ABSTRACT

BACKGROUND: The present study aimed to assess and compare regional strain of the right and left sides of interventricular septum in healthy subjects using velocity vector imaging analysis due to the importance of interventricular septum and limited basic information about the exact function of the interventricular septum. METHODS: The present study was conducted on 40 healthy subjects. Echocardiography was performed in the apical 4-chamber view in the left lateral decubitus position. Image analysis was done offline with velocity vector imaging; the longitudinal strain and strain rate were calculated during 3 cardiac cycles. Strain-time and strain rate-time curves in basal, middle, and apical segments of the left and right sides of interventricular septum were recorded; peak values and time to peak strain were determined. RESULTS: There was no significant difference between the longitudinal strain in the right and left basal (-17.7 ± 5.10% vs. -18.2 ± 5.14%, P=.550), middle (-17.1 ± 4.53% vs. -17.9 ± 4.29%, P = .197) segments, strain rate of basal (-1.1 ± 0.36 1/s vs. -1.0 ± 0.36 1/s, P = .350), and middle (-1.0 ± 0.30 1/s vs. -1.1 ± 0.32 1/s, P =0.551) segments. However, there was a significant difference between the longitudinal strain (-22.2 ± 5.55% vs. -16.6 ± 4.45%, P < .001) and strain rate (-1.5 ± 0.46 1/s vs. -1.1 ± 0.33 1/s, P < .001) of the apical segment. Time to peak strain was significantly different only in the middle segment of interventricular septum (right side: 351.0 ±11.5 ms vs.left side: 344.4 ± 13.1 ms, P = .004). CONCLUSIONS: The findings of this study suggest that the right and left function of the septum was comparable in the basal and middle segments of healthy subjects; this function was significantly different in the apical segments.

ABSTRACT

BACKGROUND: Job satisfaction is one of the most important issues related to human resources in the organization which can reduce organizational productivity. In this regard, occupational health is an important science related to the health of the workforce in any organization, paying attention to it will help health promotion. The aim of the present study was to determine the relationship between the status of occupational health management and job satisfaction among farmers with a health promotion approach. MATERIALS AND METHODS: This cross-sectional study was conducted in 2020. The number of 209 farmers from all villages of Zabol was included by census method. Data were collected using a researcher-made questionnaire that consisting of three sections of demographic information, occupational safety and health status, and standard job satisfaction questionnaire. At first, the validity and reliability of the questionnaire were evaluated. In addition, 5-10 staff working in Agricultural Jihad Organization were asked about the content, clarity, and simplicity of the questionnaire terms. Content validity index and content validity ratio indexes were calculated as 0.68 and 0.83 for occupational health management and 0.69 and 0.83 for job satisfaction questionnaires, respectively. Instrument reliability was also assessed using the Cronbach's alpha coefficient, which was 0.872. Finally, after verifying the validity and reliability of the instrument, a questionnaire was used to collect the research data. Shapiro-Wilk, Wilcoxon, and Spearman correlation tests were used to analyze the findings. The collected data were analyzed using SPSS software version 19. Armonk; NY, USA: IBM Corp. RESULTS: The results of the study indicate that occupational health management was at a moderate to a high level (3.701) and job satisfaction (3.381) was at a moderate to a high level. The standard regression coefficient between occupational health management and job satisfaction was obtained as 0.571. With a confidence of 95%, it can be stated that occupational health management in Zabol city under the supervision of jihad-e-agriculture organization has a significant impact on farmers' job satisfaction. In addition, the research findings confirmed a positive and significant relationship between occupational health management and job satisfaction of employed farmers and showed that the implementation of occupational safety and health programs in the jihad-e-agriculture organization can affect the job satisfaction of farmers. CONCLUSION: According to the evaluation of research findings, job satisfaction and occupational health management in the Agricultural Jihad Organization are relatively favorable among farmers in Zabol city. ABSTRACT

In recent years, many reforms have been made on the structure of hospital administration, most of which are proposed by Parker-Harding models. Therefore, the purpose of this study is to systematically review global relevant experiences in reforming the hospital governance structure with emphasis on the Parker-Harding model. Required information was collected using keywords autonomization, corporatization, privatization, decentralization, reform, hospital autonomy, governance model, and structural reform in databases such as EMBASE, PubMed, Scopus, SID, MagIran, and other resources. Information on the subjects under study was collected from 1990 to 2020. The content extraction method was used for data extraction and data analysis. Thirty-nine sources were included in the study. Results of searching for relevant evidence on a variety of hospital governance models (government, board, corporate, and private) based on the Parker-Harding model in four categories including strengths (31), weaknesses (30), outcomes (26), and interventions (21) are outlined. In this study, strengths, weaknesses, outcomes, and corrective interventions were presented for different models of hospital administration that could be used by healthcare policymakers. Also, According to the results of this study, governmental model less recommended.

ABSTRACT

The main purposes of this study are to evaluate the performance of graphene membranes in the separation/purification of hydrogen from nitrogen from a theoretical point of view using the molecular dynamic (MD) simulation method, and to present details about molecular mechanisms of selective gas diffusion through nanoscale pores of graphene membranes at the simulated set conditions. On the other hand, permeance and perm-selectivity are two significant parameters of such a membrane that can be controlled by several variables such as pressure gradient, pore density, pore layer angles etc. Hence, in this work, the hydrogen and nitrogen permeating fluxes as well as the H2/N2 ideal perm-selectivity are investigated from a theoretical point of view in a two-layer nanoporous graphene (NPG) membrane through classical MD simulations, wherein the effects of pressure gradient, pore density, and pore angle on the NPG membrane performance are evaluated and discussed. Simulation outcomes suggest that hydrogen and nitrogen permeating fluxes increase as a consequence of an increment of pressure gradient across the membrane and pore density.

ABSTRACT

OBJECTIVE: Patient safety walkround (PSWR) is an appropriate technique for senior managers' participation and collaboration with frontline staff to promote safety culture in hospitals. This study was conducted to assess the 5-year PSWR experience in Shahid Mahallati Hospital in Tabriz, Iran. METHODS: This was a triangulated cross-sectional study. All documents, instructions, reports, action plans, protocols, and verbal feedbacks on PSWRs were reviewed from 2013 to 2017. Data were extracted using goal-driven extraction form and analyzed using Excel 2016 software. RESULTS: A total of 174 90-minute PSWRs were conducted during the study period, and 1372 safety issues were identified. Work environment factors (32.5%), organization and management (18.14%), and tasks (18.07%) were the most frequent identified issues, respectively. About 75.5% of the issues were resolved through corrective interventions implementation. CONCLUSIONS: PSWRs provide a great opportunity to identify and resolve the safety issues in the hospitals. Senior managers' participation and their support of staffs opinions and innovations were reinforced through PSWRs. Moreover, it creates mutual trust and safety responsiveness at all organizational levels in the hospital.

ABSTRACT

PURPOSE: The purpose of this paper is to explore the validity and reliability of the Persian version of the patient satisfaction questionnaire (PVPSQ). The study addressed the communication skills section of the PSQ specifically. DESIGN/METHODOLOGY/APPROACH: This cross-sectional study was conducted in 2015 to determine the validity and reliability of the Persian version of the patient satisfaction questionnaire (PSQ). In total, 538 patients (ten in pilot, 488 in tests, 40 in retest), 14 experts and 198 residents participated. The items' impact score, content validity index, and content validity ratio were calculated. Construct validity and reliability of the scale were examined with exploratory factor analysis, Cronbach's α, and the Intraclass correlation coefficient (ICC), respectively. FINDINGS: The questionnaire demonstrated a content validity index=0.94, content validity ratio=0.84, impact score=4.61, Cronbach's α=0.93 and ICC=0.513 ( p<0.00). A single factor was found in the eigenvalue distribution of the PSQ that predicted approximately 93 percent of the variance. PRACTICAL IMPLICATIONS: The results of this study will permit researchers in all Persian-speaking countries to use a valid and reliable Persian version of the PSQ to evaluate patients' satisfaction with residents' communication skills. ORIGINALITY/VALUE: There were a few Persian questionnaires to assess patient satisfaction with physicians' communication skills, but their psychometric properties had not been reported until the time of this study. By using the PVPSQ, both researchers at Iranian universities of medical sciences and researchers in other Persian-speaking countries can assess residents' communication skills from the patient's perspective more reliably.

ABSTRACT

Defect free mats containing a cationic polysaccharide, chitosan derivative such as N-[(2-hydroxy-3-trimethylammonium)propyl] chitosan chloride (HTCC), have been prepared using electrospinning of an aqueous solution of poly(vinyl alcohol) (PVA)-HTCC blends. HTCC, a water-soluble derivative of chitosan, was synthesized via the reaction between glycidyl-trimethylammonium chloride and chitosan. Solutions of PVA-HTCC Blends were electrospun. The morphology, diameter and structure of the produced electrospun nanofibres were examined by scanning electron microscopy (SEM). The average fibre diameter was in the range of 200-600 nm. SEM images showed that the morphology and diameter of the nanofibres were mainly affected by weight ratio of the blend and applied voltage. The results revealed that increasing HTCC content in the blends decreases the average fibre diameter. These observations were discussed on the basis of shear viscosities and conductivities of the spinning solutions. Microbiological assessment showed that the PVA-HTCC mats have a good antibacterial activity against Gram-positive bacteria, Staphylococcus aureus, and Gram-negative bacteria, Escherichia coli.
