ABSTRACT

BACKGROUND: Visual vertigo occurs after vestibular and non-vestibular pathology and can be present in children and adolescents. It can be assessed by "the Visual Vertigo Analogue Scale" (VVAS), a questionnaire with a Portuguese version for adults. OBJECTIVES: To perform the adaptation to pediatric age and validation of VVAS in European Portuguese. METHODS: This prospective study involved the pediatric adaptation of the Portuguese VVAS, according to recognized guidelines. It was then completed by 30 healthy controls and 18 children with vestibulopathy. Patient caregivers also completed the Dizziness Handicap Inventory - Patient Caregivers (DHI-PC) to further explore the link between questionnaires. Groups were compared for severity of visual vertigo and VVAS test-retest reliability was tested. RESULTS: The VVAS score was significantly higher in vestibular group (p <  0.001). No statistically significant differences were found between VVAS initial and re-test scores (p = 0.33). VVAS severity scores showed a positive correlation with DHI-PC (r = 0.598, p = 0.009). CONCLUSION: The present Pediatric adaptation of VVAS in European Portuguese shows good psychometric properties for the assessment of visual vertigo. A positive correlation with the DHI-PC was showed, establishing the potential use of both questionnaires in the evaluation of vertigo children.

ABSTRACT

The management of anticoagulation in patients with ischemic stroke and atrial fibrillation (AF) poses a critical dilemma due to the inherent risk of hemorrhagic transformation. This article presents the case of an 89-year-old male with AF and recurrent ischemic strokes, highlighting the complex challenge of deciding whether to initiate or withhold anticoagulation. After the initial ischemic stroke event, the patient started a direct oral anticoagulant. Subsequent imaging revealed hemorrhagic transformation, leading to the cessation of anticoagulation. Despite multiple hemorrhagic recurrences, balancing thrombotic and bleeding risks remained challenging. Mechanical thrombectomy was performed for a subsequent ischemic stroke due to an absolute contraindication for thrombolysis. The patient's intricate clinical course involved a multidisciplinary approach, resulting in a decision to cautiously resume low-dose anticoagulation combined with left atrial appendage closure. This decision was made after careful consideration of persistent thrombotic risk despite recurrent hemorrhages. The case underscores the complex management dilemma of anticoagulation in elderly patients with AF and recurrent strokes, emphasizing the need for a multidisciplinary approach and individualized decision-making in such challenging scenarios. Further research and guidelines are warranted to establish optimal strategies for (re)initiating anticoagulation in patients with recurrent hemorrhagic transformation.

Hemorrhagic complications arising from anticoagulant use are a well-recognized concern in clinical practice. This case study presents an 84-year-old woman with multiple cardiovascular risk factors, including atrial fibrillation, who developed a perirenal hematoma after just five doses of enoxaparin, prescribed for stroke prevention. The patient exhibited altered mental status and abdominal pain, prompting imaging studies revealing the hematoma. This case highlights the importance of vigilance in patients at risk for bleeding complications, especially in the initial days of anticoagulant therapy. Diagnostic imaging, particularly CT scans or ultrasound, is crucial for early detection. Management strategies range from discontinuing anticoagulants to potential interventions like anticoagulation reversal, angiography, or surgery. The decision to resume anticoagulation presents a challenge and requires a personalized approach based on patient factors. This case underscores the need for continued vigilance, early diagnosis, and evidence-based decisions in managing patients on anticoagulants, emphasizing the necessity for further research to establish clear guidelines in such complex clinical scenarios.

ABSTRACT

Interstitial lung disease (ILD) occurs with Idiopathic Inflammatory Myopathy (IIM) as a life-threating complication and is considered the most important prognostic determinant in this disease group. The antibody anti-melanoma differentiation-associated gene 5 (anti-MDA5) is associated to rapidly progressive ILD and poor overall survival. Rituximab (RTX) is becoming a drug of choice in management of refractory IIM-ILDs and rapidly progressive ILDs, despite its low level of evidence. We report the case of a 49-year-old man with new-onset clinically amyopathic dermatomyositis (CADM) with severe respiratory symptoms and mixed radiologic pattern of non-specific interstitial and organizing pneumonia, refractory to high dose corticosteroids and intravenous immunoglobulin and oxygen dependent. He was started on RTX 375mg/m2/week of which he completed 4 perfusions, with significant clinical improvement, and has been on maintenance to date with the rheumatology RTX standard protocol with no need for oxygen supplementation. RTX may represent a rescue therapy for patients with severe anti-MDA5-related CADM-ILD refractory to conventional immunotherapies. We identified reports of a total of 12 patients treated with RTX. Infection was the only reported adverse event (25%). Respiratory improvement (defined by symptoms, imaging or PFTs) was observed in 75% of patients, with 2 (17%) having achieved clinical remission. A total of three deaths occurred (25%), all resulting from ILD progression despite treatment. No therapeutic protocol with RTX seems to be more efficient nor associated with more adverse events than the others. Comparative studies are necessary.

ABSTRACT

Solos de praças públicas são comumente contaminados por helmintos devido ao fácil acesso de cães e gatos infectados. Esses animais ao defecarem podem liberar ovos desses parasitos e, em condições ambientais favoráveis, tornam-se ovos embrionados ou larvas infectantes. O objetivo deste trabalho foi investigar a existência de larvas de helmintos no solo de duas praças públicas do município de Caxias, Maranhão, Brasil, durante a estação chuvosa e seca na região. A pesquisa foi realizada em março de 2018, considerado período chuvoso, e em outubro do mesmo ano, período seco, sendo que foram coletadas trinta amostras de areia, quinze de cada praça, nos dois períodos do ano. O material foi coletado e levado para o Laboratório de Parasitologia do Departamento de Parasitologia e Microbiologia da Universidade Federal do Piauí para análise. Na estação chuvosa, das quinze amostras analisadas na praça A, cinco foram positivas para larvas de ancilostomídeos e das quinze na praça B, três estavam contaminadas com os mesmos helmintos. No período seco, na praça A havia apenas uma amostra com essas larvas e na praça B não foram encontrados parasitos. Os resultados revelaram a presença de larvas de helmintos de caráter zoonótico no solo de praças públicas de Caxias, Maranhão, principalmente no período chuvoso, servindo de alerta à população local.(AU)


Soil in public squares is commonly contaminated by helminths due to the easy access of infected dogs and cats. These animals, when defecating, can release helminth eggs and, under favorable environmental conditions, those eggs can become embryonated or infective larvae. The purpose of this work was to investigate the existence of helminth larvae in the soil of two public squares in the city of Caxias, in the state of Maranhão, Brazil, during the rainy and dry seasons in the region. The study was carried out in March 2018, which is considered the rainy season, and in October of the same year, the dry season. A total of thirty sand samples were collected, fifteen from each square, in both periods of the year. The material was collected and taken to the Parasitology Laboratory of the Department of Parasitology and Microbiology of the Federal University of Piauí for analysis. In the rainy season, from the fifteen samples analyzed in square A, five were positive for hookworm larvae; and from the fifteen samples collected from square B, three were contaminated with the same helminths. During the dry period, only one sample from square A presented these larvae while no parasites were found in square B. The results revealed the presence of zoonotic helminth larvae in the soil of public squares in Caxias, Maranhão, mainly in the rainy season, which can be used as a warning sign to the local population.(AU)


Los suelos de las plazas públicas son comúnmente contaminados por helmintos debido al fácil acceso de perros y gatos infectados. Esos animales, al defecar, pueden liberar huevos de esos parásitos y, en condiciones ambientales favorables, convertirse en huevos embrionados o larvas infectantes. El objetivo de este trabajo fue investigar la existencia de larvas de helmintos en el suelo de dos plazas públicas de la ciudad de Caxias, Maranhão, Brasil, durante la estación lluviosa y seca de la región. La investigación se realizó en marzo de 2018, considerada época de lluvias, y en octubre del mismo año, época seca, y se recolectaron treinta muestras de arena, quince de cada plaza, en ambos períodos del año. El material fue recolectado y llevado al Laboratorio de Parasitología del Departamento de Parasitología y Microbiología de la Universidad Federal de Piauí para su análisis. En época de lluvias, de las quince muestras analizadas en la plaza A, cinco resultaron positivas a larvas de anquilostomiasis y de las quince de la plaza B, tres estaban contaminadas con los mismos helmintos. En el período poco lluvioso, en la plaza A solo hubo una muestra con esas larvas y en la plaza B no se encontraron parásitos. Los resultados revelaron la presencia de larvas de helmintos zoonóticos en el suelo de las plazas públicas de Caxias, Maranhão, principalmente en la época de lluvias, sirviendo de alerta a la población local.(AU)

Abstract Objectives: assess the implementation of child mortality surveillance in Recife/PE. Methods: an analytical evaluative study was conducted on its implementation. It was a single-case study that correlated degree of implementation with the of the result indicators surveillance. A logic model on this strategy and a matrix of indicators and judgments according to model components were drawn up. The degree of implementation was obtained from structure and process indicators and this was then correlated with result indicators, in a deductive approach based on intervention theory. Results: the structure approach presented superior results to the process in all evaluated components. This strategy was considered to have been partially implemented (75. 7%), however, the components of 'identification of deaths' (85.7%), 'epidemiological research' (88.1%) and 'referral of proposals for promotion and health care and correction of official statistics' (95.8%) were classified implemented. Regarding the relation of the degree of implantation of the surveillance and its results with the logical model, only one of the 17 indicators was considered inconsistent. Conclusions: this strategy was considered to have been partially implemented. The model of child mortality surveillance and its assessment were shown to be adequate for signaling the consistency of the interrelations between the activities proposed and the effects expected, and would be reproducible within other scenarios.


Resumo Objetivos: avaliar a implantação da vigilância do óbito infantil no Recife/PE. Métodos: realizou-se pesquisa avaliativa do tipo análise de implantação da vigilância do óbito infantil. Estudo de caso único que relacionou o grau de implantação aos indicadores de resultados da estratégia. Foi elaborado um modelo lógico da intervenção e uma matriz de indicadores e julgamento por componente do modelo. O grau de implantação foi obtido a partir de indicadores de estrutura e processo e, posteriormente, relacionado aos de resultado, em uma abordagem dedutiva baseada na teoria da intervenção. Resultados: a abordagem estrutura apresentou resultados superiores ao processo em todos os componentes. A vigilância do óbito infantil foi considerada parcialmente implantada (75,7%), entretanto, os componentes 'identificação dos óbitos' (85,7%), 'investigação epidemiológica' (88,1%) e 'encaminhamento das propostas de promoção e atenção à saúde e correção das estatísticas oficiais' (95,8%) foram julgados implantados. Quanto à relação do grau de implantação da intervenção e seus resultados com o modelo lógico, apenas um dos 17 indicadores foi considerado inconsistente. Conclusões: a vigilância do óbito infantil foi classificada como parcialmente implantada. O modelo desta estratégia e a sua avaliação mostraram-se adequados ao assinalar a consistência das inter-relações entre as atividades propostas e os efeitos esperados, passíveis de reprodutibilidade em outros cenários.

Genome scans of population differentiation identify candidate loci for adaptation but provide little information on how selection has influenced the genetic structure of these loci. Following a genome scan, we investigated the nature of the selection responsible for the outlying differentiation observed between populations of the marine mussel Mytilus edulis at a leucine/arginine polymorphism (L31R) in the antimicrobial peptide MGD2. We analysed DNA sequence polymorphisms, allele frequencies and population differentiation of polymorphisms closely linked to L31R, and pairwise and third-order linkage disequilibria. An outlying level of population differentiation was observed at L31R only, while no departure from panmixia was observed at linked loci surrounding L31R, as in most of the genome. Selection therefore seems to affect L31R directly. Three hypotheses can explain the lack of differentiation in the chromosomal region close to L31R: (i) hitchhiking has occurred but migration and recombination subsequently erased the signal, (ii) selection was weak enough and recombination strong enough to limit the hitchhiking effect to a very small chromosomal region or (iii) selection acted on a pre-existing polymorphism (i.e. standing variation) at linkage equilibrium with its background. Linkage equilibrium was observed between L31R and linked polymorphisms in every population analysed, as expected under the three hypotheses. However, linkage disequilibrium was observed in some populations between pairs of loci located upstream and downstream to L31R, generating a complex pattern of third-order linkage disequilibria which is best explained by the hypothesis of selection on a pre-existing polymorphism. We hypothesise that selection could be either balanced, maintaining alleles at different frequencies depending on the pathogen community encountered locally by mussels, or intermittent, resulting in sporadic fluctuations in allele frequency.

ABSTRACT

This paper describes the study that led to the development of a carbon dioxide emissions matrix for the Oeiras municipality, one of the largest Portuguese municipalities, located in the metropolitan area of Lisbon. This matrix takes into account the greenhouse gas (GHG) emissions due to an increase of electricity demand in buildings as well as solid and liquid wastes treatment from the domestic and services sectors. Using emission factors that were calculated from the relationship between the electricity produced and amount of treated wastes, the GHC emissions in the Oeiras municipality were estimated for a time series of 6 yr (1998-2003). The obtained results showed that the electricity sector accounts for approximately 75% of the municipal emissions in 2003. This study was developed to obtain tools to base options and actions to be undertaken by local authorities such as energy planning and also public information.

O Hospital das Clínicas da Universidade Federal de Minas Gerais-HC-UFMG é utilizado como campo de estágio para cursos de graduação e de nível médio da enfermagem. Neste contexto, o Hospital e a Escola de Enfermagem-UFMG, visando à integração ensino-serviço, instituíram a Comissão de Estágios em Enfermagem para coordenar os estágios curriculares e extracurriculares nesta área; sistematizar as visitas técnicas e o trabalho voluntário e implantar o Programa de Educação Continuada. O objetivo deste artigo é relatar a experiência de implantação desta Comissão, evidenciando a sua trajetória e os resultados alcançados. O trabalho da Comissão tem contribuído para o desenvolvimento e crescimento profissional dos estagiários e dos trabalhadores da enfermagem


The University Hospital of the Federal University of Minas Gerais is used as a training field for nursing undergraduate students. Therefore, the Hospital and the Nursing School, with a view to integrating teaching and service, created a Nurse Training Commission in order to coordinate curricular and extracurricular training in this area, systematically organizing the technical visits and the voluntary work and to implement a Continuing Education Program. The purpose of this article is to report the experience of implementing this Commission, showing its actions and the results achieved. The work of the Commission is contributing to the students' development and professional improvement as well as to the members of the nursing team


El Hospital de Clínicas de la Universidad Federal de Minas Gerais se emplea como campo de entrenamiento para cursos de graduación y nivel intermedio del curso de Enfermería. En este contexto el Hospital y la Escuela de Enfermería de la Universidad Federal de Minas Gerais designaron una Comisión de Capacitación de Enfermeros con miras a integrar enseñanza - trabajo. Las tareas asignadas a dicha comisión fueron: coordinar los entrenamientos curriculares y extracurriculares en enfermería, sistematizar las visitas técnicas y el voluntariado e implantar el Programa de Educación Continuada. El objetivo de este artículo es relatar la experiencia de implantación de tal comisión, evidenciando su trayectoria y los resultados alcanzados. El trabajo de la comisión ha contribuido al desarrollo y crecimiento profesional de los alumnos y de los trabajadores en enfermería
