ABSTRACT

The present study analyzed trends in premature mortality from Noncommunicable diseases (NCDs) between 1990 and 2019, the projections up to 2030, and the risk factors (RFs) attributable to these diseases in the Community of Portuguese Language Countries (CPLP). Estimates from the Global Burden of Disease (GBD) study and the analysis of the burden of premature mortality due to NCDs were used for nine CPLP countries, applying age-standardized rates, using RStudio. Portugal, Brazil, Equatorial Guinea, Angola, and Guinea Bissau showed declining premature mortality rates caused by NCDs, while East Timor, Cape Verde, São Tomé and Príncipe, and Mozambique showed an increase in rates. Projections indicate that none of the countries is expected to achieve the goals of reducing premature mortality due to NCDs by one third by 2030. The attributable burden of disease showed that the most important RFs in 2019 were: high systolic blood pressure (SBP), tobacco, dietary risks, high body mass index (BMI), and air pollution. It can therefore be concluded that there are profound differences in the burden of NCDs among the countries, with better results in Portugal and Brazil, and that no CPLP country is likely to reach the NCD reduction target by 2030.


Foram analisadas tendências da mortalidade prematura por doenças crônicas não transmissíveis (DCNT) entre 1990 e 2019, as projeções até 2030 e os fatores de risco atribuíveis a estas doenças na Comunidade dos Países de Língua Portuguesa (CPLP). Utilizou-se estimativas do estudo Carga Global de Doenças e análise da carga de mortalidade prematura por DCNT para nove países da CPLP, utilizando taxas padronizadas por idade, usando-se RStudio. Conclui-se pelas profundas diferenças na carga de DCNT entre os países, com melhores resultados em Portugal e Brasil e que nenhum país do CPLP deverá atingir a meta de redução das DCNT até 2030.

ABSTRACT

OBJECTIVE: To describe the indicators of smoking cessation in 2013 and 2019 for Brazil and federative units, according to sociodemographic variables, collected in the National Health Survey (PNS). METHODS: Cross-sectional, population-based and descriptive study with data from the 2013 and 2019 PNS, a household survey collected by trained interviewers. The prevalence of ex-smokers and the proportion of smokers who tried to quit smoking in the 12 months prior to the interview, and respective confidence intervals (95%CI) were calculated, according to sociodemographic variables. Additionally, the percentage variation between the years was calculated. RESULTS: In 2013, the prevalence of ex-smokers was 17.5% (95%CI 16.9;18.0) and, in 2019, 26.6% (95%CI 26.1;27.2). In 2013, 51.1% tried to quit smoking (95%CI 49.3;52.9) and, in 2019, 46.6% (95%CI 45.0;48.3). CONCLUSION: It is important to strengthen and maintain strategies for coping with tobacco use in Brazil, to increase the current smoker's willingness and ability to quit smoking.

OBJECTIVE: To describe the prevalence of smoking indicators among Brazilian students according to sociodemographic characteristics in 2019, and compare the prevalence between 2015 and 2019. METHODS: Data from the National Survey of School Health 2015 and 2019 were used. Indicators related to tobacco use were analyzed. Indicators were compared between the 2015 and 2019 editions. Prevalence and respective 95% Confidence Interval (95%IC) were calculated for the total population and according to sex, age group and type of school. RESULTS: 22.6% (95%CI 21.7-23.4) of the students had tried any cigarette and it was higher between 16 and 17 years of age (32.6%; 95% CI 31.4-33, 8) and in males (35.0%; 95%CI 33.6-36.4). The experimentation of hookah, electronic cigarette and other tobacco products are also high, with 26.9% (95%CI 26.0-27.8), 16.8% (95%CI 16.2-17.4) and 9.3% (95%CI 8.8-9.8), respectively, being higher among boys aged 16 to 17 years. It is noteworthy that there were no changes in the indicators of cigarette experimentation, smoking for the first time before the age of 13, smoking in the 30 days prior to the survey, and at least one of the smoking parents. CONCLUSION: Although smoked tobacco indicators are stable between 2015 and 2019, the high prevalence of experimentation with products such as hookah and electronic cigarettes is highlighted, drawing attention to the need for new regulatory measures.


OBJETIVO: Descrever a prevalência de indicadores do tabagismo entre escolares brasileiros segundo características sociodemográficas em 2019 e comparar as prevalências entre 2015 e 2019. MÉTODOS: Utilizaram-se dados da Pesquisa Nacional de Saúde do Escolar de 2015 e 2019. Foram analisados os indicadores referentes ao uso do tabaco, que foram comparados entre as edições de 2015 e 2019. Foram calculadas as prevalências e os respectivos intervalos de confiança de 95% (IC95%) para a população total e segundo sexo, faixa etária e tipo de escola. RESULTADOS: Dos escolares, 22,6% (IC95% 21,7-23,4) já experimentaram cigarro alguma vez, porcentagem mais elevada entre os de 16 a 17 anos de idade (32,6%; IC95% 31,4-33,8) e no sexo masculino (35,0%; IC95% 33,6-36,4). A experimentação de narguilé, cigarro eletrônico e outros produtos do tabaco também se mostra elevada, com 26,9% (IC95% 26,0-27,8), 16,8% (IC95% 16,2-17,4) e 9,3% (IC95% 8,8-9,8), respectivamente, sendo mais alta entre os escolares do sexo masculino de 16 a 17 anos. Destaca-se que não houve mudanças nos indicadores "experimentação do cigarro", "fumar pela primeira vez antes dos 13 anos", "fumar nos 30 dias anteriores à pesquisa" e "ter ao menos um dos pais fumantes" entre os anos indicados. CONCLUSÃO: Embora os indicadores de tabaco fumado estejam estáveis entre 2015 e 2019, destacam-se as elevadas prevalências de experimentação de produtos como narguilé e cigarro eletrônico, que chamam a atenção para a necessidade de novas medidas regulatórias.

ABSTRACT

This study compared indicators of care and access to health services by adults who self-reported hypertension in 2013 and 2019, analyzing those indicators according to gender, age group, schooling level, and race/color. This is an analytic study with data from the Brazilian National Health Survey (PNS), conducted in 2013 and 2019 in Brazil. The indicators to care and access to health services by individuals with arterial hypertension in both surveys were compared. For 2019, those indicators were analyzed according to sociodemographic characteristics. This study estimated the proportions, prevalence ratio (PR), and their respective 95% confidence intervals (95%CI). In total, 60,202 individuals were evaluated in 2013 and 88,531 in 2019, of these 24.4% reported arterial hypertension in 2013 and 23.9% in 2019. Women received more medical care for hypertension within the last year (PR = 1.07; 95%CI: 1.04; 1.11), had the last physician appointment at an basic health unit (PR = 1.11; 95%CI: 1.05; 1.17) than men. About race/color, black people had more hospitalization for hypertension or some complication (PR = 1.2; 95%CI: 1.05; 1.38) and intense or very intense degree of limitation in performing daily activities (PR = 1.37; 95%CI: 1.06; 1.76). In 2019, inequalities were evidenced and worse indicators were observed for males, black, with low education and young age. Therefore, investments in the Brazilian Unified National Health System, as well as public policies and strategic actions are essential to reduce inequalities, promote health care.

RESUMO Objetivo: descrever as prevalências de fatores de risco e de proteção para as doenças crônicas não transmissíveis (DCNT) em escolares brasileiros no ano de 2019 e compará-las às de 2015. Método: estudo transversal realizado com dados da Pesquisa Nacional de Saúde do Escolar (PeNSE) de 2015 e 2019. Para 2019, estimaram-se as prevalências e os intervalos de confiança de 95% (IC95%) dos indicadores alimentação, atividade física e comportamento sedentário e uso de drogas lícitas e ilícitas, considerando o sexo, a dependência administrativa da escola e a Unidade da Federação. Para comparar esses indicadores com o ano de 2015, considerouse a população total. Resultados: ao comparar 2015 com 2019, observou-se uma redução do consumo de frutas (2015: 30,9% - IC95% 29,6-32,3; 2019: 26,9% - IC95% 26,3-27,6), refrigerante (2015: 27,2% - IC95% 25,6-28,9; 2019: 17,2% - IC95% 16,6-17,8), guloseimas (2015: 40,6% - IC95% 39,0-42,1; 2019: 32,8% - IC95% 32,1-33,4) e de atividade física (2015: 31,6% - IC95% 30,1-33,2; 2019: 28,1% - IC95% 27,4-28,8); por outro lado, foi observado um aumento da embriaguez (2015: 27,2% - IC95% 25,4-28,9; 2019: 47,0% - IC95% 46,0-47,9). Conclusão: ao comparar as edições de 2015 e 2019 da Pesquisa, perceberam-se mudanças nas prevalências de fatores de risco e de proteção para as DCNT. Esses resultados reforçam a importância das estratégias e ações para promoção da saúde dos adolescentes, especialmente por ser um grupo em fase de grandes transformações e psicobiológicas e sociais.


RESUMEN Objetivo: describir la prevalencia de los factores de riesgo y protección de las enfermedades crónicas no transmisibles (ECNT) en los e studiantes brasileños en 2019 y compararla con la de 2015. Método: est udio transversal con datos de la Encuesta Nacional de Salud Escolar de 2015 y 2019. Se estimó la prevalencia y los inter valos de conf ian za del 95% (IC95%) de los indicadores de alimentación, actividad física y comportamiento sedentario y consumo de drogas lícitas e ilícitas, según sexo, dependencia administrativa del centro escolar y Unidad Federativa para 2019. Para la comparación de estos indicadores con el año 2015, se consideró la población total. Resultados: al comparar 2015 con 2019, se observó una reducción en el consumo de frutas (2015: 30,9% - IC95% 29,6-32,3; 2019: 26,9% - IC95% 26,3-27,6), refrescos (2015: 27,2% - IC95% 25,6-28,9; 2019: 17,2% - IC95% 16,6-17,8), golosinas (2015: 40,6% - IC95% 39,0-42,1; 2019: 32,8% - IC95% 32,1-33,4), la actividad física (2015: 31,6% - IC95% 30,1-33,2; 2019: 28,1% - IC95% 27,4-28,8) y el aumento de la embriaguez (2015: 27,2% - IC95% 25,4-28,9; 2019: 47,0% - IC95% 46,0-47,9). Conclusión: hubo cambios en la prevalencia de los factores de riesgo y protección de las ECNT al comparar las ediciones de 2015 y 2019 de la Encuesta. Estos resultados refuerzan la importancia de las estrategias y acciones de promoción de la salud de los adolescentes, sobre todo porque se trata de un grupo que experimenta grandes transformaciones psicobiológicas y sociales.

Conclusion: when comparing the 2015 and 2019 edition s of the Sur vey, changes were perceived in the prevalence value s of ri sk and protect ion factors for CNCDs. These results reinforce the importance of the strategies and action s to promote adolescents' health, especially for being a group undergoing a pha se marked by major psychobiological and social transformations.

ABSTRACT

RESUMO Objetivo: analisar os indicadores de consumo e exposição a bebidas alcoólicas entre escolares brasileiros em 2019 e compará-los aos de 2015. Método: estudo transversal com dados da Pesquisa Nacional de Saúde do Escolar (PeNSE), realizadas em 2015 e 2019. Em 2019, analisaram-se os indicadores referentes ao consumo e à exposição a bebidas alcoólicas, estratificadas por sexo, faixa etária, dependência administrativa, unidades da federação e região geográfica. Estimou-se as prevalências e os respectivos intervalos de 95% de confiança (IC 95%). Resultados: houve aumento na experimentação de bebidas alcoólicas antes de 13 anos (30,6% em 2015 para 34,6% em 2019); sofrer embriaguez na vida (27,2% em 2015 para 47,0% em 2019) e ter problemas com amigos devido ao consumo de bebidas alcóolicas (9,3% em 2015 para 15,7% em 2019). Todos os indicadores foram mais prevalentes entre meninas, exceto beber em binge e episódios de embriagues, que não tiveram diferenças entre os sexos, bem como foram mais elevadas entre estudantes mais velhos. Os episódios de embriaguez e ter amigos que ingerem bebida alcoólica foram mais prevalentes entre escolares de escolas públicas, enquanto o consumo de bebidas alcoólicas pelos pais e ter tido problemas com suas famílias ou amigos devido ao consumo de bebidas alcoólicas foram mais elevados em estudantes de escolas privadas. Conclusão: evidenciaram-se elevadas prevalências de experimentação, consumo e exposição a bebidas alcoólicas, mostrando que grande parcela dos adolescentes brasileiros se encontra exposta a uma carga evitável de morbimortalidade decorrente do consumo e exposição ao álcool.


RESUMEN Objetivo: analizar los indicadores de consumo y exposición a bebidas alcohólicas entre los estudiantes brasileños en 2019 y compararlos con los de 2015. Método: estudio transversal con datos de la Encuesta Nacional de Salud Escolar (PeNSE), realizada en 2015 y 2019. En 2019 se analizaron los indicadores referidos al consumo y exposición a bebidas alcohólicas estratificados por sexo, grupo de edad, dependencia administrativa, unidades federativas y región geográfica. Se estimó la prevalencia y los respectivos intervalos de confianza del 95% (IC 95%). Resultados: aumenta la experimentación con bebidas alcohólicas antes de los 13 años (30,6% en 2015 a 34,6% en 2019); sufrir borracheras en la vida (27,2% en 2015 a 47,0% en 2019) y tener problemas con los amigos por el consumo de alcohol (9,3% en 2015 a 15,7% en 2019). Todos los indicadores eran más frecuentes entre las chicas, excepto el consumo compulsivo de alcohol y los episodios de embriaguez, que no presentaban diferencias de género, además de ser más elevados entre los estudiantes de mayor edad. Los episodios de consumo de alcohol y el hecho de tener amigos que beben bebidas alcohólicas fueron más frecuentes entre los estudiantes de la escuela pública, mientras que el consumo de alcohol por parte de los padres y el hecho de haber tenido problemas con sus familias o amigos debido al consumo de alcohol fueron mayores en los estudiantes de las escuelas privadas. Conclusión: se evidenció una alta prevalencia de experimentación, consumo y exposición a bebidas alcohólicas, mostrando que una gran parte de los adolescentes brasileños está expuesta a una carga evitable de morbilidad y mortalidad resultante del consumo y exposición al alcohol.


ABSTRACT Objective: to analyze the indicators regarding consumption of and exposure to alcoholic beverages among Brazilian schoolchildren in 2018 and compare them to those from 2015. Method: a cross-sectional study conducted with data from the 2015 and 2019 National School Health Survey (Pesquisa Nacional de Saúde do Escolar, PeNSE). In 2019, the indicators referring to consumption of and exposure to alcoholic beverages were analyzed, stratified by gender, age group, administrative system, Federation Unit, and geographical region. The prevalence values and their respective 95 confidence intervals (95% CIs) were estimated. Results: there was an increase in trying alcoholic beverages before the age of 13 (from 30.6% in 2015 to 34.6% in 2019); being drunk in their lifetime (from 27.2% in 2015 to 47.0% in 2019) and having problems with friends due to alcohol consumption (from 9.3% in 2015 to 15.7% in 2019). All the indicators were more prevalent among the girls, except for binge drinking and drunkenness episodes, which presented no differences between the genders and were also higher among older students Episodes of drunkenness and having friends who drink alcohol were more prevalent among students from public schools, while consumption of alcoholic beverages by parents and having had problems with their families or friends due to alcohol consumption were higher in students from private schools. Conclusion: high prevalence of experimentation, consumption and exposure to alcoholic beverages was evidenced, showing that a large number of Brazilian adolescents are exposed to an avoidable burden of morbidity and mortality resulting from consumption of and exposure to alcohol.

ABSTRACT

INTRODUCTION: Non-Communicable Diseases (NCDs) have become the main cause of disease burden in Brazil. Our objective was to describe trends (1990 to 2019) in prevalence and attributable burden of five modifiable risk factors and related metabolic risk factors in Brazil and its states. METHODS: In Global Burden of Disease 2019 analyses, we described trends in prevalence of modifiable risk factors and their metabolic mediators as percentage change in Summary Exposure Value (SEV). We estimated deaths and disability-adjusted life years (DALYs) attributable to the risk factors. RESULTS: Age-adjusted exposures to alcohol [41.0%, Uncertainty Interval (UI): 24.2 - 63.4], red meat (61.2%, UI: 42.4-92.3), low physical activity (3.9%, UI: -5-17.5) and ambient particulate matter pollution (3.3%, UI: -48.9-128.0) have worsened. Those for smoking (-51.4%, UI: -54.7- - 47.8), diet low in fruits (-28.1%, UI: -39.1- -18.7) and vegetables (-19.6%, UI: -32.7 - -8.7), and household air pollution (-85.3%, UI: -92.9- -74.3) have improved. All mediating metabolic risk factors, except high blood pressure (0.7%, UI: -6.9-8.3), have worsened: BMI (110.2%, UI: 78.6-161.7), hyperglycemia (15.1%, UI: 9.3-21.2), kidney dysfunction (12.0%, UI: 8.4-17.2), and high LDL-c (11.8%, UI: 6.9-17.2). CONCLUSIONS: A variable pattern of progress and failure in controlling modifiable risk factors has been accompanied by major worsening in most metabolic risk factors. The mixed success in public health measures to control modifiable risk factors for NCDs, when gauged by the related trends in metabolic risk factors, alert to the need for stronger actions to control NCDs in the future.

An important reduction was identified in the exposure to RFs related to socioeconomic development, such as unsafe water, lack of sanitation, and child malnutrition. Metabolic RFs, such as high BMI, hypertension, and alcohol consumption showed an increase in the attributable burden. CONCLUSIONS: Our findings point to an increase in metabolic RFs, which are the main RFs for mortality and DALYs. These results can help to consolidate and strengthen public policies that promote healthy lifestyles, thus reducing disease and death.

RESUMO: Objetivo: Descrever a prevalência de indicadores do tabagismo entre escolares brasileiros segundo características sociodemográficas em 2019 e comparar as prevalências entre 2015 e 2019. Métodos: Utilizaram-se dados da Pesquisa Nacional de Saúde do Escolar de 2015 e 2019. Foram analisados os indicadores referentes ao uso do tabaco, que foram comparados entre as edições de 2015 e 2019. Foram calculadas as prevalências e os respectivos intervalos de confiança de 95% (IC95%) para a população total e segundo sexo, faixa etária e tipo de escola. Resultados: Dos escolares, 22,6% (IC95% 21,7-23,4) já experimentaram cigarro alguma vez, porcentagem mais elevada entre os de 16 a 17 anos de idade (32,6%; IC95% 31,4-33,8) e no sexo masculino (35,0%; IC95% 33,6-36,4). A experimentação de narguilé, cigarro eletrônico e outros produtos do tabaco também se mostra elevada, com 26,9% (IC95% 26,0-27,8), 16,8% (IC95% 16,2-17,4) e 9,3% (IC95% 8,8-9,8), respectivamente, sendo mais alta entre os escolares do sexo masculino de 16 a 17 anos. Destaca-se que não houve mudanças nos indicadores "experimentação do cigarro", "fumar pela primeira vez antes dos 13 anos", "fumar nos 30 dias anteriores à pesquisa" e "ter ao menos um dos pais fumantes" entre os anos indicados. Conclusão: Embora os indicadores de tabaco fumado estejam estáveis entre 2015 e 2019, destacam-se as elevadas prevalências de experimentação de produtos como narguilé e cigarro eletrônico, que chamam a atenção para a necessidade de novas medidas regulatórias.


ABSTRACT: Objective: To describe the prevalence of smoking indicators among Brazilian students according to sociodemographic characteristics in 2019, and compare the prevalence between 2015 and 2019. Methods: Data from the National Survey of School Health 2015 and 2019 were used. Indicators related to tobacco use were analyzed. Indicators were compared between the 2015 and 2019 editions. Prevalence and respective 95% Confidence Interval (95%IC) were calculated for the total population and according to sex, age group and type of school. Results: 22.6% (95%CI 21.7-23.4) of the students had tried any cigarette and it was higher between 16 and 17 years of age (32.6%; 95% CI 31.4-33, 8) and in males (35.0%; 95%CI 33.6-36.4). The experimentation of hookah, electronic cigarette and other tobacco products are also high, with 26.9% (95%CI 26.0-27.8), 16.8% (95%CI 16.2-17.4) and 9.3% (95%CI 8.8-9.8), respectively, being higher among boys aged 16 to 17 years. It is noteworthy that there were no changes in the indicators of cigarette experimentation, smoking for the first time before the age of 13, smoking in the 30 days prior to the survey, and at least one of the smoking parents. Conclusion: Although smoked tobacco indicators are stable between 2015 and 2019, the high prevalence of experimentation with products such as hookah and electronic cigarettes is highlighted, drawing attention to the need for new regulatory measures.

ABSTRACT

This study compared indicators of care and access to health services by adults who self-reported hypertension in 2013 and 2019, analyzing those indicators according to gender, age group, schooling level, and race/color. This is an analytic study with data from the Brazilian National Health Survey (PNS), conducted in 2013 and 2019 in Brazil. The indicators to care and access to health services by individuals with arterial hypertension in both surveys were compared. For 2019, those indicators were analyzed according to sociodemographic characteristics. This study estimated the proportions, prevalence ratio (PR), and their respective 95% confidence intervals (95%CI). In total, 60,202 individuals were evaluated in 2013 and 88,531 in 2019, of these 24.4% reported arterial hypertension in 2013 and 23.9% in 2019. Women received more medical care for hypertension within the last year (PR = 1.07; 95%CI: 1.04; 1.11), had the last physician appointment at an basic health unit (PR = 1.11; 95%CI: 1.05; 1.17) than men. About race/color, black people had more hospitalization for hypertension or some complication (PR = 1.2; 95%CI: 1.05; 1.38) and intense or very intense degree of limitation in performing daily activities (PR = 1.37; 95%CI: 1.06; 1.76). In 2019, inequalities were evidenced and worse indicators were observed for males, black, with low education and young age. Therefore, investments in the Brazilian Unified National Health System, as well as public policies and strategic actions are essential to reduce inequalities, promote health care.


Este estudo comparou indicadores de cuidados e acesso aos serviços de saúde por adultos que se autodeclararam hipertensos em 2013 e 2019, analisando esses indicadores de acordo com o sexo, grupo etário, nível de escolaridade, e rala/cor. Estudo analítico com dados da Pesquisa Nacional de Saúde (PNS), realizado em 2013 e 2019 no Brasil. Os indicadores de cuidados e acesso aos serviços de saúde por indivíduos com hipertensão arterial em ambos os inquéritos foram comparados. Para 2019, esses indicadores foram analisados de acordo com características sociodemográficas. Este estudo estimou as proporções, as razões de prevalência (RP), e os respectivos intervalos de 95% de confiança (IC95%). No total, 60.202 indivíduos foram avaliados em 2013 e 88.531 em 2019, destes 24,4% referiram hipertensão arterial em 2013 e 23,9% em 2019. As mulheres receberam mais cuidados médicos para hipertensão no último ano (RP = 1,07; IC95%: 1,04; 1,11) e a última consulta médica na unidade básica de saúde (RP = 1,11; IC95%: 1,05; 1,17) que homens. Sobre raça/cor, os negros tiveram mais hospitalização por hipertensão ou alguma complicação (RP = 1,2; IC95%: 1,05; 1,38) e grau de limitação intenso ou muito intenso na realização de atividades diárias (RP = 1,37; IC95%: 1,06; 1,76). Em 2019, foram evidenciadas desigualdades e piores indicadores foram observados em homens, negros, com baixa escolaridade e idade jovem. Investimentos no Sistema Único de Saúde brasileiro, bem como políticas públicas e ações são essenciais para reduzir as desigualdades, promover os cuidados de saúde.

En cuanto raza/color, los negros tuvieron más hospitalizaciones por hipertensión o alguna complicación (RP = 1,2; IC95%: 1,05; 1,38) y un grado intenso o muy intenso de limitación para realizar actividades diarias (RP = 1,37; IC95%: 1,06; 1,76). En 2019, se evidenciaron desigualdades y se observaron peores indicadores en hombres, los negros, con baja educación y la edad joven. Las inversiones en el Sistema Único de Salud brasileño, así como políticas públicas y acciones son esenciales para reducir las desigualdades, promover la atención sanitaria.

Objetivo: Descrever os indicadores de abandono do uso de tabaco, em 2013 e 2019, para o Brasil e as Unidades da Federação, segundo variáveis sociodemográficas, coletadas na Pesquisa Nacional de Saúde (PNS). Métodos: Estudo transversal, populacional e descritivo realizado com dados da PNS 2013 e 2019, uma pesquisa domiciliar coletada por entrevistadores treinados. Foram calculadas a prevalência de ex-fumantes e a proporção de fumantes que tentaram parar de fumar nos últimos 12 meses imediatamente anteriores à data da entrevista, e os respectivos intervalos de confiança (IC95%), segundo as variáveis sociodemográficas. Ademais, calculou-se a variação percentual entre os anos estudados. Resultados: Em 2013, a prevalência de ex-fumantes foi 17,5% (IC95% 16,9;18,0) e, em 2019, 26,6% (IC95% 26,1;27,2). Tentaram parar de fumar 51,1% (IC95% 49,3;52,9), em 2013, e 46,6% (IC95% 45,0;48,3) em 2019. Conclusão: É importante o fortalecimento e manutenção de estratégias para enfrentamento do uso de tabaco no país, de forma a aumentar a disposição e a capacidade do fumante atual de parar de fumar.

Se calculó la prevalencia de exfumadores y proporción de fumadores que intentaron dejar de fumar en los últimos 12 meses y respectivos intervalos de confianza (IC95%), según variables sociodemográficas. Además, se calculó la variación porcentual entre los años. Resultados: En 2013, la prevalencia de exfumadores fue de 17,5% (IC95% 16,9;18,0), en 2019, 26,6% (IC95% 26,1;27,2). En 2013, el 51,1% intentó dejar de fumar (IC95% 49,3;52,9), y, en 2019, el 46,6% (IC95% 45,0;48,3). Conclusión: Es importante fortalecer y mantener las estrategias de afrontamiento del tabaquismo, para incrementar la disposición y capacidad del fumador actual para dejar de fumar.

Methods: Cross-sectional, population-based and descriptive study with data from the 2013 and 2019 PNS, a household survey collected by trained interviewers. The prevalence of ex-smokers and the proportion of smokers who tried to quit smoking in the 12 months prior to the interview, and respective confidence intervals (95%CI) were calculated, according to sociodemographic variables. Additionally, the percentage variation between the years was calculated. Results: In 2013, the prevalence of ex-smokers was 17.5% (95%CI 16.9;18.0) and, in 2019, 26.6% (95%CI 26.1;27.2). In 2013, 51.1% tried to quit smoking (95%CI 49.3;52.9) and, in 2019, 46.6% (95%CI 45.0;48.3). Conclusion: It is important to strengthen and maintain strategies for coping with tobacco use in Brazil, to increase the current smoker's willingness and ability to quit smoking.

OBJECTIVE: To compare indicators of tobacco use, secondhand smoke, cessation and exposure to pro- and anti-tobacco media in 2013 and 2019, and to describe these indicators according to sociodemographic variables in 2019. METHODS: Cross-sectional study with data from the National Health Survey. The indicators of use, secondhand smoke, cessation and exposure to tobacco-related media were evaluated. Prevalence and confidence intervals (95%CI) were estimated for the total population in 2013 and 2019 and according to sociodemographic variables for 2019. Poisson regression with robust variance was used to assess differences in prevalence. RESULTS: There was an improvement in most of the indicators studied: an increase in ex-smokers, a reduction in secondhand smoke and attempts to quit smoking. All pro- and anti-tobacco media exposure indicators declined. When considering the prevalence according to sociodemographic characteristics in 2019, 43.8% (95%CI 41.6-46.0) of men tried to quit smoking, and 50.8% (95%CI 48.5-53.2) of women. Secondhand smoke at home was higher among women (10.2%; 95%CI 9.7-10.8). Among those who thought about quitting smoking because of warnings, the proportion was higher among women (48.0%; 95%CI 45.3-50.6). Tobacco use was higher among men (43.8%; 95%CI 41.6-46.0), in the population aged 40 to 59 years (14.9%; 95%CI 14.2-15.6), with a lower level of education (17.6%; 95%CI 16.8-18.4). CONCLUSION: The study showed improvement in tobacco-related indicators between the years studied. It is noteworthy that this advance was smaller in relation to the other periods previously analyzed, and therefore, greater investments in public policies to combat and control smoking in Brazil are necessary.

Objetivo: Comparar indicadores de uso do tabaco, fumo passivo, cessação e exposição a mídia pró e antitabaco, em 2013 e 2019, e descrever esses indicadores segundo variáveis sociodemográficas em 2019. Métodos: Estudo transversal com dados da Pesquisa Nacional de Saúde. Avaliou-se os indicadores de uso, fumo passivo, cessação e exposição a mídia relacionada ao tabaco. Estimou-se as prevalências e intervalos de confiança (IC95%) para população total em 2013 e 2019, e segundo variáveis sociodemográficas para 2019. Para avaliar diferenças nas prevalências usou-se a Regressão de Poisson com variância robusta. Resultados: Houve melhoria dos indicadores de uso do tabaco; aumento de ex-fumantes e redução do fumo passivo e da tentativa de parar de fumar. Todos os indicadores de exposição a mídia pró e contra o tabaco diminuíram.  Ao considerar as prevalências segundo características sociodemográficas em 2019, 43,8% (IC95%: 41,6;46,0) dos homens tentaram parar de fumar e 50,8% (IC95%: 48,5;53,2) das mulheres. O fumo passivo no domicilio foi maior nas mulheres (10,2%; IC95%: 9,7;10,8). Entre os que pensaram em parar de fumar devido as advertências, a proporção foi maior nas mulheres (48,0%; IC95%: 45,3;50,6). O uso do tabaco foi mais elevado nos homens (43,8%; IC95%: 41,6;46,0), na população de 40 a 59 anos (14,9%; IC95%: 14,2;15,6) e com menor nível de instrução (17,6%; IC95%: 16,8;18,4). Conclusão: O estudo mostrou melhoria dos indicadores relacionados ao tabaco entre os anos estudados. Ressalta-se, que esse avanço foi menor em relação a outros períodos analisados previamente, e, portanto, torna-se necessário maiores investimentos em políticas públicas de enfrentamento e controle do tabagismo no Brasil.

ABSTRACT

ABSTRACT Objective: To compare indicators of tobacco use, secondhand smoke, cessation and exposure to pro- and anti-tobacco media in 2013 and 2019, and to describe these indicators according to sociodemographic variables in 2019. Methods: Cross-sectional study with data from the National Health Survey. The indicators of use, secondhand smoke, cessation and exposure to tobacco-related media were evaluated. Prevalence and confidence intervals (95%CI) were estimated for the total population in 2013 and 2019 and according to sociodemographic variables for 2019. Poisson regression with robust variance was used to assess differences in prevalence. Results: There was an improvement in most of the indicators studied: an increase in ex-smokers, a reduction in secondhand smoke and attempts to quit smoking. All pro- and anti-tobacco media exposure indicators declined. When considering the prevalence according to sociodemographic characteristics in 2019, 43.8% (95%CI 41.6-46.0) of men tried to quit smoking, and 50.8% (95%CI 48.5-53.2) of women. Secondhand smoke at home was higher among women (10.2%; 95%CI 9.7-10.8). Among those who thought about quitting smoking because of warnings, the proportion was higher among women (48.0%; 95%CI 45.3-50.6). Tobacco use was higher among men (43.8%; 95%CI 41.6-46.0), in the population aged 40 to 59 years (14.9%; 95%CI 14.2-15.6), with a lower level of education (17.6%; 95%CI 16.8-18.4). Conclusion: The study showed improvement in tobacco-related indicators between the years studied. It is noteworthy that this advance was smaller in relation to the other periods previously analyzed, and therefore, greater investments in public policies to combat and control smoking in Brazil are necessary.


RESUMO: Objetivo: Comparar indicadores de uso do tabaco, fumo passivo, cessação e exposição à mídia pró e antitabaco em 2013 e 2019 e descrever esses indicadores segundo variáveis sociodemográficas em 2019. Métodos: Estudo transversal com dados da Pesquisa Nacional de Saúde. Avaliaram-se os indicadores de uso, fumo passivo, cessação e exposição à mídia relacionada ao tabaco. Estimaram-se as prevalências e intervalos de confiança (IC95%) para a população total em 2013 e 2019 e segundo variáveis sociodemográficas para 2019. Para avaliar diferenças nas prevalências, usou-se a regressão de Poisson com variância robusta. Resultados: Houve melhoria dos indicadores de uso do tabaco; aumento de ex-fumantes e redução do fumo passivo e da tentativa de parar de fumar. Todos os indicadores de exposição à mídia pró e contra o tabaco diminuíram. Ao se considerarem as prevalências segundo características sociodemográficas em 2019, 43,8% (IC95% 41,6-46,0) dos homens e 50,8% (IC95% 48,5-53,2) das mulheres tentaram parar de fumar. O fumo passivo no domicílio foi maior nas mulheres (10,2%; IC95% 9,7-10,8). Entre os que pensaram em parar de fumar por causa das advertências, a proporção foi maior nas mulheres (48,0%; IC95% 45,3-50,6). O uso do tabaco foi mais elevado nos homens (43,8%; IC95% 41,6-46,0), na população de 40-59 anos (14,9%; IC95% 14,2-15,6) e naquela com menor nível de instrução (17,6%; IC95% 16,8-18,4). Conclusão: O estudo mostrou melhoria dos indicadores relacionados ao tabaco entre os anos estudados. Ressalta-se que esse avanço foi menor em relação a outros períodos analisados previamente, e, portanto, torna-se necessário maiores investimentos em políticas públicas de enfrentamento e controle do tabagismo no Brasil.

ABSTRACT

This article presents the history and construction of the National Health Survey (PNS) 2019, a household survey conducted in partnership with the Brazilian Institute of Geography and Statistics. The objective was to provide the country with information on the determinants, conditions and health needs of the Brazilian population. The expected sample was 108,525 households, considering a non-response rate of 20%. The questionnaire had three parts: (i) regarding the household; (ii) to all residents of the household, focusing on the collection of socioeconomic and health information; and (iii) aimed at the selected resident (15 years or more) for whom lifestyles, chronic diseases, violence, among other topics were investigated, and anthropometric measures (sub-sample) were measured. The PNS information will serve as a basis for the (re)formulation of health policies, as well as support for existing actions and programs of the Unified Health System.


Este artigo apresenta o histórico e a construção da Pesquisa Nacional de Saúde (PNS) 2019, inquérito de base domiciliar realizado em parceria com a fundação Instituto Brasileiro de Geografia e Estatística. O objetivo da PNS 2019 foi dotar o país de informações sobre os determinantes, condicionantes e necessidades de saúde da população brasileira. A amostra prevista foi de 108.525 domicílios particulares, considerando-se uma taxa de não resposta de 20%. Seu questionário continha três partes, orientadas para (i) o domicílio, (ii) todos os moradores do domicílio, com enfoque na coleta de informações socioeconômicas e de saúde, e (iii) o morador selecionado (idade ≥15 anos), sobre o qual investigou-se estilos de vida, doenças crônicas, violências, entre outros temas, e aferiu-se medidas antropométricas (subamostra). As informações da PNS 2019 servirão de base para a (re)formulação de políticas de saúde e subsídio a ações e programas existentes do Sistema Único de Saúde.

OBJECTIVE: To analyze trend estimates on the prevalence of risk and protective behaviors for chronic noncommunicable diseases in adolescents, according to data from the National School Health Survey in 2009, 2012 and 2015. METHODS: Data from the three cross-sectional studies in Brazilian capitals and the Federal District were used. In total, 173,310 adolescents enrolled in the ninth grade of elementary school were interviewed, with average age of 14 years. The prevalence of indicators of protective (consumption of beans and fruit; physical education classes at school; practice of physical activity for 60 minutes or more) and risk factors (consumption of candies and soft drinks; use of cigarettes and alcohol in the last 30 days; drug testing) were estimated through linear regression. RESULTS: There was a significant increase (p < 0.05) in the prevalence of fruit consumption and in the reduction of bean, soft drinks and candies consumption, as well as the consumption of alcoholic beverages and cigarettes. However, an increase in the prevalence of illicit drug experimentation was observed. Discussion: Despite the tendency to reduce risk factors, prevalences are high when compared with other sociocultural realities. CONCLUSION: The school is an important area of access to the adolescent public, and it is necessary to encourage school health promotion programs to reduce health risk behaviors, as well as to stimulate protective ones.


OBJETIVO: Analisar as estimativas de tendência sobre a prevalência de comportamentos de risco e proteção para doenças crônicas não transmissíveis em adolescentes, segundo dados da Pesquisa Nacional de Saúde do Escolar em 2009, 2012 e 2015. MÉTODOS: Foram utilizados dados dos três estudos transversais nas capitais brasileiras e no Distrito Federal. No total, entrevistaram-se 173.310 adolescentes matriculados no 9º ano do ensino fundamental, com idade média de 14 anos. Foram estimadas pelo modelo de regressão linear as prevalências dos indicadores de fatores de proteção (consumo de feijão e frutas; aulas de educação física na escola; prática de 60 minutos ou mais de atividade física) e de risco (consumo de guloseimas e refrigerantes; uso de cigarro e álcool nos últimos 30 dias; experimentação de drogas). RESULTADOS: Houve aumento significativo (p < 0,05) da prevalência do consumo de frutas e redução de consumo de feijão, refrigerantes e guloseimas, assim como do uso de bebidas alcoólicas e cigarro, entretanto foi observado aumento na prevalência de experimentação de drogas ilícitas. DISCUSSÃO: Apesar da tendência de redução em alguns fatores de risco, as prevalências encontradas são altas ao comparar com outras realidades socioculturais. CONCLUSÃO: A escola é um importante espaço de acesso ao público adolescente, e faz-se necessário estimular programas de promoção da saúde escolar para reduzir comportamentos de risco à saúde, assim como incentivar comportamentos protetores.
