ABSTRACT

Thoracic hematogenic endometriosis is a rare pathology. A clinical course hasn't pathognomic symptoms, because of it, the diagnosis is established due to histological study. The article presented two cases of female patients, who were suffering from thoracic endometriosis. They were hospitalized to the department of thoracic surgery of Municipal multifield hospital No 2 in Saint-Petersburg. The first patient had a posterior mediastinum tumor with asymptomatic disease course. The second patient was with recurrent catamenial pneumothorax.

ABSTRACT

A retrospective analysis of medical history of 1489 patients with spontaneous pneumothorax was made. The patients were hospitalized to the department of thoracic surgery in municipal hospital No 2. According to the data, the incidence rate of primary spontaneous pneumothorax was noted in men during last decade in Saint-Petersburg. The number of patients with secondary spontaneous pneumothorax didn't considerably increase in spite of the growth of incidence of chronic obstructive lung disease. The secondary spontaneous pneumothorax took place in female population two times more, than in male.

ABSTRACT

Results of examination and treatment of 62 patients with spontaneous pneumothorax which appeared against the background of chronic obstructive pulmonary disease during the period from 1998 to 2009 were analyzed retrospectively. Emergent drainage of the pleural cavity was made to all the patients at admission to hospital. Surgical intervention was fulfilled in 28 (45%) patients. Complications at the postoperative period were noted in 54% of the patients. An assessment of occurrence of complications did not reveal their regular increasing along with the degree of bronchial obstruction. Differential approach and selection of high risk patients is considered to be necessary.

ABSTRACT

The work presents an experience with the diagnosing of synchronous tumors of double localization in 256 patients. Operations were performed on 146 of them. Single-stage operations were made in 55 cases. When performing single-stage operations, 20 patients were found to have cancer of the lung with distant metastases or cancer of other localization with a metastasis in the lung. In the other 35 patients the diagnosis of true polyneoplasia was made followed by radical operations. Advantages of single-stage operations are shown, performing the first stage of the operation on the lung being substantiated. The five year survival after surgical treatment of multiple primary cancer was 15%.

ABSTRACT

Surgical treatment of 135 patients with multifocal and metastatic cancer of the lungs and other organs is analyzed. The absence of absolute differential diagnostic differences between true poly-neoplasia and synchronous or metachronous solitary metastases leads to extension of indications to surgical treatment of these patients. Surgical procedures were performed in 109 patients, 37 (33.9%) of them were one-stage. One-stage procedures meet the requirements of radical surgery and are most preferable biologically and clinically. These surgeries decrease treatment duration, prolong survival to 3-5 and more years in many patients. As cytoreductive operations, one-stage resections improve the condition of the patients, enable effective polychemotherapy.

ABSTRACT

An experience with treatment of 53 patients with intrapulmonary metastases of malignant tumors of different nature included 65 surgical interventions, among them there were bilateral operations and reoperations. During the postoperative period complications took place in 5 cases (9.1%). After the operations there were no lethal outcomes. Long-term results of the surgical treatment of intrapulmonary metastases insignificantly differed from those of surgical treatment of primary lung cancer. The surgical treatment is thought to be an expedient method of treatment of intrapulmonary metastases in patients without recurrent tumors, late metastasis of another localization, satisfactory condition of the patient. High effectiveness of thoracoscopic methods for treatment of metastatic lesions of pleura is also noted.

ABSTRACT

The authors share their experiences with examination and surgical treatment of 163 patients with various neoplasms of the mediastinum. The first place among the neoplasms is occupied by tumors of the lymphatic apparatus (33%), the second--by tumors of the thymus (21%). The video-thoracoscopic technique was used in 17 of 131 operations performed. Complications after the operations took place in 6 patients (4.5%), two patients died (1.5%). A conclusion is made that the patients of this category must be treated not only by thoracic surgeons but also by hematologists, neurologists, neurosurgeons and specialists in radiation therapy.

ABSTRACT

An experience with treatment of 29 patients with bilateral malignant tumors includes primary tumors and those of metastatic origin. When large bilateral lung resections are necessary, the consecutive operations are considered to be expedient with intervals of 4-6 weeks. When the operation volume is not more than lobectomy in one side and an atypical or segmental resection in the other side, it is possible to fulfill a one-step intervention from the transsternal access. The surgical treatment for bilateral tumors should be supplemented by radio- and chemotherapy. Surgical treatment for bilateral tumors of the lungs is permissible but in the side of the maximal lesion followed by treatment of the rest of the tumor by nonsurgical methods.

ABSTRACT

An experience with treatment of 35 patients with pulmonary metastases from different tumors is described. In all, 45 surgical interventions were performed which included bilateral operations and reoperations. At the postoperative period complications were observed in 5 cases (14.7%). There were no lethal outcomes after operations. Long-term results of the surgical treatment of patients with pulmonary metastases considerably differed from those observed in patients with primary lung cancer. If there is no recurrent tumor, no distant metastases of another localization and the patient's condition is estimated as satisfactory, the surgical intervention is thought to be a valuable method of treatment for pulmonary metastases.

ABSTRACT

Expanded (13) and combined (9) pneumonectomy with laparotomy and plastics of the bronchial stump with the greater omentum on a vascular pedicle were conducted on 22 patients in the period from 1988 (June) to 1990 (April). The omentum was used for the prevention of bronchial fistulas and correction of defects in the pericardium. Laparotomy and omentopexy did not aggravate the patients' condition essentially and did not affect the terms of treatment. A defect in the bronchial stump occurred only in 2 patients in the post-operative period and closed spontaneously in one of them.
