ABSTRACT

Background: Most post-COVID-19 patients had physiological health problems, which differed for each patient. The potential treatment for post-COVID-19 syndrome must require multi-disciplinary approaches that focus on individualized treatment. Therefore, traditional Thai Medicine (TTM) clinical practice guidelines for post-COVID-19 treatment are developed by the Traditional Thai Medicine Hospital, Prince of Songkla University, to care for post-COVID-19 patients. Objective: This current study aimed to investigate the effect of the guidelines on post-COVID-19 patient's physical health. Methods: This research was a retrospective clinical study, carried out from June 2022 to July 2022. Setting: The study was conducted at the Traditional Thai Medicine Hospital, Prince of Songkla University, and La Flora Khao Lak TTM clinic, Thailand. Participants: Data (n=17) were collected from the medical record. They were included if they met the following criteria: patients were more than 18 years old, had a medical history of COVID-19 disease, had fully recovered from the disease (ATK for COVID-19 is negative) at least a week before starting the guideline, and had complete medical information such as post-COVID-19 symptoms, vital signs, pain score, cough severity score, and quality of life examination. Intervention: Approaches for post-COVID-19 treatment included herbal Thai medicine recipes, Thai massage, herbal steam inhalation, herbal hot steam, herbal hot water immersion, acupressure massage for breathing stimulation, and a breathing exercise. All procedures were applied for 7 days continually or until they have recovered. Primary Outcome Measures: The effects of TTM approaches on the symptoms of post-COVID-19 patients on pain score, quality of life, cough severity were measured using a visual analog score (VAS), SF-36 questionnaire, and cough severity index (CSI), respectively. Results: The average duration time of post-COVID-19 syndrome was 25.76 days, and the most common symptom was muscle pain/stress (10; 71.43%). Yahom-Navakot (6; 42.86%) is the most common recipe used for the treatment. Interestingly, the pain score was significantly reduced by 3 days of the treatment (4.88±2.03 vs 2.29±2.08 ). Moreover, the cough severity index (12.86±11.55 vs 3.31±6.38) was significantly lessened by 7 days of the treatment. Remarkably, most symptoms vanished entirely after 7 days of the intervention, leading to a better quality of life for the patients (53.24±22.15 vs 65.59±23.64). Conclusions: The TTM clinical practice guideline for treating post-COVID-19 syndrome effectively improved the overall physical health capacity and symptoms associated with post-COVID-19 syndrome. Therefore, the implementation of this guideline, together with other approaches, could strengthen the treatment for post-COVID-19 to be more effective.

ABSTRACT

ETHNOPHARMACOLOGICAL RELEVANCE: Hypertension is the most dominant risk factor for the development of cardiovascular, kidney, and eye diseases. In Thailand, illness and hospitalisation in the modern public health system due to high blood pressure is increasing. However, some Thai people have turned their attention to the use of herbal medicines for healthcare. Therefore, this study aimed (1) to study the folk knowledge of hypertension treatment and (2) to study plant utilisation in the treatment of high blood pressure by Songkhla folk healers. MATERIALS AND METHODS: Field surveys and semi-structured interviews about theories of disease, principles of healing, and herbal usage (plant species, parts used, preparation, and application methods) were gathered. The data were analysed by descriptive statistics. The literatures regarding medicinal plants used in any traditional medicine, antihypertension activity, and toxicity was reviewed. RESULTS: Most healers believed that hypertension was caused by the disorder of fire and wind elements in the body. The medicinal plants containing hot and mild tastes, which had the potential for treating problems in the wind element, were applied. A total of 62 species were used for hypertension treatment. Most plants were in the Asteraceae, Piperaceae, Rutaceae, or Zingiberaceae family (4 species each). Herbal medicines were preferred to be prepared by boiling (78%) and consumed by drinking 1 teacup before 3 meals each day (26%). Piper retrofractum and Cleome viscosa had the greatest Frequency of Citation (FC = 6, n = 14). Thirty-seven species have been reported for use in traditional medicine. Twenty-four and 46 species have already been investigated for antihypertension activity and toxicity, respectively. CONCLUSION: Identifying medicinal plants that have been tested by experienced folk doctors would provide an opportunity for people to choose and consume local herbs that are easy to access in their local area. However, the remaining plants that have not been studied for antihypertension activity and toxicity should be further analysed to confirm their efficacy.

However, their use as herbs for treating diseases, including gastrointestinal disorders, is rare. Therefore, the present study aimed to: (1) quantify the number of herbal weeds used for treating gastrointestinal disorders; (2) study local knowledge of weed utilization for treating gastrointestinal disorders in Songkhla and Krabi provinces; and (3) analyse quantitative data with the Informant Consensus Factor (ICF), Use Value (UV) and Fidelity Level (FL). MATERIALS AND METHODS: The study was conducted from November 2014 to January 2016 through semi-structured interviews with 35 folk healers. The main questions were designed to obtain plant information, including the local name, method of use, preparation method and medicinal properties. The data were analysed by descriptive statistics, quantitative indexes (UV, ICF as well as FL) and interpretation. RESULTS: A total of 49 species in 46 genera and 28 families were found. The most common use of weeds was as herbs (80%). The preferred part used was the whole plant (76.27%). The preferred methods of drug preparation and use were decoction and drink, respectively. The highest UV was found for Acmella oleracea (0.83). The highest FLs (100%) were found for 12 species, including Amaranthus spinosus, Amaranthus viridis, Alternanthera sessilis, Sauropus androgynus, Plantago major, and others. The highest ICF (0.93) was found for treating toothache. CONCLUSION: Overall, there are reports on the pharmacological activity of 31 species of weeds and reports on toxicity for 20 species of weeds. Therefore, awareness of the use of herbs is necessary to ensure that they are used safely and that benefits arise from the therapy. This study showed that medicinal weeds are still popularly used by folk healers. The pharmacological properties were consistent with the local uses, which supported a preliminary indication that the weed plants were effective for treating gastrointestinal diseases.

ETHNOPHARMACOLOGICAL RELEVANCE: Skin diseases are common health problems which affecting to all ages. In Thailand, the number of patients diagnosed with skin diseases is increasing every year. Nowadays, The Ministry of Public Health is supporting and promoting herbs for treating various disorders, including disorders of the skin to reduce the problem of antibiotic resistance and adverse drug reactions. This study aimed to: (1) enumerate the herbal weeds for treating skin disorders; (2) study local knowledge of weed utilization for treating skin disorders according to the folk healers in Songkhla and Krabi province; and (3) study quantitative data by Informant consensus factor (ICF), Use value (UV) and Fidelity level (FL) value. MATERIALS AND METHODS: Field surveys and Semi-structured interviews about the local names, parts of plants used, preparation and use method, as well as local properties were done. The data were further analyzed by descriptive statistics, interpretation and quantitative indexes (ICF, UV as well as FL). RESULTS: The results discovered 44 herbal species of weeds belonging to 41 genera in 25 families. The most used plant families were Amaranthaceae (6 species). Most plants were used to treat abscess (18 species; 40.91%). The highest UV was recorded for Commelina benghalensis (0.65). The highest ICF values were found in vitiligo, ringworm, tinea versicolor and burns (1.00 each). The highest FL values were recorded for Cleome gynandra, Cleome viscosa, Sphenoclea zeylanica, Acmella oleracea, Leersia hexandra, Cyperus involucratus, Phyllanthus urinaria and Iresine herbstii (100.00 each). A review of the literatures revealed that 34 plant species had already been tested for their pharmacological activities. The biological activities associated with treatment of skin diseases can be divided into four categories: antimicrobial, anti-inflammatory, wound healing and antioxidant activity. CONCLUSION: The information indicates that herbal weedy utilization is still importance to the treatment of traditional healers through accumulated experience for a long time. Therefore, this study is a guide to the conservation of folk medicinal knowledge. It might be implied as the basis for drug development and application of herbal weeds to treat skin disorders along with promoting sustainable use of natural resource.

ETHNOPHARMACOLOGICAL RELEVANCE: In Thailand, gastrointestinal diseases have occurred since the past, consequently healers know well how to use herbs for treatment. Currently, some groups of Thai people continue to be interested in treating diseases by herbs. Therefore, the aims of this study were to (1) compile a list of species of medicinal plants and their source, (2) document the diseases treated and their frequency of citation, (3) record the parts used, methods of preparation and route of administration of traditional medicines, and (4) compare the folk knowledge to that in other relevant reports. MATERIALS AND METHODS: Ten folk healers were included in the study. Semi-structured interviews were used to compile data, namely plant parts used, preparation, route of administration and properties. The data were analyzed by interpretation and descriptive statistics. RESULTS: Sixty-two species in 28 families were identified. The most used plants (11 species) were grouped in Zingiberaceae. The highest level of relative frequency of citation (RFC) was found in Zingiber officinale Roscoe (1.0). Most plants were applied for treating constipation. Powder was the most common preparation form of folk drug. A total of 174 use lists were recorded, however, more than two thirds were reported as new uses in this study. CONCLUSION: A literature review revealed that the remaining 37 species for pharmacological activities and 42 species for toxicity should be further studied to validate the folk use of these medicinal resource.

ETHNOPHARMACOLOGICAL RELEVANCE: Diabetes mellitus is a chronic disease which cannot be cured and must be suffered for whole life. Nowadays, many patients wanting to treat themselves by herbal medicine are increasing. Therefore, surveying of medicinal plants for treating diabetes and disseminating the knowledge, might give people more choice to consume herbs. The purposes of this qualitative study were to compile the medicinal plants and their folk use to treat diabetes by local healers in Chana and Nathawee district, Songkhla province. MATERIALS AND METHODS: Semi-structured interviews were used to gather about the local names, parts of plants used, preparation and use method, and local properties. Descriptive statistics, use value and interpretation were used to analyze the data. RESULTS: A total of 38 species belonging to 37 genera in 28 families were used as herbs for treating diabetes. The family mostly used as herbal medicine was Rubiaceae (4 species; 10.53%). The part of herbal medicine mostly used was the root (8 speciess; 21.05%). Decoction and drinking were the most favorite preparation form and use method. Interestingly, the most used herbal plant is Lagerstraemia speciose Pers (0.83 of Use Value). CONCLUSION: It was found that the local healers possess the different knowledge based on their experience received secretly from the family and adapted by themselves. Thus, this study is important due to assistance for collecting the variety of knowledge before disappearing.

ETHNOPHARMACOLOGICAL RELEVANCE: Gastrointestinal tract diseases commonly occur in Thailand. However, surveying for finding out traditional drugs has never been done. AIM OF STUDY: To quantify and categorize the folk medicinal remedies that are used for healing the gastrointestinal tract by the traditional healers living in Thailand's three southern border provinces. MATERIALS AND METHODS: The Pattani, Yala and Narathiwat provinces were selected. Semi-structured interviews of nine healers were conducted to collect information that included the remedy names, herbal ingredients, plant parts used, preparation, properties and treatment methods. The data were then further analyzed. RESULT: The results revealed that 39 multi-species remedies and 36 single-species remedies were used to treat gastrointestinal disorders. A total of 103 plant species and 5 other materia medica were used as therapeutics. Most of the plants used were of the Zingiberaceae, Fabaceae and Euphorbiaceae families. Furthermore, it was found that although most of the healers used different remedies for a particular disease, some of the ingredients might have been similar. For example, Caesalpinia bonduc (L.) Roxb. was an ingredient used for parasitic disease remedies, and Senna alata (L.) Roxb. was used for constipation remedies. CONCLUSION: A review of the literature revealed 57 plant species and 2 other materia medica that have already been tested for their biological activities, whereas 46 plant species and 3 materia medica have never been tested. Consequently, research should be performed to confirm the pharmacological properties of folk remedies.
