ABSTRACT

Antiphospholipid antibody syndrome (APS) has been widely recognized to be associated with various neurological complications. In addition to the classical notion of APS as a thrombotic disorder, APS has been suggested to be an autoinflammatory disease as well. We present a previously healthy 46-year-old man who concurrently developed transverse myelitis and polymyositis whose laboratory studies were significant for the elevated antiphospholipid antibodies such as anti-cardiolipin (CL)/beta2-glycoprotein I (beta 2GPI) antibody. This report further enhances the recognized clinical phenotypes of the neurological complications of APS and the understanding of its pathomechanism.

ABSTRACT

There is a disorder that is called Guillain-Barre Symdrome(GBS). Currently, plasmapheresis and high-dose immunoglobulin therapy are used for GBS. Plasmapheresis is one of the blood purifications. When the blood purification is enforced, it should be careful to decrease the blood pressure. In the Tokushima University Hospital, the hematocrit(Ht) values are measured using Crit-Line Monitor(CLM), because the Ht values are rate of red cell in blood, and are relation of the blood pressure. Hence, it is important to predict Ht value using the prediction method. There are various prediction methods, but we proposed the new prediction method. The purpose of this study is the prediction of Ht value after 1,3, and 5 minutes during the plasma exchange using the new prediction method. As the results, rms errors were small in all prediction, but the predicted Ht values are same the measured Ht value before 1,3, and 5 minutes, because the measured Ht values are strong autocorrelation.

ABSTRACT

A 66-year-old woman with SLE who recently started oral steroid was admitted to our hospital because of subacute numbness of the bilateral legs and gait disturbance, despite of the stable symptoms of SLE and its serology. Blood tests revealed hyperglycemia. Nerve conduction study showed decreased nerve conduction velocities consistent with demyelinative neuropathy. On initiation of insulin to control hyperglycemia, conduction velocities were rapidly increased in one week, suggesting hyperglycemic neuropathy. The rapid improvement of the conduction velocities was argued against mechanical "demyelination", rather suggesting axonal dysfunction, possibly associated with impaired function of Na(+)-K(+)-ATPase in hyperglycemic state.
