ABSTRACT

BACKGROUND: The main reason for dose reduction of afatinib is gastrointestinal toxicity (GT). In a phase II study, we analyzed anthropometrical, nutritional, and biochemical factors associated with GT induced by afatinib. MATERIALS AND METHODS: Patients diagnosed with non-small cell lung cancer who progressed to prior chemotherapy received 40 mg of afatinib. Malnutrition was determined by Subjective Global Assessment, and lean body mass (LBM) was determined by computed tomography scan analysis using a pre-established Hounsfield unit threshold. Toxicity was obtained during four cycles by Common Terminology Criteria for Adverse Events. RESULTS: Eighty-four patients were enrolled. Afatinib was administered as the second, third, and fourth line of treatment in 54.8%, 38.1%, and 7.12% of patients, respectively. Severe diarrhea, mucositis, and overall severe GT were present in 38.9%, 28.8%, and 57.5%, respectively. Of the patients, 50% developed dose-limiting toxicity (DLT). Patients with malnutrition have higher risk for severe GT. Patients with lower LBM and body mass index developed more DLT (71.4% vs. 18.8%). CONCLUSION: Malnutrition is associated with a higher risk of severe GT induced by afatinib. Determination of nutritional status and body composition are helpful in identifying patients at higher risk of severe GT and could allow initiating treatment with lower doses according to tolerance.

BACKGROUND: Few studies, have evaluated the prognostic impact of the quantification of mRNA expression levels in advanced non-small cell lung cancer (NSCLC). OBJECTIVE: The aim of this work was to quantify mRNA expression levels in peripheral blood through three epithelial markers in patients with stages IIIB and IV in NSCLC. METHODS: Seventy advanced NSCLC patients and ten healthy controls were included. All patients received platinum-based chemotherapy in first line treatment. Peripheral blood was obtained of each participant and mRNA expression levels present in circulating cells were quantified by molecular techniques (RT-PCR) using three epithelial markers: cytokeratin (CK)-18, CK-19 and Carcinoembryonic-Antigen (CEA). The expression levels were quantified from a standard curve using the cDNA obtained from A549 cells. Registered in ClinicalTrials.gov (NCT01052818). RESULTS: We found a significant statistical correlation between levels of CK-18, CK-19 and CEA mRNA. mRNA expression levels were lower in patients who present three or less metastasis; higher CEA mRNA expression was associated a worse progression-free survival to platinum-based chemotherapy and overall survival. CONCLUSION: RNA expression of CEA by RT-PCR is useful as a prognostic marker in advanced NSCLC.

ABSTRACT

BACKGROUND: Nitroglycerin, a nitric oxide donor agent, reduces the expression of hypoxia-inducible factor-1α (HIF-1α) and could be a normalizer of the tumor microenvironment. Both factors are associated with chemo-radio-resistance. The aim of this study was to determine the safety profile and efficacy of nitroglycerin administration with chemo-radiotherapy in patients with locally advanced non-small cell lung cancer (NSCLC). METHODS: This is a phase II trial of locally advanced NSCLC patients treated with cisplatin and vinorelbine plus concurrent nitroglycerin with radiotherapy. A 25-mg NTG patch was administered to the patients for 5 days (1 day before and 4 days after chemotherapy induction and consolidation) and all day during chemo-radiotherapy. VEGF plasmatic level was determined before and after two cycles of chemotherapy. RESULTS: Thirty-five patients were enrolled in this trial. Sixty-three percent of patients achieved an overall response after induction of chemotherapy, and 75% achieved an overall response after chemo-radiotherapy. The median progression-free survival was 13.5 months (95% CI, 8.8-18.2), and the median overall survival was 26.9 months (95% CI, 15.3-38.5). Reduction of VEGF level was associated with better OS. The toxicity profile related to nitroglycerin included headache (20%) and hypotension (2.9%). CONCLUSIONS: The addition of nitroglycerin to induction chemotherapy and concurrent chemoradiotherapy in patients with locally advanced NSCLC has an acceptable toxicity profile and supports the possibility to add nitroglycerin to chemotherapy and radiotherapy. A randomized trial is warranted to confirm these findings.
