ABSTRACT

Background In the USA, obesity in children and adolescents has become a major public health concern. Childhood obesity has been linked to various cardiometabolic comorbidities all through one's life. Owing to the significant increment in childhood obesity rates, there has been an urgent need for the identification of the correlates and antecedents of adiposity and the cardiometabolic risk to enable early prevention of obesity. As such, the objective of this study is to analyze obesity trends in American children and adolescents from 1999 to 2018 using the National Center for Health Statistics (NCHS) database, as this will enable the identification of various risk factors and early prevention of childhood obesity. Objective This study aimed to comprehensively examine demographic factors impacting obesity prevalence, including gender, age groups (two to five, six to 11, and 12-19 years), race/ethnicity, and poverty level. Methods The study conducted a retrospective analysis using the NCHS database from 1999 to 2018. Utilizing NCHS data, we examined the evolution of obesity prevalence among children and adolescents. The analysis focused on demographic variations, including gender, age, race/ethnicity, and percentage of poverty level. SPSS version 24, a statistical software by IBM Corp. (Chicago, IL, USA), was used for database summarization, graphical representation, and presenting prevalence trends across all participants. Results Temporal trends in obesity prevalence exhibited notable fluctuations from 1999 to 2018. Utilizing NCHS data, the study revealed demographic disparities in age groups, genders, race/ethnicities, and socioeconomic status categories. Gender-based obesity variations persist, with boys consistently surpassing girls in prevalence (17.5% vs. 16%, p = 0.0231). Varied age group patterns emerged, peaking at 18.7% in 12-19 years, 17.7% in six to 11 years, and 11.2% in two to five years. Racially, Hispanic individuals had the highest prevalence (22.8%), followed by Mexican (22.0%) and Black or African American-only individuals (20.6%). White-only individuals showed 14.4%, and Asian-only individuals exhibited the lowest (9.4%). Lower socioeconomic brackets correlate with higher obesity instances, particularly below the 100% poverty level (20%). The 100-199%, 200-399%, and 400% or more categories contributed 18.6%, 16.6%, and 11.6%, respectively. Conclusion Our extensive examination of obesity trends among American children and adolescents from 1999 to 2018, utilizing the NCHS database, provides valuable insights into the complex interplay of demographic factors influencing this public health concern. The study reveals age-specific variations, emphasizing unique challenges during adolescence. Gender disparities, socioeconomic influences, and racial/ethnic impacts are evident, underscoring the need for further study. Our findings present several policy implications regarding the development of interventions aimed at reducing childhood obesity rates in the USA. For instance, the findings indicate the need for policymakers to develop policy interventions aimed at enabling the prevention of obesity during early infancy stages. The findings highlight the need for interventions aimed at reducing the obesity disparities observed between genders and races/ethnic groups. Developing and executing the interventions is prone to considerably reduce the obesity prevalence rates among children and adolescents in the USA.

ABSTRACT

Myocardial bridging (MB) is a congenital coronary artery anomaly involving an overlying myocardium's partial or complete encasement of a coronary artery segment. The obstruction can lead to significant cardiac symptoms, resulting in myocardial ischemia, arrhythmia, and sudden cardiac death. Several approaches, including invasive and non-invasive methods, have been proposed to diagnose and manage MB. Invasive modalities, such as intravascular ultrasound (IVUS) and coronary angiography, offer high specificity and sensitivity. In contrast, non-invasive methods like Doppler ultrasound, multislice computed tomography (MSCT), and magnetic resonance imaging (MRI) are advantageous due to their non-invasive nature, high sensitivity and specificity, and cost-effectiveness. Treatment options for MB mainly focus on relieving symptoms and preventing adverse outcomes. The use of pharmacological agents and surgical and percutaneous interventions has been documented in numerous studies. Studies conclude that MB is a treatable cardiac anomaly, and a combined approach of diagnosis, treatment, and follow-up is necessary to reduce the morbidity and mortality associated with this condition.

ABSTRACT

Hypertension is the most common modifiable risk factor for cardiovascular and cerebrovascular diseases. In the last two decades, the guidelines have evolved tremendously from areas with no recommendations for screening or treatment to targeted recommendations for some at-risk groups. We sought to go through the literature that provided guidelines for the management of hypertension at any point in time over the last 22 years from 2000 to 2022. We searched four databases: PubMed, Embase, Google Scholar, and Cochrane, using specified search terms. The keywords used were "hypertension" and "guidelines." We combined them using the Boolean operators (AND, OR) and searched for articles. A total of 2461 publications were initially identified; 348 publications were excluded after screening for full-text availability. The full-text articles were further filtered based on title and abstract screening. Following this, a total of 1443 articles were excluded. The remaining 670 full-text articles were assessed for eligibility. Of the 670 full-text articles, 480 were excluded based on exclusion criteria, and following the full-text article screening, 190 articles met the final inclusion criteria. Most of these guideline evolutions concerned establishing and adjusting thresholds for the subgroups of the elderly population and patients with diabetic kidney disease, chronic kidney disease, and stroke. Furthermore, the medications of choice are now guided by the stage of disease, presence or absence of comorbidities, and other relevant information, as opposed to ethnicity, which was previously a heavy yardstick for medication choice.

ABSTRACT

Wellens syndrome is a precursor of left anterior descending (LAD) coronary stenosis. It is characterized by biphasic T waves in V2-V3 (type A) or negative deep T waves in V2-V4 (type B). The ability of emergency physicians, hospitalists, or primary care providers to recognize these early ECG patterns is primordial because the definitive treatment is urgent cardiac catheterization with percutaneous coronary intervention. However, failure to identify a type A or type B Wellens syndrome may lead to devastating outcomes, such as myocardial infarction or even death. We presented a clinical case of Wellens' syndrome with deep T waves in V2-V3 associated with COVID pneumonia, pleural effusions, and congestive heart failure that went to a rapid and massive myocardial infarction.

ABSTRACT

Abdominal pain is a very common presentation in the emergency department (ED). The pain is often well-characterized and leads to the diagnosis, but often, the presentation is vague and nonspecific. Superior mesenteric artery (SMA) dissection is a rare cause of abdominal pain that presents with nonspecific epigastric pain and is common in males, middle age, and patients of Asian descent. A high index of suspicion is usually helpful with imaging modalities such as computer tomography (CT) scan and ultrasonography in experienced hands. A prompt diagnosis is vital to managing this disease which may range from non-surgical intervention with supportive therapy to invasive endovascular procedures and surgery. Here, we report a case of an isolated SMA dissection presenting with vague abdominal symptoms and highlight the need to explore the vascular etiology of abdominal pain as their diagnosis is often difficult and may result in irreversible bowel injury when missed.

ABSTRACT

Distinguishing between acute kidney injury and chronic kidney disease (CKD) in an emergency setting may pose a conundrum for physicians, especially when a patient's medical history and records are unknown. Parathyroid hormone (PTH) has proved valuable as a marker of CKD and is frequently assayed for this reason. The use of PTH as a sole marker of CKD may be misleading in certain conditions, and for this reason, physicians need to interpret PTH values with caution. In patients with no existing medical records, it is vital to consider their overall clinical picture, an accurate interpretation of urinalysis and urine microscopy, and the PTH values when making the initial management decisions.
