ABSTRACT

As cicatrizes inestéticas alargadas ou hipertróficas periauriculares podem ser decorrentes da tensão sobre a sutura das incisões da ritidoplastia. Em estudo retrospectivo de cinco anos, 10 pacientes do sexo feminino portadoras de cicatrizes inestéticas foram tratadas com a técnica do anel periauricular. Utilizou-se o fio de politetrafluoretileno em seis casos e o de mononylon 2-0 duplo em quatro casos, introduzidos com agulha de fáscia lata sem bisel. Após a exerese da cicatriz, as bordas foram aproximadas, e o anel amarrado e fixado na fáscia do músculo temporal. O resultado foi satisfatório. Em dois casos os fios foram removidos sem alteração do resultado estético final das cicatrizes.

ABSTRACT

BACKGROUND: The use of CO2 or conventional erbium laser ablation or more recent nonablative laser photothermolysis for skin rejuvenation is associated with significant disadvantages. OBJECTIVE: The objective was to assess the efficacy of the erbium:YAG laser (2,940 nm) using the "ablative" fractional resurfacing mode to improve photodamaged skin. METHODS: A total of 28 patients, 27 women and 1 man, aged 28 to 72 years (mean age, 54.2 years), with Fitzpatrick Skin Types II to IV, were treated for mild to moderate actinic damage using fractional erbium:YAG laser (2,940 nm) combined with Pixel technology. Sessions were scheduled at 4-week intervals. Response to treatment was evaluated by two physicians on a five-tiered scale. RESULTS: Patients underwent one to four treatment sessions (mean, 3.2). The initial reaction consisted of erythema and minimal swelling. On clinical assessment 2 months after the final treatment, the results were rated excellent by 21 patients (75%) and good by 7 (25%). Nineteen of the 21 were also evaluated 6 to 9 months after final treatment without any significant change in the results. CONCLUSIONS: Fractional ablative photothermolysis using erbium:YAG laser (2,940 nm) is a promising option for skin resurfacing with reduced risk and downtime compared to existing laser methods.

ABSTRACT

BACKGROUND: Phosphatidylcholine injections have been used as a nonsurgical alternative to the surgical removal of undesired fat. Studies in cell culture suggest that sodium deoxycholate (SD) has a detergent action in fat reduction. OBJECTIVE: The objective was to study SD in subcutaneous human tissue. METHODS AND MATERIALS: Thirty volunteers underwent four sessions of subcutaneous abdominal injections of diluted 2.5 or 1% SD stock solutions or placebo. Clinical, hematologic, and ultrasonographic evaluations were performed for 3 months and histology at 3 and 6 months. RESULTS: Both concentrations of SD induced an inflammatory response at the injection site, with dose-dependent adipocyte lysis. Patients reported mild, localized heat, erythema, swelling, and intense pain. Microscopic evaluation revealed necrosis of adipose tissue with adipocyte lysis, fat dissolution, acute lymphomononuclear inflammatory reaction, and intense phagocytosis of fat cells by macrophages. Fibrosis was observed only at the 6-month biopsy. Nodules at the injection sites, compatible with areas of inflammation, were detected by ultrasonography 2 weeks after the first injection. Placebo injections induced no histologic changes. CONCLUSIONS: SD induces an inflammatory reaction in subcutaneous human fat and dose-dependent adipocyte lysis with acute pain and fibrosis. No systemic effects were detected. Further studies will establish the best dose-result ratio, frequency of application, and long-term safety.

ABSTRACT

Fundamentos: O câncer no mundo, atualmente é o de pele. Sua incidência vem aumentando significativamente, porém o número de casos exatos não é conhecido, especialmente no Brasil. Objetivo: Atualização do levantamento estatístico de câncer de pele feito no Serviço de Dermatologia da Faculdade de Medicina do ABC em 1987. Material e Métodos: Em um levantamento de 50.000 prontuários correspondentes ao período de janeiro de 1987 a Dezembro de 1998,realizou-se um estudo retrospectivo de 1040 pacientes que apresentavam 1194 neoplasias malignas de pele, diagnosticadas por exame anatomopatológico e posteriormente submetidas a tratamento cirúrgico no Serviço de Dermatologia da Faculdade de Medicina do ABC. Resultado: O tumor de pele mais encontrado foi o Carcinoma basocelular (CBC) 73,3%, seguido pelo carcinoma espinocelular (CEC) 18,8%, doença de Bowen 4,86% e melanoma 3,0%. Conclusões: Os resultados encontrados foram compatíveis com os relatados na literatura, exceto pelo predomínio do sexo feminino nos casos de carcinoma basocelular em nosso estudo.

ABSTRACT

O hiperparatireoidismo secundário é uma complicação cumum na Insuficiência Renal Crônica e a deficiência de vitamina D tem papel importante para o desenvolvimento dessa enfermidade. O objetivo do presente estudo foi acompanhar prospectivamente no período de doze meses os níveis séricos de paratormônio, cálcio iônico e fósforo nos pacientes em tratamento dialítico. Ao final desse período foram feitas novas dosagens e comparadas com os valores anteriormente obtidos após a administração de calcitriol
