ABSTRACT

Introduction: Malignant peritoneal mesothelioma (MPM) is a rare cancer that is associated with asbestos exposure. The diagnosis can be difficult given the nonspecific nature of presenting symptoms and the presence of concomitant confounding findings. Case Presentation: We report a 71-year-old male who presented with right lower quadrant pain and new-onset ascites. CT imaging of the abdomen/pelvis demonstrated omental stranding concerning for a possible omental infarction. Subsequent imaging showed persistent omental edema but no identifiable soft tissue mass. A biopsy of the omentum showed atypical mesothelial proliferation, but pathology was unable to determine if proliferation was a neoplastic versus reactive process. Surgical oncology performed a diagnostic laparoscopy that showed peritoneal studding of the omentum. Subsequent immunohistochemical staining of the omentum demonstrated preservation of BAP1 expression and loss of MTAP expression, consistent with peritoneal mesothelioma. Conclusion: MPM is a rare and aggressive cancer with an overall poor prognosis. The diagnosis of MPM can be difficult based on the nonspecific clinical presentation, insufficient imaging and laboratory testing, and the presence of concomitant confounding findings, such as with this patient and his admitting diagnosis of omental infarction. This case demonstrates the importance of developing a broad differential while maintaining an awareness of heuristics that can influence clinical decision-making.

ABSTRACT

Baylor Scott & White Health Central Texas displayed the diversity and growth of scholarly pursuits during Scholars Day, which debuted online on May 6, 2022. Residents and fellows, medical students, nurses, and research staff were among those showcasing their scholarly activity in areas such as medical innovation, clinical vignettes, research, and quality improvement. A selection committee chose 34 abstracts-16 select podium, 18 rapid fire. In addition, 66 abstracts were included as electronic poster presentations, an increase of 20 from 2021. Residency and fellowship program directors nominated presentations for the Excellence Awards. The Scholarship Committee chose four to receive travel awards to support the presentation of each project at a national meeting. Excellence Awards were granted to Nada A. Mohamed, MD, Irfan Shehzad, MD, Abirami Subramanian, MPH, MD, and Hadley K. Young, MD. A selection of abstracts is presented here.

ABSTRACT

Pneumoperitoneum is often a sign of a perforated viscus and is considered a surgical emergency, as it can lead to enteric contamination within the peritoneal cavity with subsequent peritonitis and sepsis. Management of asymptomatic pneumoperitoneum can pose a dilemma to physicians. Here, we present and discuss a rare case of a 60-year-old man who developed a pneumoperitoneum after a colonoscopy.

ABSTRACT

Ecthyma gangrenosum is an exceedingly rare dermatologic complication of bacteremia or fungemia in immunocompromised patients. The lesions may be single or multiple and may occur anywhere on the body. We present a case of ecthyma gangrenosum with atypical organisms and risk factors resulting in delayed diagnosis of both the condition and the underlying infectious process that caused it. Despite the atypical findings, the diagnosis was confirmed by biopsy and tissue culture.

ABSTRACT

Alterations in calcium and phosphorus levels and joint pain are a common occurrence in end-stage renal disease patients. However, metastatic calcinosis cutis is a rare diagnosis that often combines these two findings, with extensive soft tissue calcification surrounding a large joint being the hallmark of this disease. The exact mechanism behind this clinical entity is unknown. The treatment and complications can be severe and disabling. Here, we discuss the case of a 26-year-old man presenting with unusually advanced skin and joint calcification of the shoulders, neck, hand, and penis.

ABSTRACT

Nationally, health care providers wrote 259 million prescriptions for narcotic analgesics in 2012, or roughly one bottle of narcotics per US adult (1). In an effort to combat this ever-growing problem, the Drug Enforcement Administration changed the schedule of hydrocodone combination products from schedule III to schedule II on October 6, 2014. Fourteen Baylor Scott & White pharmacies encompassing a 200-mile radius in Central Texas were queried for prescription information on hydrocodone/acetaminophen, morphine, codeine/acetaminophen, and tramadol before and after the rescheduling to evaluate trends in prescription drug usage. While the rescheduling of hydrocodone combination products resulted in a reduced number of prescriptions and the total quantity dispensed of both the hydrocodone/acetaminophen 5/325 mg (Norco 5/325) and 10/325 mg (Norco 10/325) formulations, this was offset by a dramatic increase in alternative narcotic analgesics such as tramadol, codeine/acetaminophen 30/300 mg (Tylenol #3), and codeine/acetaminophen 60/300 mg (Tylenol #4), which do not have schedule II requirements. Additionally, there was no significant reduction in total pain medication prescribed after converting all agents to morphine equivalents.
