ABSTRACT
ABSTRACT: Hemorrhagic pseudotumor occurs in approximately 1% to 2% of patients with hemophilia. This lesion is exceptionally rare in nonhemophiliac individuals. The authors present a case of treated right ischial plasmacytoma with a progressively enlarging expansile hemorrhagic pseudotumor, documented on 18 F-FDG PET/CT and MRI and confirmed by histology.

ABSTRACT: The author presents 3 incidental cases of glomangiopericytoma exhibiting avidity of 99mTc-MIBI, 18F-FDG, and 11C-choline, respectively, during SPECT/CT evaluation of hyperparathyroidism, and PET/CT monitoring of metastatic melanoma and biochemical recurrence of prostate cancer. All the 3 cases show similar functional and anatomic features of tracer-avid lesion in the right nasal cavity from histologically proven glomangiopericytoma.

Focal nodular fat sparing of the liver may show, on PET/CT imaging, prominent tracer uptake over a background of less metabolic features of steatosis. This finding, already reported with F-FDG, may mimic primary or secondary neoplasms of the liver. The authors present an additional case of nodular fat-sparing hepatic parenchyma exhibiting C-choline avidity during PET/CT assessment for biochemical recurrence of prostate cancer.

Trichoblastic carcinoma is an uncommon malignant neoplasm arising from follicular germinative cells. The high-grade trichoblastic carcinoma has been reported with lymphatic and hematogenous dissemination. The authors present a 59-year-old man with gluteal cleft trichoblastic carcinoma and highlight the useful role of serial F-FDG PET/CT examinations in the demonstration of initial and subsequent nodal metastasis in the inguina and pelvis.

Somatostatin receptor scintigraphy may show benign In-octreotide uptake due to physiologic, inflammatory, or reactive processes. Awareness of these findings helps the imager to avoid pitfall interpretation of primary or secondary malignancies. To the list of previously reported potential false-positive findings of In-octreotide, the author adds a case of tracer uptake from laparoscopic wedge liver biopsy.

Necrobiotic xanthogranuloma is a non-Langerhans histiocytic disorder characterized by chronic and progressively enlarging cutaneous plaques and nodules predominantly at the head and facial regions. These lesions exhibit increased F-FDG uptake because of their inflammatory and granulomatous nature. The author presents the F-FDG PET/CT and MRI features of such an uncommon disease in a patient with associated monoclonal gammopathy.

Intraluminal masses of the azygos vein are of rare occurrence. The differential diagnosis includes bland thrombus, tumor thrombus, epithelioid hemangioendothelioma, and leiomyosarcoma. To this list, the authors add an uncommon case of intravenous lobular capillary hemangioma, also known as intravenous pyogenic granuloma, of the azygos vein, inducing a recurrent large right pleural effusion. CT, MR, and PET/CT features of this lesion are presented.

The authors present a case of 18F-FDG-avid granulomatous reaction induced by fireworks injury of the chest wall in a patient with esophageal adenocarcinoma. This hypermetabolic lesion, involving the right pectoralis muscles, appeared slightly more prominent on restaging PET/CT imaging following chemotherapy and radiation therapy. Excisional biopsy of the lesion established the diagnosis of foreign-body granulomatous-type inflammation with surrounding foci of non-polarizable black foreign material and ruled out malignancy. The patient recalled accidentally shooting himself in the chest with a Roman candle at the age of 3.

Langerhans cell histiocytosis is essentially a pediatric disorder with a large spectrum encompassing focal lytic osseous eosinophilic granuloma, chronic multifocal Hand-Schuller-Christian lesions, and fulminant disseminated Letterer-Siwe syndrome. However, adult involvement is not uncommon. The authors present a case of multifocal Langerhans cell histiocytosis in a 47-year-old woman. The PET/CT-depicted infiltrative features of soft tissue and osseous pelvic lesions with obstructive uropathy are unusual for this disease.
