ABSTRACT

AIM: To compare the efficacy of Jensen and augmented Hummelsheim procedures in the treatment of complete paralytic esotropia.METHOD: A total of 35 patients(44 eyes)who were diagnosed with complete paralytic esotropia from October 2016 to October 2020 were retrospectively analyzed, of which 15 cases(21 eyes)underwent Jensen procedure combined with recession of antagonist muscle(Jensen procedure group), and 20 cases(23 eyes)received augmented Hummelsheim procedure combined with recession of antagonist muscle(Hummelsheim procedure group). The operation time, preoperative and postoperative esotropia deviation, degree of abduction paralysis, recession of medial rectus muscle and cure rate were observed.RESULTS: Clinical data and operation time of the patients in two groups were not statistically significant(P >0.05). During the last follow-up, the esotropia deviation of Jensen procedure group decreased from 102.33±41.70PD to 3.93±4.82PD(P<0.001), and it decreased from 94.75±33.03PD to 2.85±5.96PD in Hummelsheim procedure group(P<0.001), while the degree of abduction paralysis were significantly improved from -4.81±0.40 to -1.57±0.51 in the Jensen procedure group(P<0.001)and from -4.91±0.29 to -1.22±0.42 in Hummelsheim procedure group(P<0.001). Besides, there was no statistical difference in postoperative esotropia deviation between the two groups(P>0.05), but the degree of postoperative abduction paralysis in the Hummelsheim procedure group was significantly better than that of Jensen procedure group(P<0.05). The recession of medial rectus muscle of the two groups were 7.16±2.07 and 6.37±2.34 mm, respectively(P>0.05). During the last follow-up, in the Jensen procedure group, 2 patients were undercorrection(+10PD and +12PD respectively)and 13 cases(87%)were cured. In the Hummelsheim procedure group, 1 patient was undercorrection(+25PD)and 19 patients were cured(95%), and there was no statistical significance in cure rates of the two groups(P=0.565).CONCLUSIONS: Both Jensen procedure and augmented Hummelsheim procedure can effectively treat complete paralytic esotropia, and the latter is more effective in improving the abduction paralysis.

ABSTRACT

AIM: To analyze the clinical characteristics and treatment of patients with acute acquired concomitant esotropia(AACE)in different refractive status.METHODS: A retrospective analysis was conducted on 110 patients with non-type I AACE treated from January 2020 to January 2022. The non-myopic group(30 cases, spherical equivalent>-0.5D)and the myopic group(80 cases, spherical equivalent≤-0.5D)were divided according to the refractive status. The degree of deviation, accommodative convergence and accommodation ratio(AC/A), visual function, and surgical methods were observed. RESULTS: The non-myopic group had no difference in the degree of near deviation [(47.13±23.54)△] and the degree of distant deviation [(48.90±22.59)△](P>0.05); near deviation [(40.49±26.09)△] of myopic group was less than distant deviation [(50.09±25.41)△](P<0.001); and there was no difference in the same distance between the two groups(P>0.05). AC/A in the non-myopic group(5.40±2.23)was higher than that in the myopic group(3.14±3.10; P<0.05). Patients in the myopic group had better near stereopsis than the non-myopic group(P<0.05). The non-myopic group had a variety of surgical methods, while the myopic group mostly used lateral rectus resection or/and medial rectus recession.CONCLUSION: AACE can occur in different refractive status. Non-myopic patients have the same degree of distant and near strabismus, high AC/A, and varied surgical methods. However, myopic patients have less degree of near deviation than distant deviation and have normal AC/A and better near stereopsis, and lateral rectus resection or/and medial rectus recession are commonly used.

ABSTRACT

This study aimed to systematically evaluate the efficacy and safety of different Chinese patent medicines in the treatment of idiopathic membranous nephropathy. The relevant randomized controlled trial(RCT) was retrieved from PubMed, EMbase, Cochrane Library, CNKI, SinoMed, Wanfang, and VIP with the time interval from database inception to December 2022. The Cochrane risk of bias assessment tool was employed to evaluate the quality of the included RCT, and Stata 15.0 and GEMTC to perform the Bayesian network Meta-analysis. Finally, 51 RCTs were included, involving 9 Chinese patent medicines and 3 591 patients. The results of network Meta-analysis showed that in terms of the total effective rate and the increase in plasma albumin, the top three interventions were Zhengqing Fengtongning Sustained Release Tablets + conventional western medicine, Bailing Capsules + conventional western medicine, and Tripterygium Glycosides Tablets + conventional western medicine. In terms of reducing 24-hour urine total protein, the top three interventions were Zhengqing Fengtongning Sustained Release Tablets + conventional western medicine, Shenfukang Capsules +conventional western medicine, and Huangkui Capsules + conventional western medicine. In terms of reducing serum creatinine, the top three interventions were Shenfukang Capsules + conventional western medicine, Bailing Capsules + conventional western medicine, and Zhengqing Fengtongning Sustained Release Tablets + conventional western medicine. In terms of safety, Chinese patent medicines combined with conventional western medicine had fewer adverse reactions than the control group. The results suggest that Chinese patent medicines combined with conventional western medicine can improve the therapeutic effect on idiopathic membranous nephropathy, and differentiated medications can be adopted according to the specific symptoms of patients in clinical treatment. Further validation needs to be carried out in the future with multi-center, large-sample, and high-quality RCT.

ABSTRACT

Objective To analyze the sleep status of infants and young children aged 0~35 months,in order to understand the common sleep problems and possible influencing factors of infants and young children.Methods Healthy infants aged 0~35 months who underwent physical examination in the outpatient Department of Child Health Care,Kunming Children's Hospital from June 2022 to May 2023 were selected as the study subjects.The brief infant sleep questionnaire(BISQ)was completed by their parents.Single factor analysis and Logistic regression analysis were used to analyze the sleep status of infants and young children and the factors affecting sleep problems.Results The primary sleeping mode for infants and young children is sleeping with parents(85.6%).The average daytime sleep duration is(3.51±1.75)hours,nighttime sleep is(8.84±1.41)hours,and total daily sleep is(12.37±2.07)hours.Daytime and total sleep duration decrease gradually with increasing age in each age group(P<0.05).27.7%of infants and young children may experience insufficient sleep duration;the prevalence of sleep problems is 79%,with the highest rate of difficulty falling asleep at 75.3%,followed by night waking(72.2%)and sleep rhythm disturbances(19.7%);278 cases(92.7%)involve involuntary falling asleep.Univariate analysis shows that age and sleep initiation method are related to the occurrence of sleep problems in infants and young children(P<0.05).Logistic regression analysis suggests that young age and involuntary falling asleep are risk factors for the occurrence of sleep problems.Conclusion The incidence of sleep problems in infants and young children is high,which is related to the young age and involuntary sleep onset.It is recommended to train infants and young children to fall asleep autonomously as early as possible,and strengthen the education of sleep habits in infants and young children to reduce the incidence of sleep problems in infants and young children.

ABSTRACT

Background: Despite being prone to reverse causation and having unmeasured confounding factors, many clinical observational studies have highlighted the critical association between basophils, eosinophils, and lymphocytes and atopic dermatitis (AD). Whether these cells play a causal role in AD development remains uncertain. Methods: Data were obtained from the UK Biobank and the Blood Cell Consortium, from a large publicly available genome-wide association study (GWAS) with more than 500,000 subjects of European ancestry and for AD from three independent cohorts with more than 700,000 subjects of European ancestry. We performed single-variable Mendelian randomization (SVMR), followed by multivariable Mendelian randomization (MVMR) to assess the total and direct effects of immune cell counts on AD risk. Results: SVMR estimates showed that genetically predicted higher eosinophil [odds ratio (OR): 1.23, 95% confidence interval (CI): 1.17-1.29, p = 5.85E-16] and basophil counts (OR: 1.11, 95% CI: 1.03-1.19, p = 0.004) had an adverse effect on the risk of AD, while a higher lymphocyte count (OR: 0.93, 95% CI: 0.89-0.98, p = 0.006) decreased the risk. Reverse MR analysis showed higher basophil (beta: 0.04, 95% CI: 0.01-0.07, p = 0.014) and lower lymphocyte counts (beta: -0.05, 95% CI: -0.09 to -0.01, p = 0.021) in patients with AD. In MVMR, the effects of eosinophils (OR: 1.19, 95% CI: 1.09-1.29, p = 8.98E-05), basophils (OR: 1.19, 95% CI: 1.14-1.24, p = 3.72E-15), and lymphocytes (OR: 0.93, 95% CI: 0.89-0.98, p = 0.006) were still significant. Discussion: Mendelian randomization (MR) findings suggest that an increase in the eosinophil and basophil counts and a decrease in the lymphocyte counts are potential causal risk factors for AD. These risk factors are independent of each other.

ABSTRACT

AIM: To compare the consistency and feasibility of objective ocular torsion measured with GMPE module-based optical coherence tomography(OCT)and fundus color photography(FCP).METHODS: Patients were enrolled in our strabismus clinic from December 2020 to March 2021, and the objective ocular torsion of the eyes was measured by both GMPE module-based OCT and FCP on the same day. FCP was used to measure the fovea-disc angle(FDA)manually by using the Adobe Photoshop software, while the GMPE module-based OCT software positioned automatically the macula and the center of the optic disc to measure the FDA.RESULTS: Fifty-five patients were included, the FDA measured by OCT was -6.6°±4.5° in the right eye and -8.8°±4.7° in the left eye, respectively; The FDA measured by FCP was -6.6°±4.7° in the right eye and -8.4°±4.1° in the left eye, respectively, with no statistically significant difference between the results of the two methods(Pright eye=0.90, Pleft eye=0.08). In patients with exotropia, the FDA measured by OCT was -5.8°±4.9° in the right eye and -9.1°±4.5° in the left eye, respectively, the FDA measured by FCP was -5.7°±5.0° in the right eye and -8.6°±4.3° in the left eye, respectively,(Pright eye=0.75, Pleft eye=0.15). Similarly, the patients with esotropia, the FDA measured by OCT was -9.0°±7.3° in the right eye and -11.3°±3.5° in the left eye, respectively, while the FDA measured by FCP was -10.0°±7.0° in the right and -10.1°±2.8° in the left eye(Pright eye=0.21, Pleft eye=0.10), respectively. There were no significant differences between the two methods in patients with esotropia or exotropia(P>0.05). The results of both Pearson test and Bland-Altman analysis were highly correlated(rright eye=0.93, rleft eye=0.94, P<0.01). CONCLUSION: GMPE module-based OCT can be used for objective ocular torsion measurement with high reliability and reproducibility, and is a promising clinical alternative to the fundus color photographic method.

ABSTRACT

Objective:To evaluate the implementation of the China Healthcare Improvement Initiative(CHII)from 2018 to 2020 in 143 tertiary public hospitals in China.Methods:In March 2019 and from January to March in 2021, 143 tertiary public hospitals in 31 provinces of China were investigated using the unified " medical institution questionnaire Ⅰ" and " medical institution Questionnaire Ⅱ" . The data were collected by means of hospital self-report and expert on-site scoring. Descriptive and inferential statistical analysis were used to analyze the data, and the data of two cross-sectional surveys were compared and analyzed.Results:The average score rate of implementing CHII in 143 sample hospitals in 2020 was 88.9%, which was higher than that in 2018(84.4%). The appointment diagnosis and treatment system, clinical pathway management system, day service, smart hospital and humanistic service were significantly improved. In 2020, the average score rate of logistics service, high quality nursing service and clinical pathway management system was higher than 95%, while the average score rate of day service, telemedicine system and medical social work system was lower than 85%. The total score rate of general hospitals was significantly higher than that of specialized hospitals( P<0.001). In 2020, the proportion of hospitals with full marks in 29 secondary indicators(74.4%)was more than 80%, reaching the standard level. Conclusions:The implementation level of CHII in tertiary public hospitals in China has been improved continuously and made significant progress, but some dimensions and indicators need to be further improved.

OBJECTIVE@#To study the percentage of the measured values of the main pulmonary ventilation function parameters in their predicted values based on Zapletal equation among healthy children aged 5-14 years in Kunming, China, and to provide a basis for accurate judgment of pulmonary ventilation function in clinical practice.@*METHODS@#A total of 702 healthy children aged 5-14 years (352 boys and 350 girls) from Kunming were enrolled. The Jaeger spirometer was used to measure the nine indices:forced vital capacity (FVC), forced expiratory volume in one second (FEV1), ratio of forced expiratory volume in one second to forced vital capacity (FEV1/FVC), maximal mid-expiratory flow (MMEF), forced expiratory flow at 25% of forced vital capacity (FEF25), forced expiratory flow at 50% of forced vital capacity (FEF50), forced expiratory flow at 75% of forced vital capacity (FEF75), peak expiratory flow (PEF), and maximal voluntary ventilation (MVV). The values obtained from the Zapletal equation of predicted values provided by the spirometer were used as the predicted values of children, and the percentage of measured values in predicted values was calculated.@*RESULTS@#In the 702 children, the percentages of the measured values of the main pulmonary ventilation function parameters PEF, FVC, FEV1, FEV1/FVC, and MVV in their predicted values fluctuated from 102% to 114%, 94% to 108%, 98% to 113%, 98% to 107%, and 141% to 183% respectively. As for the main airway velocity parameters, the percentages of the measured values of FEF25, FEF50, FEF75, and MMEF in their predicted values fluctuated from 98% to 116%, 85% to 102%, 71% to 98%, and 83% to 100% respectively. The percentages of the measured values of PEF, FVC, FEV1, FEV1/FVC, MVV, FEF25, FEF50, FEF75, and MMEF in their predicted values had the lower limits of normal of 88.2%, 88.4%, 92.0%, 94.4%, 118.5%, 82.9%, 70.0%, 62.1%, and 70.1% respectively.@*CONCLUSIONS@#There are differences between pulmonary ventilation function parameter levels and normal values provided by Zapletal equation in healthy children aged 5-14 years in Kunming. As for the pulmonary ventilation function parameters of PEF, FVC, FEV, FEV1/FVC, MVV, FEF25, FEF50, FEF75, and MMEF in these children, the lower limits of normal of measured values in predicted values may be determined as 88.2%, 88.4%, 92.0%, 94.4%, 118.5%, 82.9%, 70.0%, 62.1%, and 70.1% respectively.

ABSTRACT

BACKGROUND: Multiparameter, multimodality 18 F-FDG PET/MRI holds great potential for the diagnosis of cervical cancer based on the correlation between tumor glucose metabolism and imaging parameters. PURPOSE: To characterize the heterogeneity of tumor glucose metabolism by evaluating the correlation between 18 F-FDG uptake parameters and multiparametric functional MRI metrics in cervical carcinoma. STUDY TYPE: Retrospective. POPULATION: Fifty-four patients with cervical carcinoma. FIELD STRENGTH/SEQUENCE: Hybrid PET/MR (3T), multi-b DWI, and R2* mapping. ASSESSMENT: The maximum and mean standardized uptake values (SUVmax and SUVmean , respectively) from PET and functional MRI metrics (D, D*, f, and R2*) were obtained. Cervical carcinoma tissues also underwent HIF-1α, VEGF, and GLUT-1 immunohistochemical staining. STATISTICAL TESTS: Single-factor Spearman rank and Pearson correlation analysis and multiple linear regression (MLR) analysis were applied. RESULTS: R2*, D, and f have different degrees of correlation (moderate, weak, moderately strong correlation, respectively) with SUVmax and SUVmean (r = 0.530 and 0.527, and P < 0.001 for R2*; r = -0.292 and -0.291, and P < 0.05 for D; r = 0.539 and 0.520, and P < 0.001 for f, respectively). Immunohistochemical staining showed that HIF-1α expression has a moderate degree of correlation with R2* (r = 0.491; P < 0.001); GLUT-1 expression was significantly correlated with SUVmax and SUVmean (r = 0.633 and 0.622; P < 0.001), and VEGF expression had a moderately strong correlation with f (r = 0.457; P = 0.001). If SUVmax is the dependent variable, MLR yields an R-squared value after adjustment (adjusted R-squared) = 0.358, and F = 10.833 (P < 0.001), and the fitting linear equation is Y (SUVmax ) = 9.184 + 0.161X1 (R2*)+50.343X2 (f)-4.780 (D). Otherwise, MLR yields the adjusted R-squared = 0.342, and F = 10.187 (P < 0.001), and the linear regression equation is Y (SUVmean ) = 5.925 + 0.102X1 (R2*)+28.029X2 (f)-2.907X3 (D). DATA CONCLUSION: The functional MRI sequence parameters R2*, f, and D can provide information on the hypoxic condition, blood perfusion, and molecular diffusion of the tumor. 18 F-FDG PET/MR multi-imaging technique can be adopted to evaluate the heterogeneity of glucose metabolism in cervical carcinoma. ABSTRACT

The growth of solid tumors can lead to changes in the hypoxic microenvironment of local tissues,and the most important regulatory factor is the hypoxia-inducible factors (HIFs).In the case of hypoxia,the downstream target genes regulated by HIF-1α regulation mainly are vascular endothelial growth factor (VEGF) and glucose transporter 1 (GLUT-1) genes,etc.In recent years,multimodality PET and MRI imaging technologies have been widely used in diagnosis of tumor diseases,and have confirmed that the parameters obtained by imaging are associated with the expression of tumor hypoxic related molecules.The molecular expression of HIF-1α within the tumor and its downstream target genes VEGF and GLUT-1 detected by PET and multifunctional MRI sequential were reviewed in this article.

ABSTRACT

<p><b>OBJECTIVE</b>To explore occurrence and risk factors of sarcopenia in patients with hip facture.</p><p><b>METHODS</b>From May 2013 to January 2017, 187 patients with hip fractures were collected, including 99 males and 88 females aged from 50 to 95 years old with an average age of (77.40±10.58) years old. General conditions, appendicular skeletal muscle index (ASMI), total fat mass, bone mineral content (BMC), body mass index (BMI), grip strength, preoperative blood vitamin D (VITD), albumin, American Society of Anesthesiologists(ASA) classification and new mobility scores(NMS) were observed and analyzed. According to grip strength and ASMI, patients were divided into sarcopina group and non-sarcopina group, univariate and multivariate statistical methods were used to analyzed.</p><p><b>RESULTS</b>Ninty-nine patients(52.9%) were diagnosed as sarcopenia. Compared with non-sarcopenia group, occurrence of sarcopenia was associated with advanced age, high ASA, low total fat mass, low bone mineral content, low BMI, low albumin, and low NMS. Subsequent binary logistic regression analysis showed that advanced age (OR=1.804, =0.048), high ASA score (OR=3.052, =0.001), low fat content (OR=0.843, =0.006), low bone mineral salt(OR=0.203, =0.026) were risk factors of hip fracture patient with sarcopenia.</p><p><b>CONCLUSIONS</b>Old age, high ASA score, low fat content, low bone mineral content may be related risk factors for sarcopenia in hip fracture patients.</p>

ABSTRACT

BACKGROUND: Hyaluronic acid is an important component of extracellular matrix, has good biocompatibility, unique rheological properties and a variety of physiological functions. Therefore, it has wide application prospect in the tissue engineering. OBJECTIVE: To review the latest research progress in hyaluronic acid scaffold materials in different directions of tissue engineering. METHODS: Using the keywords of "hyaluronic acid, tissue engineering, regeneration, scaffold" in English and Chinese, we retrieved relevant articles published from January 2013 to September 2016 in the databases of PubMed, ScienceDirect and CNKI. RESULTS AND CONCLUSION: The application of hyaluronic acid tissue engineering scaffold materials mainly focuses on the regeneration of bone, cartilage, cardiovascular and nerve. The researchers optimize the composition, ratio of different materials, and modification or cross-linking methods to obtain the ideal scaffold materials to match the mechanical properties and physiological activity of the target tissue. However, there are not enough data on the influence of the molecular weight of hyaluronic acid and the original source of other raw materials on the scaffold properties. Further investigations are needed on the clinical transformation of hyaluronic acid tissue engineering products.

ABSTRACT

The present study aimed at determining whether berberine can enhance the antidiabetic effects and alleviate the adverse effects of canagliflozin in diabetes mellitus. Streptozotocin-induced diabetic mice were introduced, and the combined effects of berberine and canagliflozin on glucose metabolism and kidney functions were investigated. Our results showed that berberine combined with canagliflozin (BC) increased reduction of fasting and postprandial blood glucose, diet, and water intake compared with berberine or canagliflozin alone. Interestingly, BC showed greater decrease in blood urea nitrogen and creatinine levels and lower total urine glucose excretion than canagliflozin alone. In addition, BC showed increased phosphorylated 5' AMP-activated protein kinase (pAMPK) expression and decreased tumor necrosis factor alpha (TNFα) levels in kidneys, compared with berberine or canagliflozin alone. These results indicated that BC was a stronger antidiabetic than berberine or canagliflozin alone with less negative side effects on the kidneys in the diabetic mice. The antidiabetic effect was likely to be mediated by synergically promoting the expression of pAMPK and reducing the expression of TNFα in kidneys. The present study represented the first report that canagliflozin combined with berberine was a promising treatment for diabetes mellitus. The exact underlying mechanisms of action should be investigated in future studies.

Objective To study the role of dependence of stromal cell derived factor‐1(SDF‐1) on myocardial infarction an‐giogenesis and function recovery .Methods The rats were randomly divided into the test group (65 cases) and the treatment group (64 cases) ,in which 5 rats in the test group were randomly killed on 1 ,2 ,4 ,7 ,14 ,28 d before and after myocardial infarction(infarc‐tion group) and the SDF‐1 protein level was detected .At the same time the sham operation group(5 cases) was set up;64 rats in the treatment group were randomly divided into four groups ,16 cases in each group ,which were infarction+SDF‐1 group ,infarct+anti‐SDF‐1 group ,infarct+normal saline group and normal+normal saline group .After the corresponding processing in each group ,the SDF‐1 levels after myocardial infarction ,MSCs homing amount ,neovascularization density ,EF and FS levels were compared among various groups .Results The SDF‐1 level after 1-7 d in the infarction group was significantly higher than that in the sham opera‐tion group ,and reached a peak level ,then decreased gradually and restored to normal on 14 d ,the difference was statistically signifi‐cant (P<0 .05) .The MSCs homing amount and neovascularization density level in the SDF‐1 treatment groups were significantly higher than those in other groups ,which in the infarction+anti‐SDF‐1 group were significantly lower than those in the infarction+normal saline group ,the differences were statistically significant (all P<0 .05) .The cardiac function improvement situation in the infarction +SDF‐1 group was significantly better than that in other groups ,the difference was statistically significant (P<0 .05) . Conclusion SDF‐1 can promote angiogenesis in myocardial infarction ,also improve the heart function .

ABSTRACT

AlM: To explore the expression and significance of cysteine- rich 61 ( CCN1/Cyr61 ) in oxygen - induced retinal neovascularization ( RNV) of mice and study the inhibition effect of CCN1 specific siRNA on RNV. METHODS:Two hundred healthy C57BL/6J mice were chosen and randomly divided into control group, hyperxia group, hyperxia control group and CCN1 treated group, with 50 mice in each group. The hyperxia control group was treated with vector plasmids by intravitreal injection. The CCN1 treated group received CCN1 siRNA recombinant plasmids by intravitreal injection. Adenosine diphosphate-ase ( ADPase) stained retina flat-mounts was performed to assess the retinal vascular profiles, HE staining was applied to count the number of vascular endothelial cell nuclei breaking through the internal limiting membrane, protein and mRNA level expression of CCN1 were measured by immunohistochemistry, Western blot and RT-PCR. RESULTS: There were large nonperfusion area and a large number of vascular endothelial cell nuclei breaking through the internal limiting membrane ( 25. 25 ± 1. 26;23. 12 ± 1. 16 ) in the hyperxia group and the hyperxia control group. Regions of nonperfusion and vascular endothelial cell nuclei (8. 47±1. 15) were decreased in the CCN1 treated group compared to the hyperxia group and the hyperxia control group. Compared with the control group, there were high protein and mRNA expression of CCN1 in the hyperxia group and the hyperxia control group. The expression of CCN1 protein and mRNA were decreased in the CCN1 treated group compared with the hyperxia group and hyperxia control group (all P CONCLUSlON: The abnormal expression of CCN1 has close relation with RNV. The development of RNV can be markedly inhibited by RNA interference targeting CCN1, which, we believe, will provide new molecular targets and a rationale for clinical developing new strategy for ROP therapy.

ABSTRACT

Up to December of 2011, the domestic and overseas literatures of acupuncture for smoking cessation are searched and collected through Pubmed, CNKI, Wanfang and Chongqing VIP databases, which are analyzed from treatment method, action mechanism, influencing factors of efficacy and efficacy evaluation research and so on. The result shows that (1) acupuncture has positive effect on smoking cessation, and large clinical trials has been carried out. However, most of them are needling methods and the short-term effect observation, which lack of long-term efficacy research of withdrawal symptoms, especially continuous withdrawal symptoms and index of life quality. (2) Because of unified clinical treatment, manipulation standard and curative effect criteria, it is difficult to make comparison among them. (3) The study of mechanism on acupuncture for smoking cessation is not of in-depth. In future, clinical research project of acupuncture on smoking cessation should be optimized and regulated, including the unified treatment, manipulation standard and curative effect criteria, also long-term researches with large-scale samples should be launched to confirm effect ofacupuncture for smoking cessation to further discuss its action mechanism.

ABSTRACT

<p><b>BACKGROUND</b>Visfatin, a visceral fat-derived adipocytokine, plays a significant physiological function in lipid metabolism. However, the precise function of visfatin and its regulation by thyroid hormones are still unknown. This study observed the plasma visfatin concentrations in subjects with hyperthyroidism and hypothyroidism in vivo, and investigated the possible regulation mechanism between visfatin and tri-iodothyronine (T3) in vitro as a further interpretation.</p><p><b>METHODS</b>The experiment in vivo included clinical subjects (57 patients with thyroid dysfunction and 29 euthyroid healthy volunteers) and an animal model (24 Wistar rats). All subjects were divided into hyperthyroidism, hypothyroidism and euthyroidism groups, with plasma thyroid hormones, thyrotropin, visfatin and triglyceride concentrations determined. Visfatin mRNA expression in visceral fat and liver of rats was detected by quantitative real-time reverse transcription-polymerase chain reaction (RT-PCR). The experiment in vitro studied 3T3-L1 cells and visfatin mRNA expression under nine different T3 concentrations (0, 0.1, 0.25, 0.5, 1, 5, 10, 20, 100 nmol/L) using quantitative real-time RT-PCR.</p><p><b>RESULTS</b>Clinical subjects and animal models showed elevated plasma visfatin concentrations in the hyperthyroidism group (20.466 ng/ml (15.263, 26.795 ng/ml) and (1209.164±165.292) ng/L) and hypothyroidism group (12.457 ng/ml (11.115, 15.454 ng/ml) and (1205.425±109.200) ng/L) compared to euthyroidism group (6.891 ng/ml (5.888, 8.803 ng/ml) and (926.650±54.002) ng/L, P<0.001). For animal models, visfatin mRNA expression in visceral fat in the hyperthyroidism and hypothyroidism groups increased about 3.33-fold and 1.98-fold compared to the euthyroidism group (P<0.001), which was positively correlated with plasma visfatin concentrations (r=0.713, P<0.001). However, no significant group difference (P>0.05) and correlation (r=0.121, P=0.572) was found in the liver. T3 induced a remarkable increase of visfatin mRNA expression in 3T3-L1 cells at low concentrations (0-0.5 nmol/L T3) followed by a sharp decrease at higher concentrations (0.5-100 nmol/L T3), with an inflection point at 0.5 nmol/L T3.</p><p><b>CONCLUSION</b>Elevated circulating visfatin levels in subjects with hyperthyroidism and hypothyroidism are possibly due to an increase of visfatin mRNA expression in visceral fat, and a nonlinear regulation mechanism on visfatin mRNA expression under various T3 concentrations might be involved.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To compare the efficacy differences between acupuncture-moxibustion and medication in xerophthalmia.</p><p><b>METHODS</b>Eighty cases of xerophthalmia were randomly divided into an acupuncture-moxibustion group and a medication group, 40 cases in each group. In acupuncture-moxibustion group, acupuncture was applied to the local and distal points, such as Jingming (BL 1), Cuanzhu (BL 2), Taiyang (EX-HN 5) and Quchi (LI 11) etc., combined with non-smoking moxibustion. In medication group, Sodium Hyaluronate eye drops were administered, three times per day, 1 drop each time. Before and after treatment, tear secretion volume (Schirmer's test), break-up time (BUT), symptom score, visual function score and tear film grade were observed.</p><p><b>RESULTS</b>The total effective rate was 73.1% (57/78) in acupuncture-moxibustion group, and was 37.2% (29/78) in medication group, indicating significant statistical difference in comparison (P < 0.05). There was a significant difference in statistics in tear secretion volume between two groups after treatment (P < 0.05), in which, the result in acupuncture-moxibustion group was superior to that in medication group. The significant statistical differences presented in tear secretion volume, BUT, symptom score, visual function score and tear film grade in comparison before and after treatment in acupuncture-moxibustion group (all P < 0.05). The significant statistical difference presented in symptom score and tear film grade before and after treatment in medication group (both P < 0.05).</p><p><b>CONCLUSION</b>Acupuncture-moxibustion apparently relieves the symptoms of xerophthalmia, promotes tear secretion and improves the life quality of patients.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To survey the prevalence of enterotoxigenic Escherichia coli (ETEC) and Laribacter hongkongensis (LH) and their drug resistance in diarrhea patients in Guangzhou.</p><p><b>METHODS</b>We detected 646 fecal cases collected between Sep 2008 and Oct 2009 from the out-patient and emergency departments in a hospital. EC enriched culture medium was used for enrichment. MAC- and CMAC-specific culture media were used to isolate ETEC and LH from the specimens. The biochemical agents API20NE and API20E were employed for biochemical identification, and PCR was used for genetic identification. K-B disk diffusion method was used for antimicrobial susceptibility testing.</p><p><b>RESULTS</b>No LH was detected in the total 646 patients, and 38 patients were positive for ETEC, with a detection rate of 6%. Antibiotics resistance test showed that 38 strains of ETEC had a high resistance rate to penicillin, tetracycline and sulfa, but remained sensitive to cephalosporins.</p><p><b>CONCLUSIONS</b>LH may have a low prevalence in Guangzhou. The incidence of diarrhea caused by ETEC tends to decrease as compared with that a decade ago, and further multi-center survey is needed for confirmation. Consumption of aquatic products may be one of the major risk factors for ETEC infection. Cephalosporins can be used for ETEC-induced diarrhea.</p>

Objective To observe the effect of catgut embedding(CE) at "Shimen"(CV 5) on the endometrial receptivity,pinopodes expression of the endometrial surface and leukaemia inhibitory factor(LIF) immunoactivity during periimplantation period.Methods Thirty Kunming mice were randomly divided into control,CE of "Shimen"(CV 5,CE-CV 5) and medication groups(n=10/group).For mice of CE-CV 5 group,a piece of catgut was embedded into the subcutaneous tissue of "Shimen"(CV 5) on the day when the vagina-discharged epistom was found(pregnancy day 1,Pd 1).On Pd 4,mifepristone(0.1 mL,diluted in propylene glycol) was subcutaneously injected into CV 5 in rats of medication group.The expression of endometrial pinopodes(sampled on Pd 5) was observed by scanning electron microscopy.The immunoactivity of LIF of the endometrium was detected by immunohistochemistry.Results Compared with control group,the mean optical density(OD) values of LIF in CE-CV 5 and medication groups decreased significantly(P0.05).Electronic microscopic observation showed 1) abundant mature pinopodes on the endometrial surface,with a large number of well-stacked ecphyma in control group;2) fewer pinopodes and well-arranged smooth epithelial cells on the endometrial surface in catgut embedding group;and 3) no pinopodes found in the endometrium of medication group.Conclusion Catgut embedding at "Shimen"(CV 5) can suppress the establishment of the mouse endometrial receptivity,which is possibly related to its effect in inhibiting the expression of endometrial pinopodes and LIF.
