La enfermedad de Graves en relación con hipotiroidismo es excepcional, y a su diagnóstico se llega siempre a partir del estudio de la función tiroidea en un paciente con oftalmopatía compatible. Ésta, a su vez, empeora con la disfunción de la glándula. Se presenta el caso de una paciente con enfermedad de Graves y oftalmopatía, que experimentó variaciones espontáneas de la función tiroidea durante un período de seguimiento de 7 años, y que atravesó por fases de hipotiroidismo subclínico, hipotiroidismo franco, normofunción, hipertiroidismo subclínico y, quizá, hipertiroidismo franco (aunque este extremo no pudo confirmarse por interferencia de la medicación administrada). Las variaciones hormonales no repercutieron, al menos de modo agudo, en el curso de la orbitopatía, que mejoró probablemente por el simple paso del tiempo. Se destaca la dificultad diagnóstica de los casos de enfermedad de Graves con formas clínicas atípicas de presentación (AU)
The association of Graves disease and hypothyroidism is exceptional. Diagnosis is made by studying thyroid function in patients with ophthalmopathy, which in turn deteriorates with thyroid dysfunction. We describe the case of a female patient with Graves disease and ophthalmopathy who showed spontaneous variability in thyroid function during a 7-year follow-upperiod. Various phases included subclinical and overt hypothyroidism, normofunction, as well as subclinical and probably overt hyperthyroidism (the latter phase could not be confirmed due to interference of the drugs administered). These hormone variations did not acutely affect the clinical course of the orbitopathy, which improved, probably due to the passage of time. We highlight the difficulty of diagnosing Graves disease in patients with an atypical form of presentation (AU)
