ABSTRACT

A 41-year-old female with 2 years of mandibular and maxillary facial pain sought multiple medical evaluations. Symptoms were similar to those accompanying many benign temporomandibular, salivary gland, and neurological disorders. Through manual palpation, a slight swelling in the salivary gland was discovered; a malignant carcinoma was removed by parotidectomy.

ABSTRACT

The use of computed tomography (CT) in oral and maxillofacial surgery is discussed. The CT scan delineates lesions in the oral and maxillofacial complex to aid in planning of surgical treatment. Case examples are presented, and the indications for the use of CT scanning are discussed.

ABSTRACT

Dehydrated dura mater and a microfibrillar collagen preparation were used in a previously described experimental animal model system for the repair of oroparanasal communications. The clinical and histologic findings are discussed. Results support the reproducibility of the original animal model system. However, the use of these graft materials does not improve the chances for competent closure of the oroparanasal communication or provide continuity of bony contour in the guinea pig.

ABSTRACT

An experimental animal model system for reproduction of oroparanasal communications in the guinea pig is reported. Standard clinical closure techniques, both immediate and delayed, are used, as well as a new graft material-reconstituted collagen. The clinical and histologic findings are discussed. Results indicate that an oroparanasal defect in the guinea pig can be maintained, that the use of reconstituted bovine collagen as a xenograft to the defect is an aid to closure and contour, and that further study is warranted for eventual application in humans.

ABSTRACT

Data from a 1973 survey of diabetes mellitus among schoolchildren in Michigan have been analyzed to derive age-specific incidence and prevalence rates. The annual incidence of new cases of diabetes is approximately 20/100,000 children from age four through age 17, with the highest incidence rates at age nine through age 12. The prevalence rates to be expected, if the most recent incidence rates persist, are approximately 50/100,000 at age five, 150/100,000 at age ten, 270/100,000 at age 15, and 325/100,000 by age 18. The incidence rate of diabetes in Michigan children appears to have doubled between 1959 and 1972. Data from Erie County, New York, suggest that this secular trend has been evident since 1949, and data from Norway suggest that the trend could be noted 50 to 70 years ago.

ABSTRACT

To date, screening has largely failed to fulfill the hope that early detection would lead to more effective early treatment and to improved health status in the population served. The future role of screening can be much more positive but only if (1) screening is directed only at diseases and conditions for which early intervention has proven benefits; (2) valid screening methods are developed and used; (3) screening is done in settings in which diagnosis and treatment are assured; and (4) there is continuing scientific evaluation of the benefits and costs of screening.

ABSTRACT

The quest for constraint of health care costs has combined with the quest for better distribution of specialized child health services to emphasize the need for facility planning that considers the needs and resources of an entire region. This article describes the guidelines derived through one such planning effort and the process through which they were derived. The process can, we believe, be replicated or modified for planning pediatric hospital facilities or other health care facilities in other regions of the country.
