ABSTRACT

BACKGROUND: The potential morbidity and mortality of sphenoid sinus infectious processes are related to their proximity to vital structures within the orbit, such as the cavernous sinus and the brain. Involvement of the posterior orbit can result in superior orbital fissure syndrome, a rare entity affecting structures that cross this anatomic region. Early recognition of this syndrome is mandatory. Delays in adequate treatment may compromise the patient's prognosis. METHODS: We present two cases of incomplete superior orbital fissure syndrome due to infectious processes of the posterior ethmoid and sphenoid sinuses. CONCLUSIONS: In our experience, endoscopic decompression of the lamina papyracea shows complete recovery of extrinsic ocular motility in these patients and should be combined with intravenous antibiotic therapy.

ABSTRACT

Non-Hodgkin's lymphoma of the sinonasal tract is an uncommon lesion, representing 1.5 to 15% of all lymphomas. Most cases of primary non-Hodgkin's lymphoma of the sinonasal tract occur in the maxillary sinus, ethmoid sinus, and nasal cavity; its occurrence in the frontal sinus is extremely rare. We report a case of primary type B non-Hodgkin's lymphoma of the frontal sinus in a 43-year-old man. The patient complained of frontal headaches that had not improved with analgesic drugs, and he presented with a frontal bulge that involved the left upper eyelid; the bulge had progressively enlarged over a 3-month period A biopsy of the mass identified the type B non-Hodgkin's lymphoma. Immunohistochemical study not only confirmed the histologic type of the tumor, it also provided some important information about the primary tumor site. Advances in immunohistochemistry have shown that type B non-Hodgkin's lymphoma is more common in North American and European patients, whereas subtype T is more common in Asians and in some Latin Americans. The treatment of this condition is still controversial, but the combination of radiotherapy and chemotherapy has yielded the best results in all stages of the disease.

ABSTRACT

Carcinoma de pequenas células dos seios paranasais são tumores extremamente raros e agressivos. Apesar de os pulmões serem o sítio mais prevalente destes tumores, este trabalho enfoca o acometimento de um sitio extrapulmonar, os seios paranasais. Nós relatamos o caso de uma paciente com carcinoma de pequenas células primário do seio maxilar. Incluímos neste estudo uma discussão acerca de todos os casos relatados em literatura, abrangendo suas metástases, tratamento e expectativa de vida.

ABSTRACT

Os linfomas do trato nasossinusal são neoplasias incomuns, que reconhecidamente causam importantes lesões destrutivas no nariz e terço médio da face. Sua raridade pode levar os profissionais da área médica a erros no diagnóstico clínico, além de representar um verdadeiro desafio aos patologistas, por sua natureza inflamatória. OBJETIVO: O objetivo deste estudo é determinar os aspectos clínicos e histopatológicos do linfoma não-Hodgkin (LNH) do trato nasossinusal, correlacionando sítio tumoral e comportamento biológico com os subtipos do LNH. FORMA DE ESTUDO: Estudo de série. MATERIAL E MÉTODO: Foi realizada uma análise retrospectiva que incluiu 7 pacientes atendidos no ambulatório do serviço de otorrinolaringologia do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo, no período de 1985 a 2003. RESULTADOS: As linhagens de células B e T/NK têm comportamento biológico diferente, assim como o sítio e apresentação clínica, sendo o diagnóstico histopatológico de extrema importância. CONCLUSÃO: A biópsia realizada adequadamente favorecerá um diagnóstico mais precoce e preciso, instituindo rapidamente a terapêutica adequada e melhorando o prognóstico e a sobrevida destes pacientes.
