ABSTRACT

UNLABELLED: Ambulatory peritoneal dialysis is an alternative to haemo dialysis for patients with End Stage Renal Disease (ESRD). This study evaluates our experience in laparoscopic placement of Tenckhoff Catheter (TC) for peritoneal dialysis. METHODS: Between december 2000 and december 2007 in the II Surgical Clinic of Timisoara the laparoscopic implantation of the Tenckhoff catheter was performed in a lot of 56 patients with ESRD with mean age 53.04 years (limit between 17-80 years). In this lot of patients 66 laparoscopic procedures were performed--58 catheter implantation, 2 catheter changing, 2 repositioning, 2 adhesiolysis and 2 withdrawn. Mean operative time was 47.3 min. (limits 35-90 min.). RESULTS: Immediate functionality ratio (< 30 days) was 98.11% of cases. The immediate post-operative complications were: leakage of dialysis solution in three patients (5.35% of cases), bleeding from the incision in three patients (5.35% of cases) and one patient (1.78% of cases) with a migration of the TC, case who required replacement of the catheter performed also by laparoscopy. The late post-operative complications were: obstruction with omentum of the catheter in two patients (3.57% of cases), massive leakage in one patient (1.78% of cases), migration of the TC in one patient (1.78% of cases), tunnel infection in one patient (1.78% of cases) and peritonitis in two patients (3.57% of cases). In all these patients laparoscopy replacement of the catheter was required. The minor complications were: exit site infection in 3 patients (5.35% of cases), tunnel infection in 7 patients (12.5 % of cases), incisional hernia in one patient and pleural effusion in one patients (1.78% of cases). CONCLUSIONS: Laparoscopic placement of TC is a successful method compared to other procedures. The great advantage is the direct visualization and fixation of the TC in the pelvis.

ABSTRACT

This work discusses the synthesis and the fragmentation patterns for 2-(p-acetylaminosulfonamido)-2-thiono-(5,5-dimethyl-1,3,2-dioxaphosphorinane)(1) and for the p-acetylaminosulfonylamides of O,O-diethylthiophosphoric acid (2), O,O-diphenylthiophosphoric acid (3), dimethylaminocyclohexylthiophosphoric acid (4), and diethylaminophenylthiophosphoric acid (5). A thionamidic-thiolimidic structure was attributed to compounds 1-5, consistent with their IR and NMR spectra. EI mass spectra at 70 eV, high resolution (HR) mass measurements and metastable ion spectra were used to elucidate the fragmentation processes and to determine the kinetic energy release values associated with the metastable ion dissociations. HR accurate mass measurements were used to confirm the compositions of the more abundant ions.

ABSTRACT

PURPOSE: Evaluating the efficiency and safety of Alomide (lodoxamide 01.%) in the treatment of allergic conjunctivitis. MATERIAL, METHOD: 12 patients have been selected during may-august 1998, in order to be treated with Alomide 4 times daily. The clinical exam of the patient included: general information, history, ocular examination. The ocular status of each patient has been evaluated with a score: the intensity of each symptom and sign has been graded between 1-3. RESULTS: In all cases, the use of Alomide induced the improvement of the typical symptoms and signs of the allergic conjunctivitis. No side effect related to the use of the drug has been noted. CONCLUSION: Due to its dual action (inhibition of both mast cells and eosinophils), Alomide covers most of the allergic response.

ABSTRACT

The study analyzes the ocular aspects in patients receiving hemodialysis, in order to define the importance of the ophthalmological exam as prognosis and follow-up parameter. The prospective study includes 84 patients with renal insufficiency who received hemodialysis between 1994-1998. The ocular aspects and their connection with the dialysis and the basic disease are described and analyzed. The most important were the retinal vascular complications: hypertensive retinopathy, anterior optic ischaemic neuropathy, central retinal artery occlusion, diabetic retinopathy.

ABSTRACT

The current therapy of glaucoma (drugs, LASER, surgery) acts mainly on the trabecular meshwork and on the ciliary body. The loss of the visual function in glaucoma is however determined by the disfunction and death of the ganglion cells. The main theories trying to explain the death of the ganglion cells in glaucoma are analyzed. Their practical value is represented by the finding of the ideal neuroprotective drug.

ABSTRACT

The reduction of the visual performances in glaucoma may be explained by several theories. The most admitted theory, for the present, considers glaucoma as a progressive primitive optic neuropathy, having the origin at the retinal ganglion cells, associated with a disease of the sclero-corneal trabeculum, whose porosity is diminishing. The most important pathogenic hypothesis in glaucoma are: the vascular, the mechanical and the collagenic hypothesis. The vascular theory considers that the prolonged ischemia is a responsible for the neuronal alteration. In this process are participating a lot of factors: the intraocular pressure, the blood pressure and some other factors governing the vascular tone of the optic nerve (myogenic factors, metabolic factors, vasoactive factors, circulating hormones and endothelial factors). From therapeutical point of vue, these modern concepts are reflected in the new therapeutic perspectives, with a special attention for the axonal outflow and trabecular cell disfunction and also for the dysregulation of the cellular oxidoreduction.

ABSTRACT

UNLABELLED: Beside aphakia and senility myopia is one of the major factors predisposing to retinal detachment. The purpose of the study is to define the clinical features of the myopic retinal detachment with therapeutic and prognostic implications. METHOD: 100 myopic (< 6 dioptres) retinal detachments that were hospitalized in the Ophthalmological Department from Cluj-Napoca between 1996-99 are retrospectively analyzed. RESULTS, DISCUSSION: The major clinical elements having an impact in defining the anatomical and functional prognosis of the myopic retinal detachment are described and analyzed: the extension of the retinal detachment, the number and shape of the retinal ruptures, the detachment of the macula, the association of aphakia and of myopic choroidosis.

ABSTRACT

PURPOSE: To evaluate the significance of refractive errors in cases of branch retinal vein occlusion (BRVO). METHODS: We included in this study 179 patients with BRVO who attended our department between 1996-2000. We compared this group with an equal number of age, sex and associated diseases matched controls. The spherical equivalents of the refractive errors of patients in both groups were compared using the chi-square test and Student's t test. RESULTS: Hypermetropia was present in 114 patients wit BRVO (63.68%) and in 82 controls (45.8%, p = 0.00048). Myopia was present in 16 patient with BRVO (8.9%) and in 39 controls (21.8%, p = 0.0037). Emetropia was found in 49 patients (27.37%) and in 58 controls (32.4%, p = 0.299). CONCLUSIONS: Hypermetropia is significantly more common in patients with BRVO than in general population, but myopia is significantly less common in these patients than in controls.

ABSTRACT

PURPOSE: Revealing the diode laser therapy's results in the diabetic macular edema. METHOD: The diabetic retinopathies in which macular photocoagulation has been performed (between Sept. 1996-Sept. 1998) are analyzed. The cases were selected according the ophthalmoscopy appearance: clinical significant macular edema. The technical features and the clinical and functional parameters' evolution after the treatment are described. RESULTS: Visual acuity's evolution has been stationary in 80% of cases. No complication during or after the macular photocoagulation with the diode laser has occurred.

ABSTRACT

The purpose of the study is the evaluation of the therapeutical results in arterial retinal branches obstructions, according their etiology and clinical features. The study includes 30 patients with arterial retinal branch obstructions, hospitalized in the Ophthalmological Department from Cluj, between 1989-1996. The temporal branches have been significantly more frequently affected (73.33%), as compared to the nasal ones (6.66%). We noticed no difference in visual acuity's recovery according the precocity of the therapy. Visual acuity's recovery after the arterial retinal branches obstructions is under the simultaneous influence of a group of factors. In this context, the most important ones are: the etiopathogenetic mechanism and the degree of the arterial obstruction. Under these circumstances, an important consideration has to be given to the prophylactic component of the treatment.

ABSTRACT

There is no correlation between left ventricular (LV) systolic function and effort capacity of the patients with acute or old myocardial infarction (MI). On the other hand, some recent studies suggest such a relationship for LV diastolic function. Twenty-five patients with old MI were submitted to a maximal symptoms limited exercise testing (ET) and to an echo Doppler examination; functional aerobic impairment (FAI), myocardial aerobic impairment (MAI), maximal exercise capacity (METs NYHA class), isovolumic relaxation time (IVRT), E and A wave velocity, E/A rate being calculated. There is a negative significant correlation between MAI and E velocity (r = -0.68), but not between MAI and A velocity or IVRT. The data sustained the correlation between LV diastolic dysfunction and myocardial ischemia, but not with effort capacity of the patients, the last one being determined mainly by other factors. The LV diastolic function indices were modified in anterior but not in inferior MI, in relation with the amount of myocardial necrosis. It is concluded that, like LV systolic function, LV diastolic function does not correlate with the effort capacity of the patients with MI, but it represents a good predictor of the severity of myocardial ischemia mainly in anterior MI.

ABSTRACT

This study points out vitreous surgery's indications, technical means and results in various ocular traumatisms. Over one-year period, in the Charles Nicole Hospital in Rouen, vitreous surgery has been performed in 67 patients with ocular traumatisms. The surgical techniques, the intra- and postoperative complications are described and correlated to the preoperative ocular status. Results are significantly better in the group with iatrogenic traumatism than in the one with accidental traumatisms. Vitreo-retinal surgery is the only therapeutical method in various ocular traumatisms intraretinal foreign bodies, nonmagnetic intraocular foreign bodies, posttraumatic tractional retinal detachment, giant retinal tears, posttraumatic intravitreal hemorrhage without spontaneous resolution. Without vitreo-retinal surgery, visual function's loss would have occurred.

ABSTRACT

The p53 expression in peripheral lymphocytes of rats chronically exposed to atrazine was investigated. The experiment was performed in female Wistar rats. Atrazine was administrated in different doses (2.7 and 5.4 mg/kg body weight), each dose once a day, 5 days per week, for 6 and 12 months. The percentage of rats peripheral lymphocytes expressing p53 protein was evaluated by immunocytochemical technique, using a monoclonal antibody (clone PAb 122) against a common epitope, both for the wild type and the mutant p53 protein. The results indicate that in the atrazine long-term administration, the serum level of atrazine is associated with: (i) Significantly increased percentage of lymphocytes expressing p53 protein for all treated animals; (ii) different p53 intracellular compartmentalization (nucleus and cytoplasm), depending on dose and time of atrazine administration. The present study suggests that atrazine modifies the p53 expression, which could confirm the clastogenicity of this herbicide, and that the detection of the p53 protein may serve as a biomarker for the long-term exposure to atrazine.

ABSTRACT

The paper presents a case of autoimmune hemolytic anemia (AIHA) with warm IgG antibodies associated with Waldenström's disease in which in the stage of compensated hemolysis after treatment, there appeared a severe hemolytic attack induced by transitory cold agglutinins with high thermal amplitude. The case described is a novelty by the intrication of two autoantibody populations which causes autoimmune hemolysis. The difference from other similar associations is discussed.

ABSTRACT

The modern vitreous surgery has rapidly developed during the last 2 decades, becoming an effective part of the treatment, in most retinal detachments that cannot be treated by the classical means. This study points out the main types of retinal detachment in which vitrectomy is indicated, as its place in the complex therapeutical sequence. It is based on the studies of ophthalmologists with experience in vitrectomy is indicated, as its place in the complex therapeutical sequence. It is based on the studies of ophthalmologists with experience in vitreous surgery. If there continue to be controversies on vitrectomy's indications and moment, the fact that vitrectomy has radically changed the prognosis and therapeutical attitude in severe retinal detachments, is unanimously admitted. Most retinal detachments (the rhegmatogenous, uncomplicated one) continue to be treated by the classical means: scleral buckling+Rupture's blockage.

ABSTRACT

The purpose of the study is to point out the characteristic clinical, therapeutical and evolutive features in the acute ischaemic suffering of the optic nerve's preliminary portion. A retrospective study has been performed, on the patients with anterior ischaemic optic neuropathy (AION), hospitalized in the Ophthalmologic Clinic from Cluj-Napoca, between 1986-1994: 71 cases. The main functional (visual field, visual acuity) and ophthalmoscopic aspects are analyzed, as their evolution after treatment. Results are significantly better if corticosteroids are systemically used (43.3% improvement of the visual field) than in their absence (17.07% improvement of the visual field). The high percentage of failures (70%) proves the absence of a therapeutical method to cure or bring a significant visual function's improvement in AOIN.

ABSTRACT

The paper present the functional and anatomic results obtained in 376 retinal detachment operations done in the Ophthalmological Clinic from Cluj-Napoca. The following types of operations were done: cerclage Arruga, scleral indentation, surface diathermy, laser photocoagulation. The retrospective study revealed percentages of 73.41% the anatomic reapplication and 50% in functional recovery. The negative influencing factors of the prognosis were: aphakia, large and old retinal detachments, macular region affectation.
