ABSTRACT

Acute disseminated encephalomyelitis (AEM) and acute transverse myelitis (OPM) are autoimmune demyelinating diseases of the central nervous system. Two clinical observations of AEM and OPM developed after suffering acute coronavirus infection (SARS-CoV-2) are presented. Differential diagnosis was carried out with multiple sclerosis, encephalitis of an infectious nature, compressive myelopathy, and opticomyelitis. Both observations show an almost complete recovery of lost functions. The pathogenetic mechanisms of the development of AEM and OPM in patients with coronavirus infection are discussed. The onset of central nervous system dysimmune lesion in the context of coronavirus infection makes it necessary to monitor the clinical situation with the involvement of a neurologist for timely diagnosis and determination of therapeutic tactics that can reduce the degree of disability of patients.

ABSTRACT

An evaluation was done of the state of bulbar microcirculation and aggregability of blood platelets in 47 patients presenting with early manifestations of cerebral circulatory insufficiency. A correlation was established between the revealed disturbances in the microcirculatory bed and augmentation of aggregational activity of thrombocytes. Computation of the integrative guide, microcirculatory-rheologic index, reflecting objectively the degree of intravascular hemocirculatory disorders has been performed. Treatment with calcium antagonist corinfar, antihypoxic agent actovegin was of particular benefit for the patients when administered in combination; more than half of the parameters characterizing microcirculation, rheologic properties of blood, microcirculatory-rheologic index have all returned to normal.

ABSTRACT

After equiponderant radionuclide ventriculography the parametric images (phasic and amplitude) of heart function were estimated in 20 patients with dilated cardiomyopathy and in 17 patients with coronary heart disease associated with cardiomegaly. The patients with dilated cardiomyopathy and those with coronary heart disease showed principal differences in local disturbances of the amplitude of movements of left ventricle myocardium walls. While assessing synchronism of myocardial contraction in the patients with dilated cardiomyopathy the presence of the zones of myocardial asynchronism appeared to be a suggestive sign. At the same time the phenomena of asynchronism were not directly connected with impairment of heart conduction and became aggravated as dilatation of the left ventricle progressed, being often coupled with the ECG signs of cicatrical alterations in the myocardium, which came on the phasic images in the form of dyssynergia areas.

ABSTRACT

Findings of echocardiography and bicycle exercise testing were considered objective hemodynamic criteria during long-term follow-up studies of patients with mitral stenosis and early stages of cardiac insufficiency. In patients with the first degree cardiac insufficiency the most sensitive parameters were: exercise tolerance, physical adaptation index and left atrial size. In patients with the second A degree cardiac insufficiency additional information has also been obtained upon studying the ejection fraction and the rate of left ventricular circular shortening.

ABSTRACT

Thirty-four patients with rheumatic heart disease and 22 with atherosclerotic and post-infarction cardiosclerosis, signs of stage I-IIB circulatory insufficiency, and disorders of cardiac rhythm (auricular fibrillation) were kept under out-patient care for periods of 6 to 42 months. They all received cardiac glycosides per os in a maintenance dose determined in the clinic after intravenous digitalization. Clinical and echocardiographic examination of the patients was conducted every 4 to 6 months. Depending on the effect of long-term digitalization, all patients were divided into clinical groups in which the causes of the disturbed compensation were determined. The importance of regular doctor's control over proper intake of the maintenance dose of glycosides and its correction depending on each concrete clinical situation is stressed.
