ABSTRACT

In-situ liquid cell transmission electron microscopy (LCTEM) with electrical biasing capabilities has emerged as an invaluable tool for directly imaging electrode processes with high temporal and spatial resolution. However, accurately quantifying structural changes that occur on the electrode and subsequently correlating them to the applied stimulus remains challenging. Here, we present structural dissimilarity (DSSIM) analysis as segmentation-free video processing algorithm for locally detecting and quantifying structural change occurring in LCTEM videos. In this study, DSSIM analysis is applied to two in-situ LCTEM videos to demonstrate how to implement this algorithm and interpret the results. We show DSSIM analysis can be used as a visualization tool for qualitative data analysis by highlighting structural changes which are easily missed when viewing the raw data. Furthermore, we demonstrate how DSSIM analysis can serve as a quantitative metric and efficiently convert 3-dimensional microscopy videos to 1-dimenional plots which makes it easy to interpret and compare events occurring at different timepoints in a video. In the analyses presented here, DSSIM is used to directly correlate the magnitude and temporal scale of structural change to the features of the applied electrical bias. ImageJ, Python, and MATLAB programs, including a user-friendly interface and accompanying documentation, are published alongside this manuscript to make DSSIM analysis easily accessible to the scientific community.

BACKGROUND: Behavioral and psychological symptoms of dementia (BPSD) are difficult to manage and associated with poor outcome. OBJECTIVES: The aim of this study was to reach consensus on the use of antipsychotics in patients with BPSD in Spain. METHODS: A qualitative, multicenter, two-round Delphi study was carried out, with the participation of specialists involved in the care of dementia patients throughout Spain. They completed a 76-item questionnaire related to the identification of BPSD, treatment with antipsychotics, follow-up of patients, barriers for the use of atypical antipsychotics, and effects of antipsychotics on quality of life. RESULTS: A total of 162 specialists in neurology, psychiatry, and geriatrics (61% men) with a mean (SD) age of 45.9 (10) years participated in the study. Almost all participants (96.9%) strongly agreed that atypical antipsychotics are safer and better tolerated than typical antipsychotics. There was agreement on the importance to review the indication and dose of the antipsychotic drug at least every 3 months. There was consistent high rate of agreement on the beneficial impact of atypical antipsychotics on the quality of life of patients with dementia and their caregivers. A consensus was also reached on the need of detecting BPSD in patients with dementia as it decreases the quality of life of both patients and caregivers, and the need to routinely screen for dementia in elderly patients with no previous psychiatric history in the presence of suggestive symptoms of BPSD. Finally, the participants in the study agreed that administrative barriers for the prescription of atypical antipsychotics in Spain hinder the access to this drug group and favor the prescription of typical antipsychotics. CONCLUSIONS: The participants in the study agreed that atypical antipsychotics should be preferred to typical antipsychotics in the management of BPSD. Wide consensus was reached about the importance of early identification of BPSD in persons with cognitive impairment, the use and management of atypical antipsychotic drugs and their favorable impact on patients and caregiver's quality of life.

ABSTRACT

El trastorno afectivo bipolar es una enfermedad mental grave catalogada dentro de los trastornos del ánimo en el DSM 5. En el año 2013, se incorpora a la lista de los problemas de salud incluidos en las Garantías Explícitas de Salud GES, siendo un avance importante en relación a los principios que sustentan el sistema y la reforma de salud en Chile. Se realiza una reflexión crítica de la canasta GES a la luz de los componentes del sistema de Salud chileno, destacando el rol fundamental que tiene el enfermero/a psiquiátrico en la implementación de las prestaciones y recomendaciones. Finalmente, se destaca la importancia de la participación en la micro, meso y macrogestión para la profesión.


Bipolar disorder is a serious mental illness cataloged within of mood disorders in DMS 5. In 2013, it has incorporated to the list of health problems included in the explicit guarantees of health (GES), which it was an important advance in relation to the values that underpin reform and system of health in Chile. It is performed a critical reflection of the set of service GES and its relation with the components of the Chilean health system, highlighting the fundamental role that psychiatric nurses have in the implementation of the services and recomendations. Finally, it highlights the importance pf the participation in the micro, meso and macromanagement to the profession.

ABSTRACT

El grupo objetivo al cual está dirigido el presente programa psicoeducativo de salud mental corresponde a una comunidad de adultos mayores jubilados de Gendarmería de Chile. Mediante la generación de un diagnóstico participativo por parte de los integrantes de la comunidad, se constató la necesidad de desarrollar herramientas para la superación de problemas de la vida, lo que se enmarca en la promoción de la resiliencia. La realización del taller de salud mental constó de once sesiones. Primero, se abarcaron los cambios fisiológicos y patológicos del envejecimiento, para luego trabajar en conjunto la resiliencia en la adultez. Al inicio de las sesiones educativas, se aplicó la escala de resiliencia de SV-RES 60 de Saavedra y Villalta (2008), validada en nuestro país, con el fin de conocer los niveles basales de resiliencia. Luego de finalizadas las intervenciones, se realizó una nueva toma, para así poder evaluar el impacto de estas en los participantes. Se constató un aumento del nivel de resiliencia desde un 49.8% a un 75.1%, lo que se traduce en un ascenso desde un nivel promedio a un nivel alto, evidenciando la efectividad de la metodología aplicada.


The target group for this psychoeducative program in mental health is a community of elderly retired Gendarmerie of Chile. Through a participatory diagnosis with the participants, it could be possible to identify necessities about working and creating tools to resolve problems of life, in the framework of the resiliency. This workshop in mental health takes eleven sessions. At the first time, it includes physiological and pathological changes in the aging, and in the second part, the program includes working together in the resiliency. At the beginning of the sessions, it was applicated the Saavedra and Villata's resiliency scale SV-RES 60, which is validated in Chile, with the objective to know the baseline of resiliency of the group. When the workshop was finished, it was taken in a second opportunity the same scale to evaluate the impact of the sessions in the participants. It was verified an increment of the resiliency levels since the first time with a 49,8%, to a 75,1% in the finish, which it could be traduced in a increase to an average level of resiliency to a high level of resiliency, making evident the effectiveness of the methodology.
