ABSTRACT

The surgical technique, clinical and anatomic explanation of the osteoplastic part of the compression arthrodesis of a hip joint has been demonstrated. The spongiform vascularized transplant from the upper flaring portion of the illeum was used for the operation. The basis for the transplant flap is a front part of musculus gluteus medius with arteries, veins and a nerve.

ABSTRACT

Polarographic examination was conducted in 10 patients with incompetence of the sphincter ani to study the effect of biological preparation of the gracilis muscle for creation of intestinal obturator apparatus. It was established that in initial 20% reduction of pO2, in response to compression of additional vessels and training of 30 to 60 minutes not only a sufficient arterial flow of blood to the muscle through its main supplying vessel develops but also the previously nonfunctioning network of anastomoses. Stabilization of pO2 is an objective criterion of the preparedness of the muscle for transplantation. The method for preoperative training of the transplant is quite simple and reliable, which allows it to be applied in clinical practice.

ABSTRACT

Adaptive reaction to new conditions of functioning were studied morphofunctionally in an intestinal sphincteric apparatus formed by mm. gracilis, adductor longus, rectus femoris and semitendinosus in 100 experimental animals. It is established that reorganisation of vascular bed and muscular grafts mobilitive activity finishes up by 30th postoperative day and it is day 90 for morphological structures. Even at distant terms of the observation transplants preserve the structure of muscular tissue. Mm. gracilis and rectus femoris were established to be the preferable material in sphincteroplasties.

ABSTRACT

The authors give clinico-anatomical grounds of saving operations for benign and malignant tumors of the proximal part of the thigh bone with vascularized grafts on a fixed vessel-nerve pedicle. In order to make up defects of the bone the authors propose transplants from the upper flaring portion of the ilium on a "pedicle" consisting of the superior gluteal artery, veins and nerve, and a method of creation of a fascial-muscular capsule of the joint with the removal of the tumor together with the capsule and part of the muscles and the following individual prosthesis.

ABSTRACT

It is stated on the base of angioarchitectonics and innervation research of the long femoral adductor muscle, that the fascicle with the muscle branches of the long femoral adductor muscle should be considered to be the major vascular-nervous one. Only the transplants, cut out of the upper and middle one third of the muscle can be used for sphincteroplasty.

ABSTRACT

After investigations performed concerning the hemomicrocirculatory bed of the m. gracilis, m. rectus femoris, m. adductor longus and m. semitendinosus in 100 dogs transplanted on the perineum in order to form the rectal closing apparatus, according the technique elaborated, it has been stated that reaction of the blood bed of the muscle flaps in all experimental series is identical. During the restorative process of the disturbed hemocirculation in the muscle flap and in the intestinal closing apparatus built, certain phasesness in development of adaptive reactions is observed, that corresponds to the known syndrome "economization" microcirculation.

ABSTRACT

By means of micro- and macropreparation and morphometry methods in 200 corpses of children and adult persons, it has been stated that skeletotopy and syntopy of the aortal isthmus depends on the constitution of the person. Certain objective criteria have been elaborated for surgical approaches to the aortal isthmus. In little children, during early and first childhood, anterolateral thoracotomy should be performed in the III intercostal space; in persons with brachymorphose type of constitution--left-sided thoracotomy in the IV intercostal space; in persons with transitional and long-narrow thorax--left-sided thoracotomy in the V intercostal space with resection of the V rib.

ABSTRACT

Three series of chronic experiments for production of a dosed coarctation of the aorta (1/2, 2/3 and 3/4 of its lumen) have been carried out in 96 dogs. The postoperative period in all the animals had a smooth course. After the operation, by means of functional and pathomorphological investigation architectonical peculiarities of the eye ball vessels are studied. For comparison of the changes occurring in the eye ball vessels under the given pathology, its normal blood supply has been studied in 20 dogs. At various degree of the aortal coarctation already during the first week of the experiment, certain changes develop in the vascular system of the eye ball coats: spasm of the arterioles, up to the capillaries and dilatation of the venules. Vascular convolution of various degree is revealed. The changes described are directly dependent on the degree of the aorta narrowing and duration of the experiment.
