ABSTRACT
Upon the analysis of efferent combined therapy for diffuse purulent peritonitis in 130 patients, it was found that efferent detoxication promotes elimination of intoxication, correction of homeostasis abnormalities, recovery of basic systems of the body, a decrease of postoperative lethality and complications. The choice of the optimal detoxication method should be individual and adjusted for a peritonitis stage and a concrete clinical purpose.

Efferent therapy (hemosplenosorption, plasmapheresis, autologous blood radiation) was used in multimodality treatment of 45 patients with sepsis. The therapy exerted a beneficial effect on the disease, favoured the elimination of or minimized intoxication, normalized hemostasis and immunity. Indications for use of hemosplenosorption, plasmapheresis and autologous blood radiation are provided.

Results of treatment of 295 patients are presented. Among them there were 128 patients with diffuse purulent peritonitis, 109 with acute pancreatitis and 58 with destructive cholecystitis. In the complex treatment extracorporeal methods of detoxication therapy were included: plasmapheresis, hemo- or lymphosorption, perfusion ofautologous blood through the xenogenous spleen. Indications to each of the methods are considered. It was found that when using the therapy in question clinical symptoms of endotoxicosis were eliminated 1.5 times quicker and the leukocytic intoxication index was corrected more successfully as well as was the level of toxins of middle molecular mass, functional activity of blood neutrophils, immune link of homeostasis and ++cardio-hemodynamic parameters.

The authors discuss the results of studying the clinical picture, immunological values, and the leukocytic intoxication index in 25 patients with ++pyo-destructive and septic processes in the complex management of which plasmapheresis was included. With plasmapheresis intoxication was relieved 1.5-2 times more rapidly in most patients, the leukocyte intoxication index reduced, and the immune shifts were modulated.

Under analysis are data of 36 patients with perforating gastroduodenal ulcers. Five of them were children aged from 12 to 15 and 31 young patients (16-20 years of age). Specific clinical manifestations of the disease in children and young patients are considered. Special attention is given to problems of diagnosing the disease in children and tactics of treatment.

The work describes features of the clinical course and diagnostics in 19 patients with perforated ulcers of the cardial part of the stomach. Under separate consideration were patients with perforation into the free abdominal cavity (13 patients) and with an atypical clinical picture (6 patients). Problems with the diagnostics of perforation of gastric cardial ulcers are stressed. Indirect symptoms of the disease are described.

On the basis of an analysis of results of treatment of 26 patients with acute cholecystitis complicated by mechanical jaundice it was shown that plasmapheresis associated with generally accepted therapy was an effective method of detoxicating treatment. With the help of plasmapheresis these patients were prepared for operation in shorter terms and the operation was performed under more favorable conditions.

An analysis of data of 228 patients with different pathomorphological forms of acute purulent lactation mastitis has shown that clinical manifestations of purulent mastitis are due to the pathomorphological form of the inflammation. Complex treatment of acute purulent lactation mastitis including the timely radical operations followed by antibacterial and detoxicating therapy and immunotherapy ensures recovery of the patients and maximal preservation of milk producing tissues. Selection of the magnitude of operation for purulent mastitis is made with regard for their form.

The authors analysed the results of therapy with plasmapheresis of 46 patients with diffuse toxic goiter showing intolerance to antithyroid therapy for the purpose of preoperative preparation. A therapeutic effect was achieved in 45 patients. Plasmapheresis resulted in the reduction of a period of preoperative preparation, the concentration of T4 and JgG, IgA and IgM decreased and the TTH level increased.

Based on an analysis of 49 patients with acute limited gangrene of the lung the author considers the main method of treatment of this disease to be a complex conservative treatment including the antibiotic therapy, sanitation of the gangrenous cavity and bronchial tree trachea, drainage of the abscesses, correction of immune disorders of the organism and normalization of the functioning of its important systems. The operative treatment is thought to be necessary in cases with severe complications such as bleedings, pyopneumothorax, septicopyemia. Operations under these conditions give high lethality rate and are followed by severe complications.

The preoperative preparation by plasmapheresis was used in 106 patients with toxic goiter. The authors make a conclusion that plasmapheresis is an efficient method in the preoperative care of patients with thyrotoxicosis allowing to shorten this period.
