ABSTRACT

Background: Obesity and periodontitis are two commonly occurring disorders that affect a considerable amount of the world's population. Several studies have mentioned that there may be a link between the two. The purpose of this systematic review was to determine whether there was a difference in response to nonsurgical periodontal therapies (NSPTs) between obese and nonobese individuals. Materials and Methods: An online search was assembled with a combination of Medical Subject Headings terms and free-text words of the literature published up to December 2020, to identify interventional studies limited to an adult human population. Titles, abstracts, and finally full texts were scrutinized for possible inclusion by two independent investigators. Reduction in periodontal pocket depth was the primary parameter used to assess the outcome of NSPT. Results: The primary search yielded 639 significant titles and abstracts. After filtering, data extraction, and quality assessment, 34 full-text studies were selected. All studies matching inclusion criteria, suggest a positive association between obesity and periodontal disease. Conclusion: Although a possible correlation exists between periodontitis and obesity, as with other oral-systemic disease implications, some controversy exists. While some studies have reported a distinct correlation between periodontitis and obesity, other papers have suggested only moderate or no association between the two conditions at all. These results advise of a difference between response to NSPT amid obese and nonobese individuals. However, with few quality studies and variable reported findings, there is limited evidence of any significant difference in clinical practice. However, it can be a positive warning that obesity is a risk factor toward the outcome of periodontal disease treatment.

ABSTRACT

The implant-retained fixed partial denture, or bridge, is a highly efficient restorative modality. The primary challenge with this restoration, however, lies in achieving the optimal design of the pontic, which needs to be esthetic, comfortable, and easily cleansable by the patient, a factor that can directly enhance prosthesis longevity. An ovate design is desirable for the pontic area. The simplest, most efficient approach to producing an ovate pontic is to do so at the time of tooth extraction. However, usually the clinician is presented with a well-healed, rounded ridge that requires additional modification in order to create a proper ovate pontic. This article presents a simplified protocol for digital and surgical fabrication of a highly hygienic, esthetic implant bridge pontic. Using a digital design workflow, a provisional restoration is fabricated in the laboratory to ideal contours. This provisional is then utilized for a simple surgical procedure to create predictable hard- and soft-tissue contours for the pontic area. The protocol produces an extremely resilient tissue around the pontic, which also translates to an enhanced esthetic, hygienic, and functional design.

ABSTRACT

Because of their documented long-term success rates, dental implants have become a predictable treatment option to replace hopeless or missing teeth. However, full-arc transition from a hopeless dentition to a prosthesis supported by dental implants remains a challenge. One treatment option for this process is the use of transitional implants, an approach that allows for the transitioning of full-arch reconstructions from teeth to implant-supported prostheses without immediate loading the permanent implants or the need for provisional removable partial dentures. This article, which presents a long-term clinical case report, describes the steps and sequence of therapy involved in transitioning from a hopeless dentition to a maxillary full-arch implant-supported fixed prosthetic rehabilitation with the use of transitional implants, avoiding removable provisional prostheses. An assessment of the 20-year follow-up, dental implant survival rates, and marginal bone loss is also presented.

ABSTRACT

Background: Temporomandibular disorder (TMD) belongs to a heterogeneous group of musculoskeletal and neuromuscular conditions involving the temporomandibular joint complex, musculature and adjacent components. These conditions can generate signs and symptoms and be influenced by an altered biopsychosocial condition. Objective: This study aims to seek information to assist the patient in the presence of TMD signs and symptoms and Orofacial Pain, associated with the period of social isolation during the COVID-19 pandemic. Material and methods: For the preparation of this manual, a bibliographic search was performed in the databases PubMed, Latin American and Caribbean Health Sciences Literature (LILACS), BBO (BVS), Scopus, Web of Science and The Cochrane Library, using the keywords: orofacial pain, temporomandibular disorders, bruxism, stress, anxiety, biopsychosocial, diagnosis, self-care with important information on how to reduce and control the signs and symptoms of TMD and Orofacial Pain in this moment of pandemic that we are experiencing a social detachment. Results: The results show that the pandemic of COVID-19 and the need for social isolation, generates psychological impact that raises the pattern of anxiety and can directly affect patients with bruxism and TMD. Conclusion: Psychological factors associated with the pandemic can lead to an increased risk of developing, worsening and perpetuating bruxism, especially waking bruxism and TMD, so dentists should be aware of the occurrence of signs and symptoms to manage the multifactorial aspects of this condition. At that time, individual self-management strategies are advised for the patient, which consist of self-massage techniques, body education, exercise practices, sleep hygiene, meditation also the use of mobile apps and online tools that facilitate this activity. Introdução: A Desordem Temporomandibular (DTM) pertence a um grupo heterogêneo de condições musculoesqueléticas e neuromusculares envolvendo o complexo articular temporomandibular, a musculatura e os componentes adjacentes. Essas condições podem gerar sinais e sintomas e serem influenciadas por uma condição biopsicossocial alterada. Objetivo: Esse estudo teve como objetivo buscar informações que possam auxiliar ao paciente na presença de sinais e sintomas de DTM e Dor Orofacial associado ao período de isolamento social durante a pandemia do COVID -19. Material e Métodos: Para a elaboração deste manual, foi realizada uma pesquisa bibliográfica nas bases de dados PubMed, Latin American and Caribbean Health Sciences Literature (LILACS), BBO (BVS), Scopus, Web of Science e The Cochrane Library, utilizando as palavras-chaves: dor orofacial, desordem temporomandibular, bruxismo, estresse, ansiedade, biopsicossocial, diagnóstico e autocuidados, com informações importantes sobre como reduzir e controlar os sinais e sintomas de DTM e Dor Orofacial nesse momento de pandemia que estamos vivenciando um distanciamento social. Resultados: Os resultados mostram que a pandemia de COVID-19 e a necessidade de isolamento social, gera impacto psicológico que eleva o padrão de ansiedade e pode afetar diretamente pacientes com bruxismo e DTM. Conclusão: Fatores psicológicos associados à pandemia podem levar a um maior risco de desenvolver, piorar e perpetuar o bruxismo, principalmente bruxismo de vigília e DTM, por isso os cirurgiões-dentistas devem estar atentos a ocorrência de sinais e sintomas para gerenciar os aspectos multifatoriais dessa condição. Aconselha-se, nesse momento, estratégias individualizadas de autogerenciamento para o paciente que consistem em técnicas de automassagem, educação corporal, práticas de exercícios, higiene do sono e meditação. Além do uso de aplicativos digitais e ferramentas online facilitadores dessa atividade (AU)

ABSTRACT

The aim of this study was to investigate the effects of the use of an occlusal splint on postural balance considering the occlusal splint as a device for treating temporomandibular joint disorder. A randomized, controlled, prospective clinical trial was conducted. The research group consisted of 49 patients (36 as test group and 13 as control group) between 18 and 75 years old, both genders, diagnosed as temporomandibular disorder by Research Diagnostic Criteria/Temporomandibular Disorders questionnaire and magnetic resonance imaging of the temporomandibular joints. Test group was treated with orientations for physiotherapeutic exercises and occlusal splint, whereas control group received orientation for physiotherapeutic exercises only. Postural equilibrium was evaluated by means of a force plate. After 12 weeks, the groups were re-evaluated. Patients from both groups presented a significant increase in antero-posterior speed with eyes closed, test group (P < 0.001) and control group (P = 0.046). Only patients of the test group presented a significant increase in antero-posterior speed with eyes opened (P = 0.023). We concluded that the use of occlusal splint affected the postural balance.

ABSTRACT

Prevalence of peri-implant complication is expected to be on the rise with the increased number of implants being placed. Depending on the degree of osseous involvement, the clinician needs to decide if the treatment goal is to arrest the disease progression, regeneration or explantation and replacement. Host's medical status, defect configuration, aesthetic outcome, ability to access for plaque control post-treatment, and the patient's wishes are key factors to consider. The purpose of this review is to provide a contemporary synopsis on the management of peri-implantitis with emphasis on explantation. Guidance on the identification of factors/situations where salvaging an implant may be less favourable is discussed and the various techniques to remove a fractured, or peri-implantitis-affected non-mobile implant are described.

ABSTRACT

Recentes publicações clínicas e histológicas têm documentado o uso de implantes temporários, com grande aceitação, como modalidade de tratamento. A utilização de implantes temporários em pacientes parcial ou totalmente desdentados criou a possibilidade de utilizar uma prótese fixa provisória antes, durante e depois da colocação dos implantes permanentes, abolindo o uso da prótese provisória removível (dentadura), durante o período de osseointegração. Outro grande benefício da utilização dos implantes temporários é a proteção da carga sobre a crista alveolar, nas áreas em que serão feitos enxertos ósseos, e a possibilidade do Cirurgião-dentista ajustar a estética e a função antes da fabricação da prótese definitiva. Este artigo irá demonstrar as várias aplicações dos implantes temporários em diferentes tipos de restaurações com implantes, assim como o método de obtenção de um diagnóstico apropriado, a sequência para o plano de tratamento e as vantagens e desvantagens do uso de implantes temporários tanto a curto como a longo prazo
