ABSTRACT

Vasodilators (verapamil, nifedipine, and prazosin) versus hypothiazide and clofelin were studied for their effects on resting and exercise blood levels of catecholamines in 58 patients with moderate arterial hypertension. Clofelin decreased norepinephrine levels, whereas hypothiazide increased it at rest and during exercise test. The most marked effect on norepinephrine levels was produced by nifedipine, then prazosin, particularly in patients with initially enhanced total peripheral vascular resistance. Verapamil failed to substantially affect norepinephrine levels. Due to the fact that the elevated levels of norepinephrine may have a negative action, it is suggested that antiadrenergic agents should be supplemented to a long-term therapy with nifedipine or prazosin.

ABSTRACT

There have been studied the state of blood circulation in proximal parts of femur of 24 dogs (48 hip joints with simulated aseptic necrosis). In the first group of joints there have been carried out revascularization of the head by caudal epigastric artery with commitant veins. The second group have been subjected to tunneling. The third group of joints served as a control. Microcirculation disturbances, ascertained by intraosseous contrast phlebography arteriography and microscopic examination of osseous tissue, indicated that aseptic necrosis of the head of femur in an experiment is accompanied by increased intraosseous pressure, revascularization of an avascular spongy bone causes stable normalization of intraosseous hemodynamics.

ABSTRACT

The authors reviewed clinical findings from 57 adult patients with aseptic necrosis of the head of the femur. A statistical processing of the findings showed a significant direct correlation between the stages of the process and the degree of femoral muscular atrophy, degree of internal rotation and abduction limitations, thermographic activity, and intraosteal blood pressure. A pathohistological examination of osseous tissue specimens from the head of the femur revealed that at an early pre-X-ray stage, pathological changes involved only the microcirculatory bed of the bone marrow; in this consequence, extra-articular implantation of the inferior epigastric artery with concomitant veins into the neck and head of the femur is likely to be justifiable at this stage.

In the treatment of 38 patients with aseptic necrosis of the femur head, wrist bones and trophic ulcers the authors have successfully used the vascularization of tissues for the improvement of their feeding by means of transplantation of isolated vascular bundles.
