ABSTRACT

The analysis is given of the autopsy material obtained from 39 cases of progressive tuberculosis which developed against glucocorticosteroid treatment of different diseases. In 28 of 39 patients progression of tuberculosis was stated as the main cause of death. There were 12.8% and 43.6% of cases with primary and disseminated tuberculosis, respectively. Clinicoanatomical correlations related tuberculosis onset to the hormonal treatment regimens. Comparison of the final clinical and pathomorphological diagnoses showed that tuberculosis had been overlooked in 51.3% of cases.

ABSTRACT

The features of tuberculosis morbidity pattern and trends in the adult population urgently necessitate goal-oriented activities for the risk group represented by patients with nonspecific pulmonary diseases. The phthisiology service of the Chelyabinsk region could assume the responsibility for inpatient and polyclinic health care of the patients with non-specific pulmonary diseases due to the following: relatively low tuberculosis morbidity, incidence and mortality rates; available experience in exercising an integrated approach to preventive measures; an established team of highly-qualified phthisiologists; and a wide network of antituberculosis institutions in the region. An integrative emphasis is formed on the basis of guidelines, training activities, inpatient facilities of the anti-tuberculosis institutions and follow-up of the patients.

ABSTRACT

In tuberculosis dispensaries as centres for differential diagnosis of pulmonary diseases it is expedient to establish departments for diagnosis, treatment and dispensary observation of patients with sarcoidosis. This would provide verification of diagnoses with instrumental methods in 87.6 per cent of the cases and decrease of the average hospital stay for new cases of sarcoidosis and patients with relapses of the process by improving the examination and treatment procedures. At the background of the organized treatment and prophylactic measures progress in the disease was observed only in 4.7 per cent (14 persons) of the patients included into the dispensary group.
