ABSTRACT

Coletânea dedicada aos estudos das respostas rápidas do Programa Educacional em Vigilância em Saúde no enfrentamento da COVID-19 e outras Doenças Virais (VigiEpidemia). Esse tema é de extrema relevância e atualidade em nosso contexto da saúde global e na resposta as emergências em saúde pública (ESP) de forma geral. As ESP, que englobam surtos e epidemias, desastres e desassistência à população, representam desafios complexos que exigem respostas ágeis e eficazes por parte das autoridades sanitárias, profissionais da saúde e comunidades como um todo. Até o momento, a pandemia de COVID-19 foi a maior ESP do Século XXI. Ela serviu como um lembrete doloroso da vulnerabilidade da humanidade diante da ameaça de doenças virais. Esta ESP, que teve resposta catastrófica em diversos momentos, evidenciou a importância do investimento em preparação, vigilância e resposta, destacando a necessidade de sistemas de vigilância robustos, colaboração internacional, Inteligência epidemiológica e comunicação transparente para mitigar o impacto devastador das doenças infecciosas na sociedade. As lições aprendidas com a pandemia de COVID-19 são vastas e multifacetadas. A importância da pesquisa, da educação em saúde e do desenvolvimento de vacinas foi evidenciada como uma prioridade crucial na proteção da saúde pública mundial. O investimento em pesquisas e em cursos para formação de profissionais que possam estar atentos as mudanças nos padrões e comportamentos das doenças infecciosas, além de atuar na resposta rápida quando necessário, é fundamental para estarmos preparados para as futuras pandemias. A vacinação, por exemplo, sempre foi uma das ferramentas mais poderosas para evitar surtos e epidemias e, durante a pandemia de COVID-19, ajudou a controlar os óbitos pela doença e possibilitou que voltássemos a ter uma vida normal. Além da vacina contra COVID-19, as vacinas de influenza e dengue também são exemplos notáveis de avanços científicos que desempenham um papel fundamental na prevenção de futuras ESP. Ao explorar os diversos aspectos da resposta, monitoramento e controle de surtos, epidemias e pandemias, esta coletânea visa fornecer uma compreensão abrangente dos desafios enfrentados, das melhores práticas e das estratégias eficazes para mitigar os impactos adversos desses eventos. Espera-se que este trabalho não apenas informe e eduque, mas também inspire ações concretas para fortalecer a recuperação e resiliência dos sistemas de saúde e proteger o bem-estar das comunidades mais vulneráveis do nosso pais.


A collection dedicated to the study of rapid responses by the Educational Program in Health Surveillance in addressing COVID-19 and other Viral Diseases (VigiEpidemia). This theme is of utmost relevance and timeliness in our context of global health and in responding to public health emergencies (PHE) in general. PHEs, which encompass outbreaks and epidemics, disasters, and neglect of the population, represent complex challenges that require swift and effective responses from health authorities, healthcare professionals, and communities as a whole. To date, the COVID-19 pandemic has been the largest PHE of the 21st century. It served as a painful reminder of humanity's vulnerability in the face of viral disease threats. This PHE, which had catastrophic responses at various times, highlighted the importance of investing in preparedness, surveillance, and response, underscoring the need for robust surveillance systems, international collaboration, epidemiological intelligence, and transparent communication to mitigate the devastating impact of infectious diseases on society. The lessons learned from the COVID-19 pandemic are vast and multifaceted. The importance of research, health education, and vaccine development was highlighted as a crucial priority in protecting global public health. Investing in research and training courses to prepare professionals who can be attentive to changes in the patterns and behaviors of infectious diseases and act quickly when needed is essential to be prepared for future pandemics. Vaccination, for example, has always been one of the most powerful tools to prevent outbreaks and epidemics, and during the COVID-19 pandemic, it helped control disease-related deaths and allowed us to return to a normal life. In addition to the COVID-19 vaccine, influenza and dengue vaccines are also notable examples of scientific advancements that play a key role in preventing future PHEs. By exploring the various aspects of response, monitoring, and control of outbreaks, epidemics, and pandemics, this collection aims to provide a comprehensive understanding of the challenges faced, best practices, and effective strategies to mitigate the adverse impacts of these events. It is hoped that this work will not only inform and educate but also inspire concrete actions to strengthen the recovery and resilience of health systems and protect the well-being of the most vulnerable communities in our country.

ABSTRACT

As Políticas Informadas por Evidências (PIE) tem ocupado uma posição de destaque nas últimas décadas. Nota-se, portanto, um aumento do estimulo e da valorização da síntese do conhecimento cientifico para apoiar o desenvolvimento de políticas públicas de saúde baseadas em evidências. No Brasil, o Ministério da Saúde institucionalizou em 2009 a Rede para Politicas Informadas por Evidencias (Evidence-Informed Policy Network ­ EVIPNet Brasil). Essa Rede foi uma criação da Organização Mundial da Saúde (OMS) para promover o uso sistemático e transparente de evidências cientificas no desenvolvimento e na implementação de políticas de saúde, proporcionando o intercâmbio de conhecimento entre gestores, pesquisadores e representantes da sociedade civil, e facilitando a formulação, a implementação de políticas e a gestão dos serviços e sistemas de saúde. Coordenada pelo Departamento de Ciência e Tecnologia da Secretaria de Ciência, Tecnologia e Inovação e do Complexo Econômico-Industrial da Saúde do Ministério da Saúde (DECIT/SCTICS/MS), a EVIPNet Brasil, integrada por Núcleos de Evidências em Saúde (NEv), oferece periodicamente cursos diversos sobre PIE, além de materiais, como diretrizes metodológicas, para auxiliar na síntese de evidências para tomada de decisão em saúde. Com o evento da pandemia da COVID-19, a busca por evidências cientificas tornou-se mais intensa. A cobrança por resposta a curto prazo marcou esse período recente, desafiando o mundo todo a aprimorar mecanismos de elaboração, tradução e disseminação de conhecimento científico. Nesse contexto, surge o VigiEpidemia, um Programa Educacional em Vigilância e Cuidado em Saúde no Enfrentamento da COVID-19 e de outras doenças virais que oferece qualificação aos trabalhadores do Sistema Único de Saúde (SUS). Por meio desse programa, foram elaborados produtos, baseados no Portfolio de Produtos do Serviço de Produção de Evidências para Apoio a Tomada de Decisão, do DECIT, que tiveram como objetivo principal estimular o uso de evidências cientificas no cotidiano dos serviços de saúde. Este e-book oferece a leitora e ao leitor 26 pesquisas criteriosamente selecionadas com conteúdo interessante produzidos por especializandos no âmbito do VigiEpidemia, dividido em duas partes: a primeira sobre COVID-19 e outras doenças virais e a segunda sobre Dengue e outras arboviroses.


Evidence-Informed Policies (EIP) have gained prominence over the past few decades. Consequently, there has been an increase in the stimulation and appreciation of the synthesis of scientific knowledge to support the development of evidence-based public health policies. In Brazil, the Ministry of Health institutionalized the Evidence-Informed Policy Network (EVIPNet Brazil) in 2009. This network was an initiative by the World Health Organization (WHO) to promote the systematic and transparent use of scientific evidence in the development and implementation of health policies, facilitating knowledge exchange among managers, researchers, and representatives of civil society, and aiding in policy formulation, implementation, and the management of health services and systems. Coordinated by the Department of Science and Technology of the Secretariat of Science, Technology, Innovation, and the Economic-Industrial Health Complex of the Ministry of Health (DECIT/SCTICS/MS), EVIPNet Brazil, consisting of Health Evidence Centers (NEv), periodically offers various courses on EIP, along with resources such as methodological guidelines to assist in the synthesis of evidence for health decision-making. With the advent of the COVID-19 pandemic, the search for scientific evidence became more intense. The demand for short-term responses marked this recent period, challenging the world to enhance mechanisms for the elaboration, translation, and dissemination of scientific knowledge. In this context, the VigiEpidemia program emerged, an Educational Program in Health Surveillance and Care in addressing COVID-19 and other viral diseases, offering qualifications to workers of the Unified Health System (SUS). Through this program, products were developed based on the DECIT's Evidence Production Service Portfolio to Support Decision Making, primarily aimed at stimulating the use of scientific evidence in the daily operations of health services. This e-book provides the reader with 26 meticulously selected research studies with interesting content produced by specialists within the scope of VigiEpidemia, divided into two parts: the first on COVID-19 and other viral diseases, and the second on Dengue and other arboviruses.

ABSTRACT

The COVID-19 pandemic in Brazil affected mental health among healthcare workers. To objective of this study was to evaluate the mental health of healthcare workers in in the central-west region of the Brazil, estimating the prevalence of mental health disorders, and investigating associated factors, perceptions of safety, and self-perceptions about mental health in times of the COVID-19 pandemic. The questionnaire was divided into two parts that included general information and perceptions about the work process and identified symptoms using the Depression Anxiety Stress Scale-21 (DASS-21), and multiple linear regression analysis was conducted. A total of 1,522 healthcare workers participated in the survey. Overall prevalence of symptoms was calculated for depression (58.7%), anxiety (59.7%), and stress (61.7%). Physicians had 3.75 times greater risk of depression (1.59-8.85, 95% CI). Independent variables associated with depression symptoms were not feeling safe with the way services were organized (1.12:1.03-1.21, 95% CI) and self-perception of poor mental health (8.06: 4.03-16.10% CI). Working in management was protective, and married professionals had 12% lower risk of exhibiting symptoms of depression (0.79-0.99, 95% CI). Participants with self-perception of poor mental health had 4.63 greater risk for symptoms of anxiety (2.58-8.31, 95% CI). Protective factors were not having sought support for mental health (0.90: 0.82-0.99, 95% CI), having a graduate degree (0.71: 0.54-0.94, 95% CI), and not having been diagnosed with COVID-19 (0.90: 0.83-0.98, 95% CI). Perception of poor mental health was associated with 6.95-fold greater chance of developing stress symptoms. Protective factors from stress were having a degree in dentistry (0.81: 0.68-0.97, 95% CI), residing in Mato Grosso do Sul (0.91: 0.85-0.98, 95% CI), and not having sought mental health support services (0.88: 0.82-0.95, 95% CI). The prevalence of mental health disorders is high among healthcare workers, and is associated with professional category, organization of services provided, and self-perception of poor mental health, reinforcing the need for preventative measures.

ABSTRACT

Analisar a aquisição de competências para a formação generalista previstas no Programa de Estágio Supervisionado em Atenção Primária à Saúde (PESAP) do curso de medicina de uma Universidade privada, na perspectiva dos alunos e dos preceptores. Método: Estudo seccional, descritivo-analítico, com 104 alunos e 16 preceptores, por meio de questionário autoaplicável contendo 60 questões, em escala tipo Likert, distribuídas nas áreas: atenção primária à saúde (n=9), saúde do adulto e do idoso (n=13), saúde da criança e do adolescente (n=28) e saúde da mulher (n=10). Calculou-se a média aritmética, desvio padrão e classificação (incipiente, parcialmente satisfatória ou satisfatória). Resultado: Das competências relacionadas à atenção primária à saúde, 66,7% receberam classificação satisfatória para ambos os participantes. Para a saúde do adulto e do idoso, 53,8% das competências foram satisfatórias por alunos, e 61,5% por preceptores. No tocante à saúde da criança e do adolescente, 13 competências (46,4%) foram adquiridas satisfatoriamente, na ótica dos alunos e 9 (32,1%), dos preceptores, e na saúde da mulher, 4 (40%) e 5 (50%) competências receberam classificação satisfatória para ambos, respectivamente. Obteve uma média satisfatória de 51,8% e 52,6% para os alunos e preceptores, respectivamente. Conclusão: Embora a maioria das competências previstas no PESAP foram adquiridas de forma satisfatória, a classificação final foi parcialmente satisfatória. O PESAP mostrou-se propício para as áreas Atenção Primária à Saúde e Saúde do Adulto e do Idoso. Porém, ainda existem lacunas a serem aprimoradas, principalmente em relação à saúde da criança e do adolescente e saúde da mulher.

ABSTRACT

Compreender a percepção dos profissionais de saúde quanto à investigação dos óbitos de mulheres em idade fértil e maternos ocorridos entre brasileiras e estrangeiras e a atuação dos Comitês de Mortalidade Materna nos municípios na fronteira com Bolívia e Paraguai. Métodos: Trata-se de um estudo descritivo, exploratório, de abordagem qualitativa, realizado em municípios de fronteira do estado de Mato Grosso do Sul com outros países, com notificações de óbitos maternos em 2016-2017, por meio de entrevistas com profissionais atuantes na vigilância do óbito materno. Resultados: Constatou-se que a investigação dos óbitos em mulheres em idade fértil e maternos é realizada, apesar das dificuldades relacionadas à qualificação dos profissionais que integram a Rede de Vigilância do Óbito Materno; à desarticulação da Rede de Atenção à Saúde; e ao escasso reconhecimento da importância da vigilância do óbito materno para o planejamento de saúde pelos gestores. Conclusão: A qualificação dos profissionais, a efetiva implantação/implementação dos Comitês de Mortalidade Materna e o fortalecimento da Rede de Atenção Materno-Infantil nos municípios fronteiriços devem ser olhados com atenção pelos gestores, de forma que Políticas Públicas específicas sejam reelaboradas, com vistas ao cuidado e redução do óbito materno, haja vista a vulnerabilidade desses municípios.

ABSTRACT

O estudo discute as competências envolvidas na condução de rodas de conversas. Trata-se de um relato analítico e reflexivo desenvolvido a partir das experiências dos estudantes da disciplina de Promoção da Saúde de um programa de Mestrado Profissional em Saúde da Família, Mato Grosso do Sul, Brasil. As experiências foram registradas no formato de relatórios de experiências e enfatizaram o uso de rodas de conversas em Unidades de Saúde da Família. A partir do corpus documental, foram levantados o público- alvo, temáticas/objetivos e quatro competências aplicadas para o desenvolvimento das rodas de conversa, a saber: aprender a conhecer, aprender a fazer, aprender a conviver, e aprender a ser. Observou-se que o público-alvo e as temáticas desenvolvidas nas rodas de conversas foram relacionados a situação epidemiológica do território. Ainda, foi identificado que a técnica é frequentemente baseada no referencial teórico de Paulo Freire, com etapas sistematizadas. As competências de saber conhecer e saber fazer foram desafiantes. As competências saber conviver e ser destacaram- se positivamente nas experiências descritas. Por fim, as experiências revelaram que há competências que precisam ser aprimoradas para a condução de rodas de conversas, com vistas a alcançar o seu potencial transformador.

ABSTRACT

Petiveria alliacea L. is a plant used in traditional medicine harboring pharmacological properties with anti-inflammatory, antinociceptive, hypoglycemiant and anesthetic activities. This study assessed the potential cytotoxic, genotoxic and mutagenic effects of ethanolic extract of P. alliacea on Saccharomyces cerevisiae strains. S. cerevisiae FF18733 (wild type) and CD138 (ogg1) strains were exposed to fractioned ethanolic extracts of P. alliacea in different concentrations. Three experimental assays were performed: cellular inactivation, mutagenesis (canavanine resistance system) and loss of mitochondrial function (petites colonies). The chemical analyses revealed a rich extract with phenolic compounds such as protocatechuic acid, cinnamic and catechin epicatechin. A decreased cell viability in wild-type and ogg1 strains was demonstrated. All fractions of the extract exerted a mutagenic effect on the ogg1 strain. Only ethyl acetate and n-butanol fractions increased the rate of petites colonies in the ogg1 strain, but not in the wild-type strain. The results indicate that fractions of mid-polarity of the ethanolic extract, at the studied concentrations, can induce mutagenicity mediated by oxidative lesions in the mitochondrial and genomic genomes of the ogg1-deficient S. cerevisiae strain. These findings indicate that the lesions caused by the fractions of P. alliacea ethanolic extract can be mediated by reactive oxygen species and can reach multiple molecular targets to exert their toxicity.

ABSTRACT

This cross-sectional observational study that describes the epidemiological data of the first year of the COVID-19 pandemic in the Mato Grosso do Sul State, aimed to demonstrate the differences between indigenous and non-indigenous populations, characterize confirmed cases of COVID-19 according to risk factors related to ethnicity, comorbidities and their evolution and to verify the challenges in facing the disease in Brazil. SIVEP-Gripe and E-SUS-VE, a nationwide surveillance database in Brazil, from March 2020 to March 2021 in Mato Grosso do Sul state, were used to compare survivors and non-survivors from indigenous and non-indigenous populations and the epidemiological incidence curves of these populations. A total of 176,478, including 5,299 indigenous people, were confirmed. Among the indigenous population, 52.5% (confidence interval [CI] 51.2-53.9) were women, 38% (CI 36.7-39.4) were 20-39 years old, 56.7% were diagnosed by rapid antibody tests, 12.3% (CI 95%:11.5-13.2) had at least one comorbidity, and 5.3% (CI 95%:4.7-5.9) were hospitalized. In the non-indigenous patients, 56.8% were confirmed using RT-PCR, 4.4% (CI 95%:4.3-4.5) had at least one comorbidity, and 8.0% (CI 95%:7.9-8.2) were hospitalized. The majority of non-survivors were ≥60 years old (65.1% indigenous vs. 74.1% non-indigenous). The mortality in indigenous people was more than three times higher (11% vs. 2.9%). Indigenous people had a lower proportion of RT-PCR diagnoses; deaths were more frequent in younger patients and were less likely to be admitted to hospital. Mass vaccination may have controlled the incidence and mortality associated with COVID-19 in this population during the period of increased viral circulation.

ABSTRACT

OBJECTIVES: The aim of this study was to evaluate possible risk factors for death in traumatized cats based on the clinicoepidemiological profile, the Animal Trauma Triage (ATT) scale score and the Modified Glasgow Coma Scale (MGCS) score at the time of presentation to the hospital. METHODS: This was a cross-sectional study involving cats admitted to the emergency sector (ES) and the surgical clinic sector (SCS) of a veterinary teaching hospital (VTH) in northeastern Brazil over a 12-month period (2018-2019). Probable risk factors associated with non-survival after trauma were analyzed by calculating the prevalence ratio. The prediction of non-survival was evaluated by analyzing the receiver operating characteristic (ROC) curve employing the ATT scale score, MGCS score and rectal temperature. RESULTS: In 1 year, 89 traumatized cats were admitted to the VTH, of which 90% (n = 80) were young, 54% (n = 48) were male, 79% (n = 70) were intact, 59% (n = 53) were indoor cats and 92% (n = 82) were considered as mixed breed. Blunt trauma was the most prevalent cause of injuries, reported in 76% (n = 68) of the cats. Accidents involving falls (30%; n = 27) that affected the pelvic limbs (50.6%) were the most commonly reported. The prevalence of death among cats admitted to the ES was 9.44 times higher than that of animals treated at the SCS. Trauma as result of being run over and chest trauma were associated with a higher probability of death. Most of the animals that died had a lower rectal temperature than those that survived. The ATT scale score, MGCS score and rectal temperature were predictive of mortality. CONCLUSIONS AND RELEVANCE: Rectal temperature, ATT scale score and MGCS score were effective predictors of an unfavorable prognosis and point to the need for emergency procedures. Hypothermia, being run over and chest trauma were associated with a higher risk of mortality. Our findings highlight the importance of prompt and effective care for these conditions.

ABSTRACT

RESUMO Objetivo: analisar as percepções de acadêmicos e supervisores sobre a ação estratégica "O Brasil Conta Comigo". Método: estudo transversal realizado em Mato Grosso do Sul - Brasil, de dezembro de 2020 a março de 2021, em duas etapas: 1) Exploratória (quantitativa), via análise de relatórios dos 33 supervisores e 90 acadêmicos convocados, aplicação de questionário on-line; 2) Profundidade (qualitativa), utilizando a técnica do grupo focal remoto, com amostra por conveniência. Realizada análise descritiva e de conteúdo. Resultados: foram identificados os motivos de adesão e permanência à ação (bolsa, horas de estágio e responsabilidade social), os aspectos pedagógicos (capacitação, avaliação e apoio das instituições de ensino e gestão) e as contribuições da ação (assistência e formação profissional). Conclusão: as percepções gerais dos recrutados são positivas. Aspectos pedagógicos relacionados à avaliação dos acadêmicos e apoio das instituições de ensino superior devem ser revistos, para ações mais assertivas em momentos de crise sanitária.


ABSTRACT Objective: to analyze the perceptions of academics and supervisors about the strategic action "O Brasil Conta Comigo"(Brazil counts on me). Method: cross-sectional study conducted in Mato Grosso do Sul - Brazil, from December 2020 to March 2021, in two stages: 1) Exploratory (quantitative), via analysis of reports of the 33 supervisors and 90 academics convened, application of an online questionnaire; 2) In-depth (qualitative), using the remote focus group technique, with convenience sampling. Descriptive and content analysis was performed. Results: the reasons for joining and remaining with the action (scholarship, internship hours, and social responsibility), the pedagogical aspects (training, evaluation, and support from educational institutions, and management), and the contributions of the action (assistance and professional training) were identified. Conclusion: the general perceptions of the recruits are positive. Pedagogical aspects related to the evaluation of academics and support from higher education institutions should be reviewed, for more assertive actions in times of health crisis.


RESUMEN Objetivo: analizar las percepciones de estudiantes y supervisores sobre la acción estratégica " El Brasil Cuenta Conmigo". Método: estudio transversal realizado en Mato Grosso do Sul - Brasil, de diciembre de 2020 a marzo de 2021, en dos etapas: 1) Exploratoria (cuantitativa), a través del análisis de los informes de los 33 supervisores y 90 académicos convocados, aplicación de un cuestionario en línea; 2) En profundidad (cualitativa), mediante la técnica de grupo focal a distancia, con muestreo de conveniencia. Se realizó un análisis descriptivo y de contenido. Resultados: se identificaron los motivos de adhesión y permanencia a la acción (bolsa, horas de estancia y responsabilidad social), los aspectos pedagógicos (capacitación, aval y apoyo a las instituciones de enseñanza y gestión) y las contribuciones de la acción (asistencia y formación profesional). Conclusión: la percepción general de los contratados es positiva. Los aspectos pedagógicos relacionados con la evaluación de los académicos y el apoyo de las instituciones de enseñanza superior deben ser revisados, para que las acciones sean más firmes en momentos de crisis sanitaria.

ABSTRACT

ABSTRACT This cross-sectional observational study that describes the epidemiological data of the first year of the COVID-19 pandemic in the Mato Grosso do Sul State, aimed to demonstrate the differences between indigenous and non-indigenous populations, characterize confirmed cases of COVID-19 according to risk factors related to ethnicity, comorbidities and their evolution and to verify the challenges in facing the disease in Brazil. SIVEP-Gripe and E-SUS-VE, a nationwide surveillance database in Brazil, from March 2020 to March 2021 in Mato Grosso do Sul state, were used to compare survivors and non-survivors from indigenous and non-indigenous populations and the epidemiological incidence curves of these populations. A total of 176,478, including 5,299 indigenous people, were confirmed. Among the indigenous population, 52.5% (confidence interval [CI] 51.2-53.9) were women, 38% (CI 36.7-39.4) were 20-39 years old, 56.7% were diagnosed by rapid antibody tests, 12.3% (CI 95%:11.5-13.2) had at least one comorbidity, and 5.3% (CI 95%:4.7-5.9) were hospitalized. In the non-indigenous patients, 56.8% were confirmed using RT-PCR, 4.4% (CI 95%:4.3-4.5) had at least one comorbidity, and 8.0% (CI 95%:7.9-8.2) were hospitalized. The majority of non-survivors were ≥60 years old (65.1% indigenous vs. 74.1% non-indigenous). The mortality in indigenous people was more than three times higher (11% vs. 2.9%). Indigenous people had a lower proportion of RT-PCR diagnoses; deaths were more frequent in younger patients and were less likely to be admitted to hospital. Mass vaccination may have controlled the incidence and mortality associated with COVID-19 in this population during the period of increased viral circulation.

ABSTRACT

Chagas disease is a neglected tropical disease caused by the flagellate protozoan Trypanosoma cruzi (T. cruzi). Endemic in underdeveloped and developed countries, due to the migratory movement, it is considered a serious public health problem. One of the reasons for this is a weak therapeutic arsenal, represented only by the drug benznidazole (BNZ) which, although it promotes significant cure rates in the acute phase of the disease, presents serious problems of toxicity and bioavailability, mainly due to its low aqueous solubility. Several studies have presented several drug delivery systems (DDS) based on BNZ aiming at enhancing its solubility in aqueous medium and, with this, promoting an increase in the dissolution rate and, consequently, in its bioavailability. However, the present work is a pioneer in using a zeolitic imidazolate framework as a carrier agent for a DDS in order to promote a pH-sensitive modulation of the drug. Thus, this work aimed to develop a novel DDS based on BNZ and the ZIF-8 to use it in development of prolonged-release dosage forms to alternative treatment of Chagas disease. The BNZ@ZIF-8 system was obtained through an ex situ method selected due to its higher incorporation efficiency (38%). Different characterization techniques corroborated the obtainment and drug release data were analyzed by in vitro dissolution assay under sink and non-sink conditions and setting the kinetic results through both model dependent and independent methods. Under sink conditions, at pH 4.5, BNZ and BNZ@ZIF-8 showed similar release profile, but the DDS was effective in promoting a prolonged release. At pH 7.6, after 7 h, BNZ showed a lower release than BNZ@ZIF-8. On the other hand, in non-sink conditions at pH 4.5 the BNZ presented 80% of drug release in 3 h, while the DDS in 6 h. At pH 7.6, BNZ presented a release of 80% in 2 h, while the DDS reaches it in only at 12 h. Therefore, at pH 4.5 the DDS BNZ@ZIF-8 showed a faster release with a burst effect, while at pH 7.6 it showed a prolonged and controlled release. Finally, it is evident that a promising DDS pH-sensitive was obtained as a novel carrier that might be able to prolongs BNZ release in dosage forms intended for the alternative treatment of Chagas disease.

ABSTRACT

Several experiments with Anopheles darlingi Root, an important malaria vector in the Amazon region, were carried out in the laboratory, depending on the large-scale production of viable larvae and adults. Certainly, improvements in rearing conditions, including dietary requirements, can strongly affect mosquito production. In order to increase the production of this species in the laboratory, we first supplemented the regular larval diet (TetraMin Tropical Flakes) with different concentrations of vitamins and minerals and recorded several biological variables: survival and larval development time, emergence ratio, and adult longevity under a small-scale rearing condition. Second, we established an experimental design under regular lab-rearing conditions based on the concentration of vitamins and minerals that best contributed to the development of these anophelines, and evaluated the biological parameters already mentioned. Moreover, under regular rearing conditions, we recorded sex ratio, adult size, and longevity of adults fed with supplemented sucrose. The lowest concentration of vitamins (V5) and the average concentration of minerals (M3) increased larval survival and decreased larval development time compared with the control. Under regular rearing conditions, minerals provided higher larval survival and increased the longevity of adults fed with supplemented sucrose. Supplementing the regular larval diet and sucrose solutions with vitamins and minerals increased the production of immatures and the longevity of An. darlingi adults.

ABSTRACT

Chagas disease (CD), caused by the flagellate protozoan Trypanosoma cruzi, is one of the major public health problems in developing countries. Benznidazole (BNZ) is the only drug available for CD treatment in most countries, however, it presents high toxicity and low bioavailability. To address these problems this study used Zeolitic Imidazolate Framework-8 (ZIF-8), which has garnered considerable attention due to its potential applications, enabling the controlled delivery of drugs. The present work developed and characterized a BNZ@ZIF-8 system, and the modulation of BNZ release from the ZIF-8 framework was evaluated through the in vitro dialysis release method under sink conditions at different pH values. Moreover, the in vitro evaluation of cell viability and cytotoxicity by MTT assay were also performed. The dissolution studies corroborated that a pH sensitive Drug Delivery System capable of vectorizing the release of BNZ was developed, may leading to the improvement in the bioavailability of BNZ. The MTT assay showed that no statistically significant toxic effects occurred in the developed system, nor significant effects on cell viability.

BACKGROUND: Acquired Immunodeficiency Syndrome (AIDS) is a major public health problem in the world. One of the highly effective drugs in anti-HIV therapy is efavirenz (EFZ), which is classified as Class II according to the Classification System of Biopharmaceuticals, presenting low solubility and high permeability, this being an obstacle related to the drug. OBJECTIVE: This study aimed to obtain an innovative system based on EFZ and the Zeolitic Imidazolate Framework (ZIF-8) to use in the development of prolonged-release pharmaceutical forms that can circumvent this problem. METHODS: The EFZ: ZIF-8 system was obtained by a selected ex-situ method due to its higher incorporation efficiency. Different characterization techniques corroborated the obtainment of the system, and drug release was analyzed by dissolution testing under sink conditions, the profiles being adjusted to some kinetic models. RESULTS: At pH 1.2, the structure of ZIF-8 breaks down rapidly, releasing a large amount of drug within either 3h or short time. In the pH 4.5 and 6.8 medium, the EFZ release from the EFZ: ZIF-8 system obtained in ethanol was prolonged, releasing 95% of the drug in 24h at pH 4.5 and 75% medium at pH 6.8. CONCLUSION: It is evident that a promising pH-sensitive system was obtained using ZIF-8 as a novel carrier of EFZ intended for the alternative treatment of AIDS.

ABSTRACT

OBJECTIVE: This study investigated whether the presence of care workers who completed a specialization course on family health was associated with improved care and maternal and child health indicators in municipalities in the state of Mato Grosso do Sul, Brazil. METHODS: Negative binomial regression models with fixed effects were used for the 79 municipalities in the state of Mato Grosso do Sul, with repeated observations for the period 2009-2015. For our reference, the parameter "number of professionals who completed the course" calculated the proportion of professionals who completed the course, and was divided by the total number of primary health care professionals in the municipality to create a ratio. The cutoff points used represented tertile distribution: T3: high (0.35-1.00), T2: intermediate (0.02-0.33) and T1: low (0.00-0.01); to avoid biased results, the analysis was also performed for the years prior to the beginning of the course in question (2009 and 2010). RESULTS: During the study period, enrollment of pregnant women, exclusive breastfeeding for children under 4 months, and up-to-date vaccinations in children younger than 1 year to 23 months increased (high to intermediate categories) in municipalities where professionals who completed the specialization course worked. Growth in the intermediate ratio was also observed in indicators related to cervical cancer screening and new diagnoses of congenital syphilis in infants under one year of age. CONCLUSIONS: The presence of care workers who completed a specialization course on family health was seen to be associated with improved care and indicators for maternal and child health in municipalities in the state of Mato Grosso do Sul, Brazil. These findings reaffirm the importance and effectiveness of policies on training and continuing education for the Brazilian Unified Health System.

Este estudo objetiva caracterizar aspectos motivacionais de futuros médicos relativos às perspectivas profissionais e atuação na Estratégia Saúde da Família (ESF) e as estratégias indutoras do projeto político-pedagógico (PPP) dos cursos. Trata-se de um estudo descritivo-exploratório, de abordagem qualitativa, com 31 estudantes de Medicina de duas instituições de ensino. Os dados foram coletados a partir da análise dos PPPs e de entrevista. O material empírico foi analisado pela técnica de análise de conteúdo, à luz do referencial da Teoria da Autodeterminação. Emergiram três categorias analíticas: motivação intrínseca, motivação extrínseca e desmotivação. Os resultados demonstraram a ESF como oportunidade de trabalho temporário, com ideologia que cativa os acadêmicos, mas a ação das forças motivadoras extrínsecas resulta na não escolha da área para carreira. É fundamental explorar, nos estudantes, o potencial motivacional intrínseco, desenvolvendo estratégias para romper barreiras que limitam a escolha pela ESF.(AU)


This study aims at describing the motivational aspects of future doctors as to their professional perspectives of working in Family Health Strategy (ESF) and the inducing strategies of the course's Political and Pedagogical Project (PPP). This descriptive and exploratory study of qualitative approach was conducted with 31 Medicine students of two education institutions. Data were collected based on the PPP analysis and through interview. The empirical material was reviewed using the content analysis technique based on the Self-determination Theory. Three analytical categories emerged: intrinsic motivation, extrinsic motivation, and demotivation. The results showed ESF is a temporary job opportunity with an ideology that captivates academics. However, extrinsic motivations end up forcing students not to choose the area as a career. It is essential to explore the intrinsic motivational potential of students developing strategies to overcome barriers that limit choosing ESF as a career path.(AU)
