ABSTRACT

COVID-19 outbreackhas reaffirmed the need to maintain ventilation of dental environments properly. This study was carried out to encourage a reflective analysis of the risks of air contamination in Brazilian dental clinics. A narrative review of the literature was carried out on the recommendations of heating, ventilation and air conditioning (HVAC) systems in dental environments, considering the risks of air contamination in Brazilian dental teaching clinics. The literature research was conductedin the PubMedand Google Scholar databases and the main studiesthat evaluated the climatization systems and portable high efficiency particulate air filtration (HEPA)units in dental environments were included. Furthermore, it were analyzed the guidelines of the Centers for Disease Control and Prevention, the Brazilian National Health Surveillance Agency, the Brazilian Ministry of Health and important institutions of the different countries. After the initial research, a review of the guidelines and articles that assess the use of HEPA units to improve air cleanliness. This analysis was done by different groups of researchers. All documents mentioned that the air quality of the dental offices must follow the protocols in current legislation to ensure the safety of the environs. Theyreaffirmed that the COVID-19 pandemic makes it imperative that dental environments equipped with air conditioning have mechanical air renewal devices. An alternative for Brazilian dental teaching clinics equipped with mini-splits or window air conditioning could be the installation of exhaust fans and portable HEPA filter units to exchange air and reduce aerosols inside the environments (AU).


O surto de COVID-19 reafirmou a necessidade de manter adequadamente a ventilação dos ambientes odontológicos. Este estudo foi realizado para incentivar uma análise reflexiva sobre os riscos de contaminação do ar em clínicas de odontologia. Foi realizada uma revisão narrativa da literatura sobre as recomendações dos sistemas de aquecimento,ventilação e ar-condicionado (AVAC) em ambientes odontológicos, considerando os riscos de contaminação do ar nas clínicas de ensino odontológico brasileiras. A pesquisa bibliográfica foi realizada nas bases de dados PubMed e Google Acadêmico e foram incluídos os principais estudos que avaliaram os sistemas de climatização e unidades portáteis com filtragem de partículas de alta eficiência(HEPA)em ambientes odontológicos. Além disso, foram analisadas as diretrizes do Centro de Controle e Prevenção de Doenças, da Agência Nacional de Vigilância Sanitária, do Ministério da Saúde e de importantes instituições de diferentes países. Após a pesquisa inicial, foi realizada uma revisão das diretrizes e dos artigos que avaliaram o uso de unidades HEPA para melhorar a limpeza do ar. Essa análise foi feita por diferentes grupos de pesquisadores. Todos os documentos mencionaram que a qualidade do ar dos consultórios odontológicos deve seguir os protocolos da legislação vigente para garantir a segurança dos ambientes. Eles reafirmam que a pandemia da COVID-19 torna imprescindível que os ambientes odontológicos, equipados com ar-condicionado, tenham dispositivos mecânicos de renovação de ar. Uma alternativa para as clínicas de ensino odontológicas brasileiras equipadas comminisplitsou ar-condicionado de janela poderia ser a instalação de exaustores e unidades portáteis de filtro HEPA para fazer as trocas de ar e reduzir os aerossóis no interior dos ambientes (AU).

ABSTRACT

BACKGROUND: Neurofibromatosis type 1 (NF1) is an autosomal dominant genetic disorder with a prevalence of 1:3000 births and a wide variety of clinical manifestations. Cutaneous neurofibromas (cNF) are among the most common visible manifestations of NF1 and present a major clinical burden for patients. NF1 patients with cNF often report decreased quality of life, emotional well-being and physical comfort. Developing effective medical therapies for cNF has been identified as a priority for the majority of adults with NF1. METHODS: The study was an open, controlled and prospective proof-of-concept clinical trial. The topical treatment consisted of two steps: cNF microporation using a laser device followed by topical application of one drop of diclofenac 25 mg/mL on the surface of the cNF (T neurofibroma = treatment) or physiological saline (C neurofibroma = control) and reapplied twice daily for 3 days. Neurofibroma assessments included visual and dermatoscopy observations noting color and presence of necrosis, presence of flaccidity, measurements in two dimensions, photographs, and histopathology after excision. The primary efficacy variable was the presence of tissue necrosis. The primary safety variable was the occurrence of treatment-related adverse events. RESULTS: Six patients were included in the study. The treatment resulted in transitory topical changes (healing of the microporation grid with formation of scintillating tissue layer, hyperemia and desquamation), with no statistically significant variation in the dimensions of the T and C neurofibromas in relation to pretreatment measurements. There was no necrosis in the T or C neurofibromas. In the histopathological analysis, there was no significant difference in the distribution of chronic (lymphocytic) inflammatory infiltrate in the papillary reticular dermis (subepithelial), type of infiltrate (diffuse, perivascular, or both), presence of fibrosis, and presence of atrophy among the T and C neurofibromas. No adverse events attributable to the use of diclofenac were reported during the treatment period. CONCLUSIONS: Treatment did not result in significant alterations in terms of presence of tissue necrosis, size, or histopathological features in the T neurofibromas or in comparison to the C neurofibromas. Topical diclofenac with laser microporation was well-tolerated, with no adverse events attributable to diclofenac reported. Whether these observations are due to minimal systemic and neurofibroma exposure remain to be explored in dosage studies with larger patient groups. ABSTRACT

PURPOSE: Urinary antiseptics including methenamine and methylene blue are used in the symptomatic treatment of urinary tract infections (UTIs). PATIENTS AND METHODS: This was a prospective, double-blind, randomized, double-dummy safety and efficacy study of 2 urinary antiseptic combinations in the symptomatic treatment of recurrent cystitis: methenamine 120mg + methylene blue 20mg (Group A) versus acriflavine 15mg + methenamine 250mg + methylene blue 20mg + Atropa belladonna L. 15mg (Group B). All subjects underwent pretreatment urine culture and antibiotic sensitivity tests prior to 3-day oral treatment with study drug, followed by 3 days of antibiotic therapy (based on urine culture) + study drug treatment. Efficacy was evaluated using the Urinary Tract Infection Symptoms Assessment Questionnaire (UTISA). The primary endpoint was the percentage of patients presenting improvement in cystitis manifestations on the UTISA domain "Urination Regularity" at Visit 2. The primary safety variable was the incidence of treatment-related adverse events. RESULTS: A total of 144 subjects were randomized per group and 272 completed the study. Primary endpoint analysis demonstrates homogeneity between treatment groups, with 69.4% and 72.2% subjects, respectively, showing improvement in the score of the urinary regularity UTISA domain after 3 days of treatment (p= 0.87). At Visit 2, incidence of treatment-related adverse events was higher in Group B (Group A: n= 11, Group B: n= 31, p= 0.0057). CONCLUSION: Both treatments were effective in reducing UTI symptoms assessed by UTISA questionnaire after 3 days of treatment. The two regimens were comparable in incidence of adverse events, but the combination of methenamine + methylene blue resulted in fewer treatment-related adverse effects.

ABSTRACT

PURPOSE: We report the results of low back pain treatment using a combination of nucleotides, uridine (UTP), cytidine (CMP) and vitamin B12, vs a combination of vitamins B1, B6, and B12. PATIENTS AND METHODS: Randomized, double-blind, controlled trial, of a 60-day oral treatment: Group A (n=317) receiving nucleotides+B12 and Group B (n=317) receiving B vitamins. The primary endpoint was the percentage of subjects in each group presenting adverse events (AEs). Secondary endpoints were visual analog scale (VAS) pain scores at Visit 2 (day 30) and Visit 3 (day 60) in relation to pretreatment values, Roland-Morris Questionnaire (RMQ) scores and finger-to-floor distance (FFD) (percentage of subjects per group presenting improvement ≥5 points and ≥3cm, respectively). RESULTS: Seventy-five (24%) and 105 (33%) subjects (P=0.21) presented 133 and 241 AEs, with 3159% of subjects presenting ≥2 AEs (P=0.0019) in Group A and Group B, respectively. Twenty-four subjects in Group B were discontinued due to AEs, while no AE-related discontinuations occurred in Group A (P<0.0001). VAS score reduction after 30 and 60 days of treatment was statistically significant (P<0.0001) in both groups, with Group A showing greater reduction at Visit 2 (P<0.0001). RMQ score improvement ≥5 points occurred in 99% of subjects from each group, and FFD improvement ≥3 cm occurred in all subjects. CONCLUSION: Treatment with nucleotides+B12 was associated with a lower number of total AEs, fewer AEs per subject, and no AE-related treatment discontinuation. Pain intensity (VAS) reduction was superior at 30 days of treatment in the nucleotides+B12 group and equivalent between groups at 60 days of treatment. Improvements in efficacy measures RMQ and FFD were observed in both groups at treatment days 30 and 60.

BACKGROUND: Motion sickness can be triggered in a variety of situations and is characterized primarily by nausea and vomiting. Ginger is widely used in treating conditions including chemotherapy-associated gastrointestinal symptoms, morning sickness, postoperative nausea, and motion sickness. OBJECTIVES: The primary study objective was to evaluate Zingiber officinale extract in the treatment of motion sickness. Secondary objectives were to evaluate treatment effect on Motion Sickness Assessment Questionnaire (MSAQ) score and subscores before and after treatment, and to evaluate treatment tolerability. METHODS: Open-label, single-arm study assessing motion sickness outcomes with and without pre-travel oral treatment with Zingiber officinale 160 mg extract (containing 8 mg gingerols). All patients answered the MSAQ on 4 separate occasions following a trip of at least 15 minutes in duration: Trip 1 (pretreatment) and Trips 2, 3, and 4 (after oral treatment with study medication). The primary end point was percentage of patients presenting improvement ≥20 score points on the MSAQ during Trip 2, Trip 3, and Trip 4 in comparison to pretreatment score (Trip 1). Secondary end points included percentage of patients presenting improvement in MSAQ subscores during Trips 2, 3, and 4; percentage of patients presenting treatment-related adverse events; and pre- and posttreatment physician assessment scores. RESULTS: One hundred eighty-four patients were included and 174 completed treatment. A reduction of ≥20 points in total MSAQ score points occurred in 26.52%, 29.89%, and 29.31% of patients from Trips 2, 3, and 4, respectively. There was no significant difference at Trips 2, 3, and 4 in number of patients presenting improvement ≥20 score points (P = 0.9579). There was a significant reduction in total MSAQ scores from Trips 2, 3, and 4 (P < 0.0001) compared with Trip 1. Total MSAQ scores did not vary at each trip taken under treatment (P = 0.28). There were significant (P < .001) improvements in all domain subscores from Trips 2, 3, and 4 in relation to scores from Trip 1. There was a significant improvement in physician assessment scores at Visit 2 (P < .0001). Adverse events were reported among 31 patients, mainly affecting the gastrointestinal system. Twenty-four patients (13.04%) reported 39 adverse events considered related to treatment. No significant change in physical exam was noted at Visit 2 in relation to Visit 1. CONCLUSIONS: These open label, historically controlled study results suggest the need for randomized, blinded, placebo and active substance controlled clinical trials. BACKGROUND: Phlebotonics have beneficial effects on some symptoms related to chronic venous disease (CVD) of the lower limbs. The most commonly used one is diosmin, available in a pure semisynthetic form or as a micronized purified flavonoid fraction. Patients and Methods. The primary objective of this single-blind, randomized, parallel-group, prospective study was to assess the clinical noninferiority of nonmicronized diosmin 600 mg once daily (D-group) compared to micronized diosmin 900 mg plus hesperidin 100 mg once daily (D/H-group) over a 6-month treatment period. Adult patients with a symptomatic CVD of the lower limbs (C0-C3 grade; 20-60 mm on a 100 mm visual analog scale (VAS)) were included. The primary endpoint was the change (from baseline to last postbaseline value) of the intensity of the lower-limb symptoms on VAS. RESULTS: 114 patients (mean age, 44.4 years; women, 90.4%) were randomized in the per-protocol analysis (D-group, n = 57; D/H-group, n = 57; D/H-group, p < 0.0001) in the D-group and -22.8 mm (p < 0.0001) in the D-group and -22.8 mm (p < 0.0001) in the D-group and -22.8 mm (. CONCLUSION: Nonmicronized diosmin 600 mg was proven to have a noninferior efficacy compared to micronized diosmin 900 mg plus hesperidin 100 mg, associated with greater ease in swallowing the tablet.

INTRODUCTION: Neurofibromatosis type 1 (NF1) is one of the most common autosomal dominant genetic disorders. Some clinical manifestations are present at birth, while some develop during childhood, and others can occur at any age. Given the early age at which patients develop clinical features, diagnosis is often made during childhood. The most prevalent features of NF1 are café au lait spots, dermal and plexiform neurofibromas, and learning disability. A variety of skeletal problems may be seen in NF1, including scoliosis, short stature, and pseudoarthrosis. Reduced skeletal bone mass has been documented to be a common phenomenon in children and adults with NF1. Decreased serum 25-hydroxyvitamin D (vitamin D) levels have been noted in adults and children with NF1 and have been reported to be inversely correlated with the number of dermal neurofibromas in adults. However, the actual correlation of vitamin D level to bone density and dermal neurofibroma number in children with NF1 remains unclear. OBJECTIVES: The primary objective of this study was to evaluate vitamin D levels among children and adolescents with NF1. The secondary objective was to describe the levels of vitamin D among children and adolescents with NF1, to verify in which age group there is a higher frequency of vitamin D alterations, and to explore vitamin D level correlations between age, gender, sun exposure, number of neurofibromas, and number of plexiform neurofibromas. METHODS: This was an observational, cross-sectional, hospital-based study. We obtained a convenience sample of individuals with confirmed diagnosis of NF1 from patients attending the Medical Genetics Service of the IPPMG-UFRJ and Santa Casa de Misericórdia of Rio de Janeiro over a 24-month period. We evaluated vitamin D levels in blood samples of patients with NF1 by a chemiluminescent immunoassay method, and we correlated the results with gender, age, number of neurofibromas, number of plexiform neurofibromas, and satisfactory sun exposure. RESULTS: Of the 55 patients, 28 (50.9%) were female and 27 (49.1%) were male. Patient ages ranged from a minimum of 1.2 to a maximum of 19.6 years (mean age 10.95 years) and the median was 11.11 years. Median and mean body mass index (BMI; z score) were -0.09 (minimum value -1.63 and maximum of 4.62) and 0.16, respectively. The mean value of vitamin D was 30.82 ng/mL (±12.31) and the median was 29 ng/mL (minimum value of 10.40 ng/mL and maximum of 79.19 ng/mL). CONCLUSIONS: The levels of vitamin D did not differ according to gender, age group, or the presence or number of cutaneous neurofibromas. Among patients with adequate sun exposure, there was a higher incidence of sufficient serum vitamin D levels. Patients with cutaneous neurofibromas in the 0 to 11 age group had a greater tendency to vitamin D sufficiency in relation to patients aged 11 to 19 years.

ABSTRACT

Human African trypanosomiasis (HAT) caused by the protozoan Trypanosoma brucei gambiense and Trypanosoma brucei rhodesiense, and transmitted by the tsetse fly (genus Glossina), affects 36 Sub-Saharan African countries with considerable public health impact. Despite approximately 15,000 infected individuals and 70 million at risk, in recent years the World Health Organization has mentioned removal of HAT from the list of Neglected Tropical Diseases by 2020, due to the decrease in cases over the last two decades. When untreated, the disease presents high lethality rates and the available treatments are complicated to administer, highly toxic, and do not guarantee cure, especially in the advanced stages of the disease. Further, there is no prospect for vaccine development in the near future. The present review compiles information on the history of the clinical aspects of HAT, as well as its epidemiology, diagnosis, therapy, and prophylaxis, as well as updating information on the current panorama and perspectives regarding the disease.

ABSTRACT

Case report of Community-acquired pneumonia in a male patient without co-morbidities. Empiric antibiotic treatment did not resolve the clinical picture of productive cough, and a chest computerized tomography and sputum culture with antibiogram were performed, identifying S. maltophilia infection with sensitivity to levofloxacin and sulfamethoxazole/trimethoprim. Treatment with levofloxacin (500 mg/day for 15 days) resulted in resolution of the clinical picture.

ABSTRACT

Meningococcal disease is the acute infection caused by Neisseria meningitidis, which has humans as the only natural host. The disease is widespread around the globe and is known for its epidemical potential and high rates of lethality and morbidity. The highest number of cases of the disease is registered in the semi-arid regions of sub-Saharan Africa. In Brazil, it is endemic with occasional outbreaks, epidemics and sporadic cases occurring throughout the year, especially in the winter. The major epidemics of the disease occurred in Brazil in the 70's caused by serogroups A and C. Serogroups B, C and Y represent the majority of cases in Europe, the Americas and Australia. However, there has been a growing increase in serogroup W in some areas. The pathogen transmission happens for respiratory route (droplets) and clinically can lead to meningitis and sepsis (meningococcemia). The treatment is made with antimicrobial and supportive care. For successful prevention, we have some measures like vaccination, chemoprophylaxis and droplets' precautions. In this review, we have described and clarify clinical features of the disease caused by N. meningitidis regarding its relevance for healthcare professionals.

ABSTRACT

CONTEXT: This paper reports on the results of treatment of compressive neuralgia using a combination of nucleotides (uridine triphosphate trisodium [UTP] and cytidine monophosphate disodium [CMP]) and vitamin B12. OBJECTIVES: To assess the safety and efficacy of the combination of nucleotides (UTP and CMP) and vitamin B12 in patients presenting with neuralgia arising from neural compression associated with degenerative orthopedic alterations and trauma, and to compare these effects with isolated administration of vitamin B12. METHODS: A randomized, double-blind, controlled trial, consisting of a 30-day oral treatment period: Group A (n=200) receiving nucleotides + vitamin B12, and Group B (n=200) receiving vitamin B12 alone. The primary study endpoint was the percentage of subjects presenting pain visual analog scale (VAS) scores ≤20 at end of study treatment period. Secondary study endpoints included the percentage of subjects presenting improvement ≥5 points on the patient functionality questionnaire (PFQ); percentage of subjects presenting pain reduction (reduction in VAS scores at study end in relation to pretreatment); and number of subjects presenting adverse events. RESULTS: The results of this study showed a more expressive improvement in efficacy evaluations among subjects treated with the combination of nucleotides + vitamin B12, with a statistically significant superiority of the combination in pain reduction (evidenced by VAS scores). There were adverse events in both treatment groups, but these were transitory and no severe adverse event was recorded during the study period. Safety parameters were maintained throughout the study in both treatment groups. CONCLUSION: The combination of uridine, cytidine, and vitamin B12 was safe and effective in the treatment of neuralgias arising from neural compression associated with degenerative orthopedic alterations and trauma.

Objective: To assess results of the finger-to-floor distance (FFD) and the Schober test performed during the DOLOR study, and to verify correlation between Visual Analog Pain Scale scores (VAS) with these measures. Research design and methods: Previously tabulated data from the Clinical Research Forms of the DOLOR study were analyzed (statistical significance defined with a two-tailed p value < 0.05 and confidence interval of 95%). For continuous variables, the Student's T- test or analysis of variance (ANOVA) was used, and differences between pre-treatment and Visits 2, 3, and 4 in the absolute number and percentage of patients with no change, improvement, or worsening of the Schober test and the FFD test scores were calculated, and the results were analyzed with the Chi-squared test. Spearman non-parametric correlation was used for correlating VAS scores with FFD and Schober test scores at each study visit. Main outcome measures: FFD, measured in centimeters; Schober test scores. Results: Throughout the treatment period, there was a statistically significant correlation between the VAS scores and the FFD in the total patient population and within treatment groups. This was not observed for the correlation between the Schober's test scores and the VAS scores. FFD scores within treatment groups improved progressively at each study visit, as did the Schober Test scores. Conclusions: The results of this post-hoc analysis show that combination therapy with diclofenac plus vitamins B1, B6, and B12 had additional positive effects on mobility restoration among the patients of the DOLOR study and serve to highlight the correlation between mobility and pain intensity among patients presenting with low back pain. The two fundamental goals of low back pain therapy are to provide improvements in pain and function. In this sense, the combination of diclofenac with the B vitamins was particularly effective in achieving both of these goals.

Chronic Venous Disorder (CVD) is a term used to represent all abnormal clinical changes that result from venous disease of the lower extremities, and that have a chronic pattern. This disease has a documented socioeconomic impact, involving a significant part of the western populations, and consuming 2-3% or more of societies? health budgets. This review of the literature focuses on diosmin, a benzopyrone phlebotonic, specifically mechanisms of action as well as preclinical and clinical evidence.

ABSTRACT

This is a qualitative-quantitative study based on hospital records of female patients of reproductive age, presenting sexual dysfunction, and treated with 250 mg Tribulus terrestris extract (1 tablet thrice daily for 90 days). Safety monitoring included vital signs, physical examination, laboratory tests, and occurrence of adverse events. Efficacy analysis included results of the Female Sexual Function Index (FSFI), dehydroepiandrosterone (DHEA) levels together with total and free testosterone, and the patient and physician assessments. There was a statistically significant improvement in total FSFI scores (P < 0.0001) post-treatment, with improvement among 106 (88.33%) subjects. There was a statistically significant (P < 0.0001) increase in the level of DHEA, while the levels of both serum testosterone (P = 0.284) and free testosterone decreased (P < 0.0001). Most adverse events recorded were related to the gastrointestinal tract. Physical examination showed no significant changes post-treatment. Based on the results, it is concluded that the T. terrestris extract is safe and effective in the treatment of female sexual dysfunction.

ABSTRACT

Herpes é uma infecção causada por dois vírus da família Herpesviridae (herpes simples tipos 1 e 2; HSV-1 e HSV-1), que apresenta curso clínico variável e para o qual atualmente não existe cura. As manifestações da infecção por HSV-1 incluem herpes simples orofacial primário e recorrente, enquanto as do HSV-2 em geral ocorrem na forma de herpes simples genital, embora casos de lesões genitais pelo HSV-1 e orais pelo HSV-2 possam ocorrer. As infecções pelo vírus herpes simples (HSV-1 e HSV-2) representam as doenças sexualmente transmissíveis mais comuns a nível global, alcançando uma soroprevalência de 80% em adultos. Nesta revisão da literatura, abordaremos os aspectos clínicos da infecção pelo HSV, incluindo a epidemiologia, etiologia, manifestações clínicas, métodos diagnósticos e tratamento, bem como uma breve descrição da imunogenética da infecção pelo HSV


Herpes is an infection caused by two viruses in the Herpesviridae family (herpes simplex types 1 and 2; HSV-1 and HSV-2), which presents a variable clinical course and for which there is currently no cure. The manifestations of HSV-1 infection include primary and recurrent orofacial herpes simplex, while HSV-2 infection usually manifests in the form of genital herpes simplex, although cases of genital lesions from HSV-1 infection and oral lesions form HSV-2 infection can occur. Infections by the herpes simplex virus (HSV-1 and HSV-2) represent one of the most common sexually transmitted diseases globally, reaching a serum prevalence of 80% in adults. In this review of the literature, we discuss the clinical aspects of HSV infection, including epidemiology, etiology, clinical manifestations, diagnosis and treatment, as well as a brief description of the immunogenetics of HSV infection.

ABSTRACT

Vacuoles of yeast Saccharomyces cerevisiae are functionally analogous to mammalian lysosomes. Both are cellular organelles responsible for macromolecular degradation, ion/pH homeostasis, and stress survival. We hypothesized that undefined gene functions remain at post-endosomal stage of vacuolar events and performed a genome-wide screen directed at such functions at the late endosome and vacuole interface - ENV genes. The immunodetection screen was designed to identify mutants that internally accumulate precursor form of the vacuolar hydrolase carboxypeptidase Y (CPY). Here, we report the uncovering and initial characterizations of twelve ENV genes. The small size of the collection and the lack of genes previously identified with vacuolar events are suggestive of the intended exclusive functional interface of the screen. Most notably, the collection includes four novel genes ENV7, ENV9, ENV10, and ENV11, and three genes previously linked to mitochondrial processes - MAM3, PCP1, PPE1. In all env mutants, vesicular trafficking stages were undisturbed in live cells as assessed by invertase and active α-factor secretion, as well as by localization of the endocytic fluorescent marker FM4-64 to the vacuole. Several mutants exhibit defects in stress survival functions associated with vacuoles. Confocal fluorescence microscopy revealed the collection to be significantly enriched in vacuolar morphologies suggestive of fusion and fission defects. These include the unique phenotype of lumenal vesicles within vacuoles in the novel env9Δ mutant and severely fragmented vacuoles upon deletion of GET4, a gene recently implicated in tail anchored membrane protein insertion. Thus, our results establish new gene functions in vacuolar function and morphology, and suggest a link between vacuolar and mitochondrial events.

ABSTRACT

As infecções causadas pelos vírus herpes simplex 1 e 2 representam uma das doenças sexualmente transmissíveis mais comuns em nível global e apresentam um amplo espectro de acometimento clínico. A Uncaria tomentosa é uma planta popularmente conhecida como unha-de-gato, cujo sextratos são amplamente utilizados por suas propriedades farmacológicas, que influenciam com ações benéficas nas alergias, bem como possuem ação antiinflamatória,antimicrobiana e antioxidante. Objetivo: o objetivo geral foi avaliar o uso tópico do extrato de Uncaria tomentosa na forma de gel 50 mg/gcomo adjuvante no tratamento e controle das lesões orais e genitais decorrentes da infecção com herpes simplex. Os objetivos específicos foram avaliara eficácia e segurança do uso tópico do extrato como adjuvante no tratamento e controle das lesões orais e genitais decorrentes da infecção com herpessimplex, através de avaliações clínicas das lesões herpéticas e questionários realizados pelo paciente e pelo médico investigador; exame físico; incidênciade eventos adversos; incidência de alteração de parâmetros laboratoriais. Métodos: estudo clínico em pacientes apresentando lesões decorrentes de infecção por HSV-1 ou HSV-2, tratados com o extrato de Uncaria tomentosa 50 mg/g na forma de gel tópico, durante um mínimo de 5 dias e um máximo de 10dias. As avaliações de eficácia foram realizadas a partir de dados coletados antes do tratamento (pré-tratamento), após 5 dias de tratamento (visita 2), e após 10 dias de tratamento (visita final). As medidas primárias de eficácia incluíram a cura (definida como a perda da crosta dura) das lesões e a presençade dor e prurido nas lesões. As medidas secundárias de eficácia incluíram a presença de edema e/ou rubor nas lesões, e a incidência de desenvolvimento denovas lesões durante o período de tratamento, bem como a Avaliação do Médico, a Avaliação do Paciente e a Avaliação Global de Eficácia. A tolerabilidade clínica geral e a segurança foram analisadas pela incidência de todos os eventos adversos; pelos eventos adversos sérios e/ou hospitalizações; e síndromesde abstinência devidas a qualquer evento adverso; pela incidência de alteração clinicamente significativa nos exames laboratoriais realizados, bem como pela Avaliação Global de Segurança. Os resultados foram comparados para cada avaliação (pré-tratamento, avaliação 2, avaliação 3). Resultados: umtotal de 150 pacientes foi incluído na análise. Após 5 dias de tratamento (visita 2), 45 (30%) pacientes apresentavam cura total de todas as lesões, sendo considerados como sucesso terapêutico. Ao final de 10 dias e tratamento, 69 (68,3%) dos 101 pacientes que permaneceram no tratamento apresentavam cura total de todas as lesões. Foi observada uma redução estatisticamente significativa na porcentagem de pacientes apresentando dor quanto na visita 2(p < 0,0001) tanto na visita 3 (p < 0,0001). Também foram observadas reduções estatisticamente significativas (p < 0,0001 para todos) na porcentagem de pacientes apresentando prurido, eritema e ardência na visita 2 e na visita 3. Na visita 3 foi observada uma redução estatisticamente significativa (p =0,0032) na porcentagem de pacientes apresentando rubor. Não houve alteração nos sinais vitais registrados na visita 2 e visita 3 em relação aos valores prétratamento.Eventos adversos (EAs) foram registrados entre dez pacientes, sendo estes de intensidade branda ou moderada e transitórios. Na Avaliação do Paciente realizada durante a visita final, 70 pacientes (69,3%) responderam à avaliação com pontuação de 8-10. Na Avaliação do Médico realizada durantea visita final, 66 pacientes (65,3%) foram avaliados com 8-10 pontos. Na Avaliação Global de Eficácia realizada pelo médico investigador, a eficácia do medicamento foi avaliada como Muito Bom em 57 pacientes (38,3%). Na Avaliação Global de Segurança realizada pelo médico investigador ao final do período de tratamento, a tolerabilidade do medicamento foi considerada Muito Boa em 81 (54,4%) dos pacientes. Conclusão: com base nos resultados obtidos nesta avaliação, concluímos que o extrato de Uncaria tomentosa na forma de gel 50 mg/g foi eficaz e seguro no tratamento e controle das lesões orais e genitais decorrentes da infecção com HSV-1 e HSV-2, na população de pacientes avaliada.

ABSTRACT

The primary objective of this double-blind, randomized, controlled clinical trial was to assess the use of azithromycin dihydrate in oral suspension form in the treatment of impetigo in children. The secondary objectives were to compare the efficacy and safety of two presentations of azithromycin dihydrate in the treatment of impetigo in children, on wound healing and on wound pruritus. After screening and obtaining informed consent of the parents or legal guardians, a total of 100 patients ranging in age from 2-8 years old and presenting impetigo were randomized to one of two groups for a 3-day treatment period using azithromycin dihydrate in oral suspension in single doses of 10mg/kg/day: Group A (manufactured by Merck S.A.), and Group B (manufactured by Pfizer). Patients returned to the study center at the end of the 3-day treatment (Visit 2) and 7 days after the Pretreatment visit (Visit 3) for efficacy assessments and safety monitoring. Pretreatment demographic data and impetigo characteristics (type, location, number of lesions, pruritus) were homogenous between treatment groups. At the end of the study, all patients in both groups presented either ?improvement? or ?cured? lesions, with the majority (72.9%) of the patients presenting ?cured? lesions. We observed a statistically significant decrease in pruritus severity at Visit 2 and Visit 3 in relation to pretreatment, with no significant between-group difference at either study visit. Reported adverse events were transient and mild-to moderate in severity in both treatment groups, with no serious adverse events reported during the study. Based on the data collected during this study, we conclude that the two presentations of azithromycin were safe and effective in the treatment of impetigo in the population evaluated.

JUSTIFICATIVA E OBJETIVOS: A tosse persistente representa uma condição comum e causa frequente de procura por atendimento médico, podendo alterar a rotina diária do paciente afetado. O objetivo deste estudo foi avaliar a resposta clínica e laboratorial à terapia de tosse persistente, utilizando uma combinação de eucaliptol, gomenol, guaifenesina, cansilato de sódio e cloridrato de lidocaína.MÉTODO: Foram obtidos dados referentes a 60 pacientes atendidos no Serviço de Pneumologia do Hospital das Clínicas de Teresópolis Constantino Otaviano, incluindo informações demográficas, resultados de exames físicos, laboratoriais e de espirometria, avaliações do médico e do paciente, bem como uma avaliação através do Cough-specific Quality of Life Questionnaire (CQLQ) que foram analisadas antes, durante e após sete dias de terapia. RESULTADOS: Não houve alterações clinicamente significativas nos exames físicos. Os exames laboratoriais e testes de espirometria permaneceram dentro das respectivas faixas de normalidade e nenhuma diferença significativa nos valores entre as avaliações foi observada. Vinte e nove eventos adversos foram registrados em 22 pacientes, todos com gravidade leve a moderada. As pontuações das avaliações de condição geral do médico e do paciente melhoraram significativamente na Avaliação 3 em relação à Avaliação 1 (Qui-quadrado = 121,1; GL= 16; p < 0,0001, e Qui-quadrado = 61,86; GL = 16; p < 0,0001, respectivamente). As pontuações no CQLQ da Avaliação 3 melhoraram de forma significativa em relação às da Avaliação 1 (t = 6,746; GL = 58; p < 0,0001), indicando melhora substancial na qualidade de vida.CONCLUSÃO: Com base nos resultados obtidos, concluiu-se que a combinação de eucaliptol, gomenol, guaifenesina, cansilato de sódio e cloridrato de lidocaína foi segura e eficaz em pacientes com processos pulmonares produtivos acompanhados de tosse, com impacto positivo na qualidade de vida dos pacientes tratados.


BACKGROUND AND OBJECTIVES: Persistent cough represents a common ailment for which medical attention is often sought, and may cause significant disruption of the affected patient's routine. The objective of this study was to evaluate clinical and laboratory response to persistent cough therapy using a combination of eucalyptol, gomenol, guaifenesin, sodium camsilate, and lidocaine hydrochloride. METHOD: We collected data from 60 patients attended at the Pneumology Service of the Hospital de Clínicas de Teresópolis Constantino Otaviano, including demographic information, physical exams, laboratory, and spirometry exam results, physician and patient evaluations, and a quality-of-life assessment (CQLQ) which were analyzed before, during, and after seven days of therapy. RESULTS: There were no clinically significant changes regarding physical exams. Laboratory exams and spirometry tests remained within the respective normal ranges, and no statistically significant difference in values among study assessments was observed. Twenty-nine adverse events were recorded among 22 patients, all of which were mild to moderate in severity. The scores of the physician and patient evaluation of overall condition improved significantly in Assessment 3 in relation to Assessment 1 (Chi-square = 61.86; Degree of Freedom = 16; p < 0.0001 and Chi-square = 121.1; DF= 16; p < 0.0001, respectively). CQLQ total scores from Assessment 3 improved significantly in relation to those of Assessment 1 (t = 6.746; DF = 58; p < 0.0001), indicating a substantial improvement in quality-of-life. CONCLUSION: Based on the results of this analysis, we conclude that the combination of eucalyptol, gomenol, guaifenesin, sodium camsilate, and lidocaine hydrochloride, was safe and effective in patients with productive pulmonary processes accompanied by cough, positively impacting quality-of-life of the treated patients.
