ABSTRACT

A patient complained of photopsia and vision loss in the left eye for two days, with visual acuity of 20/32. Right eye was normal. Funduscopy revealed foveal granularity and gray-white lesions in the posterior pole, mainly temporal to the fovea. The lesions (dots and spots), along with a few other areas surrounding them, showed hyperautofluorescence on autofluorescence imaging. Fluorescein angiogram (FA) depicted some early hyperfluorescent dots with late staining. Indocyanine green angiogram (ICGA) showed hypofluorescent lesions in a greater number compared with funduscopy, autofluorescence, and FA. Thirty days later, BCVA was 20/20 in both eyes and the complimentary exams were almost normal, despite an ICGA that showed few small hypofluorescent lesions. This case supports the hypothesis that the choroidal involvement occurs primarily in MEWDS, with secondary involvement of the RPE and the neurosensory retina.

ABSTRACT

PURPOSE: To compare total protein concentration in the aqueous humor of primary open-angle glaucoma and non-glaucomatous patients. METHODS: Aqueous humor samples were obtained from 22 patients just before trabeculectomy for clinically uncontrolled primary open angle glaucoma (POAG group). Aqueous humor (0.1 mL) was aspirated by inserting a 26-gauge needle into the anterior chamber. The same procedure was performed in 22 non-glaucomatous patients just before cataract surgery (control group). Immediately after collection, the aqueous humor was stored at -20 degrees C. Aqueous humor total protein concentration was determined using a colorimetric assay. RESULTS: The geometric mean of total protein concentration of the aqueous humor samples was 32 mg/dL (range: 8-137 mg/dL) in the primary open angle glaucoma group and 16 mg/dL (range: 2-85 mg/dL) in the control group. The ratio of the protein concentration between the two groups was 2.0 (95% confidence interval: 1.3 to 3.2; p=0.003). CONCLUSIONS: The total protein concentration in primary open-angle glaucoma aqueous humor was approximately two times higher than that in non-glaucomatous subjects.

ABSTRACT

OBJETIVOS: Comparar a concentração total de proteínas no humor aquoso entre pacientes com glaucoma primário de ângulo aberto e sem glaucoma. MÉTODOS: Foram coletadas amostras de humor aquoso de 22 pacientes com glaucoma primário de ângulo aberto (grupo GPAA) no momento da trabeculectomia. Na coleta, 0,1 mL de humor aquoso foi aspirado da câmara anterior através de uma agulha de calibre 26, no início do procedimento cirúrgico. Coleta semelhante foi realizada em 22 pacientes sem glaucoma no início da cirurgia de catarata (grupo controle). A amostra de humor aquoso foi armazenada a -20°C após a coleta. A concentração total de proteínas no humor aquoso foi determinada por meio de um teste colorimétrico. RESULTADOS: A média geométrica da concentração total de proteínas no humor aquoso foi de 32 mg/dL (amplitude: 8-137 mg/dL) no grupo glaucoma primário de ângulo aberto e de 16 mg/dL (amplitude: 2-85 mg/dL) no grupo controle. A razão da concentração total de proteínas no humor aquoso entre estes dois grupos foi de 2,0 (intervalo de confiança de 95 por cento: 1,3 a 3,2; p=0,003). CONCLUSÕES: A concentração total de proteínas no humor aquoso de pacientes com glaucoma primário de ângulo aberto foi aproximadamente duas vezes maior quando comparada aos pacientes sem glaucoma.

ABSTRACT

We compared the concentration of hyaluronic acid in the aqueous humor of primary open-angle glaucoma (POAG) patients and non-glaucomatous patients. Aqueous humor samples were obtained from 22 patients just before trabeculectomy for clinically uncontrolled POAG (POAG group). Aqueous humor (0.1 mL) was aspirated by inserting a 26-gauge needle into the anterior chamber. The same procedure was performed for 22 non-glaucomatous patients just before cataract surgery (control group). Immediately after collection, the aqueous humor was stored at -20 degrees C. The concentration of hyaluronic acid was determined by a sensitive, noncompetitive and nonisotopic fluoroassay. The median (range) concentrations of hyaluronic acid of the POAG and control groups were 298.4 microg L(-1) (99.0-743.7 microg L(-1)) and 545.1 microg L(-1) (145.0-2366.0 microg L(-1)), respectively. The difference in concentrations of hyaluronic acid between the groups was statistically significant (P<0.001). In conclusion, the concentration of hyaluronic acid in the aqueous humor in POAG patients is lower than in non-glaucomatous patients. Further studies are necessary to determine the role of hyaluronic acid in the pathophysiology of POAG.

ABSTRACT

Objetivo: Avaliar as alterações da pressão intra-ocular (Po) em pacientes submetidos a tratamento de miopia e astigamtismo miópico por maio da técnica de Laser in Situ Keratomileusis (LASIK) e correlacioná-las a alteração de ceratometria e espessura máxima de ablação. Pacientes e Métodos: estudo clínico prospectivo envolvendo 50 olhos (26 pacientes) submetidos à correção da miopia e astigmatismo miópico pelo método de LASIK utilizando o Laser VISX Star S3. A medida da Po, por meio da tonometria de aplanação de Goldmann, e a ceratometria automatizada foram realizadas no pré-operatório e 30 dias após a cirurgia. A estimativa da espessura máxima de abação fornecida pelo laser e as alterações ceratométricas foram correlacionadas a alteração da Po. Resultados: Observou-se uma redução média de 1,7 +/- 1,5mmHg (p<0,001)da Po, e uma redução da ceratometria média de 4,02 +/- 1,94 dioptrias (p<0,001) entre as medidas pré e pós-operatórias. A média da espessura máxima de ablação foi de 65,2 +/- 33,6 µm. Não houve correlação estatisticamente significante entre a espessura máxima de ablação e alteração da Po (r+0,040; p=0,782), bem como entre a alteração da Po e a redução ceratométrica (r=0,109; p=0,504). Conclusão: Houve uma redução dos valores da Po após a cirurgia de LASIK quando comparados a valores pré-operatórios, porém esta alteração da Po não foi correlacionada de forma estatisticamente significante à alteração da ceratometria e espessura máxima de ablação.
