ABSTRACT

A 38-year-old man without a severe traumatic history reported to the outpatient department (OPD) with wrist pain at the dorsal aspect, mild swelling, stiffness, and restricted mobility at the left wrist joint. The patient had been experiencing these symptoms for a year. There was sharp tenderness, graded as 4 above the lunate bone, on examination of the left wrist joint. Advanced imaging, which is magnetic resonance imaging (MRI), and radiographs suggested that the patient had Kienbock's illness. Typically, the surgical approach for Kienbock's used is wrist fusion or proximal row carpectomy. However, in this case, a novel strategy of bone grafting, scaphoid-capitate fusion, and lunate excision was adopted. This case report explains the outcome of our scaphoid-capitate arthrodesis, which was done to maintain functional mobility and relieve discomfort by halting the progression of carpal collapse and carpal-ulnar translation.

ABSTRACT

This article presents a planar, non-angular, series-fed, dual-element dipole array MIMO antenna operating at 28 GHz with the metasurface-based isolation improvement technique. The initial design is a single-element antenna with a dual dipole array which is series-fed. These dipole elements are non-uniform in shape and distance. This dipole antenna results in end-fire radiation. The dipole antenna excites the J1 mode for its operation. Further, with the view to improve channel capacity, the dipole array expands the antenna to a three-element MIMO antenna. In the MIMO antenna structure, the sum of the J1, J2, and J3 modes is excited, causing resonance at 28 GHz. This article also proposes a metasurface structure with wide stopband characteristics at 28 GHz for isolation improvement. The metasurface is composed of rectangle-shaped structures. The defected ground and metasurface structure combination suppresses the surface wave coupling among the MIMO elements. The proposed antenna results in a bandwidth ranging from 26.7 to 29.6 GHz with isolation improvement greater than 21 dB and a gain of 6.3 dBi. The antenna is validated with the diversity parameters of envelope correlation coefficient, diversity gain, and channel capacity loss.

ABSTRACT

Distal radius fractures (DRFs) are prevalent among all hand injuries, commonly due to a fall on an outstretched hand. Not being treated properly can cause many complications such as malunion, non-union, reduced range of motion, and muscle strength. This case report presents a multidisciplinary approach to the physiotherapeutic management of a DRF treated with closed reduction internal fixation using K-wires. The rehabilitation protocol incorporated matrix rhythm therapy (MRT), a novel therapeutic technique, in combination with targeted therapeutic exercises. The study outlines the patient's journey from injury to recovery, detailing the integration of MRT sessions alongside conventional physiotherapy exercises. The comprehensive rehabilitation aimed to enhance pain relief, restore range of motion, and improve functional outcomes. The case highlights the synergistic benefits of incorporating MRT into traditional rehabilitation strategies through assessment, personalized treatment planning, and regular progress evaluations. The outcomes underscore the potential of this combined approach in optimizing the recovery process.

ABSTRACT

Broadband antennas with a low-profile generating circular polarization are always in demand for handheld/ portable devices as CP antennas counter multipath and misalignment issues. Therefore, a compact millimeter-wave antenna is proposed in this article. The proposed antenna structure comprises two circular rings and a circular patch at the center. This structure is further embedded with four equilateral triangles at a 90° orientation. The current entering the radiator is divided into left and right circular directions. The equilateral triangles provide the return path for current at the differential phase of ±90°, generating circular polarization. Structural development and analysis were initially performed through the characteristic mode theory. It showed that Modes 1 to 4 generated good impedance matching from 20 to 30 GHz and Modes 1 to 5, from 30 to 40 GHz. It also demonstrated the summation of orthogonal modes leading to circular polarization. The antenna-measured reflection coefficient |S11| > 10 dB was 19 GHz (23-42 GHz), and the axial ratio at -3 dB was 4.2 GHz (36-40.2 GHz). The antenna gain ranged from 4 to 6.2 dBi. The proposed antenna was tested for link margin estimation for IoT indoor conditions with line-of-sight (LOS) and non-line-of-sight (NLOS) conditions. The communication reliability with co- and cross-polarization was also studied under these conditions, and the results proved to be satisfactory.

ABSTRACT

Friedreich's ataxia (FA) is a debilitating, multisystemic disease caused by the depletion of frataxin (FXN), a mitochondrial iron-sulfur (Fe-S) cluster biogenesis factor. To understand the cellular pathogenesis of FA, we performed quantitative proteomics in FXN-deficient human cells. Nearly every annotated Fe-S cluster-containing protein was depleted, indicating that as a rule, cluster binding confers stability to Fe-S proteins. We also observed depletion of a small mitoribosomal assembly factor METTL17 and evidence of impaired mitochondrial translation. Using comparative sequence analysis, mutagenesis, biochemistry, and cryoelectron microscopy, we show that METTL17 binds to the mitoribosomal small subunit during late assembly and harbors a previously unrecognized [Fe4S4]2+ cluster required for its stability. METTL17 overexpression rescued the mitochondrial translation and bioenergetic defects, but not the cellular growth, of FXN-depleted cells. These findings suggest that METTL17 acts as an Fe-S cluster checkpoint, promoting translation of Fe-S cluster-rich oxidative phosphorylation (OXPHOS) proteins only when Fe-S cofactors are replete.

ABSTRACT

Organophosphate poisoning (OPP) results from the occupational, accidental, or suicidal intake of organophosphate pesticides (OPs). There is a huge limitation in the available literature on OPP cases and the role of physiotherapy in such cases. We report a case of a 33-year-old farmer, found in a state of ingested insecticide and admitted to the emergency department. The patient had two to three episodes of vomiting, associated with continuous tremors in his hands and legs. Immediately infused with atropine, the patient's general condition deteriorated, and he was intubated with an endotracheal tube. With the signs of long-term intubation, a tracheostomy was done. A respiratory therapy consult was taken for indications of intermediate syndrome and to achieve early weaning. The patient's referral was received in view of the long-term requirement of bronchial hygiene and the need for early ambulation as well as psychological support. Informed consent was taken from the family prior to the commencement of the treatment. Respiratory therapy interventions included body positioning, bronchial hygiene techniques, chest proprioceptive neuromuscular facilitation (PNF) techniques, and mobility exercises to achieve early ambulation. Physiotherapists have the appropriate training, knowledge, and skills to deliver the exercise components and help patients return to their activities of daily living. Significant levels of improvement have been seen in the general condition of the patient. The overall functioning of the patient's health was seen as improved on the scales of consciousness, early mobility in the step-down unit, and quality of life.

Background Chronic obstructive pulmonary disease (COPD) is a heterogeneous lung condition, yielding various respiratory symptoms and categorized under several descriptors: early, mild, young, pre-COPD, and preserved ratio impaired spirometry. COPD is synonymous with symptoms such as dyspnea and cough, in addition to others like exercise intolerance, which result from respiratory muscle weakness. Therefore, the emergence of respiratory strength assessment tools for such patients is not surprising. However, evidence is limited regarding the impact of respiratory muscle strength on the physical performance of COPD patients. Therefore, this study employs the MicroRPM device (Medikart HealthCare Systems Pvt. Ltd., Delhi, India) to measure maximum inspiratory and expiratory pressure utilizing mouth pressure. Methodology We recruited a total of 40 patients for the study. All patients received a thorough assessment for hemodynamic stability and were categorized according to the Global Initiative for Chronic Obstructive Lung Disease criteria of COPD. The patients then underwent a training session for the MicroRPM device. We took each patient's inspiratory and expiratory pressure measurements, then determined their six-minute walk distance and modified the Borg scale rating. Results We observed no significant correlation between maximum inspiratory pressure (Pimax) and six-minute walk distance (r=-0.023, p=0.890) or modified Borg scale (r=-0.044, p=0.788); additionally, the correlation between maximum expiratory pressure (Pemax) and modified Borg scale was not significant (r=-0.192, p=0.235). However, the correlation between Pemax and six-minute walk distance was both negative and significant (r=-0.384, p=0.014). Conclusion Based on our results, respiratory muscle strength can influence the aerobic performance of COPD patients.

ABSTRACT

Catamenial pneumothorax is one of the most common extra-pelvic presentations of endometriosis, with the gastrointestinal tract being the most common location. Catamenial pneumothorax is defined as spontaneous recurrent pneumothorax occurring in women of reproductive age in a temporal relationship with menses. Symptoms include dyspnea, sharp chest pain, and hypoxemia. A much rarer presentation is the involvement of endometriosis with the diaphragm. In this case, we present a 31-year-old female who presented with signs of pneumothorax. She has had multiple episodes leading to suspicion of catamenial pneumothorax. However, it wasn't until her surgery that the extent of diaphragmatic involvement, characterized by numerous holes secondary to endometriosis, was discovered. She was surgically treated, which led to a drastic improvement in symptoms and a reduction in subsequent episodes. We hope that this case can add to the current limited literature on diaphragmatic endometriosis cases. Since this patient presented with mainly catamenial pneumothorax symptoms, we urge clinicians to still consider diaphragmatic involvement as a primary cause in patients with recurrent episodes of pneumothorax.

ABSTRACT

Basal cell carcinoma (BCC) is one of the most common skin malignancies in the United States. It commonly presents in the neck and head as a papulo-nodular lesion that is slow-growing with rare metastasis. BCC has a high curative rate often with Mohs surgery or excision of the lesion. In this case, we present an 80-year-old male who presented to the emergency department with a necrotizing wound on his left hand and met sepsis criteria. His necrotizing wound was identified as severe BCC of his hands which he has a 10-year history diagnosis of. He received appropriate sepsis measures with fluids, antibiotics, and blood transfusions but denied any further care for his BCC. Our case is unique because hands are an extremely uncommon presentation location of BCC. It is also unique because he presented as an extremely aggressive case while most cases of BCC are relatively innocuous. Early management of BCC is important, especially with a multidisciplinary approach as it can significantly reduce mortality risk.

Iron-sulfur clusters (ISC) are essential cofactors that participate in electron transfer, environmental sensing, and catalysis. Amongst the most ancient ISC-containing proteins are the ferredoxin (FDX) family of electron carriers. Humans have two FDXs- FDX1 and FDX2, both of which are localized to mitochondria, and the latter of which is itself important for ISC synthesis. We have previously shown that hypoxia can eliminate the requirement for some components of the ISC biosynthetic pathway, but FDXs were not included in that study. Here, we report that FDX1, but not FDX2, is dispensable under 1% O2 in cultured human cells. We find that FDX1 is essential for production of the lipoic acid cofactor, which is synthesized by the ISC-containing enzyme lipoyl synthase. While hypoxia can rescue the growth phenotype of either FDX1 or lipoyl synthase KO cells, lipoylation in these same cells is not rescued, arguing against an alternative biosynthetic route or salvage pathway for lipoate in hypoxia. Our work reveals the divergent roles of FDX1 and FDX2 in mitochondria, identifies a role for FDX1 in lipoate synthesis, and suggests that loss of lipoic acid can be tolerated under low oxygen tensions in cell culture.

ABSTRACT

There are many presentations of iron-deficiency anemia associated with pica in adults, but there is a lack of literature summarizing its different presentations. In this scoping review, we sought to identify the various presentations and if treatment of iron-deficiency anemia resolved the presenting symptoms of pica. This review was conducted by completing the Preferred Reporting Items for Systematic Review and Meta-Analysis extension for Scoping Reviews (PRISMA-Scr) checklist. The following electronic databases were searched for potentially eligible articles: PubMed, ProQuest, and Bielefeld Academic Search Engine (BASE). Study screening procedures were completed with a narrative synthesis. The data is synthesized and interpreted by sifting, charting, and sorting based on organ systems. Twenty articles met the inclusion criteria and were included in the scoping review. Regardless of other clinical presentations, the identification of pica symptoms allowed treatment for iron deficiency and led to the resolution of all symptoms in all 20 articles. Therefore, it is imperative to map the available evidence to inform clinicians and allow for better patient care.

ABSTRACT

The increased traffic in e-commerce, cloud-based processing, social media, and online video streaming demands higher data rates. The current 4G has reached the bottleneck, due to which it may not be able to fulfill the high data demand, so the focus is drifting toward millimeter wave (mmWave). The mmWave spectrum ranging from 30 to 300 GHz offers wide bandwidth with low latency, which finds its application in various communication fields, including 5G cellular. Despite its atmospheric attenuation and non-line-of-sight (NLOS) propagation, most countries are currently adopting mmWave 5G at the 28/38 GHz band due to less atmospheric attenuation, low path loss exponent, and low signal spread at these bands. The single-element patch antenna is a compact solution for mmWave applications, but its performance is inferior in terms of bandwidth, gain, and radiation efficiency. The array antennas have overcome these demerits, as it has shown a significant increase in bandwidth, gain, and radiation efficiency. Still, it has a limitation on data rate support. As a result, Multiple-Input-Multiple-Output (MIMO) technology can increase the data rate to 1000 times through spatial diversity and multiplexing techniques. So, to refine the performance further, there is a need to comprehend the MIMO antenna structures designed so far at mmWave. This paper presents the planar MIMO antenna structures developed so far, categorized here as slot, coplanar waveguide, defected ground structures, tapered/Vivaldi, meta-surface/metamaterial, dielectric resonator, and flexible antennas. The performance of these designs is compared based on bandwidth, gain, isolation, efficiency, and radiation pattern. This article also discusses the effects of slots, partial ground, and decoupling structures on impedance matching, bandwidth, and isolation levels. Also, a thorough discussion of the design issues and future work to be undertaken is discussed in this here.

Precautionary measures such as physical distancing, wearing a mask, and hand hygiene to suppress virus transmission necessitated a shift in the communication paradigm.The study aimed to check the effects of wearing masks (N95, surgical, and cloth) due to the COVID-19 pandemic on interpersonal communication in audiology-speech-language pathology clinical setup from a clinician perspective. A total of 105 participants, 17 males, and 88 females, in the age range of 19 to 29 years (Mean age = 21.41 years; S.D = 1.6), participated in the study. A questionnaire consisting of 15 close-ended questions grouped into five major categories, Communication Effectiveness (3 questions), Visual Cues (5 questions), Physiological Effect (4 questions), Palliative Effect (1 question), and Environment Effect (2 questions) was framed. Procedure: Participants rated the questions using a binary forced-choice as either Yes or No adapted into a google form. Results showed that most questions in all five categories received an above-average "yes" response. A significant association between questions in communication effectiveness with visual cues and physiological effects was noticed, leading to the conclusion that wearing face masks impacted overall communication by affecting various parameters of speech, majorly, the voice. It was also seen that of all the participants, 60% used N95, 32.4% used cloth, and only 7.6% used surgical face masks. Speech-language pathologists have a significant role in facilitating oral/ verbal communication when such barriers are encountered in clients with communication disorders and fellow professionals with strategies to strengthen oral/ verbal communication.

In this current era of communications and networking, The Internet of things plays the main role in the making of smart communication and networking. In this article, we have focused on the literature survey on wireless sensor networks which are energy efficient. Various standard protocols are reviewed along with some enhanced protocols which makes the network energy efficient. The comparison of the standard and enhanced protocols with respect to various applications in wireless sensor networks is thoroughly done in this article. The outcomes of the enhanced protocols are also briefly discussed. For easier analysis to future researchers, a comparative table which lists the enhanced protocols which are compared with standard counterparts along with the factors for energy efficiency of the protocols. This article also comments on the issues and challenges of the protocols which can be further analyzed for making the wireless sensor network more energy efficient.

ABSTRACT

Advancement in smartwatch sensors and connectivity features demands low latency communication with a wide bandwidth. ISM bands below 6 GHz are reaching a threshold. The millimeter-wave (mmWave) spectrum is the solution for future smartwatch applications. Therefore, a compact dual-band antenna operating at 25.5 and 38 GHz is presented here. The characteristics mode theory (CMT) aids the antenna design process by exciting Mode 1 and 2 as well as Mode 1-3 at their respective bands. In addition, the antenna structure generates two traverse modes, TM10 and TM02, at the lower and higher frequency bands. The antenna measured a bandwidth (BW) of 1.5 (25-26.5 GHz) and 2.5 GHz (37-39.5 GHz) with a maximum gain of 7.4 and 7.3 dBi, respectively. The antenna performance within the watch case (stainless steel) showed a stable |S11| with a gain improvement of 9.9 and 10.9 dBi and a specific absorption rate (SAR) of 0.063 and 0.0206 W/kg, respectively, at the lower and higher bands. The link budget analysis for various rotation angles of the watch indicated that, for a link margin of 20 dB, the antenna can transmit/receive 1 Gbps of data. However, significant fading was noticed at certain angles due to the shadowing effect caused by the watch case itself. Nonetheless, the antenna has a workable bandwidth, a high gain, and a low SAR, making it suitable for smartwatch and IoT applications.

Introduction COVID-19 is an infectious illness that first appeared in Wuhan, China in December 2019, and subsequently spread over the entire world. Those who were affected suffered from fever, cough, weakness, and breathlessness, along with the probability of developing pneumonia which sometimes leads to respiratory failure. The older population with co-morbidities was at higher risk. Methods In this experimental study, 59 subjects with COVID-19 were included according to pre-decided inclusion and exclusion criteria. Pre-vitals before the treatment were noted such as heart rate, respiratory rate, and oxygen saturation (SPO2), and then the chest proprioceptive neuromuscular facilitation (PNF) technique was given as a treatment regime, and its immediate effect was calculated. Post-vitals were noted immediately after the treatment session. The statistical analysis was made using the student's paired t-test. Results We found a mean difference of 9.45±5.27 in heart rate before and after the intervention provided which is suggestive of a statistically significant increase within normal limits. Also, the mean value of respiratory rate immediately after the intervention was shown as 22.91 and pre-intervention was 19.18 with a mean difference of 2.93±3.95, showing a statistically significant increase in respiratory rate. Additionally, the mean value of SPO2 immediately after the intervention was shown as 92.76 and pre-intervention was 94.64 and had a mean difference of 1.88±1.67 suggestive of high improvement in oxygen saturation immediately after the intervention. Conclusion The present study concludes from the available statistical analysis that chest PNF techniques such as intercostal stretch and anterior basal stretch lift have an immediate impact on improvement in oxygen saturation in COVID-19 patients and can be incorporated as an inpatient treatment regime in the rehabilitation protocol.

Objectives: The aim of this study was to investigate quality, reliability, and comprehensiveness of YouTube videos about oral self-examination to detect oral cancer and precancerous lesions and to assess whether the source, duration, quality, reliability, and/or comprehensiveness of videos influence their visibility and popularity. Materials and Methods: Videos on YouTube were searched using eight keywords similar to oral self-examination. The first 100 videos for each search term were included. Strict inclusion and exclusion criteria were adhered to, and videos were assessed for quality, reliability, and comprehensiveness. Upload date, views, likes, dislikes, duration, and source of the videos were noted. Viewing rate and interaction index were calculated for each video. Results: A total of 800 videos were analyzed, 24 of which met the inclusion criteria. Majority of videos (87%) were uploaded by the healthcare group. The visibility, quality, reliability, and comprehensiveness were higher in videos uploaded by the healthcare group when compared with the non-healthcare group, but not at a level of statistical significance. The mean interaction index score of the non-healthcare group was found to be higher than the healthcare group, with statistically significant difference. Duration of videos showed an impact on the comprehensiveness of the videos. Conclusion: There are relatively few videos on oral self-examination on YouTube, and most do not have satisfactory quality, reliability, and comprehensiveness. There is a potential to increase public awareness about oral self-examination by utilizing this tool. Videos with complete and accurate information regarding oral self-examination must be uploaded to YouTube, which is currently an important source of information for the general public.

ABSTRACT

With increasing urbanisation, the dengue disease burden is on the rise in India, especially in large cities such as Mumbai. Current dengue surveillance in Mumbai includes municipal corporation carrying out specific activities to reduce mosquito breeding sites and the use of insecticides to suppress the adult mosquito populations. Clinical cases remain either underreported or misreported due to the restriction to government clinics, missing the large private health care sector. There is a need for an integrated approach to manage dengue outbreaks in Mumbai. There are various novel strategies available for use that can be utilised to improve disease detection, mosquito surveillance, and control of mosquito-borne diseases. These novel technologies are discussed in this manuscript. Given the complex ecosystem of mosquito-borne diseases in Mumbai, integrating data obtained from these technologies would support the ongoing mosquito control measures in Mumbai.
