ABSTRACT

The results of segidrin administration to 46 patients with malignant and 6 patients with benign tumors of the brain are presented. Pronounced therapeutic effect for the whole group was 63.5% and 73%, if partial regression of neurologic symptoms in the entire brain and separate foci is considered. These indexes for patients with malignant tumors only were 61 and 71.7%, respectively. Since segidrin has virtually no significant untoward side-effects, it is considered a most safe medicine for managing brain tumors. It is recommended in cases of inoperable tumor and for post-operative adjuvant chemotherapy with a view to extending the patient's survival time and improving the quality of life.

ABSTRACT

Two rare observations of sinus pericranii are presented, characterized by a soft intumescence containing venous blood and growing in size if the head is bent or during tension. The condition may be diagnosed by local introduction of a contrast agent into the intumescence. Surgical treatment of this developmental venous defect is recommended.

ABSTRACT

In patients with cerebral meningiomas, hemorrhages to the subarachnoidal space and tumorous tissues are an extreme rarity. The difficulties encountered in the clinical diagnosis may be, as a rule, accounted for by an uneventful course, marked by a dramatically occurring intracranial hemorrhage as the first disease manifestation. Carotid angiography and computer-aided tomography enable a correct diagnosis to be made. Radical excision of the tumor provides favourable results, with the exception of comatose patients.

ABSTRACT

The authors had under observation 21 patients with diastematomyelia--a rare congenital disease marked by the formation of a bony spicule in the vertebral canal, which splits the spinal cord and its meninges. The disease may be manifested by localized (at the level of the bony spicule) skin lesions on the back (hypertrichosis), developmental anomalies of the locomotor apparatus, and disorders of spinal cord function of various degree. The diagnosis is established on the basis of radiological examination (plain and contrast spondylographs). During surgical intervention (laminectomy) the bony spicule is removed, the dura mater surrounding it is cut, and a common sheath is formed from the dura mater for the locally split spinal cord.

ABSTRACT

The analysis of the clinical, laboratory and roentgenological symptoms of 15 patients with the recurrent course of suppurative meningitis and nasal liquorrhea (11 patients) has made it possible to establish that the main factors underlying this disease include craniocerebral trauma, the formation of liquor fistulas and focal infection in the adnexal sinuses. It has been shown that surgery is the cardinal and radical method of treatment determining the disease prognosis.

ABSTRACT

The authors analyse the course of the immediate postoperative period in 27 patients with ruptured arterial aneurysms of the cerebral vessels. The dynamics of the local cerebral blood flow and electric resistance of the brain matter were examined for a comprehensive study of changes in cerebral circulation and metabolism. Intracranial pressure (ventricular and subdural) and mechanical resistance of the brain matter were measured with the M-34 mingograph. Such combination of the examination methods and the findings of neurological examination made it possible to study over time the pathological processes which are interconnected (circulation-metabolism-edema and swelling of the brain) and determine the course of the postoperative period. A dependence between the preoperative condition of patients and the course of the immediate postoperative period was revealed, adequate methods for correction of the pathological reactions are suggested, and the specific features of a complex clinico-physiological control in this category of patients and the informativeness of the different methods of examination are discussed.

ABSTRACT

The authors describe the technique and explain the reason for the use of an intravascular balloon catheter technique in the treatment of vasospasm in cases of subarachnoid haemorrhage due to rupture of a cerebral aneurysm. In 33 patients with SAH 105 major cerebral arteries were dilated (in the system of internal carotid and basilar arteries). Only in one case vasodilatation could not be accomplished. The effect of dilatation of vasospastic arteries was stable and the functional state of the brain was ameliorated, local and general cerebral dysfunctional signs and symptoms due to vasospasm regressed. The authors discuss indications and contraindications for intravascular balloon dilatation of spastic arteries, the time for the operation, the length of arteries to be dilated, and the possible mechanism of amelioration of cerebral blood flow to the brain stem.

ABSTRACT

Employing the Soviet thermal scanner "Rubin", 59 patients aged 20 to 65 years suffering from lumbosacral radiculitis were examined. Clinical examinations were supplemented with spondylography , epidurography, pneumomyelography and myelography. Operation performed on 29 patients showed a slipped disk in 26 and hypertrophy of the yellow ligament in three patients. The level and the side of compression of one or another neural root by the slipped disk detected at operation correlated with the thermographic data in 72% of all cases. An analysis of the thermograms in 59 patients with lumbosacral radiculitis demonstrated that thermographically the given disease was manifested by the zone of hyperthermia in the lumbosacral area and the disruption of the thermal flow in the form of hypothermia in the lower extremities which in 72% of cases was distributed according to the stripe type. The side and the level of compression of the neural root are also outlined.

ABSTRACT

The article deals with the substantiation, techniques, and possibilities of treating long-term spasm of the cerebral vessels in patients with ruptured arterial aneurysms by means of an intravascular intervention, dilatation of the spastic vessels with a balloon catheter. Thirty-three large cerebral vessels (internal carotid arteries, the basilar artery, and their branches) were dilated in 13 patients. Vasodilation could not be accomplished in only one patient in the group discussed. The increase in the vascular lumen proved to be stable, and the functional state of the brain in patients subjected to surgical treatment of spasm of the cerebral vessels improved. The authors discuss the indications for and the performance of the suggested intervention, the time of its performance, the boundaries of vasodilatation, and possible mechanisms of circulation improvement in the brain stem. The duration of the effect of dilatation of the spastic cerebral vessels is, according to the authors, the most complex problem.
