ABSTRACT

Resumo Apesar do crescente interesse pela dimensão da religiosidade na área da saúde, muçulmanos têm sido negligenciados pelo campo psi. Por um lado, profissionais pouco conhecem essa religião e seus seguidores; por outro, a população muçulmana cresce tanto pelas reversões quanto por imigração/refúgio, e adentra nas clínicas psicológicas. Para suprir tal descompasso, este artigo tem como objetivo apresentar pontos de atenção a serem considerados quando se trata da saúde mental de mulheres muçulmanas e das especificidades do cuidado direcionado a elas. Para tanto, tomou-se como disparador a série turca 8 em Istambul, que expõe os meandros de um atendimento psicológico a uma mulher muçulmana que usa hijab, o véu islâmico. A análise do corpus fez emergir três eixos reflexivos, que apontam: (1) a necessidade de psicoeducação da comunidade muçulmana diante das tensões e dos estigmas cristalizados; (2) o etnocentrismo e a islamofobia como obstáculos ao processo psicoterapêutico; e (3) a importância da escuta sensível para uma prática psicológica aberta à diversidade cultural-religiosa. Este estudo, em confluência com pesquisas etnográficas conduzidas em campo islâmico brasileiro, permite destacar possibilidades, desafios e aspectos éticos da relação entre religião e cuidado em saúde mental.


Abstract Despite the growing interest in the dimension of religiosity in the health area, Muslims have been neglected in the psychology field. Mental health professionals know little about this religion and its followers. However, the Muslim population is growing, both through reversions and immigration/refuge, leading them to seek psychological care services. To address this divergence, this article aims to present points that should be considered about Muslim women's mental health. To this end, we considered the Turkish series Ethos, which depicts the psychotherapeutic process of a Muslim woman who wears the hijab, the Islamic veil. The analysis of the corpus led to three axes, pointing to (a) the need for psychoeducation of the Muslim community in the face of tensions and stigmas; (b) ethnocentrism and Islamophobia as obstacles to the psychotherapeutic process; and (c) the importance of sensitive listening for a psychological practice open to cultural-religious diversity. The study, in confluence with ethnographic research conducted in the Brazilian Islamic field, allows us to highlight possibilities, challenges, and ethical aspects of the relationship between religion and mental health care.

RESUMO A humanização visa a transformação do modelo institucional de saúde por meio da valorização dos aspectos subjetivos, históricos e socioculturais dos processos de saúde e doença. Entretanto, por carregar múltiplos sentidos, sua implementação tem sido um desafio em diversos serviços públicos de saúde. Diante disso, este trabalho teve como objetivo apreender as representações sociais sobre humanização na pediatria entre profissionais de saúde que atuam neste contexto. Trata-se de uma pesquisa qualitativa, realizada em um hospital universitário do Nordeste, que contou com a participação de 12 profissionais de saúde de oito categorias. Para apreensão dos dados recorreu-se à entrevista semiestruturada, cujos dados foram analisados por meio da análise categorial temática. Os dados permitiram a construção de três categorias temáticas: o contexto da pediatria, práticas assistenciais e humanização. Constatou-se que embora seja gratificante trabalhar na pediatria, existem dificuldades nas relações entre os próprios profissionais e entre os profissionais e as cuidadoras das crianças. As práticas assistenciais respaldam-se principalmente em valores humanísticos expressos nas vinculações afetivas e no uso eficaz do raciocínio clínico e dos materiais. A humanização, por sua vez, foi representada predominantemente por valores humanísticos, embora existam alguns elementos representacionais que referenciem aspectos da Política Nacional de Humanização. Constatou-se a ausência de argumentos pautados no direito do usuário a uma assistência de qualidade. Fazem-se necessárias, portanto, ações que possam promover reflexões e estratégias em humanização, no sentido de favorecer ressignificações no modo de pensar e concretizar a proposta da humanização em saúde.


RESUMEN La humanización tiene como objetivo transformar el modelo institucional de salud mediante la valoración de los aspectos subjetivos, históricos y socioculturales de los procesos de salud y enfermedad. Sin embargo, debido a que tiene múltiples significados, su implementación ha sido un desafío en varios servicios de salud pública. Por lo tanto, este estudio tuvo como objetivo aprehender las representaciones sociales de la humanización en pediatría entre los profesionales de la salud que trabajan en este contexto. Es una investigación cualitativa, realizada en un hospital universitario del noreste, con la participación de doce profesionales de la salud de ocho categorías. Para aprehender los datos, se utilizó una entrevista semiestructurada, cuyos datos se analizaron mediante análisis categórico temático. Los datos permitieron la construcción de tres categorías temáticas: el contexto de la pediatría, las prácticas de atención y la humanización. Se descubrió que, aunque es gratificante trabajar en pediatría, existen dificultades en las relaciones entre los propios profesionales y entre los profesionales y los cuidadores de los niños. Las prácticas de atención se basan principalmente en valores humanísticos expresados ​​en lazos afectivos y en el uso efectivo del razonamiento clínico y los materiales. La humanización, a su vez, estuvo representada predominantemente por valores humanistas, aunque hay algunos elementos de representación que se refieren a aspectos de la Política Nacional de Humanización. Se encontró la ausencia de argumentos basados en el derecho del usuario a una asistencia de calidad. Por lo tanto, es necesario tomar acciones que puedan promover reflexiones y estrategias en humanización, a fin de promover nuevos significados en la forma de pensar y concretar la propuesta de humanización en salud.


ABSTRACT. Humanization aims at transforming the institutional model of health by valuing the subjective, historical and socio-cultural aspects of health and disease processes. However, because it carries multiple meanings, its implementation has been a challenge in several public health services. Therefore, this study aimed to apprehend the social representations about humanization in pediatrics among health professionals who work in this context. It is a qualitative research, carried out in a university hospital in the northeast, with the participation of twelve health professionals from eight categories. To apprehend the data, a semi-structured interview was used, whose data were analyzed through thematic categorical analysis. The data allowed the construction of three thematic categories: the context of pediatrics, care practices and humanization. It was found that although it is gratifying to work in pediatrics, there are difficulties in the relationships between the professionals themselves and between the professionals and the caregivers of the children. Care practices are mainly supported by humanistic values ​​expressed in affective ties and in the effective use of clinical reasoning and materials. Humanization, in turn, was represented predominantly by humanistic values, although there are some representational elements that refer to aspects of the National Humanization Policy. The absence of arguments based on the user's right to quality assistance was found. Therefore, it is necessary to take actions that can promote reflections and strategies in humanization, in order to promote new meanings in the way of thinking and concretizing the proposal of humanization in health.

Resumo O SUS trouxe os princípios de universalidade, integralidade e equidade para nortear as ações e os serviços de saúde, que devem ser livres de preconceito e discriminação. A rede formal de cuidados foi compreendida como a que está institucionalizada e a rede informal é uma rede afetiva. O objetivo da pesquisa foi compreender o acesso à rede de cuidados à saúde na percepção de transexuais em um município da Zona da Mata mineira, por meio da abordagem metodológica qualitativa com estudo descritivo. Foram realizadas entrevistas semiestruturadas com oito pessoas transexuais. As informações foram analisadas por meio da hermenêutica-dialética. Foram identificadas como barreiras à rede formal de cuidados o desrespeito ao uso do nome social nos serviços de saúde e o preconceito dos profissionais. Buscam-se cuidados e informações sobre uso de hormônios nas redes informais. A rede informal acolhe, mas a busca por cuidados somente nessas redes pode trazer riscos à saúde. A pesquisa evidenciou a existência de uma forte rede afetiva no município e a necessidade do estabelecimento de uma sólida rede formal de cuidados para a população transexual com ampla participação da rede informal. A rede formal é instável e não atende as necessidades de saúde dessa população.


Abstract SUS brought the principles of universality, integrality, and equity to guide health actions and services, which must be free from any form of prejudice and discrimination. The formal care network was understood as the one that is institutionalized, and the informal network is an affective network. This research aimed to understand the access to the health care network in the perception of transsexuals in a municipality in Zona da Mata, Minas Gerais, through the qualitative methodological approach with descriptive study. Semi-structured interviews were conducted with eight transsexual people. The information was analyzed using hermeneutics-dialectics. Barriers to the formal care network were disrespect to the use of the social name in health services, prejudice, and unpreparedness of professionals. Therefore, care and information on hormone use are sought in informal networks. The informal network welcomes, but the search for care only in these networks can bring health risks. The research showed the existence of a strong affective network in the municipality and the need to establish a solid formal care network for the transsexual population with wide participation in the informal network, since the formal network is unstable and does not meet health needs of that population.

ABSTRACT

Objective: To evaluate the efficacy of intracervical injection of liposomal bupivacaine for postoperative pain control among women undergoing minimally invasive supracervical hysterectomy. Methods: A randomized double-blinded placebo-controlled trial of intracervical injection of combination liposomal bupivacaine and bupivacaine for postoperative pain among patients undergoing laparoscopic and robotic supracervical hysterectomy. Patients were enrolled between October 1, 2018 and April 30, 2019. The primary outcome was pain at 12 hours postoperatively using a numeric rating scale from zero to 10. Pain scores were also recorded pre-operatively, immediately postoperatively, at 12, 24, and 48 hours postoperatively. The secondary outcome was the number of patients who required opioid analgesic medications up to 48 hours postoperatively. Results: Sixty participants were randomized into the control (n = 30) and intervention (n = 30) groups. Pain scores were 1 and 1.75 (p = 0.89) immediately postoperatively, 3 and 3.5 (p = 0.85) at 12 hours, 3.5 and 5 (p = 0.22) at 24 hours, and 2.75 and 4 (p = 0.18) at 48 hours for the control and intervention groups, respectively. Within the first 24 hours, 10 patients in the control and 14 patients in the intervention group used narcotics (p = 0.37). From the 24 to 48 hours window, 6 and 8 patients in the control and intervention groups used narcotics (p = 0.74), respectively. Conclusion: There was no statistically significant difference in pain scores between patients receiving combination liposomal bupivacaine and bupivacaine intracervical block and those receiving placebo in the first 48 hours after surgery. There was no difference in analgesic use between the two study groups.

Resumo Os estudos de gênero e sexualidade vêm se destacando nas pautas atuais de pesquisas realizadas em campo islâmico. Existe dentro da religião um incentivo aos prazeres que contraria a visão estereotipada da opressão e repressão da mulher muçulmana, bem como a relação problemática entre feminismo e religião que muitas acadêmicas e militantes muçulmanas vêm tentando desconstruir. A partir da etnografia realizada pelas autoras, são tecidas reflexões sobre a sexualidade no Islã a partir de uma perspectiva decolonial. Busca-se ressaltar as práticas e os sentidos que as mulheres muçulmanas brasileiras revertidas ao Islã atribuem à sexualidade, dentro dos seguintes eixos: (a) vestimenta islâmica e a fetichização da muçulmana; (b) lícito/ilícito e a capacidade de agência e protagonismo dessas mulheres; (c) conhecimento religioso e empoderamento feminino. A pesquisa permite destacar dimensões das experiências das mulheres muçulmanas que diferem da posição de subalternidade a qual são constantemente submetidas.


Resumen Los estudios de género y sexualidad se han destacado en la actual agenda de investigación llevada a cabo en el campo islámico. Dentro de la religión existe un incentivo al placer que va en contra la visión estereotipada de la opresión y represión de las mujeres musulmanas, así como de la relación problemática entre el feminismo y la religión que muchas académicas y militantes musulmanas están intentando deconstruir. A partir de la etnografía realizada por las autoras, las reflexiones sobre la sexualidad en el Islam se realizan desde una perspectiva decolonial. El artículo busca resaltar las prácticas y los significados que las mujeres musulmanas brasileñas que volvieron al Islam atribuyen a la sexualidad, dentro de los siguientes ejes: (a) vestimenta islámica y fetichización de las musulmanas; (b) lícito/ilícito y la capacidad de agencia y protagonismo de estas mujeres; (c) conocimiento religioso y empoderamiento femenino. La investigación nos permite destacar dimensiones de las vivencias de mujeres musulmanas que se diferencian de la posición subordinada a la que están constantemente sometidas.


Abstract Gender and sexuality studies are prominent topics in current research regarding the Islamic field. There is an incentive to pleasure within religion that contradicts the oppressed and repressed Muslim woman stereotype, as well as the problematic relationship between feminism and religion that many Muslim academics and militants have been trying to deconstruct. Reflections on sexuality in Islam from a decolonial perspective derives from the ethnography conducted by the authors. The article seeks to highlight the practices and meanings that Brazilian Muslim women who reverted to Islam attribute to sexuality, within the following axes: (a) Islamic clothing and the fetishization of Muslim women; (b) licit/illicit and the capacity of agency and protagonism of these women; (c) religious knowledge and women's empowerment. The research shows dimensions of the experiences of Muslim women that differ from the position of subalternity to which they are constantly submitted.

ABSTRACT

BACKGROUND: A scheduled pre-viable hysterectomy is a treatment option for women with early diagnosed placenta accreta spectrum who do not wish future fertility. A minimally invasive hysterectomy with pregnancy in situ for placenta accreta spectrum has not been previously reported. CASE: A patient with evidence of placenta accreta spectrum on prenatal imaging underwent an elective robot-assisted laparoscopic hysterectomy at 16 weeks of gestation. The procedure was uncomplicated and she was discharged on postoperative day 1. Pathology was consistent with placenta percreta. CONCLUSION: Robot-assisted laparoscopic hysterectomy with pregnancy in situ is feasible in a patient with placenta accreta spectrum in the second trimester. TEACHING POINTS: 1. Early diagnosis of placenta accreta spectrum is important for surgical planning and management. 2. We present a technique for minimally invasive hysterectomy in a patient with placenta accreta spectrum diagnosed before viability.

OBJECTIVE:: To identify symptoms of anxiety, depression, and feelings of hopelessness in patients in outpatient treatment for substance dependency and to test for correlations with various aspects of their quality of life. METHODS:: A cross-sectional study of a sample of 25 men in recuperation from substance dependency, selected by convenience. We assessed symptoms of depression (Beck Depression Inventory-II), anxiety (Beck Anxiety Inventory), hopelessness (Beck Hopelessness Scale), and quality of life (World Health Organization Quality of Life instrument-Abbreviated version [WHOQOL-Bref]), and also analyzed sociodemographic profile, substance abuse, and family history. Categorical variables were expressed as frequencies and percentages and quantitative variables as means and standard deviations or as medians and interquartile ranges. We also analyzed Spearman correlations to a 5% significance level. RESULTS:: The study revealed prevalence rates of 32% for depression, 24% for anxiety, and 12% for hopelessness, at a moderate/severe level. Correlations between Beck scales and WHOQOL-Bref were significant; but impacts differed in the four areas evaluated. CONCLUSIONS:: Overall, we observe global negative impacts on subjects' lives, affecting their psychiatric symptoms and quality of life and their relationships and occupational factors to a similar degree. The results show that the lower the scores on these scales, the better the quality of life in some areas, indicating that there is a negative correlation between psychiatric symptoms and quality of life.

Resumo Objetivo: Identificar sintomas de ansiedade, depressão e sentimentos de desesperança em pacientes dependentes químicos em tratamento ambulatorial e verificar a existência de correlações com os diferentes âmbitos da qualidade de vida. Métodos: Estudo transversal com 25 homens em recuperação para dependência química, selecionados por conveniência. Foram avaliados sintomas de depressão (Inventário de Depressão de Beck-II), ansiedade (Inventário de Ansiedade de Beck) desesperança (Escala de Desesperança de Beck) e qualidade de vida (World Health Organization Quality of Life instrument-Abbreviated version [WHOQOL-Bref]), bem como perfil sociodemográfico, uso de substâncias e histórico familiar. Variáveis categóricas foram apresentadas como frequências e percentuais, e variáveis quantitativas, como médias e desvios-padrão ou medianas e intervalos interquartis. Foram utilizadas também correlações de Spearman com nível de significância de 5%. Resultados: O estudo revelou prevalência de 32% para depressão, 24% para ansiedade e 12% para desesperança, em níveis moderado/grave. As correlações entre os inventários Beck e o WHOQOL-Bref foram significantes; entretanto, os impactos diferiram nos quatro domínios avaliados. Conclusões: De modo geral, observaram-se prejuízos globais na vida dos sujeitos, tanto frente a sintomas psiquiátricos e qualidade de vida quanto sobre relacionamentos e questões ocupacionais, em níveis semelhantes. Os resultados apontam que, quanto menor a pontuação nesses inventários, melhor a qualidade de vida em alguns domínios, indicando haver correlação negativa entre sintomas psiquiátricos e qualidade de vida.

ABSTRACT

Objetivo Identificar o estado nutricional e hábitos alimentares de pacientes masculinos em recuperação de dependência química em acompanhamento ambulatorial de uma unidade de adição. Métodos Estudo transversal com 25 pacientes adultos em tratamento ambulatorial para dependência química. Foram aferidos parâmetros antropométricos (peso, estatura, circunferência da cintura e índice de massa corporal) e de composição corporal (bioimpedância elétrica), e foram investigados hábitos alimentares (Questionário de Frequência Alimentar – QFA). As variáveis categóricas são apresentadas como frequências e percentuais e as variáveis contínuas, como média e desvio-padrão ou como mediana e intervalo interquartil. Resultados Observou-se índice de massa corporal médio de 27,73 ± 4,15 kg/m2, com prevalência de sobrepeso de 80% e obesidade de 8%. A média da circunferência da cintura foi de 96,60 ± 9,84 cm e a de percentual de gordura corporal, de 23,24 ± 6,44. A maior parte da amostra estudada [20 (80%)] refere realizar quatro ou mais refeições por dia e 72% referiram aumento do consumo alimentar no período de abstinência. A preferência por alimentos específicos no período de abstinência foi relatada por 12 (48%) pacientes. Em relação ao consumo de alimentos ultraprocessados, destaca-se o consumo diário de pão francês (68%), pães brancos de forma (16%), sucos artificiais (48%), refrigerantes (32%), queijo amarelo (36%), embutidos com alto teor de gordura (36%) e balas e chicletes (32%). Conclusão Os resultados mostram prevalência elevada de sobrepeso e obesidade, além de medida de circunferência da cintura alterada, relato de aumento da ingestão alimentar e consumo diário e semanal elevado de alimentos ultraprocessados. .


Objective To identify the nutritional status and dietary habits of male patients recovering from chemical dependency in an ambulatory treatment of an addition unit. Methods Crosssectional study with 25 male patients in ambulatorial treatment for chemical dependency. Anthropometric parameters (weight, height, waist circumference and body mass index) and body composition (bioelectrical impedance) were measured and dietary habits (Food Frequency Questionnaire) were investigated. Categorical variables are shown as frequencies and percentages and continuous variables as mean and standard deviation or as median and interquartile range. Results It was found a body mass index mean of 27.73 ± 4.15 kg/m2, with a prevalence of overweight in 88% of the sample. The waist circumference mean was 96.60 ± 9.84 cm and a percentual body fat mass of 23.24 ± 6.44. The sample majority [20 (80%)] refers do four or more meals a day and 72% reported an increased food intake during the period of abstinence. The preference for specific foods during the abstinence period was reported by 12 (48%) patients. Regarding the consumption of ultra-processed foods, there is a highlight daily consumption of French bread (68%), white pan bread (16%), artificial juices (48%), soft drinks (32%), high-fat cheese (36%), high-fat processed meat (36%), candy and gum (32%). Conclusion The study reveals a high prevalence of overweight and obesity, as well as altered waist circumference, increased food intake and daily and weekly consumption of ultra-processed foods. .

OBJECTIVES: The purpose of this study was to determine the interobserver reproducibility of renal volume measurement performed by 3-dimensional (3D) sonography. METHODS: We conducted an exploratory, quantitative, observational, and descriptive cross-sectional study. We calculated ranges, means, and standard deviations of the renal volumes obtained with 3D sonography by 2 different examiners from a sample of 30 patients. The interobserver reproducibility was assessed by calculating intraclass correlation coefficients (ICCs) and by construction of Bland-Altman plots. RESULTS: The ICC between the right 3D renal volumes obtained by examiners 1 and 2 was 0.8552 (95% confidence interval, 0.7153-0.9292), showing excellent reproducibility (P < .0001). For the left 3D renal volumes obtained by examiners 1 and 2, the ICC was 0.6411 (95% confidence interval, 0.3652-0.8135), showing average to good reproducibility (P = .0001). The Bland-Altman plots showed mean differences ± SD of -4.8% ± 23.9% between the right renal volumes obtained by examiners 1 and 2 and -2.2% ± 38% between the left renal volumes. The 95% limits of agreement between the right renal volumes obtained by examiners 1 and 2 were -28.7% to 19%, whereas those between the left renal volumes were -40.2% to 35.7%. CONCLUSIONS: Three-dimensional sonography showed good interobserver reproducibility, which was better for the right kidney. It appears necessary to establish standardized techniques for acquisition of sonographic renal volumes.

ABSTRACT

Obstructive sleep apnea (OSA) is characterized by recurrent upper airway obstruction occurring at the level of the pharynx during sleep. Although cephalometric analysis is an important method in the diagnosis of craniofacial deformities, CT and magnetic resonance imaging have been highlighted as the major imaging methods to investigate the possible causes of OSA, which, in most cases, is multifactorial. Magnetic resonance and CT both allow an excellent evaluation of the various anatomical planes of the site of obstruction, which enables better clinical assessment and surgical approach. This pictorial essay aims to describe the aspects that must be evaluated in the diagnostic imaging of patients presenting with the major predisposing factors for OSA.

A apneia obstrutiva do sono (AOS) é caracterizada por obstruções recorrentes das vias aéreas superiores durante o sono que ocorrem no nível da faringe. Apesar de a análise cefalométrica ser um importante método no diagnóstico das deformidades craniofaciais, a TC e a ressonância magnética vêm se destacando como os principais métodos de imagem para a investigação das eventuais causas da AOS que, na maioria das vezes, é multifatorial. Esses métodos permitem uma excelente avaliação nos diversos planos anatômicos do eventual sítio da obstrução, o que permite uma melhor avaliação clínica e abordagem cirúrgica. O presente ensaio pictórico tem como objetivo descrever os aspectos que devem ser avaliados no diagnóstico por imagem dos principais fatores predisponentes para a AOS.

Estudo longitudinal e prospectivo que procurou quantificar a variabilidade intra e interobservadores da medida da altura uterina (AU) e verificar a contribuição de um curso capacitatório para enfermeiros e graduandos de Enfermagem no viés de aferição da medida. Foram selecionados dois participantes, que realizaram duas medidas em vinte gestantes antes e após o curso. Calculou-se a variabilidade intra e interobservadores, o coeficiente de correlação intraclasse, o erro técnico de medida absoluto e relativo e a fiabilidade da medida. O teste t pareado mostrou redução estatisticamente significante (p=0,0396) no viés de aferição. Conclui-se que a capacitação de enfermeiros e graduandos de Enfermagem na técnica de medição da AU reduz significativamente a variabilidade intraobservador e interobservadores.

Fueran seleccionados dos participantes que realizaron dos mediciones de cada 20 mujeres embarazadas antes y después del curso. Se calculó la variabilidad intra y inter-observador, el coeficiente de correlación intraclase, el error técnico de medición absoluta y relativa y la fiabilidad de la medida. El teste t de Student mostró reducción estadísticamente significativa (p=0,0396) en el sesgo. Se concluye que la formación de enfermeras y estudiantes de enfermería en la técnica de medición de la AU redujo significativamente la variabilidad intra y inter-observador.
