ABSTRACT

INTRODUCTION: Accidents currently rank first among causes of complex lower limb injuries. The most common open long bone fractures are tibial fractures. Estimates indicate that there are 492,000 new fractures every year, with a prevalence of 100,000 cases that do not heal. CLINICAL CASE: Young, female patient who underwent emergency surgery for a Gustilo III A open comminuted fracture of the left proximal tibia. Surgical toilette was performed several times until the soft tissues improved. Then internal fixation of large fragments was performed with a cortical screw and Professor Rodrigo Alvarez Cambras' external fixator was used to heal the fracture. The combined approach consisting of internal and external fixation is discussed within the setting of these difficult fractures.

ABSTRACT

OBJECTIVE: To present the results obtained in the treatment of chronic osteomyelitis of the distal third of the tibia using an external fixator. CLINICAL CASE: Male, young patient with chronic osteomyelitis of the left tibia resulting from a Gustilo IIIB complicated fracture. We performed resection of the osteomyelitis focus. RESULTS: Good clinical and radiologic results were obtained with this technique; a 9 cm effective bone transport was achieved. CONCLUSIONS: Bone transport is a very useful method for the treatment of osteomyelitis that involves bone resection; the RALCA external fixator is technically useful.

ABSTRACT

Objetivo. Evaluar los resultados de la magnetoterapia en pacientes con fracturas recientes de Colles. Diseño. Estudio prospectivo, comparativo, con un modelo de casos y controles. Material y método. La investigación se realizó en el Policlínico Docente Ernesto Guevara del municipio Niquero, provincia Granma, en el período comprendido desde septiembre del 2009 hasta febrero del 2010. Del universo de trabajo se crearon aleatoriamente 2 grupos, a uno de ellos se le aplicó el tratamiento convencional y al otro se le aplicó además electromagnetoterapia durante los siguientes 30 días. Los resultados de ambos grupos se reflejaron en tablas y se realizó un estudio comparativo entre ambos utilizando pruebas estadísticas. Resultados. Fueron valorados como positivos ya que en el grupo de estudio y mediante la utilización de la electromagnetoterapia, la consolidación clínico-radiológica se produjo, en un 93,3% de los pacientes, mucho antes del tiempo de aquellos que siguieron solo el tratamiento convencional. Resultados. Obtuvimos la consolidación clínico-radiológica, antes del tiempo descrito según el tratamiento convencional, en un 93,3% de los pacientes. Conclusiones. El uso del campo magnético continuo a baja frecuencia logra una recuperación mucho más pronta en los pacientes afectados con fracturas recientes de Colles, y es una conducta que puede tomarse sin rebasar los límites de la atención primaria (AU)


Objective. To analyse the results of the use electromagnetic therapy in patients with recent Colles fractures. Design. Prospective, comparative and case/control study. Material and methods. The study was conducted in the Ernesto Guevara Polyclinic in Niquero city, province of Granma (Cuba), from September 2009 to April 2010. Two groups were randomly chosen from all the patients with recent Colles fracture. Both were treated conventionally, but one of them (the study group) was also treated with electromagnetic therapy for the following 30 days. At the end of treatment, the groups were compared, statistical tests were applied and conclusions reached. Results. The results were positive in the study group, as by using electromagnetic therapy, the clinical and radiological consolidation was complete in the 93.3% of patients, earlier than the patients who followed only the conventional treatment. Conclusions. We conclude that the use of low frequency magnetic field improves the treatment of recent Colles fractures, and this treatment can be applied within primary care (AU)

ABSTRACT

Objetivo. Conocer el comportamiento de las sepsis respiratorias en los pacientes ingresados en nuestro servicio. Material y método. El estudio, con un diseño descriptivo retrospectivo, se realizó en el Hospital Clínico Quirúrgico Docente Gelasio Calaña de la Hera, Niquero, durante el primer cuatrimestre del año 2008. La muestra de estudio quedó constituida por todos los pacientes portadores de sepsis respiratoria ingresados en nuestro servicio, y los datos se extrajeron de las historias clínicas de estos. Se aplicaron pruebas estadísticas y se llegó a conclusiones. Resultados. Se conoció la incidencia de la morbilidad en el municipio y se identificaron los tipos de sepsis respiratorias más frecuentes así como las principales afecciones crónicas presentes en los pacientes estudiados. El grupo más afectado resultó el de los provenientes del área urbana. Prevalecieron las sepsis respiratorias en el grupo de 65 y más años, y de ellos, las mujeres fueron las más afectadas. Durante el mes de marzo fue ingresado el mayor número de pacientes. Como dato final obtuvimos que la bronconeumonía bacteriana prevaleció sobre el resto de las entidades con un 54,3% y, de las enfermedades crónicas de base, la EPOC registró mayor incidencia con un 35,1%. Conclusiones. Concluimos que a pesar de los esfuerzos, las infecciones respiratorias agudas siguen siendo un serio problema de salud y por ello constituyen el grupo más numeroso entre los pacientes ingresados. Se deben tomar medidas para mejorar los índices de morbimortalidad de estas, sobre todo en los grupos más vulnerables, como son los ancianos y los portadores de afecciones de base (AU)


Objective. To know behavior of respiratory sepses in patients admitted to our department. Material and methods. The study, with a retrospective and descriptive design, was conducted in Gelasio Calaña La Hera clinical-surgical-teaching hospital in Niquero city, from January to April 2008. All the patients admitted to our Department with Respiratory Disease were included and the data were obtained from their clinical records. Statistical tests were applied and conclusions reached. Results. The incidence of morbidity and the most frequent respiratory sepses as well as the principal chronic conditions in the patients studied were identified. The group that was the most affected came from the urban area. Respiratory sepses prevailed in the 65 year and older group, women being the most affected. The greatest number of patients were admitted during the month of March. As final data, we found that that bacterial bronchopneumonia prevailed over the other conditions with a result of 54.3%. Of the chronic baseline diseases, COPD recorded the greatest incidence, that is, 35.1%. Conclusions. We conclude that despite the efforts made, respiratory sepses continue to be a serious health problem in our community. Its makes up the largest group among the hospitalized patients. Steps are needed to improve their morbidity-mortality indexes, above all in the most vulnerable groups, as are the elderly and the carriers of baseline conditions (AU)
