ABSTRACT

En los últimos años se ha popularizado el uso de Internet, lo que unido a la evolución a la denominada Web 2.0 ha hecho que su impacto en nuestra vida cotidiana y profesional sea cada vez más importante. En el presente artículo se citan algunos de los cambios más relevantes registrados y se recopilan una serie de herramientas que pueden ser de utilidad en el desempeño profesional de los farmacéuticos de Atención Primaria. Sin ánimo de ser exhaustivos, y a sabiendas de que cuando estas líneas se publiquen su contenido habrá quedado desfasado, con este artículo se pretende despertar la curiosidad de aquellos que aún no han explorado las posibilidades que hoy ofrece la Red desde una óptica profesional (AU)


In recent years, the use of Internet has become popular. This, together with the evolution of the so-called Web 2.0, has made its impact on our daily and professional life increasingly more important. In the present article, some of the most important changes recorded are mentioned and a series of tools that can be useful in the professional performance of the primary care pharmacists are collected. Without trying to provide an exhaustive study and knowing that when these lines are published, their content will already be outdated, the purpose of this article is to stir up the curiosity of those who have still not investigated the possibilities offered by the Network from a professional point of view (AU)

ABSTRACT

Solitary fibrous tumor of the pleura (TFSP) is an uncommon and mostly benign neoplasm. It grows slowly and presents with scant symptoms. Recurrence after surgical excision has been described. The aim of this report is to show our experience in managing this disease. Clinical as well as radiological, surgical, histopathological and follow-up data were analyzed in a clinical series. Forty one cases of TFSP that underwent surgery at our institution between 1991 and 2004 were retrospectively analyzed; 27 were females and 14 males with a median age of 60.4 years old; 24.4% of the cases were asymptomatic. All tumors were surgically excised, in average they weighed 945 g and 36.5% of the lesions showed a pedicle. Most of the lesions were originated in the visceral pleura. Surgical morbidity was 12.1% and no mortality was recorded. Three tumor recurred. TFSP is usually a benign neoplasm but it can recur. Complete surgical excision is the treatment of choice of primary and recurrent tumors.


Introducción: El tumor fibroso solitario de la pleura (TFSP) es una neoplasia infrecuente, considerada benigna, de crecimiento lento y poco sintomática pero que puede recidivar tras la resección quirúrgica. El objetivo de este estudio es mostrar la experiencia de nuestro centro con el manejo de estos tumores. Pacientes y Método: Análisis retrospectivo de 41 pacientes con TFSP intervenidos en nuestro Servicio entre 1991 y 2004. Se analizan datos clínicos, radiológicos, quirúrgicos, morbimortalidad, recurrencia y seguimiento. Resultados: De los 41 pacientes con TFSP resecados, 27 eran mujeres y 14 hombres. Edad media 60,4 años. 24,4% de los pacientes fueron asintomáticos. Todos los pacientes fueron intervenidos quirúrgicamente y se realizó estudio histopatológico en todas las piezas. El peso promedio de los tumores fue de 954 g y 36,5% eran tumores pediculados, el resto de base ancha. La mayoría (75,6%) se originaban de la pleura visceral. No hubo mortalidad operatoria y la morbilidad alcanzó el 12,1%. Tres tumores recidivaron. Conclusiones: El TFSP es una neoplasia rara generalmente benigna, pero que puede recidivar. El tratamiento es la resección quirúrgica completa y las recidivas deben ser resecadas nuevamente.

Los antagonistas de los receptores de la angiontensina II (ARA II) son un subgrupo de antihipertensivos muy polémicos desde su comercialización, tanto por su elevado precio como por la gran presión promocional dela industria farmacéutica a todos los niveles, pero también por su lugar en la terapéutica de las indicacionesautorizadas. En la actualidad su uso se ha extendido, y han logrado desplazar a otros subgrupos terapéuticos,en particular a los inhibidores de la enzima conversora de la angiotensina (IECA), sin que este hechoesté basado en evidencias sólidas. El objetivo de esta revisión es analizar los ensayos clínicos más importantesrealizados con los ARA II, resumir su contenido en forma de tablas para facilitar su consulta por partede los farmacéuticos de atención primaria en su quehacer diario, y determinar el lugar que ocupan en laterapéutica estos fármacos en sus distintas indicaciones a partir de dichos estudios


Drugs containing angiotensin II (AII) receptor antagonists are a subgroup of antihypertensive agents thathave been highly controversial ever since they became commercially available because of their high cost, themarked promotional pressure exerted at every level by the pharmaceutical industry and their place in therapeuticswith respect to the approved indications. At the present time, their use has become increasinglywidespread, displacing other therapeutic subgroups, especially angiotensin converting enzyme (ACE) inhibitors,despite the absence of solid evidence-based reasons. The objective of the present review is to analyzethe most important clinical trials carried out with AII receptor agonists, summarize their content in tableform for use by primary care pharmacists in their routine practice and determine the place of these drugsin the treatment of the conditions for which they are indicated, according to said studies

ABSTRACT

El tumor fibroso de la pleura es una neoplasia poco habitual cuya presentación clínica es inespecífica, obligando al diagnóstico diferencial con otras patologías respiratorias más frecuentes. El diagnóstico definitivo es histopatológico. Su tratamiento es quirúrgico y debe realizarse seguimiento a largo plazo por la posibilidad de recidiva. Se comunica caso de una paciente portadora de un tumor fibroso de la pleura, que se manifestó por dificultad respiratoria, dolor torácico y signología clínica de ocupación pleural. El diagnóstico se determinó mediante biopsia por punción percutánea del tumor. La paciente fue sometida a toracotomía, realizándose extirpación completa del tumor, con buena evolución postoperatoria

ABSTRACT

We developed a model of lesion of Schaffer collaterals in hippocampal organotypic slice cultures to analyse the capacity for sprouting and functional recovery expressed in young (one week old) and old (four week old) slice cultures. Slice cultures were sectioned at different ages of maturation in two separate half-slices and maintained in co-culture. Functional recovery was assessed by measuring synaptic responses elicited across the lesion seven days after the lesion and sprouting was evaluated by biocytin labeling of the regenerating fibers seen under the same conditions. Sprouting and functional recovery were found to be markedly reduced and delayed in old vs young cultures. Preparation of co-cultures between young CA3 and old CA1 half-slices resulted in a significant reduction in the capacity for sprouting and regeneration of the young CA3 neurons. Conversely, co-cultures prepared between old CA3 and young CA1 half-slices showed a markedly enhanced capacity for sprouting and functional recovery of old CA3 neurons. These results indicate that the age-dependent impairment in sprouting and regeneration expressed in cortical regions can be improved by and depends upon the presence of a favourable environment.
