ABSTRACT
In a controlled study in 109 female rats we evaluated the effect of soluble D-Penicillamine in doses of 10 mg/ml or 100 mg/ml on capsular formation around semipermeable 2 cm3 mini-prostheses. This was compared with methylprednisolone 1 mg/ml or 10 mg/ml, and a group given saline served as controls. The drugs were injected into the lumen. Capsular wet weight and capsular tensile strength were measured after a period of 40 days. Rats given D-Penicillamine showed a significant, dose-dependent, reduction in wet weight and tensile strength compared with the saline group. There was no significant difference between the groups given D-Penicillamine and those given steroids. Topical treatment with diffused D-Penicillamine can significantly reduce the amount of capsular formation around silicone implants. This drug, which is highly specific for the systemic treatment of fibrotic diseases, should be evaluated further to use in reducing capsular formation.

A retrospective report on experience with patients with hemorrhagic necrotizing pancreatitis treated surgically with the new therapeutic concept of programmed lavage, so-called, is presented. With the exception of one fatality, using an unchanged intensive care therapy program, 7 patients were successfully treated with the new concept. Since the results obtained with the therapeutic concept used prior to the technique of programmed lavage, as also reports in the international literature, are associated with a very poor prognosis, it is suggested that this new treatment concept be employed with the aim of improving the prognosis. A definitive assessment of this concept will, however, require a large number of cases treated in this way.

Two case reports are given of patients harboring big gastric leiomyoma with central ulceration. X-ray, endoscopic and intraoperative findings are described; the surgical procedure is discussed.

On the basis of clinical observation the significance of the juxtapapillary leiomyoma is discussed. In spite of the potential malignancy of the leiomyoma the lesion has to be treated as malignoma. The leading clinical symptom is the history of occult bleeding. For ensuring the diagnosis the procedures are described. It is further discussed which surgical managements are necessary and indicated.

From a series of 28 patients subjected to aortic valve replacement with the Björk-Shiley tilting disc valve prosthesis 3 cases are reported in which massive thrombosis with valve dysfunction and fatal outcome occurred in the very early postoperative period. The autopsy findings and the examination of the blood coagulation state during the first 24 hours postoperatively disclosed in all 3 cases a hypercoagulability being responsible for the massive clot formation. The early recognition and establishment of the diagnosis of a hypercoagulability state and an immediate anticoagulant therapy with heparin are necessary for preventing such early fatal complications in patients with cardiac valve replacement.
