ABSTRACT

BACKGROUND: Factors specific to sub-Saharan Africa could have an impact on tympanic membrane perforation characteristics. Obtaining precise information on these characteristics and determinants of hearing loss severity would enable better management. OBJECTIVE: To determine the relationship between characteristics of tympanic membrane perforation and subsequent hearing impairment. METHODS: A cross-sectional study of consenting patients presenting with tympanic membrane perforation was conducted. They were examined using otoendoscopy with a digital camera to obtain precise measurements, followed by pure tone audiometry. RESULTS: Eighty-six cases of tympanic membrane perforation were included. Mean tympanic membrane perforation proportion was 34.1 ± 18.4 per cent. Medium-sized tympanic membrane perforations were predominant (47.7 per cent). Median tympanic membrane perforation duration was 20 years. Tympanic membrane perforation size was found to be a predictor of hearing loss severity (odds ratio = 2.5, 95 per cent confidence interval = 1.02-6.13, p = 0.04). CONCLUSION: Tympanic membrane perforation size was a predictor of hearing loss severity in our setting. Site, duration and aetiology seem to have no impact on hearing loss severity.

ABSTRACT

Une etude retrospective a ete realisee dans le service ORL de l'hopital general de Douala au Cameroun; d'avril 1998 a juin 2004 soit 6 ans. 18 cas de carcinome epidermoide du larynx ont ete colliges. L'age moyen des patients etait de 53 ans. Les patients au moment du diagnostic etaient au stade avance de leur maladie : T4 : 10 ; T3 : 3 ; T2 : 4 : T1 : 1. Les traitements administres ont ete adaptes aux capacites financieres reduites des patients ainsi radiotherapie exclusive pour 6 malades; laryngectomie totale suivie d'irradiation pour 3 et association radio-chimiotherapie pour 1. Ces faits confirment la necessite de subvention pour les examens et les traitements. L'importance de creation des centres d'information et de prevention sur le cancer

Costen syndroma or temporo-mandibular joint disorders is not welknow in the ENT service, despite the common presentation with otalgia. We therefore present 60 cases diagnose in our hospital and present clinical and therapeutic aspect.

ABSTRACT

OBJECTIVE: The aim of this work is to describe the ENT clinical manifestations by which HIV positive patients inaugurate their AIDS-illness and to classify these manifestations in respect to the biological state of the infection. MATERIAL AND METHOD: It was a multicenter, prospective study carried out in the ENT services of Yaounde (Cameroon), between the period of September 2000 and June 2002. Included in this study were HIV patients who started their AIDS with an ENT illness. The stage of evolution of the HIV infection was evaluated using the CD4 and CD8 T lymphocyte counts. This was carried out using the Flux cytometric technique of Fascount/Becton. RESULTS: We thereafter recruited 76 patients. The incidence rate of ENT early manifestations in HIV positive patients was 11.5%. Pharyngeal and oral candidosis represented the most observed manifestation (30.60%), followed by peripheral facial paralysis (11.13%) and rhino-sinusitis (10.58%). Parotid gland hypertrophy represented 8.23% of the affections. Persistant cervical lymphadenopathy was observed in 7.05% of cases. Kaposi sarcoma and cervical lymph node tuberculosis represented each 3.53% of cases of the sample. The correlation of lymphocytic count and initial ENT manifestations showed that these manifestations were observed in all the biological states of the HIV/AIDS infection. CONCLUSION: The above mentioned ENT manifestations in the patient should motivate the request of the HIV screening tests.
