a male patient with third-degree electrical burns on his right palm, whose reconstruction was performed on the 14th day postinjury using the reverse posterior interosseous flap of the forearm after conservative debridement. The patient presented good postoperative evolution, without long-term complications or functional sequelae. Conclusion: The reverse posterior interosseous flap of the forearm permits adequate coverage of palm injuries, preserving its functionality.

ABSTRACT

OBJECTIVE: To assess and compare radiographic, tomographic and functional pre and postoperative parameters of patients submitted to treatment of nonunion of the scaphoid waist with locked plate for scaphoid. METHODS: This is a case series of scaphoid waist nonunion, operated from March 2017 to March 2018 at the Institute of Orthopedics and Traumatology of HCFMUSP, using the technique of Fisk-Fernandez and osteosynthesis with locked plate for scaphoid APTUS®Hand from Medartis. The patients were submitted to radiographs, computed tomography and functional evaluation by the occupational therapy team in the pre and postoperative periods. RESULTS: There was consolidation of the scaphoid waist nonunion in all cases, improvement in the parameters of carpal alignment in the imaging examinations, but functionally we observed reduction of the range of motion and grip strength in relation to the contralateral limb. CONCLUSIONS: The treatment of the scaphoid waist nonunion with locked plate was effective, with a high index of consolidation and improvement of the carpal alignment in the imaging tests, although with a reduction in the range of motion and grip strength in relation to the contralateral limb when evaluated with 12 weeks postoperatively. OBJETIVOS: Avaliar e comparar parâmetros radiográficos, tomográficos e funcionais pré e pós-operatórios de pacientes submetidos ao tratamento de pseudoartrose do colo do escafoide com placa bloqueada para escafoide. MÉTODOS: Esta é uma série de casos de pseudoartrose do colo do escafoide, operados no período de março de 2017 a março de 2018 no Instituto de Ortopedia e Traumatologia do HCFMUSP, utilizando-se a técnica de Fisk-Fernandez e osteossíntese com placa bloqueada para escafoide APTUS®Hand da Medartis. Os pacientes foram submetidos a radiografias, tomografia computadorizada e avaliação funcional pela equipe de terapia ocupacional no pré e pós-operatório. RESULTADOS: Houve consolidação da pseudoartrose do colo do escafoide em todos os casos e melhora nos parâmetros de alinhamento carpal nos exames de imagem, porém, funcionalmente, observamos redução da amplitude de movimento e força de preensão em relação ao membro contralateral. CONCLUSÃO: O tratamento da pseudoartrose do colo do escafoide com placa bloqueada se mostrou efetivo, com elevado índice de consolidação e melhora do alinhamento carpal nos exames de imagem, embora com redução da amplitude de movimento e força de preensão em relação ao membro contralateral, quando avaliados com 12 semanas de pós-operatório. OBJECTIVE: To analyze 10 consecutive cases of microsurgical arteriovenous loops created to reconstruct complex injuries from March 2011 to May 2012. METHODS: This observational cohort-type study conducted by the Hand and Microsurgery Group at the HC-FMUSP included patients who were candidates for microsurgical reconstruction as a last alternative to amputation of the limb with proven absence of adequate recipient vessels for primary microsurgical anastomosis, in a prospective and consecutive manner. We analyzed 14 variables (epidemiological, clinical, procedure-related, and outcome) in patients who underwent reconstruction using an arteriovenous loop utilizing a single-stage or two-stage procedure. RESULTS: The injuries were mostly traumatic (80%). The success rate of the single-stage procedure was 75%, and 17% for the two-stage procedure. The rate of preservation for the injured limb was 44%. CONCLUSION: This study reinforces the more recent understanding that the indication for single-stage or two-stage reconstruction should be individualized; our findings favor the single-stage reconstruction. This technique should be used in selected cases, as a last reconstructive alternative before amputation, and further studies are necessary to confirm its safety and efficacy in our practice. OBJETIVO: Analisar dez casos consecutivos de alças vasculares microcirúrgicas realizadas para reconstrução de feridas complexas no período de março de 2011 a maio de 2012. MÉTODOS: Estudo de observação, analítico do tipo coorte, realizado pelo Grupo de Mão e Microcirurgia do Instituto de Ortopedia e Traumatologia do HC-FMUSP. Foram incluídos, prospectivamente e de maneira consecutiva, os pacientes candidatos à reconstrução microcirúrgica como última alternativa à amputação do membro, com comprovada ausência de vasos receptores adequados para anastomose microcirúrgica primária. Foram analisadas 14 variáveis (epidemiológicas, clínicas, relacionadas ao procedimento e resultados) nos pacientes submetidos à reconstrução com alça em tempo único e em dois tempos. RESULTADOS: As lesões foram de etiologia traumática em sua maioria (80%). A taxa de sucesso do procedimento em tempo único foi de 75%, e em dois tempos de 17%. A taxa de manutenção do membro lesado foi de 44%. CONCLUSÃO: Este estudo reforça o entendimento mais recente de que a indicação de reconstrução em um ou dois tempos deve ser individualizada, e nossos resultados favorecem a reconstrução em tempo único. A técnica deve ser usada em casos selecionados, como última opção reconstrutiva à amputação, sendo necessários mais estudos para atestar sua segurança e eficácia em nosso meio. ABSTRACT

OBJECTIVE: To evaluate postoperative epidemiological and functional factors in patients operated to treat duplicate thumb. METHODS: This retrospective case series evaluated 20 patients (23 thumbs) treated from January 2012 to December 2016 at our service. Epidemiological and clinical factors were studied, including Tada score. RESULTS: Of the 34 children who were treated surgically, only 20 appeared for the functional evaluation and were included in the study. Of the operated cases, 60% were Wassel type 4. All evaluated cases had good functional results (Tada score ≥5, mean score: 6.65). The most frequently used surgical technique was resection of the radial thumb with reconstruction of the radial collateral ligament (47.8% of cases). CONCLUSION: Surgical correction of duplicated thumb yields good results, as long as attention is paid to abnormalities in bones, ligaments, and tendons. Level of Evidence IV; Case series.


OBJETIVO: Avaliar os fatores epidemiológicos e funcionais pós operatórios dos pacientes operados por duplicação de polegar. MÉTODOS: Essa é uma série retrospectiva de casos em que foram avaliados 20 pacientes (23 polegares), operados entre janeiro de 2012 e dezembro de 2016, no Hospital Menino Jesus/ SP. Foram estudados fatores epidemiológicos e clínicos, dentre os quais o score funcional de Tada. RESULTADOS: Das 34 crianças operadas, apenas 20 compareceram para avaliação funcional. Dos casos operados, 60% eram Wassel tipo 4. Todos os casos avaliados obtiveram resultado funcional bom (Tada maior ou igual a 5), com score médio de 6,65. A técnica cirúrgica mais utilizada foi a ressecção do polegar radial com reconstrução do ligamento colateral radial, presente em 47,8% das vezes. CONCLUSÃO: A correção cirúrgica da duplicação de polegar cursa com bons resultados, desde que se atente para as anormalidades osteoligamentares e tendíneas. Nível de Evidência IV; Série de casos.

ABSTRACT

Síndrome do choro assimétrico é uma condição congênita secundária à hipoplasia ou ausência do músculo depressor do ângulo da boca. Trata-se de uma condição não tão incomum que pode cursar com assimetria facial ao chorar e sorrir, além de poder estar associadas a outras malformações congênitas. Crianças com essa deformidade podem sofrer dificuldades psicossociais e introversão. O arsenal terapêutico dessa condição já foi estudado e discutido na literatura com ênfase em abordagens cirúrgicas e invasivas. Relatamos aqui um caso de uma criança de 9 anos com essa síndrome, tratada, de forma menos invasiva, com toxina botulínica, com um bom resultado e satisfação.


Asymmetric crying face syndrome is a congenital condition secondary to hypoplasia or absence of the depressor muscle at the mouth angle. It is a common condition that presents with facial asymmetry while crying and smiling and may be associated with other congenital malformations. Children with this deformity may experience psychosocial difficulties and introversion. The therapeutic arsenal of this condition has already been studied and discussed in the literature with an emphasis on surgical and invasive approaches. We report here a case of a 9-year-old child with this syndrome, treated less invasively with botulinum toxin, with good result and satisfaction.

ABSTRACT Objective To evaluate postoperative epidemiological and functional factors in patients operated to treat duplicate thumb. Methods This retrospective case series evaluated 20 patients (23 thumbs) treated from January 2012 to December 2016 at our service. Epidemiological and clinical factors were studied, including Tada score. Results Of the 34 children who were treated surgically, only 20 appeared for the functional evaluation and were included in the study. Of the operated cases, 60% were Wassel type 4. All evaluated cases had good functional results (Tada score ≥5, mean score: 6.65). The most frequently used surgical technique was resection of the radial thumb with reconstruction of the radial collateral ligament (47.8% of cases). Conclusion Surgical correction of duplicated thumb yields good results, as long as attention is paid to abnormalities in bones, ligaments, and tendons. Level of Evidence IV; Case series.

ABSTRACT Objective: To analyze 10 consecutive cases of microsurgical arteriovenous loops created to reconstruct complex injuries from March 2011 to May 2012. Methods: This observational cohort-type study conducted by the Hand and Microsurgery Group at the HC-FMUSP included patients who were candidates for microsurgical reconstruction as a last alternative to amputation of the limb with proven absence of adequate recipient vessels for primary microsurgical anastomosis, in a prospective and consecutive manner. We analyzed 14 variables (epidemiological, clinical, procedure-related, and outcome) in patients who underwent reconstruction using an arteriovenous loop utilizing a single-stage or two-stage procedure. Results: The injuries were mostly traumatic (80%). The success rate of the single-stage procedure was 75%, and 17% for the two-stage procedure. The rate of preservation for the injured limb was 44%. Conclusion: This study reinforces the more recent understanding that the indication for single-stage or two-stage reconstruction should be individualized; our findings favor the single-stage reconstruction. This technique should be used in selected cases, as a last reconstructive alternative before amputation, and further studies are necessary to confirm its safety and efficacy in our practice. RESUMO Objetivo: Analisar dez casos consecutivos de alças vasculares microcirúrgicas realizadas para reconstrução de feridas complexas no período de março de 2011 a maio de 2012. Métodos: Estudo de observação, analítico do tipo coorte, realizado pelo Grupo de Mão e Microcirurgia do Instituto de Ortopedia e Traumatologia do HC-FMUSP. Foram incluídos, prospectivamente e de maneira consecutiva, os pacientes candidatos à reconstrução microcirúrgica como última alternativa à amputação do membro, com comprovada ausência de vasos receptores adequados para anastomose microcirúrgica primária. Foram analisadas 14 variáveis (epidemiológicas, clínicas, relacionadas ao procedimento e resultados) nos pacientes submetidos à reconstrução com alça em tempo único e em dois tempos. Resultados: As lesões foram de etiologia traumática em sua maioria (80%). A taxa de sucesso do procedimento em tempo único foi de 75%, e em dois tempos de 17%. A taxa de manutenção do membro lesado foi de 44%. Conclusão: Este estudo reforça o entendimento mais recente de que a indicação de reconstrução em um ou dois tempos deve ser individualizada, e nossos resultados favorecem a reconstrução em tempo único. A técnica deve ser usada em casos selecionados, como última opção reconstrutiva à amputação, sendo necessários mais estudos para atestar sua segurança e eficácia em nosso meio. As lesões mutilantes de mão são um desafio para o cirurgião de mão e o paciente. O cirurgião deve tomar decisões desde o debridamento inicial, escolhendo quais dedos e articulações serão preservadas e uso apropriado das partes a serem retiradas. A reconstrução tardia é a segunda parte dessa difícil tarefa. Dificuldade que se atribui à particularidade de cada lesão, do grande número de possibilidades de tratamento e seus diversos níveis de complexidade que devem ser adequados à necessidade e motivação pessoal de cada paciente. Este relato de caso apresenta uma reconstrução tardia de mão com perda de indicador e dedo médio com transplante de articulação metacarpofalângica de indicador para função de interfalangeana proximal de dedo médio.


Mutilating hand injuries are a challenge to both the hand surgeon and the patient. The surgeon must make decisions ranging from the initial debridement to which fingers and joints will be preserved and the appropriate use of the parts to be removed. Late reconstruction constitutes the second part of this difficult task. The difficulty attributed to the characteristics of each lesion, the large number of treatment possibilities, and the different levels of complexity must be adapted to the personal needs and motivation of each patient. This case report describes a late hand reconstruction with index and middle finger loss, using metacarpophalangeal joint transplantation of the index finger to gain the proximal interphalangeal function of the middle finger.

ABSTRACT

OBJECTIVE: To evaluate the factors that influence the survival rate of replantation and revascularization of the thumb and/or fingers. METHODS: We included fifty cases treated in our department from May 2012 to October 2013 with total or partial finger amputations, which had blood perfusion deficit and underwent vascular anastomosis. The parameters evaluated were: age, gender, comorbidities, trauma, time and type of ischemia, mechanism, the injured area, number of anastomosed vessels and use of vein grafts. The results were statistically analyzed and type I error value was set at p <0.05 . RESULTS: Fifty four percent of the 50 performed replantation survived. Of 15 revascularizations performed, the survival rate was 93.3%. The only factor that affected the survival of the amputated limb was the necessity of venous anastomosis. CONCLUSION: We could not establish contraindications or absolute indications for the replantation and revascularization of finger amputations in this study.
