ABSTRACT

Background: Different questionnaires have been developed globally to assess and compare the impact of food allergy on the quality of life. The aim of this study was to validate a Spanish translation of the Food Allergy Independent Measure (FAIM) and the EuroPrevall Food Allergy–Quality of Life Questionnaire–Teenage Form (FAQLQ-TF) for adolescents aged 13–17 years. Methods: Sixty adolescents diagnosed with immunoglobulin E-mediated allergy to food completed the questionnaires. Cronbach’s alpha was used to assess internal consistency; correlation between FAQLQ-TF and FAIM was used to test construct validity. The discriminant validity was evaluated by comparison with the number of offending foods, the perceived impact on social life, the diagnosis of anaphylaxis, and the previous prescription of adrenaline auto--injectors (AAI). Results: No question fulfilled criteria to be removed from the questionnaire. For FAIM, Cronbach’s alpha = 0.763, and for the four domains of FAQLQ-TF, Cronbach’s alpha = 0.797–0.847. A significant correlation existed between FAQLQ-TF and FAIM, and of both of them with anaphylaxis and the prescription of AAI (P ˂ 0.05 for all comparisons). Conclusions: This Spanish translation of FAIM and FAQLQ-TF for adolescents had good internal consistency and construct validity as well as ability to discriminate patients according to the number of foods to avoid, impact on social life, diagnosis of anaphylaxis, and AAI prescription (AU)

Background Different questionnaires have been developed to measure quality of life (QoL) in patients with food allergy. Our aim was to validate a Spanish translation of the Food Allergy Independent Measure (FAIM) and the EuroPrevall Food Allergy Quality of Life Questionnaire—Child Form (FAQLQ-CF) for children aged 8–12 years.Methods Sixty children with a diagnosis of IgE-mediated allergy to food completed the questionnaires. The internal consistency was evaluated with Cronbach’s alpha. The correlation of FAQLQ-CF with FAIM was assessed to test construct validity. We compared both values with the diagnosis of anaphylaxis to evaluate discriminant validity.Results Cronbach’s alpha was in the range of 0.654–0.863 for the four domains of FAQLQ-CF and 0.779 for FAIM. There were no criteria to remove questions from the questionnaires. Significant correlations could be found between FAQLQ-CF and the number of offending foods and the impact on social life (all r > 0.33, P < 0.01), and between FAIM and anaphylaxis.Conclusions The Spanish translation of FAQLQ-CF showed acceptable internal consistency, good construct validity, and capacity to discriminate patients depending on the number of foods to avoid and the impact on social life. FAIM showed good discriminant capacity for anaphylaxis (AU).

ABSTRACT

Los abscesos odontogénicos son un motivo de consulta frecuente en Pediatría, y tienen como causa principal la caries, por deficiente higiene bucal. Sin embargo, en casos de refractariedad al tratamiento antibiótico o empeoramiento de la clínica se deben barajar otras entidades, entre las que se encuentra la miasis oral. Esta patología, aunque es anecdótica en nuestro medio, debe conocerse para poder instaurar de forma precoz el tratamiento, que consiste en la retirada total de las larvas, profilaxis antibiótica de amplio espectro y, en caso de miasis profundas o extensas, ivermectina. Presentamos el caso de una miasis en cavidad bucal por Musca domestica (familia Muscidae) (AU)


Odontogenic abscesses are a frequent reason for consultation in pediatrics, and their main cause is dental caries, due to poor oral hygiene. However, in cases of refractoriness to antibiotic treatment or worsening of the symptoms, other entities should be considered, among which is oral myiasis. This pathology, although it is anecdotal in our environment, must be known in order to establish early treatment, which consists of total removal of the larvae, broad-spectrum antibiotic prophylaxis and, in the case of deep or extensive myiasis, ivermectin. We present the case of a myiasis in the oral cavity caused by Musca domestica (Muscidae family) (AU)

ABSTRACT

Las adenopatías son la tumoración cervical benigna más frecuente en los niños, pero se deben sospechar otras patologías en caso de localización atípica, crecimiento rápido, consistencia pétrea, signos inflamatorios o supuración espontánea. La fístula del seno piriforme es una malformación congénita infrecuente de los arcos branquiales. Tras una infección, da lugar a una pseudotumoración que normalmente cursa con signos inflamatorios locales, fiebre y odinofagia, por lo que el tratamiento del proceso agudo es la antibioterapia sistémica de amplio espectro asociada en ocasiones a drenaje. El estudio de toda masa cervical debe iniciarse con una ecografía, a completar con una tomografía computarizada con contraste en caso de hallazgos sugestivos de absceso adyacente al tiroides o inflamación del mismo, que pueden orientar hacia fístula del seno piriforme. El método diagnóstico de confirmación es el examen endoscópico en el quirófano bajo anestesia general, que permite además realizar el tratamiento quirúrgico definitivo. Presentamos el caso clínico de una paciente que acude remitida desde Atención Primaria tras detectársele una tumoración indurada a nivel paramedial izquierda como única manifestación, consecuencia de una fístula de seno piriforme


Lymphadenopathy is the most common benign cervical tumor in children, but other pathologies should be suspected in case of atypical location, rapid growth, tougher consistency, inflammatory signs or spontaneous suppuration. Pyriform sinus fistula is an unusual congenital malformation of the branchial arches. After an infection, it results in a pseudotumoration that usually occurs with local inflammatory signs, fever and odynophagia, so that the treatment of the acute process is broad-spectrum systemic antibiotic therapy sometimes associated with drainage. The study of any cervical mass should begin with an ultrasound, to be completed with a CT scan with contrast in case of suggestive findings of abscess adjacent to the thyroid or inflammation of the thyroid, that can point towards a pyriform sinus fistula. The diagnostic method of choice is the endoscopic examination in the operating room under general anesthesia, that also allows the definitive surgical treatment. We present the clinical case of a patient who comes from Primary Care after detecting an indurated tumor at the left paramedial level as the only manifestation, consequence of a pyriform sinus fistula

ABSTRACT

Las enfermedades exantemáticas suelen tener un patrón definido típico que facilita el diagnóstico clínico, aunque ocasionalmente surgen dudas por similitud de patologías y es preciso conocer el agente etiológico. La enfermedad boca mano pie siempre ha sido una de las patologías sencillas de reconocer por su clásica afectación cutánea y mucosa; sin embargo, en los últimos años se ha visto un viraje en la distribución, extensión y morfología de las lesiones, que le otorgan gran capacidad de simulación. En los casos donde la clínica es atípica, y un erróneo etiquetado diagnóstico puede privar a un paciente de la vacunación frente a la varicela, la serología puede ser de utilidad. Presentamos un caso de enfermedad boca-mano-pie atípico en un paciente sin dermopatía previa


The exanthematic diseases usually have a typical defined pattern that facilitates the clinical diagnosis, although occasionally doubts arise due to similarity of pathologies and it is necessary to know the etiological agent. The Hand-foot-and-mouth disease has always been one of the easiest pathologies to be recognized for its classic skin and mucous involvement; however, nowadays there has been a change in the distribution, extension and morphology of lesions, which give it great simulation capacity. In cases where the clinical manifestation is atypical and a wrong labeling can deprive a patient from chickenpox vaccine, serology can be useful. We present a case of atypical Hand-foot-and-mouth disease in a patient without previous dermopathy
