La Nefrología española es una de las especialidades médicas que ha experimentadoun mayor desarrollo en los últimos años.Definida inicialmente como «necesariamente hospitalaria», en la actualidad enel Libro Blanco de la Nefrología Española se hace una llamada de atención sobrela necesidad de un cambio de orientación asistencial.Se llamaba la atención sobre el escaso desarrollo de las actividades de prevencióny el retraso en la llegada del paciente al Servicio de Nefrología.Para acercarnos a esta realidad hay que dar prioridad a la asistencia del enfermoen el propio ámbito extrahospitalario y dotar a su atención integral de una lógicacontinuidad entre niveles asistenciales.Las iniciativas asistenciales que se han llevado a cabo en el territorio españolson escasas y todavía insuficientes. El aumento progresivo en la incidencia y prevalenciade la IRC asociado con una mala evolución y elevados costes está convirtiendoa la IRC en un importante problema de salud pública, que requiere unosplanes específicos de detección y prevención.Por todo ello es necesaria la mejora de la coordinación entre Asistencia Primariay Nefrología. Las iniciativas de coordinación entre los dos niveles deberían centrarseen impulsar estudios epidemiológicos que clarifiquen la significación real dela patología renal, en elaborar Guías de Actuación sobre patología renal prevalerte,en educar al Médico de Atención primaria sobre la referencia precoz al nefrólogode los pacientes con Insuficiencia Renal y en diseñar planes conjuntos paraabordar patologías nefrológicas con importante significado epidemiológico
The Spanish Nephrology is one of the medical specialties that have experienceda greater development in the last years. Defined initially as an activity in thehospital and consolidated in this sense, at the present time the «Libro Blanco ofthe Spanish Nephrology» makes a call of attention on the necessity of a changeof welfare direction. The attention was called on the little development of the preventionactivities and the delay in the arrival of the patient to the Service of Nephrology,which causes a worsening of his pathology. In order to approach us thisreality it is necessary to give priority to the attendance of the patient out of thehospital and to guarantee a logical continuity between welfare levels. The welfareinitiatives that have been carried out in the Spanish territory are scarce and stillinsufficient. The progressive increase in the incidence and prevalence of the ERDassociated with a bad evolution and elevated costs is converting ERD in an imartiportantproblem of public health, that requires specific plans of detection and prevention.The improvement of the coordination between general practioners andNephrologists is necessary. Multiple papers show a delayed reference of the patientswith Renal Disease to the Nephrologists. The initiatives of coordination betweenboth levels should be centered in studies epidemiologists that clarify the realmeaning of the renal pathology and in educating the GP on the early referenceto the nephrologist of the patients with Renal Failure
